Abstract

Characterization of the genetic landscape of Alzheimer's disease (AD) and related dementias (ADD) provides a unique opportunity for a better understanding of the associated pathophysiological processes. We performed a two-stage genome-wide association study totaling 111,326 clinically diagnosed/'proxy' AD cases and 677,663 controls. We found 75 risk loci, of which 42 were new at the time of analysis. Pathway enrichment analyses confirmed the involvement of amyloid/tau pathways and highlighted microglia implication. Gene prioritization in the new loci identified 31 genes that were suggestive of new genetically associated processes, including the tumor necrosis factor alpha pathway through the linear ubiquitin chain assembly complex. We also built a new genetic risk score associated with the risk of future AD/dementia or progression from mild cognitive impairment to AD/dementia. The improvement in prediction led to a 1.6- to 1.9-fold increase in AD risk from the lowest to the highest decile, in addition to effects of age and the APOE ε4 allele.

Abstract

Objective: Rural unpaid caregivers experience different opportunities and challenges than their urban counterparts. Our aim was to understand rural unpaid caregivers' experiences of challenges and opportunities during the Covid‐19 pandemic in New Zealand. Setting: Rural northern New Zealand during the Covid‐19 pandemic. Participants: A convenience sample of 10 unpaid rural caregivers, most supporting an older person living with dementia. Design: A qualitative descriptive design. Data included interviews and online workshops, and a thematic analysis. Results: Rural unpaid caregivers observed that some aspects of living in a rural context were helpful during the Covid‐19 pandemic. However, tasks such as shopping and long‐distance travel and communication (both telephonic and digital) became more complex. Abrupt changes to routine and closure of resources and support groups created challenges. Unpaid caregivers worried about what would happen if they were hospitalised themselves. There was a diversity of experience both across caregivers, of familiar routines and rural context as resources, increased challenges requiring resourcefulness to adapt, and experiences of crisis. Many experienced all three states at different times. Conclusion: This research provides valuable insights into challenges and opportunities experienced by rural unpaid caregivers during the Covid‐19 pandemic and may help policy‐makers and healthcare professionals develop improved emergency preparedness. Multilevel, multisector responses are needed to ensure effective health and social support and access to resources including health care, access to groceries and medication, transport, and communications (telephone and digital). Enhancing rural access to effective and inexpensive online services is particularly important, as is recognition of the value of rural sites or hubs of connection and social interaction. Abstract

The concept 'performance expectancy' is used to theorize about older people's day-to-day use of technology mediated communication (telephone, text/emails and video calls) in family relationships and the impact on social isolation and loneliness. Using theoretical and methodological triangulation six mediation models are conceptualized, followed by empirical validation using a nationally representative dataset of older people (≥ 65 years) living in Wales, UK (N = 2,099). The results reveal that frequency of telephone and video calls mediate the relationship between proximity of family and social isolation, and telephone calls mediate the relationship with loneliness. Text/emails have no significant mediating effect. Qualitative data analysis provides a deeper understanding of conditions that facilitate technology mediated communication. Demonstrating the impact of lifecourse habits, social context and environment on technology mediated communication shifts the focus from individual deficits to other reasons for non-adoption. Understanding the link between the individual, the social-cultural and social structural environment, social interaction, and loneliness requires an understanding of what is important to older people. For example, knowledge concerning the role of embodied presence in alleviating loneliness can be used to better understand the likelihood of implementing successful interventions. 30. Imagery and Imaginary of Islander Identity: Older People and Migration in Irish Small-Island Communities

Abstract

This article examines the imagery and imaginaries of islander identity and makes an original contribution to the fields of gerontology and nissology. Drawing on data collected through in-depth interviews with 19 older residents of two small-island communities located off the island of Ireland, we address the central roles played by older people in creating and sustaining islander identities. Reflecting both public and private representations of islander identity, the article contrasts an island "imagery" with an island "imaginary", resulting in a complex "imag(in)ery" of islander identity. We explore three main themes. To what extent do older residents of island communities perceive an "imag(in)ery" of islander identity? In what ways do older islanders contribute to, substantiate or perpetuate the imag(in)ery of the islander identity? Are there alternative imag(in)eries of the islander identity for different groups of older people who live in island communities? Our analysis identified two imag(in)eries of islander identity. An historical islander identity was structured by the shared hardships and enforced self-sufficiency associated with residence in remote communities. Contemporary islander identities are founded on the positively perceived isolation of islands, an historical and cultural sense of belonging, frequent social interaction within cohesive, safe and secure communities, and a persistence of "traditional" values. Older people were actively engaged in the (re)production of islander identity, such as helping visitors discover their island origins, producing traditional cultural artefacts, passing knowledge of culture down through the generations, and acting to maintain the civic life of the island community. Knowledge of local and traditional skills imbued some older islanders with the ability to perform island-specific symbolic rituals. Our study revealed subtle forms of differentiation between over-arching categories of island residents based on migration histories. In particular, older people's narratives revealed a hierarchy in relation to claims to islander status. (Contains 1 table, 1 figure, and 6 notes.)

32. Research on Rural Ageing: Where Have We Got to and Where Are We Going in Europe?

Abstract

This paper examines the extent to which rural studies conducted in Europe (compared to other countries in the Global North) have addressed the phenomenon of rural ageing. Through a review of the literature published on rural ageing research in the last decade, it compares the research goals identified by the International Rural Ageing Project (IRAP) in 1998 with progress in this area. In addition to the key themes for rural ageing research identified by IRAP, the authors believe that there are others topics that will emerge and impact on rural ageing in Europe in the coming decade. These have been highlighted consistently in futures studies as "global challenges" and can be grouped as social, economic and political, technological, relating to climate change, or related to agriculture and food security. In addition to the literature review, the authors undertook a consultation exercise with more than 50 eminent academics and directors of key organisations who were interested in rural issues, "global challenges" or rural ageing. Their feedback is included within a framework for future rural research. The article concludes with a discussion of emerging areas for rural ageing in the European context and the challenges that the EU may potentially face over the coming decades. (Contains 2 tables.)

33. Training Older Volunteers in Gerontological Research in the United Kingdom: Moving towards an Andragogical and Emancipatory Agenda

Abstract

In the UK, very few studies have engaged older people in two or more elements of the research process (design, conduct, dissemination). Although there is a body of work on educational gerontology, there are few publications that specifically focus on training older people as coresearchers. This paper reports upon the training program undertaken as part of the Rural North Wales Initiative for the Development of Support for Older People (RuralWIDe). The RuralWIDe training program was built upon principles of andragogy: the art and science of teaching adults. The trainers used lecturing; modeling (observation of another person conducting a task); and active participation (structured guidance, instructions or trial and error, during which the student discovers for him or herself how to perform the task) to teach older volunteers social gerontology, research methods, interview techniques, and data analysis. This paper describes the methods used in RuralWIDe and highlights instances when these were both effective and ineffective. It assesses the study against Rachal's (2002) criteria for andragogy (voluntary participation, adult status, collaboratively determine objectives, performance-based assessment of achievement, measuring satisfaction, appropriate adult learning environment, and technical issues), and it describes when pedagogical assumptions were made by the research team.
