Abstract

BackgroundStress is highly prevalent and known to be a risk factor for a wide range of physical and mental disorders. The effectiveness of digital stress management interventions has been confirmed; however, research on its economic merits is still limited. ObjectiveThis study aims to assess the cost-effectiveness, cost-utility, and cost-benefit of a universal digital stress management intervention for employees compared with a waitlist control condition within a time horizon of 6 months. MethodsRecruitment was directed at the German working population. A sample of 396 employees was randomly assigned to the intervention group (n=198) or the waitlist control condition (WLC) group (n=198). The digital stress management intervention included 7 sessions plus 1 booster session, which was offered without therapeutic guidance. Health service use, patient and family expenditures, and productivity losses were self-assessed and used for costing from a societal and an employer’s perspective. Costs were related to symptom-free status (PSS-10 [Perceived Stress Scale] score 2 SDs below the study population baseline mean) and quality-adjusted life years (QALYs) gained. The sampling error was handled using nonparametric bootstrapping. ResultsFrom a societal perspective, the digital intervention was likely to be dominant compared with WLC, with a 56% probability of being cost-effective at a willingness-to-pay (WTP) of €0 per symptom-free person gained. At the same WTP threshold, the digital intervention had a probability of 55% being cost-effective per QALY gained relative to the WLC. This probability increased to 80% at a societal WTP of €20,000 per QALY gained. Taking the employer’s perspective, the digital intervention showed a probability of a positive return on investment of 78%. ConclusionsDigital preventive stress management for employees appears to be cost-effective societally and provides a favorable return on investment for employers. Abstract

Adequate soft-tissue dimensions have been shown to be crucial for the long-term success of dental implants. To date, there is evidence that placement of dental implants should only be conducted in an area covered with attached gingiva. Modern implant planning software does not visualize soft-tissue dimensions. This study aims to calculate the course of the mucogingival borderline (MG-BL) using statistical shape models (SSM). Visualization of the MG-BL allows the practitioner to consider the soft tissue supply during implant planning. To deploy an SSM of the MG-BL, healthy individuals were examined and the intra-oral anatomy was captured using an intra-oral scanner (IOS). The empirical anatomical data was superimposed and analyzed by principal component analysis. Using a Leave-One-Out Cross Validation (LOOCV), the prediction of the SSM was compared with the original anatomy extracted from IOS. The median error for MG-BL reconstruction was 1.06 mm (0.49–2.15 mm) and 0.81 mm (0.38–1.54 mm) for the maxilla and mandible, respectively. While this method forgoes any technical work or additional patient examination, it represents an effective and digital method for the depiction of soft-tissue dimensions. To achieve clinical applicability, a higher number of datasets has to be implemented in the SSM.

Abstract

Purpose: Modern virtual implant planning is a time-consuming procedure, requiring a careful assessment of prosthetic and anatomical factors within a three-dimensional dataset. In order to facilitate the planning process and provide additional information, this study examines a statistical shape model (SSM) to compute the course of dental roots based on a surface scan. Material and methods: Plaster models of orthognathic patients were scanned and superimposed with three-dimensional data of a cone-beam computer tomography (CBCT). Based on the open-source software “R”, including the packages Morpho, mesheR, Rvcg and RvtkStatismo, an SSM was generated to estimate the tooth axes. The accuracy of the calculated tooth axes was determined using a leave-one-out cross-validation. The deviation of tooth axis prediction in terms of angle or horizontal shift is described with mean and standard deviation. The planning dataset of an implant surgery patient was additionally analyzed using the SSM. Results: 71 datasets were included in this study. The mean angle between the estimated tooth-axis and the actual tooth-axis was 7.5 ± 4.3° in the upper jaw and 6.7 ± 3.8° in the lower jaw. The horizontal deviation between the tooth axis and estimated axis was 1.3 ± 0.8 mm close to the cementoenamel junction, and 0.7 ± 0.5 mm in the apical third of the root. Results for models with one missing tooth did not differ significantly. In the clinical dataset, the SSM could give a reasonable aid for implant positioning. Conclusions: With the presented SSM, the approximate course of dental roots can be predicted based on a surface scan. There was no difference in predicting the tooth axis of existent or missing teeth. In clinical context, the estimation of tooth axes of missing teeth could serve as a reference for implant positioning. However, a higher number of training data must be achieved to obtain increasing accuracy.

Abstract

The recently introduced 5G New Radio is the first wireless standard natively designed to support critical and massive machine type communications (MTC). However, it is already becoming evident that some of the more demanding requirements for MTC cannot be fully supported by 5G networks. Alongside, emerging use cases and applications towards 2030 will give rise to new and more stringent requirements on wireless connectivity in general and MTC in particular. Next generation wireless networks, namely 6G, should therefore be an agile and efficient convergent network designed to meet the diverse and challenging requirements anticipated by 2030. This paper explores the main drivers and requirements of MTC towards 6G, and discusses a wide variety of enabling technologies. More specifically, we first explore the emerging key performance indicators for MTC in 6G. Thereafter, we present a vision for an MTC-optimized holistic end-to-end network architecture. Finally, key enablers towards (1) ultra-low power MTC, (2) massively scalable global connectivity, (3) critical and dependable MTC, and (4) security and privacy preserving schemes for MTC are detailed. Our main objective is to present a set of research directions considering different aspects for an MTC-optimized 6G network in the 2030-era.

13. Dataset size versus homogeneity: A machine learning study on pooling intervention data in e-mental health dropout predictions

Abstract

Objective This study proposes a way of increasing dataset sizes for machine learning tasks in Internet-based Cognitive Behavioral Therapy through pooling interventions. To this end, it (1) examines similarities in user behavior and symptom data among online interventions for patients with depression, social anxiety, and panic disorder and (2) explores whether these similarities suffice to allow for pooling the data together, resulting in more training data when prediction intervention dropout. Methods A total of 6418 routine care patients from the Internet Psychiatry in Stockholm are analyzed using (1) clustering and (2) dropout prediction models. For the latter, prediction models trained on each individual intervention's data are compared to those trained on all three interventions pooled into one dataset. To investigate if results vary with dataset size, the prediction is repeated using small and medium dataset sizes. Results The clustering analysis identified three distinct groups that are almost equally spread across interventions and are instead characterized by different activity levels. In eight out of nine settings investigated, pooling the data improves prediction results compared to models trained on a single intervention dataset. It is further confirmed that models trained on small datasets are more likely to overestimate prediction results. Conclusion The study reveals similar patterns of patients with depression, social anxiety, and panic disorder regarding online activity and intervention dropout. As such, this work offers pooling different interventions’ data as a possible approach to counter the problem of small dataset sizes in psychological research.

Abstract

Providing reliable low latency wireless links for advanced manufacturing and processing systems is a vision of Industry 4.0. Developing, testing and rating requires accurate models of the radio propagation channel. The current 3rd generation partnership project (3GPP) new radio (NR) model as well as the quasi deterministic radio channel generator (QuaDRiGa) lack the propagation parameters for the industrial indoor scenario. To close this gap, measurements were conducted at 2.37 GHz and 5.4 GHz at operational Siemens premises in Nuremberg, Germany. Furthermore, the campaign was planned to allow the test and parameterization of new features of the QuaDRiGa channel model such as support for device-to-device (D2D) radio links and spatial consistency. A total of 5.9 km measurement track was used to extract the statistical model parameters for line of sight (LOS) and Non-LOS propagation conditions. It was found that the metallic walls and objects in the halls create a rich scattering environment, where a large number of multipath components arrive at the receiver from all directions. This leads to a robust communication link, provided that the transceivers can handle the interference. The extracted parameters can be used in geometric-stochastic channel models such as QuaDRiGa to support simulation studies, both on link and system level.

22. Right ventricular function and mechanics in chemotherapy- and radiotherapy-naïve cancer patients.

Abstract

The present research evaluated right ventricular (RV) structure, function and mechanics in the cancer patients before initiation of chemo- or radiotherapy, and the association between cancer and decreased RV longitudinal strain. This retrospective investigation included 101 chemo- and radiotherapy-naïve patients with solid cancer and 38 age- and gender-matched controls with similar cardiovascular risk profile. Echocardiographic examination and strain evaluation was performed in all participants. RV structure and RV systolic and diastolic function estimated with conventional echocardiographic parameters were similar between the cancer patients and controls. However, RV global longitudinal strain (− 22.7 ± 2.6% vs. − 21.1 ± 2.4%, p < 0.001) was significantly decreased in the cancer patients than in controls. The same was revealed for RV free wall endocardial (− 33.6 ± 4.3% vs. − 31.4 ± 4.0%, p = 0.006) and mid-myocardial (− 25.2 ± 3.6% vs. − 23.7 ± 3.8%, p = 0.035) longitudinal RV strains, whereas difference was not found in RV free wall epicardial longitudinal strain. The presence of cancer was independently of age, gender, body mass index, left ventricular hypertrophy, diabetes, hypertension and pulmonary pressure associated with reduced RV global longitudinal strain (OR 3.79; 95% CI 2.18–10.92, p < 0.001), as well as with decreased free wall RV longitudinal strain (OR 5.73; 95% CI 3.17–9.85, p < 0.001). RV strain is deteriorated in the chemo- and radiotherapy-naïve cancer patients. Endocardial and mid-myocardial layers are more affected than epicardial strain in the cancer patients. The presence of cancer is independently of other clinical parameters associated with reduced RV longitudinal strain

23. Left Ventricular Strain in Chemotherapy-Naive and Radiotherapy-Naive Patients With Cancer

Abstract

Background: We sought to investigate left ventricular (LV) function and mechanics in patients with cancer before they received chemotherapy or radiotherapy, as well as the relationship between cancer and reduced LV multidirectional strain in the whole study population. Methods: The retrospective study involved 122 chemotherapy- and radiotherapy-naive patients with cancer and 45 age- and sex-matched controls with a cardiovascular risk profile similar to that of the patients with cancer. All the patients underwent echocardiographic examination before introduction of chemotherapy or radiotherapy. Results: LV longitudinal (−19.1% ± 2.1% vs −17.8% ± 3.5%; P = 0.022), circumferential (−22.9% ± 3.5% vs −20.1% ± 4.1%; P < 0.001), and radial (40.5% ± 8.8% vs 35.2% ± 10.7%; P = 0.004) strain was significantly lower in the patients with cancer than in the control group. Endocardial and midmyocardial longitudinal LV strain was significantly reduced in the patients with cancer compared with the controls, whereas epicardial longitudinal strain was similar between these groups. Endocardial, midmyocardial, and epicardial circumferential strain was significantly lower in the chemotherapy- or radiotherapy-naive patients with cancer than in the controls. Cancer was associated with reduced longitudinal (odds ratio [OR], 9.0; 95% confidence interval [CI], 2.20-23.50; P < 0.001), reduced circumferential (OR, 7.1; 95% CI, 3.80-20.40; P < 0.001), and reduced radial strain (OR, 7.2; 95% CI, 3.41-25.10; P < 0.001) independent of age, sex, body mass index, diabetes, and hypertension. Conclusions: LV mechanics was impaired in the patients with cancer compared with the controls even before initiation of chemotherapy and radiotherapy. Cancer and hypertension were associated with reduced LV multidirectional strain independent of other clinical parameters. The present results indicate that cancer itself potentially induces cardiac remodelling independent of chemotherapy and radiotherapy.

Abstract

Automation of incoming invoices processing promises to yield vast efficiency improvements in accounting. Until a universal adoption of fully electronic invoice exchange formats has been achieved, machine learning can help bridge the adoption gaps in electronic invoicing by extracting structured information from unstructured invoice formats. Machine learning especially helps the processing of invoices of suppliers who only send invoices infrequently, as the models are able to capture the semantic and visual cues of invoices and generalize them to previously unknown invoice layouts. Since the population of invoices in many companies is skewed toward a few frequent suppliers and their layouts, this research examines the effects of training data taken from such populations on the predictive quality of different machine-learning approaches for the extraction of information from invoices. Comparing the different approaches, we find that they are affected to varying degrees by skewed layout populations: The accuracy gap between in-sample and out-of-sample layouts is much higher in the Chargrid and random forest models than in the LayoutLM transformer model, which also exhibits the best overall predictive quality. To arrive at this finding, we designed and implemented a research pipeline that pays special attention to the distribution of layouts in the splitting of data and the evaluation of the models.

25. Biotic and Abiotic Stress Factors Induce Microbiome Shifts and Enrichment of Distinct Beneficial Bacteria in Tomato Roots

Abstract

Crops are often simultaneously threatened by abiotic and biotic stress factors but the stress response of the plant holobiont is not well understood, despite the high importance of this response to ensure future plant production. Therefore, the aim of this study was to assess the impact of individual and combined abiotic (ionic and osmotic) and biotic (Verticillium dahliae and Fusarium oxysporum) stress factors on plant performance and on the bacterial composition of the root endosphere in tomato. Structure and function of the microbiota was analyzed by 16S ribosomal RNA gene amplicon sequencing and a complementary cultivation approach, including in vitro and in vivo assays. Under all stress conditions, tomato growth and photosynthetic activity was reduced. Combined abiotic stressors with F. oxysporum but not with V. dahliae infection led to an additive negative effect on plant performance. All stress conditions induced a microbiome shift, and changed the relative abundance of phyla such as Firmicutes and classes of Proteobacteria. Endophytes identified as Bacillus, Paenibacillus, and Microbacterium spp. showed tolerance to abiotic stress conditions and plant beneficial effects. Stressor-specific enrichments of beneficial bacteria in the root were discovered (e.g., Paenibacillus in roots infected with F. oxysporum and Microbacterium in roots infected with V. dahliae). Interestingly, endophytes that were able to promote plant growth were obtained only from roots exposed to individual biotic and combined abiotic and biotic stress conditions but not individual abiotic stressors. Our study revealed stressor-specific enrichment of beneficial bacteria in tomato roots, which has implications for novel plant protection strategies.

27. Right ventricular strain in heart failure: Clinical perspective

Abstract

The number of studies demonstrating the importance of right ventricular remodelling in a wide range of cardiovascular diseases has increased in the past two decades. Speckle-tracking imaging provides new variables that give comprehensive information about right ventricular function and mechanics. In this review, we summarize current knowledge of right ventricular mechanics in heart failure with reduced ejection fraction and preserved ejection fraction. We searched PubMed, MEDLINE, Ovid and Embase databases for studies published from January 2000 to December 2016 in the English language using the following keywords: "right ventricle"; "strain"; "speckle tracking"; "heart failure with reduced ejection fraction"; and "heart failure with preserved ejection fraction". Investigations showed that right ventricular dysfunction is associated with higher cardiovascular and overall mortality in patients with heart failure, irrespective of ejection fraction. The number of studies investigating right ventricular strain in patients with heart failure with reduced ejection fraction is constantly increasing, whereas data on right ventricular mechanics in patients with heart failure with preserved ejection fraction are limited. Given the high feasibility, accuracy and clinical implications of right ventricular strain in the population with heart failure, it is of great importance to try to include the evaluation of right ventricular strain as a regular part of each echocardiographic examination in patients with heart failure. However, further investigations are necessary to establish right ventricular strain as a standard variable for decision-making

28. Cross-reactions of immunoglobulin M and G antibodies with enterovirus-specific viral structural proteins

Abstract

We analysed the reactivity of enterovirus-specific human IgM and IgG antibodies with the structural proteins of different enteroviruses by the immunoblot technique. In general, all immunoglobulin G antibodies of the tested sera reacted with capsid polypeptide VP 1 of the viruses tested (echoviruses 9 and 11, coxsackievirus B3 and poliovirus 2). In contrast, enterovirus specific immunoglobulin M antibodies of adults reacted with capsid polypeptides VP 1, VP 2, and/or VP 3 of the viruses mentioned above. The reactions with VP 2 and/or VP 3 were often stronger than with VP 1. IgM antibodies from sera of newborns infected by echovirus 11 reacted with VP 1 and VP 2/3 of echovirus 11 and also with VP 2 and VP 3 of poliovirus 2. Preabsorption experiments indicate that cross-reactive IgG antibodies react with epitopes of VP 1 not present on the surface of intact virus particles. Abstract

An M-antibody capture radioimmunoassay (MACRIA) for anti-rubella IgM was developed. Under optimum conditions positive serum specimens bound up to 20 times as much radioactivity as negative specimens. Positive reactions were expressed in arbitrary units/ml by comparison with a calibration curve derived from results obtained with dilutions of a standard serum. The specificity of the assay was confirmed by testing IgM and IgG rich fractions of positive sera. One hundred and forty specimens from blood donors, patients whose sera contained rheumatoid factor and patients with acute, non-rubella, virus infections were tested by MACRIA. No significant non-specific reactions were detected. Paired sera from acute rubella (25 patients) and individual sera from suspected rubella (69 patients) were tested for anti-rubella IgM by MACRIA and by haemagglutination inhibition following sucrose-density-gradient fractionation. There was close agreement between the two methods. The capture assay was more sensitive and could be used to detect the weak IgM response in women given RA 27/3 vaccine. After the natural infection, the MACRIA was strongly positive for two months and remained weakly so for a further two months. Repeat testing of sera demonstrated good reproducibility of the assay. MACRIA proved a simple, sensitive and specific test for anti-rubella IgM and compared favourably with currently used techniques

33. Personalization strategies in digital mental health interventions: a systematic review and conceptual framework for depressive symptoms

Abstract

IntroductionPersonalization is a much-discussed approach to improve adherence and outcomes for Digital Mental Health interventions (DMHIs). Yet, major questions remain open, such as (1) what personalization is, (2) how prevalent it is in practice, and (3) what benefits it truly has.MethodsWe address this gap by performing a systematic literature review identifying all empirical studies on DMHIs targeting depressive symptoms in adults from 2015 to September 2022. The search in Pubmed, SCOPUS and Psycinfo led to the inclusion of 138 articles, describing 94 distinct DMHIs provided to an overall sample of approximately 24,300 individuals.ResultsOur investigation results in the conceptualization of personalization as purposefully designed variation between individuals in an intervention's therapeutic elements or its structure. We propose to further differentiate personalization by what is personalized (i.e., intervention content, content order, level of guidance or communication) and the underlying mechanism [i.e., user choice, provider choice, decision rules, and machine-learning (ML) based approaches]. Applying this concept, we identified personalization in 66% of the interventions for depressive symptoms, with personalized intervention content (32% of interventions) and communication with the user (30%) being particularly popular. Personalization via decision rules (48%) and user choice (36%) were the most used mechanisms, while the utilization of ML was rare (3%). Two-thirds of personalized interventions only tailored one dimension of the intervention.DiscussionWe conclude that future interventions could provide even more personalized experiences and especially benefit from using ML models. Abstract

Die der Entstehung des Prostatakarzinoms zugrundeliegenden bzw. mit einem Progreß vergesellschafteten genetischen Veränderungen sind noch weitgehend unbekannt. Ziel der vorliegenden retrospektiven Untersuchung war die Erfassung genetischer Aberrationen an paraffineigebettetem Material primärer Prostatakarzinom-Biopsien und die Korrelation der Daten zum klinischen Verlauf. In 28 karzinomtragenden primären Prostatabiopsien wurden die Tumorareale gekennzeichnet. Nach Dissektion der Tumorzellen wurden die Proben mittels "comparative genomic hybridization" (CGH) analysiert. Daten zum klinischen Verlauf der Patienten wurden erfaßt. Die mittlere Nachbeobachtungszeit betrug 36 Monate, alle Patienten wurden hormonablativ behandelt. 11 Patienten (39 %) wiesen unter laufender Therapie einen Tumorprogreß auf. In 25 (88 %) der 28 untersuchten Tumorareale konnten genetische Aberrationen detektiert werden, im Mittel fanden wir 4,8 Aberrationen je Tumor. Das Verhältnis Verlust / Zugewinn betrug 1,3 / 1. Folgende chromosomale Veränderungen wurden häufig festgestellt: Verluste von 6q in 40 %; von 8p in 32 %; von 16q in 21 %; von 18q in 21 %; Zugewinne von 8q in 32 %; von 17 in 43 %. In Korrelation zum klinischen Verlauf fand sich ein Zugewinn von 8q signifikant häufiger bei Patienten, welche im Beobachtungszeitraum einen Tumorprogreß entwickelten (64 % vs. 12 %; p = 0,0001). In der primären Tumorbiopsie waren bei progredienten Patienten im Mittel 5,3, bei Patienten ohne Tumorprogreß 3,8 Aberrationen je Tumor nachzuweisen. Unsere Ergebnisse zeigen, daß Veränderungen der Chromosomen 6q, 8, 16q, 17 und 18q regelmäßig beim Prostatakarzinom auftreten. Karzinome mit klinisch ungünstigem Verlauf weisen bereits in der primären Biopsie eine hohe genetische Instabilität auf.

36. MIMOSA - a dual approach to detailed land mobile satellite channel modeling

Abstract

As multiple-input-multiple-output (MIMO) transmission technologies are common in terrestrial mobile communications and getting attention for mobile broadcasting satellite systems, the need for a comprehensive MIMO channel model for the dual-polarized land mobile satellite (LMS) channel arises. In the course of the Characterisation of the MIMO Channel for Mobile Satellite Systems (MIMOSA) project, two complementary modeling approaches have been developed that form a dual model. They provide a scalable modeling solution, which permits a user selectable level of simulation complexity and detail. The models have been evaluated by comparing their results with data gathered from the extensive field trials carried out within the MIMOSA project. This paper describes the dual wideband-modeling and narrowband-modeling approaches, provides examples of the usage and provides some evaluation results.

Abstract

BACKGROUND This study was undertaken to determine whether use of the direct renin inhibitor aliskiren would reduce cardiovascular and renal events in patients with type 2 diabetes and chronic kidney disease, cardiovascular disease, or both. METHODS In a double-blind fashion, we randomly assigned 8561 patients to aliskiren (300 mg daily) or placebo as an adjunct to an angiotensin-converting-enzyme inhibitor or an angiotensin-receptor blocker. The primary end point was a composite of the time to cardiovascular death or a first occurrence of cardiac arrest with resuscitation; nonfatal myocardial infarction; nonfatal stroke; unplanned hospitalization for heart failure; end-stage renal disease, death attributable to kidney failure, or the need for renal-replacement therapy with no dialysis or transplantation available or initiated; or doubling of the baseline serum creatinine level. RESULTS The trial was stopped prematurely after the second interim efficacy analysis. After a median follow-up of 32.9 months, the primary end point had occurred in 783 patients (18.3%) assigned to aliskiren as compared with 732 (17.1%) assigned to placebo (hazard ratio, 1.08; 95% confidence interval [CI], 0.98 to 1.20; P = 0.12). Effects on secondary renal end points were similar. Systolic and diastolic blood pressures were lower with aliskiren (between-group differences, 1.3 and 0.6 mm Hg, respectively) and the mean reduction in the urinary albumin-to-creatinine ratio was greater (between-group difference, 14 percentage points; 95% CI, 11 to 17). The proportion of patients with hyperkalemia (serum potassium level, = 6 mmol per liter) was significantly higher in the aliskiren group than in the placebo group (11.2% vs. 7.2%), as was the proportion with reported hypotension (12.1% vs. 8.3%) (P CONCLUSIONS The addition of aliskiren to standard therapy with renin-angiotensin system blockade in patients with type 2 diabetes who are at high risk for cardiovascular and renal events is not supported by these data and may even be harmful.
