Gouty panniculitis is caused by the deposition of urate crystals in the subcutaneous tissue, accompanied by a lobular panniculitis. It presents as subcutaneous nodules, most commonly located on the lower extremities. Being an unusual clinical presentation of gout, the sonographic findings of gouty panniculitis have been scarcely described in the literature. Our report describes the ultrasound features we found in a case of gouty panniculitis and the usefulness of this technique for diagnosis and monitoring of this disease.

5. Osteochondroma of the Thoracic Spine and Scoliosis

Results. The patient was treated with surgery in an attempt to remove the tumor and correct the aesthetic deformity. The results were satisfactory, with an improvement of the thoracolumbar scoliosis from 45° to 18°. Conclusions. Osteochondromas of the vertebral column may cause scoliosis. Computed tomography and magnetic resonance imaging are necessary for evaluating the origin, size, and characteristics of the tumor. In this case, surgical management involved resection of the tumor and correction of the scoliotic deformity. [Key words: computed tomography, magnetic resonance imaging, osteochondroma, scoliosis] Spine 2001;26:1082‐1085 Osteochondromas, also known as osteocartilaginous exostoses, are common tumors of bone that arise from any zone of endochondral bone formation in the context of a dysplastic disorder. They represent 9.3% of all bone tumors and 40% of all benign tumors. They may be solitary or multiple. Multiple osteochondromas can appear sporadically, but they usually occur as part of the genetic disorder known as hereditary multiple exostoses. This article describes a case of osteochondroma arising in T11 that caused rigid thoracolumbar scoliosis of 45° and presents a review of the literature.

La infección por Helicobacter pylori es una de las causas más frecuentes de infección bacteriana crónica en el ser humano. A través de distintos factores patogénicos (adhesinas, ureasa, proteínas citotóxicas cagA y vacA) puede producir cambios que se pueden apreciar en una endoscopia digestiva alta. Estos incluyen la aparición de lesiones ulcerosas, mucosa en empedrado y palidez alternando con eritema, entre otros. Sus dianas preferentes en la edad pediátrica son el antro gástrico y el duodeno proximal. Este artículo hace una revisión de los hallazgos endoscópicos relacionados con la infección por H. pylori en niños, así como de sus posibles causas y consecuencias clínicas, H. pylori infection is one of the most frequent chronic bacterial infections in human beings. Through different pathogenic factors (adhesins, urease, cagA and vacA cytotoxic proteins) it may produce mucosal changes that can be seen in an upper gastrointestinal endoscopy. These changes include, among others, ulcerative lesions, cobblestone mucosa and pallor alternating with erythema. Its favorite targets in children are the gastric antrum and proximal duodenum. This article reviews the endoscopic findings related to H. pylori infection in children and their possible causes and clinical consequences

11. Sellar xanthogranuloma.

- Abstract
We report the case of a 16-year-old girl with a history of idiopathic precocious puberty and normal results on pituitary imaging scan. Ten years later, a new cranial magnetic resonance imaging scan was performed due to worsening of episodes resembling Horton's headache and a lesion suggestive of pituitary bleeding was detected. The headaches diminished with glucocorticoid administration but a severe complication, steroid psychosis, occurred. Surgical treatment and pathological study of the lesion led to the differential diagnosis between craniopharyngyoma and xanthogranuloma of the sella turcica. The clinical progression of the tumor (not visualized 10 years previously), together with preservation of pituitary and visual function both before and after surgery, gross total removal of the tumor (difficult to achieve with craniopharyngioma) and the absence of recurrence provide strong support for the diagnosis of xanthogranuloma of the sella turcica., (Copyright © 2008 Sociedad Española de Endocrinología y Nutrición. - Abstract
Objectives: To analyze the evolution of prostate cancer (PCa) in our community health area over the last years, in terms of incidence, clinical-epidemiological characteristics, and cancer-specific mortality., Methods: Retrospective analysis of the medical records of patients with PCa in our hospital over the 1991-2003 period. We calculated the annual incidence rates for our community health area as well as its trend over the period of study. Changes of several clinical-epidemiological parameters over time were studied. The evolution of PCa mortality was also analyzed., Results: 730 patients with PCa were evaluated. Median age and PSA at diagnosis were 69 years and 11.4 ng/ml, respectively. 60.5% tumors were detected in a localized clinical stage, and 18.6% were metastasic. 30.4% of PCa in the study (the most numerous group) had a Gleason score between 5 and 6. A constant increase of PCa incidence was observed with an annual percentage increase of 5.5% (p < 0.001). Standardized incidence rate for the year 2003 Spanish population pyramid was 100.9 cases/100,000 males. A significant trend to a greater proportion of PCa cases diagnosed by uncontrolled screening was observed, being the cases currently detected after symptoms only a small proportion (p < 0.001). Decreases in the age at diagnosis (p < 0.001), PSA level (p < 0.001), proportion of advanced clinical stages (p < 0.001), as well as proportion of cancers with well-differentiated Gleason score (2-4) (p < 0.001) were also detected. PCa mortality adjusted by the population pyramid was 8.7 deaths/100,000 males in the year 2003., Conclusions: We found an increase in the incidence of prostate cancer in our community health area over the last years, parallel to an increase in the proportion of cases detected by screening in asymptomatic population. Because of that, tumors currently detected tend to appear in younger ages, with lower PSA levels, and localized clinical stages.
