WOS: 000326410100017 Background: This study aims to investigate the efficacy of polytetrafluoroethylene (PTFE) graft use in decreasing complications of bullous lung diseases. Methods: Between January 2004 and December 2012, 22 patients with bullous lung disease complicated with spontaneous pneumothorax were included. The patients were divided into two groups. Ten patients in group 1 had heterogeneous emphysematous disease and the 12 patients in group 2 had homogenous emphysematous disease. Both groups underwent muscle-saving thoracotomy. Wedge resection was performed by using stapler. The resection line was supported with an expandable PTFE graft in six patients in group 1 and in five patients in group 2. Treatment results and complications were compared between the groups. Results: Subtype of emphysema was the most important criterion which determined the result of the treatment. Although the use of PTFE graft support had no effect on complications in both emphysema subtypes, it shortened the duration of the chest tube withdrawal and the length of hospital stay. Conclusion: We concluded that the most important criterion which determined the treatment outcome was subtype of emphysema and this led to reduced intubation time and length of hospital stay, although use of PTFE support in both emphysema types had no effect on complications.

WOS: 000310402100019 Background: This study aims to compare the trauma scores by evaluating chest traumas both with the injury severity score (ISS) and the new injury severity score (NISS) and to assess the risk factors affecting the prognosis. Methods: A total of 1126 patients with chest trauma (898 males, 228 females; mean age 45.9 years; range 9 to 97 years) monitored in the Thoracic Surgery Clinic between January 1998 and December 2008 were retrospectively analyzed. Patients with blunt trauma were classified as group 1, and patients with penetrating trauma were classified as group 2. Patients were evaluated for their gender, age, type of trauma, comorbidities, habits, clinical signs, extrathoracic injuries, type of treatment, morbidity, and mortality. The ISS and NISS scores were estimated according to the trauma scales. Results: Of the patients, 82.3% (n=927) had blunt trauma and 17.7% (n=199) had penetrating trauma. The mean ISS of the patients with blunt trauma was 15 +/- 8.8 (range; 2-66), while the mean NISS was 19 +/- 10.6 (range; 3-66). The mean ISS of the patients with penetrating trauma was found to be 10 +/- 5.6 (range;2-35), while the mean NISS was 14 8.8 (range; 2-59). WOS: 000265616500013 The relationship between lung cancer and bullous lung diseases has been described in the literature. Herein, we presented a 45-year-old male patient who was asymptomatic when bilateral bullae and bilateral Synchronous primary lung adenocarcinoma were detected. Computed tomography revealed a giant bulla in the upper right lobe with a nodular lesion in the bulla wall, and multiple bullae in the upper left lobe with a nodule in the parenchyma adjacent to the bulla. Initially, malignancy was not considered. Pulmonary function test was restrictive. Thoracotomy was performed for the giant bulla in the right upper lobe. Frozen section analysis of the nodular lesion in the bulla wall revealed poorly differentiated adenocarcinoma. Upper right lobectomy was performed. Mediastinal lymphadenopathy was not observed. Two weeks later, the patient underwent a left thoracotomy for the intraparenchymal nodule. Adenocarcinoma was reported on frozen section analysis and upper left lobectomy was performed. Following surgery, the patient received chemotherapy. No recurrences were detected within a follow-up period of 20 months. This is an interesting case showing the relationship between bullous disease and lung cancer, reminding that bilateral synchronous primary lung cancer may be encountered in bullous lung disease.

WOS: 000268223800004 Background: The aim of the study was to evaluate the prognostic factors influencing survival following pulmonary metastasectomy and the importance of thoracic computed tomography (TCT) in detecting pulmonary metastases. Methods: The study included 25 patients (16 males, 9 females; mean age 46.4 years) who underwent metastasectomy for secondary lung tumors and 10 control patients (6 males, 4 females; mean age 56.5 years) who were not eligible for surgery. Primary tumors included carcinoma in 17 patients (48.6%), sarcoma in 17 patients, and malignant melanoma in one patient (2.9%). Staging was made according to the system of the International Registry of Lung Metastases. A total of 31 operations were performed (24 wedge resections, 5 lobectomies, 1 enucleation, 1 debulking surgery). The two groups were compared with respect to survival and prognostic factors for metastasectomy patients were assessed. Results: Operative mortality was seen in one patient (3.2%). The mean number of nodules detected by preoperative TCT was 2.7 +/- 2.4, compared to 7.6 +/- 10.5 nodules found at surgery (p=0.04). The accuracy of TCT in detecting nodules was 54.8%. One-year survival rate was 68% in metastasectomy patients, and 70% in the control group (p=0.707). The corresponding survival rates for three years were 44% and 0% (p=0.171).
