11. Partial replication of stable individual factors in resting state functional brain networks

Abstract

Functional brain organisation is widely studied today, however with diverging methods which lead to hard to compare results. For example, in modelling resting state functional networks, one may choose a hard-parcellation which assumes a fixed spatial map of brain areas which are temporally synchronous in their activity. On the other hand, soft-parcellations may assume a fixed temporal signal which can vary across spatial areas with different weights. In this modelling approach a single brain area may then belong to several networks. With such fundamental differences in models of functional organisation, questions may be raised as to whether certain results in the field are model dependent or translate across models. The following project is aimed at investigating stability of resting state functional brain networks at the individual level, partially replicating Gratton et al. (2018) though using a different dataset and different functional brain network model. Instead of a hard-parcellation of 9 densely sampled individuals (MSC; Gordon et al., 2017), we use the HCP test-retest dataset (n = 45) and estimate individual level networks with a soft-parcellation algorithm known as Probabilistic Functional Modes (PROFUMO; Harrison et al., 2015; 2020). Lastly, instead of comparing connectivity matrices of a network we compare the spatial topography of networks.

Abstract

Introduction: Human immunodeficiency virus (HIV) pre‐exposure prophylaxis (PrEP) is largely underutilized in the Southern United States. Given their community presence, pharmacists are well positioned to provide PrEP within rural, Southern regions. However, pharmacists' readiness to prescribe PrEP in these communities remains unknown. Objective: To determine the perceived feasibility and acceptability of prescribing PrEP by pharmacists in South Carolina (SC). Methods: We distributed a 43‐question online descriptive survey through the University of South Carolina Kennedy Pharmacy Innovation Center's listserv of licensed SC pharmacists. We assessed pharmacists' comfort, knowledge, and readiness to provide PrEP. Results: A total of 150 pharmacists responded to the survey. The majority were White (73%, n = 110), female (62%, n = 93), and non‐Hispanic (83%, n = 125). Pharmacists practiced in retail (25%, n = 37), hospital (22%, n = 33), independent (17%, n = 25), community (13%, n = 19), specialty (6%, n = 9), and academic settings (3%, n = 4); 11% (n = 17) practiced in rural locales. Pharmacists viewed PrEP as both effective (97%, n = 122/125) and beneficial (74% n = 97/131) for their clients. Many pharmacists reported being ready (60% n = 79/130) and willing (86% n = 111/129) to prescribe PrEP, although over half (62% n = 73/118) cited lack of PrEP knowledge as a barrier. Pharmacists described pharmacies as an appropriate location to prescribe PrEP (72% n = 97/134). Conclusions: Most SC pharmacists surveyed considered PrEP to be effective and beneficial for individuals who frequent their pharmacy and are willing to prescribe this therapy if statewide statutes allow. Many felt that pharmacies are an appropriate location to prescribe PrEP but lack a complete understanding of required protocols to manage these patients. Further investigation into facilitators and barriers of pharmacy‐driven PrEP is needed to enhance utilization within communities. [ABSTRACT FROM AUTHOR]

Abstract

Background Human immunodeficiency virus (HIV) pre-exposure prophylaxis (PrEP) is underutilized in the southern United States. Rapid identification of individuals vulnerable to diagnosis of HIV using electronic health record (EHR)-based tools may augment PrEP uptake in the region. Methods Using machine learning, we developed EHR-based models to predict incident HIV diagnosis as a surrogate for PrEP candidacy. We included patients from a southern medical system with encounters between October 2014 and August 2016, training the model to predict incident HIV diagnosis between September 2016 and August 2018. We obtained 74 EHR variables as potential predictors. We compared Extreme Gradient Boosting (XGBoost) versus least absolute shrinkage selection operator (LASSO) logistic regression models, and assessed performance, overall and among women, using area under the receiver operating characteristic curve (AUROC) and area under precision recall curve (AUPRC). Results Of 998 787 eligible patients, 162 had an incident HIV diagnosis, of whom 49 were women. The XGBoost model outperformed the LASSO model for the total cohort, achieving an AUROC of 0.89 and AUPRC of 0.01. The female-only cohort XGBoost model resulted in an AUROC of 0.78 and AUPRC of 0.00025. The most predictive variables for the overall cohort were race, sex, and male partner. The strongest positive predictors for the female-only cohort were history of pelvic inflammatory disease, drug use, and tobacco use. Conclusions Our machine-learning models were able to effectively predict incident HIV diagnoses including among women. This study establishes feasibility of using these models to identify persons most suitable for PrEP in the South. [ABSTRACT FROM AUTHOR]

24. Understanding Retention in Pre-Exposure Prophylaxis Care in the South: Insights from an Academic HIV Prevention Clinic.

Abstract

HIV pre-exposure prophylaxis (PrEP) is poorly utilized in the southern United States. We examined PrEP retention in care and sexually transmitted infections (STIs) through a retrospective review of the Duke University PrEP Clinic from January 1, 2015 to October 15, 2019. We evaluated short-term (3 months), long-term (additional 8–12 months), and longitudinal retention in care in our clinic. Adjusted odds ratios (aOR) were generated to explore demographics associated with retention. Kaplan–Meier curves were generated to view retention longitudinally. STIs were examined at baseline (1 year before initial PrEP visit) and while retained in care. Of a total of 255 patients; 88% were men, 37% were black, and 73% were men who have sex with men (MSM). Short- and long-term retention in care were met by 130/237 (55%) and 80/217 (37%) patients, respectively. MSM were more likely to be retained in the short term (aOR = 5.22, 95% confidence interval [CI] = 1.57–17.32). Self-referred patients were more likely to be retained in the long term (aOR = 2.18, 95% CI = 1.12–4.23). Uninsured patients were less likely to be retained in the long term (aOR = 0.32, 95% CI = 0.11–0.91). STI diagnoses include 42 infections at baseline and 69 infections during follow-up. STI diagnosed while in PrEP care was associated with longer retention in care over time. Patients discontinue PrEP care over time and STIs were frequently encountered. Abstract

1,2,3,4,6-Penta-O-(trans-3,4-dialkoxycinnamoyl)-(D)-glucopyranoses (GP) were synthesized using 3,4-dialkoxybenzaldehydes as starting material. The phase structures of GP were determined by optical microscopy, differential scanning calorimetry and X-ray powder diffraction. The results revealed a short and a long-chained homologue of the alpha and beta anomers of GP upon irradiation, which result in two intramolecular [2+2] cycloaddition reactions between cinnamoyl double bonds of GP.

Abstract

Twenty articles describing innovative courses and teaching methods are included in this first annual collection. Techniques for English courses are described in four articles covering the uses of media, the tutorial approach, teaching the art of questioning, and using the steps of film production as analogy in writing instruction. Two articles describe interdisciplinary courses in the sciences and humanities, while a third discusses one means of involving faculty in interdisciplinary programs. Biofeedback training and the use of counselors as instructors are considered in two articles on personal development courses. Other descriptions of innovative approaches include the use of art in teaching western civilization; simulation in nursing education; a course in geometry based on inductive reasoning; ways of involving students in American Indian history; using corporate annual reports to teach the accounting language of business; competency-based instruction in art history; teaching analytic thinking in introductory sociology; computer-assisted instruction in biology; and using federal census data as primary sources in history instruction. This collection of 15 articles on teaching and learning by community college faculty explores the following topics: (1) using modern treatments of standard literary themes and texts to enhance "relevance"; (2) coordination of a course based on Jacob Bronowski's "The Ascent of Man"; (3) planning a "Library Awareness Day"; (4) use of a management development program to establish a sense of community often lacking at the large "commuter" school; (5) panning for gold as an exercise to teach sedimentation to students of introductory geology; (6) an actor's approach to the study of plays; (7) maintaining standards in travel/study courses with emphasis on courses that take place in limited time periods; (8) a thematic approach to the teaching of western civilization; (9) experience-based instruction in archaeology courses; (10) an argument in favor of focusing on creativity in art appreciation courses; (11) use of audio-tape descriptions to assist students in choosing a community field work placement in mental health programs; (12) structuring history instruction to promote understanding of past, present, and future; (13) teaching literature to non-literary students; (14) an interdisciplinary approach to teaching the skills and procedures of the library research paper; and (15) a model for student government at the community college. This collection of 17 articles dealing with community college education begins with Marvin Feldman's discussion of the origins and future of the community college. Then, Faye Lenarcic examines the relation between sexism and language. Next, Mary Loomba discusses the problem of burnout in relation to librarians. Howard Himelstein outlines the faculty's role in reducing stress among adult students. After Ralph Szwenda's presentation of tips for working with deaf students, Maryanne Vent describes the use of nonfiction works in a literature elective. Next, Harold Cantor provides perspectives on general education in the community college, and John Cadigan presents methods of integrating the humanities and business education. The significance of study-abroad programs for community college students is examined by Jonathan Lambert prior to Sally Walters' article on the role of library services in continuing education courses. Following Sally Carr's discussion of inmate education programs, Shirley Stanley and Eugene Mozier outline the development of a constructive geometry and trigonomety program. Thomas Giometti then reviews the establishment of a language institute at Mohawk Valley Community College. Next, Libby Bay describes the application of learning theories in a Freshman Composition course. Suzon Kister outlines changes in library technology prior to Shirley Kurtzberg and Eileen Walsh's presentation of an alternative approach to general biology. Ten articles by community college faculty of the State University of New York on instruction-related topics make up this annual publication. First, Harold Cantor argues that the future of community colleges hinges on the development of a cadre of generalists who will foster general education. After the Life Skills Program of Rockland Community College is described by Barbara Viniar in terms of its application of adult development theory, Nancy Jacobs Grossman reviews student activities programs in light of curriculum structure.
