This note extends previous work of the authors modelling the Wheatley valve by using six intersecting and contiguous ellipses to obtain a generalized mathematical representation of the Wheatley valve: this provides a number of free parameters that could be employed to obtain an optimal design. Since optimality is multi-objective with many of the objectives conflicting we focus on the stresses imposed on the valve by a constant force field. Three distinctly different designs are chosen and an analysis of the stresses is undertaken, conclusions are drawn and results are discussed. [ABSTRACT FROM AUTHOR]

The optimal design of gas-lubricated contacts is addressed, with emphasis on appropriate strategies for the management of constraints that usually arise from physical or manufacturing considerations. The necessary tools for evaluating the objective function (which is the load capacity in this case) and its gradient are recalled. The presentation aims at 'black-box' implementations in which an optimization package is coupled with a completely independent Reynolds-equation solver. In our implementation DONLP2 is used as optimizer, while the solver is a Galerkin finite-element code. Two applications are reported: A fixed-volume planar slider bearing and a three-pad compliant journal bearing. In both cases the robustness of the approach is demonstrated and interesting aspects of the optimal gap profiles are presented. [DOI: 10.1115/1.1631011]

- Abstract
BACKGROUND: Lung ultrasound (LUS) is a useful tool for assessing lung status in critically ill patients, but is not widely applied in intensive care units (ICUs). The aim of this study was to investigate the use of LUS in Italian ICUs. METHODS: A questionnaire was distributed to 116 ICU representatives belonging to the Italian GiViTI network, participating in a national project on continuous quality of care assessment. The answers to the questionnaire were analyzed with the data collected in the course of said project. RESULTS: 87 ICUs took part in the study. LUS was performed in 94.3% of them, generally by an internal intensivist. The mean number of tests per bed per week was 0.97 (SD, 1.0). A standardized reporting format was present in only 11% of ICUs. LUS was generally used to investigate known or suspected diseases and as a screening tool in only 10%. The number of testes per bed per week was inversely correlated to the number of beds per physician (P<0.001) and to the provider performing the LUS (P<0.001), but not to other structural, organizational or performance data. CONCLUSIONS: LUS is performed in the majority of ICUs and is generally used to investigate known or suspected diseases. Frequency of use appears to be low and related to selected organizational factors. There is generally no standardized format for reporting LUS findings.

SUMMARYThe prompt availability of reliable epidemiological information on emerging pandemics is crucial for public health policy-makers. Early in 2013, a possible new H1N1 epidemic notified by an intensive care unit (ICU) to GiViTI, the Italian ICU network, prompted the re-activation of the real-time monitoring system developed during the 2009–2010 pandemic. Based on data from 216 ICUs, we were able to detect and monitor an outbreak of severe H1N1 infection, and to compare the situation with previous years. The timely and correct assessment of the severity of an epidemic can be obtained by investigating ICU admissions, especially when historical comparisons can be made.

- Abstract
The prompt availability of reliable epidemiological information on emerging pandemics is crucial for public health policy-makers. Early in 2013, a possible new H1N1 epidemic notified by an intensive care unit (ICU) to GiViTI, the Italian ICU network, prompted the re-activation of the real-time monitoring system developed during the 2009-2010 pandemic. Based on data from 216 ICUs, we were able to detect and monitor an outbreak of severe H1N1 infection, and to compare the situation with previous years. The timely and correct assessment of the severity of an epidemic can be obtained by investigating ICU admissions, especially when historical comparisons can be made.

Il sistema carsico di Sant’Angelo Muxaro si sviluppa in corrispondenza del rilievo gessoso su cui sorge l’abitato omonimo, nel centro Sicilia, in provincia di Agrigento. Tale sistema è costituito da due inghiottitoi attivi (Grotta di Sant’Angelo Muxaro o localmente Grotta Ciavuli e Inghiottitoio Infantino), che si aprono al termine di due valli cieche alla base del versante meridionale del rilievo, e da una risorgenza attiva, ubicata alle pendici del versante nord-occidentale dello stesso rilievo. Le prime esplorazioni speleologiche e l’attività di ricerca in questo sistema nei gessi messiniani della Sicilia, escludendo le segnalazioni del geografo Olinto Marinelli, sono cominciate tra la fine degli anni ’80 e la metà degli anni ‘90, quando è stata topografata e descritta la Grotta di Sant’Angelo Muxaro ed esplorato l’Inghiottitoio Infantino. Solo a partire dal 2004 le ricerche si sono rivolte anche alla risorgenza. Nell’ambito di studi di carattere geomorfologico effettuati nell’area, è stato realizzato un nuovo rilievo topografico e geomorfologico della Grotta di Sant’Angelo Muxaro. In questa occasione sono stati scoperti alcuni rami nuovi, caratterizzati dalla presenza di particolari speleotemi gessosi. In questo contributo verrà descritto il sistema carsico nel suo complesso, anche alla luce delle nuove scoperte.

SUMMARYAims– The aim of this work is to present the main discrepancies, as evidenced by the SIEP-DIRECT's Project, between the evidence-based NICE guidelines for schizophrenia and the usual practices of the Italian mental health services in order to promote the recovery of patients with schizophrenia.Methods– Starting from the main NICE recommendations on recovery promotion, 41 indicators were developed. These were experimented in 19 participating Italian Mental Health Departments (MHD) or Psychiatric Services through self-evaluation of the activities carried out to promote patient recovery with the aim of assessing the level of adherence to the recommendations. The data required by most of the indicators were obtained from the psychiatric informative system or from the Direction of the MHD. Moreover, specific research was carried out on the clinical records and on representative patient samples. Furthermore, for 14 indicators, there was requested an assessment by the part of “multidisciplinary” or “specialistic” focus groups who then attributed a score according to a defined “ad hoc” scale.Results– According to the data obtained, although the mental health services seem to care about the physical condition of their patients, they do not routinely examine principle parameters such as blood pressure, glycaemia etc., and collaboration with general practitioners is often complex or not uniformly practiced. Most psychiatrists and psychologists possess the basic communication skills but not enough competences in cognitive-behavioural treatments; such treatments, and every other form of structured individual psychotherapy, are seldom carried out and seem to have become marginal activities within the Services. Also family psycho-educational interventions are under-used. The Services are very active in the care of multi-problem schizophrenia patients, who make up a large percentage (almost a quarter, on average) of the patients in their care. These patients are offered specific and integrated treatment plans with the involvement of other health services and social agencies operating in the territory. The strategies adopted by the services for the pharmacological treatment in the prevention of relapses and for patients with frequent crises or with treatment-resistant schizophrenia are all in line with the NICE recommendations. Finally, the Services promote activities of vocational training and supported employment, but the outcomes of these are often unsatisfactory.Conclusions– The results of the study show a picture of the Italian mental health services with bright yet also dark areas as regards recovery promotion activities. The Services seem to guarantee adequate pharmacological evidence-based treatments, an integrated assistance and good management of multi-problem patients. They have difficulty, however, with respect to the monitoring of the physical health of the patients, psychotherapeutic activities, including those for families, and the promotion of supported employment. Moreover, they still show problems regarding the structuring and formalizing of care processes. To improve this situation, they should make greater use of professional guidelines, protocols and written procedures.Declaration of Interest:None.

Aims - This paper aims at presenting the most significant results emerging from the work carried out by the focus groups of the multi-centre Project SIEP-DIRECT'S. The Project is aimed at assessing the existing discrepancies between the evidence-based NICE guidelines for schizophrenia and the usual practices of care given by Italian mental health services. Each focus group was requested to give an evaluation on: a) appropriateness of the English NICE guidelines in the context of the Italian mental health services; b) clarity and usefulness of the 103 indicators developed on the basis of the NICE recommendations to measure their level of application within the services. Methods - In each of the 19 mental health departments or psychiatric services participating in the Project there were organized "multidisciplinary" focus groups and "specialistic" focus groups. The former included, amongst others, professional operators of the mental health services, patients, their relatives, representatives of patient organizations and general practitioners. They examined the recommendations and indicators upon which the participants could express their opinion or judgment based on their knowledge, experience or information in their possession. The latter group, composed only of psychiatrists, examined the recommendations and indicators relative to pharmacological treatments that regarded the specific competences of their professional category. Results - Most NICE recommendations seemed appropriate to the working context of the Italian services. However, some perplexity emerged as regards specific organizational models of the services, such as the specific services for psychotic onsets or the assertive outreach teams, which were believed not to be strictly pertinent to the traditional organization of mental health care in our Country. SUMMARYAims– To highlight the major discrepancies that emerged between evidence and routine practice in the framework of the SIEP-DIRECT's Project (DIscrepancy betweenRoutine practice andEvidence in psychiatricCommunityTreatments onSchizophrenia). The Project was conducted in 19 Italian mental health services (MHS), with the aims of: a) evaluating the appropriateness of the NICE Guidelines for Schizophrenia in the Italian context, b) developing and testing a set of 103 indicators that operationalised preferred clinical practice requirements according to the NICE Guidelines, and c) evaluating their actual application in Italian MHSs.Methods– The indicators investigated five different areas: common elements in all phases of schizophrenia; first episode treatment; crisis treatment; promoting recovery; the aggressive behaviour management.Results– The NICE recommendations examined were judged in most instances to be appropriate to the Italian MHS context, and the indicators fairly easy to use. The more severe and frequently encountered evidence-practice discrepancies were: lack of written material, guidelines, and information to be systematically provided to users; lack of intervention monitoring and evaluation; difficulty in implementingspecific and structured forms of intervention; difficulty in considering patients' family members as figures requiring targeted support themselves and who should also be regularly involved in the patient care process.Conclusions– The key actions to be undertaken to favour implementation of evidence-based routine practices are: focussing on mental illness onset and family support/involvement in care; planning training activities aimed at achieving specific treatment goals; encouraging MHS participation in evaluation activities; identifying thresholds for guideline application and promoting specific guideline implementation actions; and activating decision making and resource allocationprocesses that rely more strictly on evidence and epidemiological assessment. AIMS: To evaluate the quality of acute psychiatric care concerning the management of violent behaviour and rapid tranquilization. METHODS: Data concerning 13 indicators, drawn from NICE recommendations, were collected in 19 Departments of Mental Health, in the frame of the SIEP-DIRECT'S Project, to evaluate the implementation of NICE recommendations in Italian Mental Health Services. Aims - The aim of this work is to present the main discrepancies, as evidenced by the SIEP-DIRECT'S Project, between the evidence-based NICE guidelines for schizophrenia and the usual practices of the Italian mental health services in order to promote the recovery of patients with schizophrenia. Methods - Starting from the main NICE recommendations on recovery promotion, 41 indicators were developed. These were experimented in 19 participating Italian Mental Health Departments (MHD) or Psychiatric Services through self-evaluation of the activities carried out to promote patient recovery with the aim of assessing the level of adherence to the recommendations. The data required by most of the indicators were obtained from the psychiatric informative system or from the Direction of the MHD. Moreover, specific research was carried out on the clinical records and on representative patient samples. Furthermore, for 14 indicators, there was requested an assessment by the part of "multidisciplinary" or "specialistic" focus groups who then attributed a score according to a defined "ad hoc" scale. Results - According to the data obtained, although the mental health services seem to care about the physical condition of their patients, they do not routinely examine principle parameters such as blood pressure, glycaemia etc., and collaboration with general practitioners is often complex or not uniformly practiced. Most psychiatrists and psychologists possess the basic communication skills but not enough competences in cognitive-behavioural treatments; such treatments, and every other form of structured individual psychotherapy, are seldom carried out and seem to have become marginal activities within the Services. Also family psycho-educational interventions are under-used. The Services are very active in the care of multi-problem schizophrenia patients, who make up a large percentage (almost a quarter, on average) of the patients in their care. These patients are offered specific and integrated treatment plans with the involvement of other health services and social agencies operating in the territory. The strategies adopted by the services for the pharmacological treatment in the prevention of relapses and for patients with frequent crises or with treatment-resistant schizophrenia are all in line with the NICE recommendations. Finally, the Services promote activities of vocational training and supported employment, but the outcomes of these are often unsatisfactory. Conclusions - The results of the study show a picture of the Italian mental health services with bright yet also dark areas as regards recovery promotion activities. The Services seem to guarantee adequate pharmacological evidence-based treatments, an integrated assistance and good management of multi-problem patients. They have difficulty, however, with respect to the monitoring of the physical health of the patients, psychotherapeutic activities, including those for families, and the promotion of supported employment. Moreover, they still show problems regarding the structuring and formalizing of care processes. To improve this situation, they should make greater use of professional guidelines, protocols and written procedures.

Aims - To evaluate the quality of psychiatric care in Italian community-based services and the discrepancy between real practices and NICE recommendations for the treatment of schizophrenia concerning the elements common to all phases of care and the first episode of psychosis. Methods - Data concerning 14 indicators on common aspects of care in all phases and 11 indicators concerning psychosis onset, drawn from NICE Recommendations, were collected in 19 Departments of Mental Health. Results - An optimistic attitude seems to prevail in the staff in all phases of care, while remarkable discrepancies between service practice and recommendations have been found in relation to systematic assessment, availability of informative leaflets and support to relatives. Concerning the treatment of first episode, a lack of specific services and differentiated activities, and paucity of practices based on specific guidelines has been detected. However, Italian community based services proved to have a good capacity to provide help quickly to those seeking help for a psychotic onset, to maintain regular contact with them in the subsequent year, and provide pharmacological treatment reasonably in line with the scientific evidence. Instead, little specific support is provided to the relatives. Conclusions - The results show critical points concerning capacity of assessment and treatment standardization, in all aspects of care and specifically in the treatment for the first episode. Differentiation of activities specifically dedicated to the patients at their first episode should be promoted as well as strategies to support relatives in a more specific way

- Abstract
We discuss in this paper some implementation aspects of a finite element formulation for the incompressible Navier-Stokes equations which allows the use of equal order velocity-pressure interpolations. The method consists in introducing the projection of the pressure gradient and adding the difference between the pressure Laplacian and the divergence of this new field to the incompressibility equation, both multiplied by suitable algorithmic parameters. The main purpose of this paper is to discuss how to deal with the new variable in the implementation of the algorithm. Obviously, it could be treated as one extra unknown, either explicitly or as a condensed variable. However, we take for granted that the only way for the algorithm to be efficient is to uncouple it from the velocity-pressure calculation in one way or another. Here we discuss some iterative schemes to perform this uncoupling of the pressure gradient projection (PGP) from the calculation of the velocity and the pressure, both for the stationary and the transient Navier-Stokes equations. In the first case, the strategies analyzed refer to the interaction of the linearization loop and the iterative segregation of the PGP, whereas in the second the main dilemma concerns the explicit or implicit treatment of the PGP., Peer Reviewed, Postprint (author’s final draft)

En este trabajo se estudia el fenómeno de migración de hidrógeno y formación de blisters de hidruro ante el contacto entre tubos de presión y tubos calandria en reactores tipo CANDU. Este fenómeno es actualmente admitido como uno de los principales factores limitantes de la vida Útil de este tipo de reactores, desde que ocasionara el incidente de Pickering en 1983. Se desarrolla un método numérico basado en la regularización de las ecuaciones constitutivas que, con el tratamiento usual de elementos finitos y con un esquema de Newton-Raphson, permite resolver el citado problema sobre redes generales. Se incluyen resultados unidimensionales y bidimensionales, que muestran buen acuerdo con soluciones cuasi-analíticas disponibles (caso l-D) e ilustran la capacidad del método empleado.

- Abstract
Surface tension in multi-phase fluid flow engender pressure discontinuities on phase interfaces. In this work we present two finite element methods to solve viscous incompressible flows problems, especially designed to cope with such a situation. Taking as a model the two-dimensional Stokes system, we consider stable solution methods based on piecewise linear approximations of both the velocity and pressure, with either velocity bubble or divergence free penalty enrichment. However a modification of the pressure space is employed in order to represent interface discontinuities. A priori error analyses point to optimal convergence rates for both approaches, which justify observations from plentiful numerical experiments. [ABSTRACT FROM AUTHOR]

State‐of‐the‐art magnetic storage devices have head‐to‐disk distances of about 300 Angstrom, for which compressibility, slip‐flow and roughness effects are significant. Since the head and the disk are in relative motion, the air‐gap thickness when both surfaces are rough varies rapidly in both space and time. A rigorous homogenization of the transient compressible Reynolds equation appropriate for such situation is presented in this article. If ℇ is the roughness length and pℇ the pressure field for that roughness, the existence of p0∈L2(0,T,H1(Ω)) such that pℇ→p0 strongly in L2(Ω×]0,T[) when ℇ→0 is proved. A homogenized problem for p0 is introduced together with a uniqueness result under remarkably weak assumptions, i.e., p0∈L2(0,T,H1(Ω)) and ∂p0/∂t∈L2(0,T,H-1(Ω)). Interestingly, no time derivatives appear in the auxiliary local problems, which are thus computed as in the steady state case. The role of the time variable is to parameterize the relative positions of the roughness shapes, and the homogenized coefficients result from averaging all such positions. To our knowledge, this is the first rigorous treatment of lubrication problems accounting for roughness on both surfaces. [ABSTRACT FROM AUTHOR]

50. Modeling and scaling of aeration bubble plumes: A two-phase flow analysis

- Abstract
In this paper, we focus on bubble plumes aimed at providing oxygen to, and at mixing unstratified water bodies. First, we discuss scaling relations obtained from a three-dimensional (3D) theoretical model for multi-component fluids.An expression for a length scale that represents a balance between inertia and buoyancy is derived, and it is used to scale vertical as well as horizontal (radial) distances. This length scale is then analyzed in terms of previous bubble-plume non-dimensional numbers. For the practical case in which the bubble diameter remains almost constant throughout the bubble plume, a one-dimensional theoretical model—which can be derived by integrating the 3D model in a horizontal plane—is used to show that the length scale (D) is well suited for defining zones in bubble plumes. These zones include a region near the diffuser where the size of the diffuser is important, an intermediate zone where the diffuser size and D determine the flow features, and an asymptotic region where plume variables approach a universal behavior. Procedures for scaling entire reservoirs and portions of reservoirs are developed. These procedures are evaluated with results from previous experiments and new observations in a large tank and a scaled physical model. Scaling of turbulence statistics in bubble plumes is also investigated.
