12. Clinical and paraclinical evolution of term and near term neonates with persistent pulmonary hypertension, treated with treprostinil and/or epoprostenol, hospitalized in NICU.

Abstract

Persistent pulmonary hypertension of the newborn (PPHN) is a serious disease that occurs in 1.9 per 1000 live births. Epoprostenol and treprostinil, witch are prostacyclin analogues, are used by some care teams in the treatment of PPHN, in absence of established proof of their efficacy in this indication. The main objective of this retrospective multicenter study was to evaluate clinical and paraclinical evolution of newborns treated with treprostinil and/or epoprostenol during neonatal period. Inclusion of neonates ≥ 34 SA and aged ≤ 28 days, with clinical signs and ≥ 1 ultrasound sign of pulmonary hypertension, treated with treprostinil and/or epoprostenol between 01/01/17 and 31/12/22 in 4 French teaching hospitals. Data collected included clinical, biological and ultrasound parameters. Seventy patients were included, with a mean age of 39 SA + 2.5 days and a mean birth weight of 3200 g, including 40 congenital diaphragmatic hernia (57%). On arrival in NICU over 90% of newborns had iso or supra-systemic pulmonary hypertension. The introduction of prostacyclin analogues appears to have a beneficial effect on the pre- and post-ductal saturation differential as well as on the echocardiographic evolution of pulmonary hypertension. The temporality of evolution of pulmonary hypertension seems to depend on the underlying etiology due to the different pathophysiological mechanisms. Moreover, our study shows that epoprostenol and treprostinil can be used in neonates suffering from iNO refractory PPHN without significant adverse effects. We report few adverse effects of epoprostenol and treprostinil in neonates treated for in iNO refractory PPHN. However, the heterogeneity of practices between centers requires further studies to establish recommendations for the use of these molecules in severe neonatal pulmonary hypertension. Abstract

International audience; The multidrug-resistant Staphylococcus capitis NRCS-A clone is responsible for sepsis in preterm infants in neonatal intensive care units (NICUs) worldwide. Here, to retrace the spread of this clone and to identify drivers of its specific success, we investigated a representative collection of 250 S. capitis isolates from adults and newborns. Bayesian analyses confirmed the spread of the NRCS-A clone and enabled us to date its emergence in the late 1960s and its expansion during the 1980s, coinciding with the establishment of NICUs and the increasing use of vancomycin in these units, respectively. This dynamic was accompanied by the acquisition of mutations in antimicrobial resistance- and bacteriocin-encoding genes. Furthermore, combined statistical tools and a genome-wide association study convergently point to vancomycin resistance as a major driver of NRCS-A success. We also identified another S. capitis subclade (alpha clade) that emerged independently, showing parallel evolution towards NICU specialization and non-susceptibility to vancomycin, indicating convergent evolution in NICU-associated pathogens. These findings illustrate how the broad use of antibiotics can repeatedly lead initially commensal drug-susceptible bacteria to evolve into multidrug-resistant clones that are able to successfully spread worldwide and become pathogenic for highly vulnerable patients.

Abstract

Enterotoxigenic strains of Escherichia coli H10407 retain pathogenicity even after being exposed to sunlight, and entering the viable but nonculturable state. When exposed to sunlight, the direct counts of viable cells fail to change. Escherichia coli cells are culturable in the absence of sunlight only when exposed to seawater. Ganglioside-enzyme-linked immunosorbent assay (GM1-ELISA) shows that Escherichia coli continues to produce enterotoxin after exposure to sunlight. GM1-ELISA is found to be superior to the conventional rabbit intestinal loop assay.

Abstract

Objectives: We investigated the epidemiological, clinical, microbiological and genetic characteristics of linezolid-resistant (LZR) Staphylococcus capitis isolates from French ICUs, and compared them with LZR S. capitis isolates from other European countries.Methods: All LZR isolates were subjected to antimicrobial susceptibility testing (AST) and the presence of cfr and optrA genes as well as mutations in the 23S rRNA and ribosomal proteins were investigated using specific PCR with sequencing. The genetic relationship between isolates was investigated using PFGE and WGS. Epidemiological data concerning LZR S. capitis were collected retrospectively in French microbiology laboratories.Results: Twenty-one LZR isolates were studied: 9 from France, 11 from Greece and 1 from Finland. All were resistant to methicillin and aminoglycosides. In addition, this unusual AST profile was identified in S. capitis isolates from seven French hospitals, and represented up to 12% of the S. capitis isolates in one centre. A G2576T mutation in 23S rRNA was identified in all isolates; cfr and optrA genes were absent. All isolates belonged to the same clone on the basis of their PFGE profiles, whatever their geographical origin. WGS found at most 212 SNPs between core genomes of the LZR isolates.Conclusions: We identified and characterized an LZR S. capitis clone disseminated in three European countries, harbouring the same multiple resistance and a G2576T mutation in the 23S rRNA. The possible unrecognized wider distribution of this clone, belonging to a species classically regarded as a low-virulence skin colonizer, is of major concern not least because of the increasing use of oxazolidinones. Abstract

Objectives: The Staphylococcus capitis clone NRCS-A has recently been described as a frequent cause of late-onset sepsis (LOS) in pre-term neonates worldwide. Representatives of this clone exhibit non-susceptibility to vancomycin, the first-line agent used in LOS. Cases of prolonged S. capitis LOS despite vancomycin treatment have been reported. We investigated whether NRCS-A strains exhibit faster adaptation to vancomycin pressure as compared with other staphylococci.Methods: Strains of S. capitis NRCS-A, S. capitis non-NRCS-A and Staphylococcus epidermidis (n = 2 each, all with vancomycin MICs ≤2 mg/L) and the prototype vancomycin-heteroresistant Staphylococcus aureus Mu3 were subcultured daily for 15 days with 0.25-32 mg/L vancomycin. Regression coefficients of daily log2 MICs on time were used to estimate the kinetics of resistance development. Changes in bacterial cell-wall thickness were measured by transmission electron microscopy. To assess the stability of resistance and the emergence of cross-resistance, vancomycin, teicoplanin, daptomycin and linezolid MICs were measured before and after vancomycin treatment, as well as after nine additional subcultures without antibiotics.Results: All strains developed a stable resistance to vancomycin, but this occurred significantly faster in S. capitis NRCS-A than in S. capitis non-NRCS-A (P < 0.001) and other species (P < 0.0001). Vancomycin resistance in S. capitis NRCS-A was associated with significant cell-wall thickening and an increase in MICs of daptomycin and teicoplanin, but not linezolid.Conclusions: S. capitis NRCS-A rapidly adapts to vancomycin pressure as compared with potential niche competitors, a feature that might contribute to its success in neonatal ICUs where vancomycin is widely prescribed. ABSTRACTMultiresistant Staphylococcus capitispulsotype NRCS-A has been reported to be a major pathogen causing nosocomial bacteremia in preterm infants. We report that the NRCS-A strain CR01 harbors a novel 60.9-kb composite staphylococcal cassette chromosome mec(SCCmec) element, composed of an SCCmecwith strong homologies to Staphylococcus aureusST398 SCCmecand of an SCCcad/ars/copharboring resistance genes for cadmium, arsenic, and copper. Whole-genome-based comparisons of published S. capitisstrains suggest that strain CR01 acquired the two elements independently.

49. Infecciones neonatales víricas

Abstract

Dentro de las infecciones neonatales, las infecciones víricas presentan un amplio abanico de situaciones clínicas de gravedad variable. La contaminación vírica puede producirse durante el embarazo a través de la placenta. En este caso, algunos virus pueden provocar un retraso de crecimiento o malformaciones, como por ejemplo el caso de la rubéola. Otros virus producirán lesiones viscerales, incluso la muerte fetal in utero. Es el caso en particular de los virus de la varicela, del citomegalovirus (CMV) o también del parvovirus B19. Algunos virus pueden contaminar al recién nacido durante el parto, por el contacto con las secreciones genitales maternas contaminadas. Es el caso del herpes simple o del virus de la inmunodeficiencia humana (VIH). Para estos casos de riesgo, se pueden aplicar medidas de prevención orientadas (considerar una cesárea, antiviral específico durante el parto, tratamiento del recién nacido). Por último, el recién nacido, debido a la inmadurez inmunológica, está expuesto en las primeras semanas de vida al riesgo de desarrollar una infección grave en caso de contaminación vírica. Están implicados casi todos los virus: los virus respiratorios, los enterovirus, el virus de la varicela (en ausencia de inmunidad materna). Se deben aplicar medidas de aislamiento y de barrera para limitar el riesgo de contaminación neonatal. Quedan todavía por estudiar numerosos ejes en relación con la investigación de las infecciones víricas. Un mejor conocimiento de los fenómenos de contaminación y de respuesta inmunológica podrían permitir desarrollar medios de prevención (vacunación en particular). Por otra parte, en caso de infección, es importante realizar un seguimiento de los niños a lo largo de varios años mediante cohortes, con el fin de detectar precozmente la aparición de complicaciones infrecuentes o tardías. Como conclusión, las infecciones víricas neonatales representan una importante causa de morbilidad neonatal y pueden ser responsables de secuelas o discapacidad a largo plazo. Por lo tanto, deben tratarse con el mismo cuidado que las infecciones bacterianas neonatales.
