Abstract

Background: Utilization of oral anticoagulants for acute ischemic stroke (AIS) prevention in patients with atrial fibrillation (AF) increased in the United States over the last decade. Whether this increase has been accompanied by any change in AF prevalence in AIS at the population level remains unknown. The aim of this study is to evaluate trends in AF prevalence in AIS hospitalizations in various age, sex, and racial subgroups over the last decade. Methods: We used data contained in the 2010–2020 National Inpatient Sample to conduct a serial cross-sectional study. Primary AIS hospitalizations with and without comorbid AF were identified using International Classification of Diseases Codes. Joinpoint regression was used to compute annualized percentage change (APC) in prevalence and to identify points of change in prevalence over time. Results: Of 5,190,148 weighted primary AIS hospitalizations over the study period, 25.1% had comorbid AF. The age- and sex-standardized prevalence of AF in AIS hospitalizations increased across the entire study period 2010–2020 (average APC: 1.3%, 95% confidence interval (CI): 0.8–1.7%). Joinpoint regression showed that prevalence increased in the period 2010–2015 (APC: 2.8%, 95% CI: 1.9–3.9%) but remained stable in the period 2015–2020 (APC: –0.3%, 95% CI: –1.0 to 1.9%). Upon stratification by age and sex, prevalence increased in all age/sex groups from 2010 to 2015 and continued to increase throughout the entire study period in hospitalizations in men 18–39 years (APC: 4.0%, 95% CI: 0.2–7.9%), men 40–59 years (APC: 3.4%, 95% CI: 1.9–4.9%) and women 40–59 years (APC: 4.4%, 95% CI: 2.0–6.8%). In contrast, prevalence declined in hospitalizations in women 60–79 (APC: –1.0%, 95% CI: –0.5 to –1.5%) and women ⩾ 80 years over the period 2015–2020 but plateaued in hospitalizations in similar-aged men over the same period. Conclusion: AF prevalence in AIS hospitalizations in the United States increased over the period 2010–2015, then plateaued over the period 2015–2020 due to declining prevalence in hospitalizations in women ⩾ 60 years and plateauing prevalence in hospitalizations in men ⩾ 60 years. 14. The 'war of images' between Louis XIV and Leopold I: Jean-Baptiste Lully's music and Il pomo d'oro in Vienna in 1667.

Abstract

The current article uncovers a new concordance for an enigmatic dance suite titled Balletti francesi that was copied in Vienna in 1667, showing that all eight dances are arrangements of Lully's music from Hercule amoureux. This finding has permitted new insights into how and why Lully's music appeared in Vienna so early. I argue that the unusual titles of the suite, particularly the four dances for animated statues, connect it to the iconography of the opera Il pomo d'oro and to the political rivalry between Austria and France. Statue iconographies in opera and ballet differed between the imperial and French courts as part of complex strategies of monarchical representation. While much remains uncertain about the transmission and performance of the Lully Balletti francesi , the suite opens a new window on how the dominant two states of Europe engaged in strategies of cultural appropriation and political theatre, while mobilizing music within a so-called 'war of images'. Abstract

Neutron imaging is a non-destructive technique with many applications in diverse fields such as industrial quality assurance, archaeology, and border security. However with the high cost of the standard fill gas 3He and the high cost of scaling conventional digital imaging systems to large areas its applications are limited. Here is presented the proof of concept for a gaseous neutron imaging system utilizing a 10B4C solid state converter and THGEM technology and 2D passive delay line readout. THGEMs used for signal amplification can be produced cost effectively and in large areas by PCB manufacturers. This combined with the reduced channel processing requirements of delay lines over individual pixel readouts results in a cost-effective and scalable system when compared to similar designs using solid state multipliers such as silicon photomultipliers. Here is presented a proof of concept of this imaging system with data acquisition accomplished by digitization and offline image reconstruction achieving mean X and Y resolution of = (1.37+-0.24) mm and = (1.15+-0.13) mm respectively. Studied in parallel with this system is the effectiveness of gadolinium oxide based paint as a thermal neutron shield and image contrast agent. Thesis Doctor of Philosophy (PhD) In this work is presented the development process of a novel and cost-effective neutron imaging system capable of imaging soft biological and dense materials that X-rays are unable to penetrate. Such a system may be scaled to large areas for many applications including the study of large archeological objects or employed as a security measure to monitor border checkpoints for transportation of controlled radioactive materials.

Abstract

As of writing this thesis there are two detectors at TRIUMF’s DRAGON facility which directly measure the kinetic energy (E) of heavy ions at the focal plane of its recoil separator. These are an ionization chamber and a double-sided-silicon-strip detector (DSSSD). The ionization chamber has in- ferior resolution to the DSSSD but can discriminate isobaric contaminants in a E-∆E spectrum. A DSSSD has superior energy resolution and timing but cannot discriminate isobaric contaminants in many cases. A hybrid ioniza- tion chamber / double-sided-silicon-strip detector has been designed using the GEANT4 simulation package that combines the strengths of both these detec- tor types. This hybrid detector design consists of an ionization chamber set in front of a DSSSD positioned at the end of the beamline of the recoil separator. The design presented here is specific to DRAGON’s needs but can conceiv- ably be re-purposed in other environments requiring heavy ion detection and identification and may be useful in fields such as health physics. They were members of Team 3 in the course titled Human Factors and Work Analysis (IE 486). This course explores the application of engineering, computer sciences, information sciences, and psychological principles and methods to the analysis and design of human work systems. In this article, the students describe their development of an application prototype to improve the airport travel experience.

36. Cavernous Thrombophlebitis Secondary to Cavernous Internal Carotid Mycotic Aneurysm

Abstract

We report a case of a 22-year-old male with a history of intravenous drug use presenting with cavernous sinus syndrome secondary to cavernous thrombophlebitis. The source of the thrombophlebitis was from a mycotic aneurysm in the setting of fungal endocarditis. With antifungal therapy and aortic valve replacement, the patient had full resolution of cranial nerve deficits. Descriptions of mycotic aneurysms of the cavernous portion of the internal carotid artery are limited to case reports and case series. Most have been nonendocarditic in etiology with poor prognosis. We present a unique case with endocarditic etiology and an excellent prognosis.

Furthermore, when a patient presents with intractable seizures after metformin initiation, the clinician should be prompted to include mitochondrial disorders that may inhibit NADH dehydrogenase (such as MELAS) in the differential. Although MELAS is a rare disease, clinicians should be aware of this increased risk and recognize that the initiation of metformin is reported to unmask MELAS in previously healthy patients. Abstract

Over 50 million people undergo surgeries each year in the United States, with over 70% of them filling opioid prescriptions within one week of the surgery. Due to the highly addictive nature of these opiates, a post-surgical window is a crucial time for pain management to ensure accurate prescription of opioids. Drug prescription nowadays relies primarily on self-reported pain levels to determine the frequency and dosage of pain drug. Patient pain self-reports are, however, influenced by subjective pain tolerance, memories of past painful episodes, current context, and the patient's integrity in reporting their pain level. Therefore, objective measures of pain are needed to better inform pain management. This paper explores a wearable system, named Painometry, which objectively quantifies users' pain perception based-on multiple physiological signals and facial expressions of pain. We propose a sensing technique, called sweep impedance profiling (SIP), to capture the movement of the facial muscle corrugator supercilii, one of the important physiological expressions of pain. We deploy SIP together with other biosignals, including electroencephalography (EEG), photoplethysmogram (PPG), and galvanic skin response (GSR) for pain quantification. From the anatomical and physiological correlations of pain with these signals, we designed Painometry, a multimodality sensing system, which can accurately quantify different levels of pain safely. We prototyped Painometry by building a custom hardware, firmware, and associated software. Our evaluations use the prototype on 23 subjects, which corresponds to 8832 data points from 276 minutes of an IRB-approved experimental pain-inducing protocol. Using leave-one-out cross-validation to estimate performance on unseen data shows 89.5% and 76.7% accuracy of quantification under 3 and 4 pain states, respectively.

Abstract

The animated statue represented one of the central magical figures in French musical theater of the seventeenth and eighteenth centuries. During the period covered by this dissertation, 1661-1748, animated statues appeared in more than sixty works of musical theater of almost every available genre. This number does not include the many works containing statues that demonstrated magical or otherworldly properties through means other than movement or song. Some of the works of this period that feature living statues are well-known to musicologists—e.g. Molière/Jean-Baptiste Lully’s comedy-ballet Les Fâcheux (1661), Lully’s opera Cadmus et Hermione (1673), and Jean-Philippe Rameau’s one-act ballet Pigmalion (1748)—while others have received little recognition. This dissertation is the first study to consider the history of animated statues on the French stage during this period, and the first to reveal music as a defining feature of these statues. Over the course of nearly ninety years, music assumed an increasingly important role in the theatrical treatments of these figures that operated in the space between magic and mechanics. At the beginning of Louis XIV’s reign, animated statues appeared with some frequency in both public and court spectacles. By the mid-eighteenth century, the animated statue had become the central focus of many works and had transformed into a potent symbol of, among other ideas, the power of music and dance, as most dramatically realized in Rameau’s Pigmalion. This dissertation traces the history of this transformation.

Abstract

In the late-eighteenth century, not long before Ludwig van Beethoven began to experience the first symptoms of hearing loss, an important historical shift in the understanding of deafness took place in Europe. This shift resulted from the birth of deaf education, of the deaf community, and of deafness itself as a social category. The history of deafness, and more broadly of disability, has been neglected by musicologists, and in this thesis I map out points of intersection between Beethoven reception history and deaf history to show that these histories can enlighten each other. I focus on three “documents” of Beethoven reception—the Heiligenstadt Testament, Richard Wagner’s 1870 Beethoven essay, and twentieth-century Beethoven biopics—to show that Beethoven has been used to normalize hearing and construct deafness as its pathological opposite, even as his example shows that hearing loss does not negate music.
