Purpose: This study investigates whether men who engage in dual harm while imprisoned are disproportionately involved in committing misconduct during a 1‐year follow‐up period. It also examines whether dual harm is significantly associated with future involvement in misconduct, when other known risk factors for misconduct are considered, and whether this relationship varies depending on the type of misconduct examined. Methods: Drawing on the administrative records of 430 men who were imprisoned during the 1‐year follow‐up period, a combination of descriptive statistics and negative binominal regressions was used to analyse the data. Results: Roughly one‐in‐four men engaged in dual harm while imprisoned and were responsible for over half of all misconduct incidents recorded during the follow‐up period. A significant relationship between dual harm, as well as violence‐only harm compared to no harm, and future involvement in misconduct was also observed even when other known risk factors for misconduct were considered but only for violent and disorder‐related misconduct, demonstrating this relationship varied by harm history and type of misconduct examined. Conclusion: These findings address previous gaps in knowledge, advancing our understanding of the relationship between dual harm and misconduct. Possible explanations for why, compared to no‐harm history, dual harm as well as violence‐only harm was only related to violent and disorder‐related misconduct are offered, alongside possible implications of this research for policy and practice. For decades, researchers have sought to understand the impact of imprisonment; yet we have a limited understanding of the lived experience of cell-sharing. To address this gap in knowledge, this paper draws on 37 semi-structured interviews with imprisoned adult men in Northern Ireland. While demonstrating that, for most, cell-sharing was a negative experience, imbued with discomfort, unease and distress, a new conceptual framework is presented that seeks to understand the tactics people use to manage cellsharing, influences on their choice of tactics and the potential repercussions of these tactics. Purpose: This study examines the prevalence of dual harm (i.e. self‐harm and violence) among imprisoned adult men in Northern Ireland, the relationship between dual harm and non‐violent misconduct, while controlling for other known risk factors for misconduct, as well as how those who engage in dual harm may differ from other groups. Methods: Using the administrative records of 892 adult men, descriptive statistics assessed the prevalence of dual harm. A negative binominal regression followed by predicted margins examined the relationship between dual harm and non‐violent misconduct accounting for controls. Additionally, a multinomial logistic regression was utilised to identify if those engaged in dual harm differed from others in terms of their characteristics and in‐prison experiences. Results: The findings indicate that 1‐in‐5 adult men were engaged in dual harm, with these men accounting for 72% of all non‐violent misconduct incidents examined. Dual harm was significantly related to an increased involvement in non‐violent misconduct compared to other harm histories (self‐harm only, violence only, or no harm) even when other known risk factors were considered. Those engaged in dual harm were also discovered to possess a number of characteristics that differ significantly from other groups. Conclusion: These findings strengthen emerging research indicating those who engage in dual harm are a distinct group that can be challenging to manage due to their increased involvement in misconduct and their multiple needs, which existing services and supports may be ill suited to address. This paper finds qualified support for the use of Skarbek's (2011, 2014) governance theory to understand the emergence of prison gang-like groups in Kyrgyzstan, Northern Ireland and Brazil. However, Skarbek's (2011, 2014) governance theory has little to say about how many prison gangs emerge and how they organise comparatively outside the US context. This paper argues that variation in the number of gangs and their monopolization of informal governance can only be explained by considering importation and deprivation theories alongside governance theories. These theories factor in variation in prison environments and pre-existing societal divisions imported into prison, which affect the costs on information transmission and incentives for gang expansion. In particular, the paper pays attention to the wider role social and political processes play in influencing whether monopoly power by prison gangs is supported and legitimized or not. The present study uses a prospective longitudinal research design to examine whether previously identified risk factors for prison interpersonal violence can predict violent prison misconduct in Northern Ireland (NI). Administrative data drawn from the records of 429 adult males imprisoned on November 22, 2017 were used to predict involvement in violent prison misconduct during a 1-year follow-up period. The results revealed that only a small number of previously identified risk factors were found to be significant in the NI context. Nationality, neighborhood deprivation, history of addiction, submission of prison complaints, past involvement in prison misconduct, and number of incarcerations emerged as significant, while religion, head injury/epilepsy, property offences, and prison visits were significant at the marginal level. Given the variation in risk factors identified as significant in the NI context compared to previous research, it is argued that cultural context matters when attempting to generalize the risk factors for prison interpersonal violence from one jurisdiction to another. These results offer some support for the importation theory, although it should be noted that the inclusion of prison environmental factors was limited due to the nature of the data. It is argued that specialist services and supports should be provided to address the factors contributing to interpersonal prison violence, including interventions to improve feelings of fairness, identify and treat underlying medical issues, as well as support visitation. Studies are increasingly interested in how different categories of prison misconduct may affect reoffending. Yet few studies investigate the distinct factors driving different types of misconduct and those that do are often cross-sectional or retrospective. This study uses a prospective longitudinal design to address this gap by investigating what factors influence future involvement in misconduct and if these factors vary by type. Administrative data drawn from the records of 429 imprisoned men are examined to predict misconduct during a 1-year follow-up period. Findings reveal distinct differences by type and suggestions for targeted support and tailored interventions are made to help prevent future infractions within, and potentially after, incarceration. Despite a growth in desistance research, our understanding of how local external factors may affect the desistance process as people transition from prison to society remains limited. Drawing on qualitative data from two neighbourhoods in Northern Ireland, it is argued that subcultural factors in neighbourhoods can play a key role in influencing desistance during reintegration by shaping the opportunities that individuals have to access, use and sell drugs, their experiences of social integration/exclusion and processes of surveillance and collective efficacy. It is argued that there is a need to better incorporate the effects of local external factors, including subcultural norms and values, into existing desistance theories if we are to fully understand how neighbourhood context can affect desistance during reintegration. Background: A respectful, person‐centered philosophy of maternity care has been emerging over several decades. Research conducted on behalf of the International Confederation of Midwives (ICM) to identify essential competencies for midwifery practice also identified the knowledge, skills, and professional behaviors that should be hallmarks of respectful maternity care practices among the global community of midwives. Methods: A three‐round, online, modified Delphi survey was conducted between April 2016 and October 2016. A total of 895 individuals from 90 of the then‐current 105 ICM member countries participated, with good representation across English, French, and Spanish speakers, high‐income, medium‐income, and low‐income countries, and educators and clinicians. Results: A total of 115 respectful maternity care (RMC)‐related items were endorsed by participants in Round 1 or 2. These items received average scores of between 90.24% and 99.10%, well above the 85% threshold required to be identified as within the scope of global midwifery practice. These items were compared with the 12 domains of RMC identified by Shakibazadeh and colleagues that defined respectful care during childbirth in health facilities globally, and with similar RMC frameworks, and were found to be highly congruent, thus demonstrating the high value of RMC within the core of midwifery practice. Discussion: ICM survey items were endorsed across all 12 RMC domains proposed by Shakibazadeh et al, and the findings affirmed that across ICM countries and regions, the philosophy of RMC was integrally related to the knowledge, skills, and professional behaviors that emerged as essential for basic midwifery practice. The objective of this study was to evaluate the effectiveness of drinking water quality verification monitoring as a means of improving preventive measures on drinking water quality management in regional New South Wales (NSW), Australia. Water sampling and E. coli detection data were obtained from the NSW Drinking Water Database. Statistical analysis was performed using Incidence Rate Ratios to compare the relationship between the proportion of samples collected to the tests allocated based on population served (sampling adequacy), E. coli detection and the relationship between sampling adequacy and E. coli detections over time. Sampling adequacy and E. coli detections significantly improved during the study period. Sampling adequacy was significantly lower in smaller populations (IRR = 0.83, p = 0.036). E. coli detections were significantly increased in smaller communities (IRR = 4.3, p = 0.01) and in summer (IRR = 2.7, p = < 0.001). There was a strong inverse correlation between improved sampling adequacy and decreased E. coli detections (Spearman's rho = −0.821; p < 0.0001). This research has highlighted the value of continued assistance to water utilities in the implementation of drinking water management systems to improve drinking water safety. [ABSTRACT FROM AUTHOR]

Introduction: The International Confederation of Midwives (ICM) represents 132 midwifery associations in 113 countries. The ICM disseminates the Essential Competencies for Basic Midwifery Practice (EC) that describes the global scope of midwifery practice. The basic (core) and expanded (additional or optional) role of midwives in providing abortion-related care services was first described in 2010. A literature review about three items that are particularly critical to access to abortion services was conducted. Findings that emerged in the recent 2016–2017 update study about these three items are presented. Methods: A modified Delphi study was administered via the Internet in a series of three rounds. Thirty-seven statements of abortion-related knowledge and skill were presented. Results: A total of 895 individuals participated. The total of respondents across all three rounds represented 90 of the 105 member countries at the time of the study. The role of midwives in providing comprehensive abortion care, including referral for abortion and provision of postabortion family planning, achieved the necessary 85% agreement to be designated as essential (basic) knowledge or skill for the global scope of midwifery practice. The provision of medication abortion and performance of manual vacuum aspiration abortion were designated as optional for midwives who wished to provide these services. Endorsement of these latter practices was highest in both Francophone and Anglophone regions of Africa, Asian Pacific countries, and countries at a lower state of economic development. Conclusion: The role of midwives in provision of abortion-related care services was reaffirmed in the recent Delphi study to inform the update to the EC. The role of midwives as direct providers of medical and vacuum aspiration abortions was reaffirmed for those individual midwives who wish to obtain the requisite competency to provide those services, in jurisdictions where these services are legally authorized. Although testing accommodations for standardized assessments are available for students with disabilities, interpretation remains challenging. The authors explored resilience to see if it could contribute to the interpretation of academic success for students who are deaf or hard of hearing or blind or have low vision. High school students (30 deaf, 11 blind) responded to the Resiliency Scales and a schoolspecific resilience survey. Student ratings were within 1 SD of those who were not deaf or blind; resilience was positively correlated with academic success (grade point average). Resilience information may contribute to the interpretation of academic success in students who are deaf or blind and may provide insight into potential interventions. This is the first time students who are deaf or blind have been assessed on a published measure of resilience. Results defy stereotypes and affirm that individuals who are deaf or hard of hearing or blind or have low vision have more in common with the hearing and seeing world than is often recognized. Community-based restorative justice projects in Northern Ireland have garnered international attention due to their role in conflict transformation and peace building. Yet, little is known about how changing State policies has helped or hindered their ability to use restorative justice to reduce crime and paramilitary violence. The findings indicate that changing State policies and how these policies are implemented has hindered this aspect of the project's work. It is argued that State policies seeking to control and regulate the use of restorative justice may undermine its ideological basis and potential effectiveness. During imprisonment, fathers are separated from their families and contact is limited. When delivering a prison based parenting programme, providing an opportunity to rehearse newly acquired parenting skills can be key for mastering the performance of these skills and using these skills to improve father-child relationships. This paper takes an in-depth look at how one parenting programme in Northern Ireland sought to overcome this challenge by providing additional opportunities to parent via increased telephone contact and special family friendly visits. Using a combination of in-depth interviews and observations, how fathers and their families responded to this increased contact is explored, as well as the extent to which this increased contact facilitated the acquisition of the parenting skills being taught on the programme. It is argued that while prison based parenting programmes can improve parenting skills and father-child relationships, their potential long-term effectiveness may be limited by wider prison policies, procedures and practices surrounding prison visitation, telephone access and the progression of fathers following the completion of such programmes. Background: Rates of normal birth have been declining steadily over the past 20 years, despite the evidence of the benefits to mother and baby. This is most obvious in steadily increasing caesarean section rates across countries and studies of the factors involved suggest it may be more to do with the organization of maternity care and the preferences of healthcare providers than changes in maternal or demographic conditions. The proportion of women in British Columbia (BC) receiving care from a midwife continues to grow and there is a particular focus on promoting and supporting normal pregnancy and birth in the midwifery philosophy of care. In BC, women receiving care from a midwife are less likely to have a caesarean section and other birth interventions.Methods: An interpretive approach, based on interpretive phenomenology was used to explore the experiences of midwives in BC of normal birth and the strategies that they use to keep birth normal. Fourteen experienced midwives were purposively selected from across the range of practice, geographical, and rural/urban contexts to participate in depth interviews. Data were analyzed using Thematic Network Analysis.Results: Seven key themes were identified in the data: working with women from the early pregnancy, informing choice, the birth environment, careful watching and waiting, managing early labour, helping the woman to cope with labour, and tools in the tool kit.Conclusions: Midwives in BC work closely with women from early pregnancy to prepare them for a normal birth, and as "instruments of care" they adopt a range of approaches to support women to achieve this. Background: Acute flaccid paralysis (AFP) surveillance rates are used as an indicator of surveillance sensitivity to detect poliomyelitis with an expected rate of =1 case per 100 000 population in children under 15 years of age. Methods: The Australian AFP detection rates at sub-national (statistical local area) level were analysed using χ² goodness of fit tests and exact Poisson probabilities for the combined years 2001-2015 to detect 'silent areas', which may require improved AFP detection efforts, and areas with greater than expected rates, which may indicate unexplained clusters such as those due to enterovirus infection. Results: Eight (n=8/87, 9%) local areas had AFP surveillance detection rates that were less than expected, and eighteen local areas (n=18/87, 21%) had rates that were greater than expected. However, based on available evidence, it is unlikely that these indicated previously unidentified, enterovirus clusters. Conclusions: While Australia has regularly met the national AFP surveillance performance indicators, at the subnational level nine per cent of local areas demonstrated statistically significant lower AFP detection rates. All countries, even those with relatively small populations, should actively identify silent AFP areas to prompt surveillance improvements. [ABSTRACT FROM AUTHOR]

Problem The emergence of Zika virus has challenged outbreak surveillance systems in many at-risk, low-resource countries. As the virus has been linked with Guillain-Barre syndrome, routine data on the incidence of acute flaccid paralysis (AFP) may provide a useful early warning system for the emergence of Zika virus. Approach We documented all Zika virus outbreaks and cases in 21 Pacific Islands and territories for the years 2007 to 2015. We extracted data from the Global Polio Eradication Initiative database on the reported and expected annual incidence of AFP in children younger than 15 years. Using a Poisson probability test, we tested the significance of unexpected increases in AFP in years correlating with Zika virus emergence. Data were analysed separately for each Pacific Island country and territory. Local setting In most Pacific Island countries, early warning surveillance for acute public health threats such as Zika virus is hampered by poor health infrastructure, insufficient human resources and geographical isolation. Relevant changes Only one example was found (Solomon Islands in 2015) of a significant increase in reported AFP cases correlating with Zika virus emergence. Lessons learnt We found no conclusive evidence that routinely reported AFP incidence data in children were useful for detecting emergence of Zika virus in this setting. More evidence may be needed from adult populations, who are more likely to be affected by Guillain-Barre syndrome. Reporting of AFP may be deficient in regions certified as polio-free. Background Flying foxes (megachiroptera) and insectivorous microbats (microchiroptera) are the known reservoirs for a range of recently emerged, highly pathogenic viruses. In Australia there is public health concern relating to bats' role as reservoirs of Australian Bat Lyssavirus (ABLV), which has clinical features identical to classical rabies. Three deaths from ABLV have occurred in Australia. A survey was conducted to determine the frequency of bat exposures amongst adults in Australia's most populous state, New South Wales; explore reasons for handling bats; examine reported practices upon encountering injured or trapped bats or experiencing bat bites or scratches; and investigate knowledge of bat handling warnings. Methods A representative sample of 821 New South Wales adults aged 16 years and older were interviewed during May and June 2011, using a computer assisted telephone interview (CATI) method. Frequencies, proportions and statistical differences in proportion were performed. Using an α-value of 0.05 and power of 80%, it was calculated that a sample size of 800 was required to provide statistical significance of +/− 5% for dichotomous variables. Results One-hundred-and-twenty-seven (15.5%) respondents indicated that they had previously handled a bat, being 22% (48/218) rural and 13% (78/597) urban respondents (χ² = 9.8, p = 0.0018). Twenty one percent of males (63/304) had handled bats compared with 12% (64/517) of females (χ² = 10.2, p = 0.0014). Overall, 42.0% (n = 345) of respondents reported having seen or heard a warning about handling bats. If faced with an injured or trapped bat, 25% (206/821) indicated that they would handle the bat, with 17% (36/206) saying that they would use their bare hands. For minor scratches, 14% (117/821) indicated that they would ignore the injury while four respondents would ignore major scratches or bites. Conclusions Previous human-bat interactions were relatively common. Bat exposures most frequently occurred with sick or injured bats, which have the highest risk of ABLV. On encountering an injured or sick bat, potentially high risk practices were commonly reported, particularly among rural males. Abstract: The lack of reperfusion of myocardium after prolonged ischaemia that may occur despite opening of the infarct-related artery is termed “no reflow”. No reflow or slow flow occurs in 3–4% of all percutaneous coronary interventions, and is most common after emergency revascularization for acute myocardial infarction. In this setting no reflow is reported to occur in 30% to 40% of interventions when defined by myocardial perfusion techniques such as myocardial contrast echocardiography. No reflow is clinically important as it is independently associated with increased occurrence of malignant arrhythmias, cardiac failure, as well as in-hospital and long-term mortality. Previously the no-reflow phenomenon has been difficult to treat effectively, but recent advances in the understanding of the pathophysiology of no reflow have led to several novel treatment strategies. These include prophylactic use of vasodilator therapies, mechanical devices, ischaemic postconditioning and potent platelet inhibitors. As no reflow is a multifactorial process, a combination of these treatments is more likely to be effective than any of these alone. In this review we discuss the pathophysiology of no reflow and present the numerous recent advances in therapy for this important clinical problem. [Copyright &y& Elsevier]

Abstract: Treatment of racemic 2-hydroxy-3-(1H-indol-3yl)propionic acid methyl ester (5) with isopropyl magnesium chloride provided the title compound 1 and its isomer, 3-hydroxy-1-(indol-3-yl)-4-methylpentan-2-one (9). Both enantiomers (>96% ee) of each component were obtained via semi-preparative chiral supercritical fluid chromatography (SFC). In contrast to previous reports, these compounds, as well as their acetate derivatives, were not active or very weakly active against 16 bacterial strains, including Escherichia coli, Bacillus subtilis and Staphylococcus aureus. Abstract: DNA spike clearance methods were used to demonstrate improved clearance factors on anion exchange and hydrophobic interaction columns used in the production of human therapeutic proteins. DNA clearance at large-scale was first measured for a monoclonal antibody expressed in Chinese Hamster Ovary (CHO) cells and an antibody fragment expressed in Escherichia coli. Small-scale spike experiments were then performed on individual chromatographic steps using host-specific DNA paired with TaqMan PCR assay methods. This approach has advantages of improved specificity, sensitivity, cost and throughput compared to other types of spike clearance methods. The anion exchange column used in the monoclonal antibody process was shown to have very high capacity for CHO DNA, resulting in greater than 7.1 log reduction. The anion exchange and hydrophobic interaction columns used in the antibody fragment process were shown to have high E. coli DNA clearance capability, with greater than 5.1 and 5.3 logs clearance, respectively. Compared to the large-scale process, higher log reduction values were achieved in small-scale spike clearance studies by challenging the chromatographic steps with load DNA levels 2–5 logs higher than the large-scale process levels. Using highly specific and sensitive spike clearance methods, we demonstrated consistently high DNA clearance factors for each of the production processes that meet industry and regulatory standards for human therapeutics. The method is applicable to a broad range of industrial scale processes where demonstration of the robustness of DNA clearance is necessary to support development or licensure of biopharmaceutical products. Whereas cognitive deficits in multiple sclerosis (MS) were documented in the original clinicopathological description by Charcot, the extent and nature of cognitive dysfunction was poorly understood in the U.S. until over a century later. Our objective was to discern reasons for this misunderstanding and to examine forces shifting this topic to center stage of research and increased awareness in clinical practice. We hypothesized that during the 19th century, conflict between psychiatrists and neurologists over control of treatment of the mentally ill fueled a misunderstanding of the nature of MS which led neurologists to treat diseases of the body, and psychiatrists, diseases of the mind. The importance of cognitive deficits in MS was thus minimized until scientific breakthroughs could once again bring them to light. As the rift between disciplines peaked, awareness of cognitive deficits diminished, and vice versa. Self-report research suggests that much violence is triggered by perceived insults and disrespect. This may be particularly true in the context of a prison or another environment of acute deprivation, whereby individuals have little other recourse to means of reputation enhancement. This paper presents the findings of two studies conducted with prisoner volunteers inside a Category C (minimum security) prison in England. In the first study, the authors randomly assigned a sample of 89 prisoners to one of two conditions: the experimental group were asked to discuss times they have been disrespected by authority figures inside and outside the prison; the control group were asked more neutral questions. Both groups then completed several measures of cognitive beliefs, distortions, and hostile attribution biases. None of the measures differed across the two groups except the measure of excuse and justification acceptance. Controlling for other factors, the experimental group endorsed these rationalisations at a significantly higher rate than the control group. This finding suggests that raising the salience of disrespect - reminding prisoners of times they have been made to feel unworthy of consideration - may raise the risk that prisoners will engage in violence by providing prisoners with justifications or excuses for actions they might not otherwise endorse. These findings received some additional validation in the second study, a qualitative analysis of offender accounts of violence and aggression within the prison. This article examines the occurrence of fights, assaults, arguments and threats of violence between adult male prisoners in an English category C prison. The self-narratives of 40 men are analysed to investigate whether some prisoners engage in more confrontations than others due to a psychological need to protect their identity. The findings indicate that how an individual understands and constructs their self-narrative can influence their involvement in aggressive behaviour. This article examines the meaning of respect in the interpersonal relationships within Her Majesty's Prison Service. It is argued that respect-as-esteem and respect-as-consideration are often confused and unequally emphasised in modern society. This confusion is especially evident within the prison context where, due to the prison service's ‘decency agenda’, the respectful treatment of inmates has become a topical issue. What does respect mean in prison? Why is it important? How can respectful relationships be established between staff and inmates? This article discusses these questions and proposes that there are different forms of respect possible between people. It is argued that there needs to be a recognition of the nuances of meaning when we use the word respect and that ‘respect-as-consideration’ may be the form of respect most consistently achievable, at the present time, within interpersonal relationships in English and Welsh prisons. The work presented in this second part of a two-part series on the accelerated transformation of erythromycin in superheated water focuses on the chemical nature and resultant antimicrobial implications of the overall reaction observed. Analyses of reactor effluents employing infrared spectroscopy and liquid chromatography/mass spectrometry indicated that the initial step in the decomposition pathway is one of dehydration. Subsequent hydrolysis in the presence of an acetate buffer appeared to result in the loss of the sugar cladinose. Chemical transformation under superheated conditions was tied to the loss of antibiotic function by an agar dilution test The sensitivity of this test was verified by determination of the minimum inhibitory concentration (MIC) of erythromycin corresponding to each of two test microorganisms. MIC values for the selected strains of Escherichia coil and Bacillus subtilis were 35 and 0.5 mg/L, respectively. The presence of antibiotics and other pharmaceuticals in the environment is of increasing concern. The stringent treatment of point discharges of antibiotic wastes holds promise as an approach for curtailing growing trends of microbial resistance. The work described in this two-part series explores the use of superheated water as a medium for the accelerated transformation and deactivation of a specific target antibiotic, erythromycin. Part 1 of the series focuses on parent compound conversion, and Part 2 examines transformation mechanisms and reaction products. This paper, Part 1, highlights the results of reactor studies performed in both batch and flow-through modes. The data presented confirm that accelerated conversion of erythromycin, based on parent compound disappearance as measured by high-pressure liquid chromatography, occurs in water under superheated conditions. Given an initial erythromycin concentration of 50 mg/L, greaterthan 85% apparent conversion was achieved within 30 mm in the batch system at all temperatures investigated in the range from 125 to 200 °C. The presence of dissolved organic matter extracted from two natural soil materials, at concentrations of 2.5-20 mg/L, was shown to have little effect on the overall extent of transformation of erythromycin in the batch system. The rates of decomposition observed were found to be best described by a psuedo-first-order expression, one in which the rate coefficient increased linearly with increasing initial concentration of the antibiotic. First-order rate behavior was verified in subsequent flow reactor experiments. The temperature dependence of rate was also examined, and an activation energy of 68.8 kJ/mol was determined. A thorough understanding of Multiple Sclerosis (MS) is necessary to offer individuals informed options for treatment and planning. To assist in this quest, the following historical analysis examined how MS has been conceived from the 14th century through the early 20th century. Primary sources were consulted whenever possible, and many of the original archival materials were accessed by the first author (MB) during an on-site visit to the Rare Book Room of the New York Academy of Medicine. There is a striking similarity between how MS symptoms have presented throughout history compared with the 21st century. Sensorimotor and cognitive sequelae have been observed in patients since the 1800s. Cognitive symptoms were acknowledged in the 1800s, but disregarded in the early 1900s and were not given recognition again until the latter part of the 20th century. If conceptualizations of MS are inaccurate, patients will not be served well. In contrast to the shared symptomatology across time, early conceptualizations of etiology and treatment choices differed dramatically from today, a genuine reflection of the times in which they were created. Porins are essential pore-forming proteins found in the outer membrane of several gram-negative bacteria. Investigating the relationships between molecular structure and function involves an extremely time-consuming and labor-intensive purification procedure. We report a method for rapid extraction of the outer membrane protein, OmpF, from freeze-dried Escherichia coli cells using valeric acid, alleviating the effort and time in sample preparation. Extraction results in a highly enriched fraction containing OmpF as 76% of the total protein content. The apparent molecular mass determined by SDS-PAGE mobility was 38,900, similar to that of the monomeric form of OmpF. N-terminal sequencing yielded 23 amino acids with 100% identity to the published OmpF sequence. The trimeric form of OmpF was observed in unheated samples run on SDS-PAGE and analysis of these samples by periodic acid/silver staining revealed the presence of unbound lipopolysaccharides. Furthermore, this method should prove useful for isolating other outer membrane proteins. [Copyright &y& Elsevier]

Pulmonary macrophages (PM) are involved in the clearance of inhaled particulate matter from the lung. PM also are capable of metabolizing xenobiotics such as benzo[a]pyrene (BaP). The objective of this investigation was to measure the ability of PM isolated from dogs to metabolize BaP coated onto diesel exhaust particles and to compare this metabolism with that of BaP in solution. PM were isolated from male beagle dogs and incubated with 1 μM [ 14 C/BaP (solution or diesel particle coated) for select times up to 48 h. After incubation of PM with [ 14 C]BaP, both the cells and the media were individually analyzed for [ 14 C]BaP metabolites by high‐performance liquid chromatography. Total quantities of [ 14 C]BaP metabolites in both the media (125 pmol/10 6 cells) and cells (45 pmol/10 6 cells) increased with incubation time for up to 48 h. BaP‐9, 10‐diol and BaP‐7,8‐diol were the major metabolites in organic extracts from the culture media, whereas BaP‐7,8‐diol and BaP‐4,5‐diol were the major metabolites in extracts of cells. Small quantities of BaP phenols and BaP quinones were detected in both the cells and media. Total quantities of BaP metabolites (20–30 pmol/10 6 cells) were not significantly different when PM were incubated for 24 h with either / 14 C]BaP in solution or [C]BaP coated on diesel particles. The data suggest that particles retained in lungs are capable of being acted upon by PM metabolizing enzymes and that the ensuing metabolism may play an important role in the metabolic fate of organic material inhaled on particulate matter. In order to investigate the tests selected by neuropsychologists to make up clinical batteries, a large survey of neuropsychological test usage was cluster analyzed. This provided groupings of test that are endorsed in common. Theoretical orientation within neuropsychology also was included in the analysis to determine which tests and clusters of tests are more and less associated with the reported orientation of the neuropsychologist. Fifteen clusters of tests were found. Strong and appropriate associations with the eclectic, hypothesis testing, process approach, Halstead­Reitan, Luria, and Benton orientations were seen. It is now widely accepted that living with domestic abuse (DA) can significantly affect children, with the effects of DA continuing to be felt into adulthood. The common conceptualisation of children as passive 'witnesses' of DA and the failure to recognise them as direct 'victims' in their own right, separate from adult victims, can act as a major barrier for professionals responding to children within this context. The first professionals with whom many child victims of DA come into contact often are members of the police. Yet, little is still known about how children and police officers experience these encounters. The aim of this paper is to conduct a narrative review of the existing literature on the police response to children at DA call outs through the lens of Emotional Intelligence (Goleman 1996). Reviewed studies state that a large number of police officers reported feelings of overwhelm and uncertainty at incidents of DA involving children. Children reported significant differences in empathy of officers, a key tenet of EI, which impacted their feelings of safety and visibility at incidents of DA. DA incidents are an emotionally challenging aspect of police work and most officers do not appear to have the skills to manage this effectively. The significance of speaking with children in these traumatic situations cannot be understated. Background: Flying foxes (megachiroptera) and insectivorous microbats (microchiroptera) are the known reservoirs for a range of recently emerged, highly pathogenic viruses. In Australia there is public health concern relating to bats' role as reservoirs of Australian Bat Lyssavirus (ABLV), which has clinical features identical to classical rabies. Three deaths from ABLV have occurred in Australia. A survey was conducted to determine the frequency of bat exposures amongst adults in Australia's most populous state, New South Wales; explore reasons for handling bats; examine reported practices upon encountering injured or trapped bats or experiencing bat bites or scratches; and investigate knowledge of bat handling warnings.Methods: A representative sample of 821 New South Wales adults aged 16 years and older were interviewed during May and June 2011, using a computer assisted telephone interview (CATI) method. Frequencies, proportions and statistical differences in proportion were performed. Using an α-value of 0.05 and power of 80%, it was calculated that a sample size of 800 was required to provide statistical significance of +/- 5% for dichotomous variables.Results: One-hundred-and-twenty-seven (15.5%) respondents indicated that they had previously handled a bat, being 22% (48/218) rural and 13% (78/597) urban respondents (χ2 = 9.8, p = 0.0018). Twenty one percent of males (63/304) had handled bats compared with 12% (64/517) of females (χ2 = 10.2, p = 0.0014). Overall, 42.0% (n = 345) of respondents reported having seen or heard a warning about handling bats. If faced with an injured or trapped bat, 25% (206/821) indicated that they would handle the bat, with 17% (36/206) saying that they would use their bare hands. For minor scratches, 14% (117/821) indicated that they would ignore the injury while four respondents would ignore major scratches or bites.Conclusions: Previous human-bat interactions were relatively common. Bat exposures most frequently occurred with sick or injured bats, which have the highest risk of ABLV. On encountering an injured or sick bat, potentially high risk practices were commonly reported, particularly among rural males. Objectives: The aim of this study was to describe dietary intake and eating behaviours of obese children and adolescents, and also to determine how these differ in Indigenous versus non-Indigenous children at enrolment in an obesity programme. Methods: Baseline dietary intake and eating behaviour records were assessed from those enrolled in a clinical unblinded randomised controlled trial of a multi-disciplinary intervention. The setting was a community-based obesity programme in Taranaki, New Zealand. Children or adolescents who were enrolled from January 2012 to August 2014, with a BMI ≥98th percentile or >91st centile with weight-related comorbidities were eligible. Results: 239 participants (45% Māori, 45% NZ Europeans, 10% other ethnicities), aged 5–17 years were assessed. Two-thirds of participants experienced hyperphagia and half were not satiated after a meal. Comfort eating was reported by 62% of participants, and daily energy intake was above the recommended guidelines for 54%. Fruit and vegetable intake was suboptimal compared with the recommended 5 servings per day (mean 3.5 [SD = 1.9] servings per day), and the mean weekly breakfasts were less than the national average (5.9 vs 6.5; p<0.0001). Median sweet drink intake amongst Māori was twice that of NZ Europeans (250 vs 125 ml per day; p = 0.0002). Conclusions: There was a concerning prevalence of abnormal eating behaviours and significant differences in dietary intake between obese participants and their national counterparts. Ethnic differences between Indigenous and non-Indigenous participants were also present, especially in relation to sweet drink consumption.
