Background First pass effect (FPE), defined as single-pass complete or near complete reperfusion during endovascular thrombectomy (EVT) for large vessel occlusion (LVO) strokes, is a critical performance metric. Atrial fibrillation (AF)-related strokes have different clot composition compared with non-AF strokes, which may impact thrombectomy reperfusion results. We compared FPE rates in AF and non-AF stroke patients to evaluate if AF-related strokes had higher FPE rates. Methods We conducted a post-hoc analysis of the DIRECT-SAFE trial data, including patients with retrievable clots on the initial angiographic run. Patients were categorized into AF and non-AF groups. The primary outcome was the presence or absence of FPE (single-pass, single-device resulting in complete/near complete reperfusion) in AF and non-AF groups. We used multivariable logistic regression to examine the association between FPE and AF, adjusting for thrombolysis pre-thrombectomy and clot location. Results We included 253 patients (67 with AF, 186 without AF). AF patients were older (mean age: 74 years vs 67.5 years, p=0.001), had a higher proportion of females (55% vs 40%, p=0.044), and experienced more severe strokes (median National Institutes of Health Stroke Scale (NIHSS) score: 17 vs 14, p=0.009) than non-AF patients. No differences were observed in thrombolytic agent usage, time metrics, or clot location. AF patients achieved a higher proportion of FPE compared with non-AF patients (55.22% vs 37.3%, adjusted odds ratio 2.00 (95% CI 1.13 to 3.55), p=0.017). Conclusions AF-related strokes in LVO patients treated with EVT were associated with FPE. This highlights the need for preparedness for multiple passes and potential adjuvant/rescue therapy in non-AF-related strokes. [ABSTRACT FROM AUTHOR]

Background: Following reperfusion treatment in ischemic stroke, computed tomography (CT) imaging at 24 h is widely used to assess radiological outcomes. Even without visible hyperattenuation, occult angiographic contrast may persist in the brain and confound Hounsfield unit-based imaging metrics, such as net water uptake (NWU). Aims: We aimed to assess the presence and factors associated with retained contrast post-thrombectomy on 24-h imaging using dual-energy CT (DECT), and its impact on the accuracy of NWU as a measure of cerebral edema. Methods: Consecutive patients with anterior circulation large vessel occlusion who had post-thrombectomy DECT performed 24-h post-treatment from two thrombectomy stroke centers were retrospectively studied. NWU was calculated by interside comparison of HUs of the infarct lesion and its mirror homolog. Retained contrast was quantified by the difference in NWU values with and without adjustment for iodine. Patients with visible hyperdensities from hemorrhagic transformation or visible contrast retention and bilateral infarcts were excluded. Cerebral edema was measured by relative hemispheric volume (rHV) and midline shift (MLS). Results: Of 125 patients analyzed (median age 71 (IQR = 61–80), baseline National Institutes of Health Stroke Scale (NIHSS) 16 (IQR = 9.75–21)), reperfusion (defined as extended-Thrombolysis-In-Cerebral-Infarction 2b–3) was achieved in 113 patients (90.4%). Iodine-subtracted NWU was significantly higher than unadjusted NWU (17.1% vs 10.8%, p < 0.001). In multivariable median regression analysis, increased age (p = 0.024), number of passes (p = 0.006), final infarct volume (p = 0.023), and study site (p = 0.021) were independently associated with amount of retained contrast. Iodine-subtracted NWU correlated with rHV (rho = 0.154, p = 0.043) and MLS (rho = 0.165, p = 0.033) but unadjusted NWU did not (rHV rho = –0.035, p = 0.35; MLS rho = 0.035, p = 0.347). Conclusions: Angiographic iodine contrast is retained in brain parenchyma 24-h post-thrombectomy, even without visually obvious hyperdensities on CT, and significantly affects NWU measurements. Adjustment for retained iodine using DECT is required for accurate NWU measurements post-thrombectomy. Future quantitative studies analyzing CT after thrombectomy should consider occult contrast retention. [ABSTRACT FROM AUTHOR]

26. An Evaluation of the Occupationally Oriented Basic Education Program In Waterbury, Connecticut.

- Abstract
The Waterbury Board of Education is currently operating a Manpower Development and Training Act (MDTA) project. It includes the Adult Basic Education Program and Occupational Skill Training. The program provides basic elementary education to individuals functioning at or below the third grade level in arithmetic and English, as well as those who are unable to speak, read, or write the English language. The objective of the project is to train unemployed and underemployed youth and adults who have inadequate skills, to obtain and hold jobs, so they may become productive, functional members of society. The subjects were selected from terminees of the program who had been given the Basic Education Test (BET) upon entry and the Adult Basic Learning Exam (ABLE) during the training period. Analysis of variance and trend analysis were conducted and the results indicate that the program was effective in giving its trainees an increase in educational attainment with a linear trend. Trial Trench Evaluation The Trial Trench Evaluation encompassed two parcels of land over several fields comprising land package C25091 (centred on NGR 462916, 231057) comprising a total of 3.25ha. The evaluation comprised 21 trenches (1 - 21). The geophysical survey identified the presence of seven of the 17 features excavated, comprising three ditches, a limestone outcrop, in which two quarry pits were recorded and two furrows. It also uncovered features that were not evident in the results of the geophysical survey, comprising part of a former Roman road that linked Alchester and Towcester (Margary 160a, Via Glareata), two pits, a large posthole, one ditch, one gully, three more furrows and a tree throw. The results of the Trial Trench Evaluation identified three main phases of activity. The basal remains of the former Roman road were identified in Trench 3 and a ditch in the same trench may represent an associated roadside drainage ditch. Another, possible agricultural boundary ditch in Trench 2 may have respected the alignment of the road and been of contemporary date. A medieval to post-medieval furrow in Trench 7 yielded a single sherd of Roman pottery. This may have derived from an underlying pit, which was cut by the furrow, and very tentatively may also be ascribed a Roman period origin. In addition, also tentatively, a V-Shaped ditch at the southern end of Trench 7, aligned broadly east / west, could also be of Roman origin; however, a piece of modern bottle glass was recovered from it which could either date it to the modern period, or could be intrusive. Elsewhere agricultural activity, comprising five broadly north-east / south-west aligned furrows were recorded in Trenches 6, 7, 12 and 15. These are likely to date to the wider medieval to post-medieval periods. A possible agricultural boundary ditch was identified in Trench 17, on an east / west alignment, which may represent the remains of an agricultural boundary ditch. This may also date to the medieval or post-medieval period, though this interpretation remains tentative since no dating evidence was recovered. These agricultural features are likely to be associated with agricultural practices in medieval and postmedieval Newton Purcell, or possibly the medieval moated manor located c. 400m to the south-west of the Site. A posthole of probable modern origin, its wooden post in situ and broadly intact, and two probably modern quarry pits, were recorded in Trench 2. These may well be associated with the development of the Great Central Railway. Finally, there are the remains of a gully in Trench 19 and an elongated pit in Trench 5, which are difficult to ascribe by morphology or location to any specific period, and so remain undated. With the exception of the single redeposited Roman period sherd from the furrow in Trench 7, in situ post and piece of bottle glass, no dating evidence was recovered from any of the features excavated during the Trial Trenching Evaluation, on this basis firm dating of the majority of features is unconfirmed and the above phases have been defined on morphological grounds and the preceding evidence of non-intrusive investigations and desk-based assessment.

- Abstract
A 26-year-old female, G4 P2012 presented for an anatomy scan at 18 weeks. Multiple macrocysts were seen in the left fetal lung, which lead to a diagnosis of congenital pulmonary airway malformation (CPAM) type II. A fetal MRI examination performed at 24 weeks of gestation confirmed the diagnosis of CPAM type II. A genetic amniocentesis was done to rule out a fetal chromosomal abnormality and the fetus was found to have mosaic Klinefelter syndrome. Fetal CPAM is not usually associated with chromosomal abnormalities unless there are other fetal malformations present. This is the first known case where a fetus with CPAM and no other malformation was found to have mosaic Klinefelter syndrome. Therefore, we believe it is prudent to offer prenatal diagnostic testing whenever a fetus with CPAM is identified with ultrasound. [ABSTRACT FROM AUTHOR]

- Abstract
One part of the curriculum installation system outlined in this series of five booklets calls for an assessment procedure to measure pupil attainment of instructional objectives, whether cognitive, affective, or behavioral. Activities for the school administrator to schedule prior to the actual involvement of his faculty in the development of a pupil assessment system are presented. These activities are aimed at assessing faculty members' ability to use pupil-instruction objectives. Instruments appropriate to this purpose are cited, along with resource material for use in areas where the faculty is found to be deficient. A discussion of procedures which should be followed in development of the assessment system stresses the need for continual faculty involvement. Extensive resource information is listed, and models for preworkshop, workshop, and on-going activities are suggested. (JY)

Background: CT perfusion may improve diagnostic accuracy in posterior circulation stroke. The posterior circulation Acute Stroke Prognosis Early CT score (pc-ASPECTS) on Computed Tomography Angiography source images (CTA-SI) predicts functional outcome in patients with basilar artery occlusion. Aims: We assessed the prognostic value of pc-ASPECTS on CT perfusion in patients with vertebral and basilar artery occlusion (VBAO) in comparison with CTA-SI. Methods: Whole-brain CT perfusion from consecutive stroke patients with VBAO at four stroke centers was retrospectively analyzed. pc-ASPECTS – a 10-point score assessing hypoattenuation on CTA-SI – was calculated from CT perfusion parameters as focally reduced cerebral blood flow or cerebral blood volume, focally increased time to peak of the deconvolved tissue residue function (Tmax) or mean transit time. Two investigators independently reviewed the images. Reliability was assessed with intraclass correlation coefficient. Good outcome was defined as modified Rankin scale ≤3 at three months. Results: We included 60 patients with VBAO. After assessment of four CT perfusion maps simultaneously, area-under-ROC curve (AROC) was 0.83 (95%CI 0.72–0.93) for cerebral blood volume, 0.76 (95%CI 0.64–0.89) for cerebral blood flow, 0.77 (95%CI 0.64–0.89) for Tmax, 0.70 (95%CI 0.56–0.84) for mean transit time versus area-under-ROC curve 0.64 (95%CI 0.50–0.79) for CTA-SI. Cerebral blood volume had greater accuracy compared with CTA-SI for poor outcome (p = 0.04). In logistic regression analysis, cerebral blood volume pc-ASPECTS≤8 was independently associated with poor outcome (OR 9.3 95%CI 2.2–41; p = 0.003, adjusted for age and clinical severity). Inter-rater agreement was substantial for cerebral blood volume pc-ASPECTS (intraclass correlation coefficient 0.82 95%CI 0.71–0.90 versus 0.67 for CTA-SI 95%CI 0.43–0.81). Conclusions: Cerebral blood volume pc-ASPECTS may identify VBAO patients at higher risk of disability. [ABSTRACT FROM AUTHOR]

50. Safety of Endovascular Thrombectomy for Acute Ischaemic Stroke in Anticoagulated Patients Ineligible for Intravenous Thrombolysis.

Background/Aim: Endovascular thrombectomy may be performed in anticoagulated patients taking vitamin-K antagonists (VKA) or direct-acting oral anticoagulants (DOAC) in whom the use of intravenous tissue plasminogen activator (tPA) is contraindicated. We aimed to investigate the efficacy and safety of mechanical thrombectomy specifically in anticoagulated patients ineligible for thrombolysis. Methods: We performed a retrospective analysis of a prospectively collected database of consecutive ischaemic stroke patients undergoing mechanical thrombectomy from January 2008 to June 2017. Patients receiving any dose of intravenous or intra-arterial thrombolysis were excluded. Patients taking oral anticoagulants (VKAs or DOACs) were compared with non-anticoagulated patients. Outcomes compared between groups included the rate of intracerebral haemorrhage (ICH) on follow-up imaging (ICHany), symptomatic ICH, functional independence at 90 days (modified Rankin scale score, 0–2), mortality, and post-treatment recanalization (Thrombolysis in Cerebral Infarction score ≥2b). Results: In all, 102 patients undergoing mechanical thrombectomy without prior thrombolysis were included in the study. Sixty-six (64.7%) patients were not anticoagulated, 23 (22.5%) patients were taking VKAs, and 13 (12.7%) patients were taking DOACs. There were no significant differences in the rate of ICHany (11.1 vs. 13.6%, p = 0.93) or sICH (2.8 vs. 1.5%, p = 0.14) in anticoagulated patients compared to non-anticoagulated patients. No cases of sICH were observed among patients taking DOACs. After 90 days of follow-up, the rates of functional independence (50.0 vs. 43.1%) and mortality (27.8 vs. 25.8%) were also similar between the anticoagulation and the non-anticoagulation groups. Conclusion: Mechanical thrombectomy appears to be safe and effective in anticoagulated patients ineligible for thrombolysis, with observed haemorrhage rates similar to those of patients not on anticoagulant therapy. However, further multicentre prospective studies are needed, due to the rising number of patients on warfarin and DOACs worldwide. [ABSTRACT FROM AUTHOR]
