Abstract

Integrated assessment models that incorporate biodiversity and ecosystem services could be an important tool for improving our understanding of interconnected social-economic-ecological systems, and for analyzing how policy alternatives can shift future trajectories towards more sustainable development. Despite recent scientific and technological advances, key gaps remain in the scientific community’s ability to deliver information to decision-makers at the pace and scale needed to address sustainability challenges. We identify five research frontiers for integrated social-economic-ecological modeling (primarily focused on terrestrial systems) to incorporate biodiversity and ecosystem services: 1) downscaling impacts of direct and indirect drivers on ecosystems; 2) incorporating feedbacks in ecosystems; 3) linking ecological impacts to human well-being, 4) disaggregating outcomes for distributional equity considerations, and 5) incorporating dynamic feedbacks of ecosystem services on the social-economic system. We discuss progress and challenges along each of these five frontiers and the science-policy linkages needed to move new research and information into action.

7. Towards a common definition of surgical prehabilitation: a scoping review of randomised trials

Abstract

BACKGROUND: There is no universally accepted definition for surgical prehabilitation. The objectives of this scoping review were to (1) identify how surgical prehabilitation is defined across randomised controlled trials and (2) propose a common definition. METHODS: The final search was conducted in February 2023 using MEDLINE, Embase, PsycINFO, Web of Science, CINAHL, and Cochrane. We included randomised controlled trials (RCTs) of unimodal or multimodal prehabilitation interventions (nutrition, exercise, and psychological support) lasting at least 7 days in adults undergoing elective surgery. Qualitative data were analysed using summative content analysis. RESULTS: We identified 76 prehabilitation trials of patients undergoing abdominal (n=26, 34%), orthopaedic (n=20, 26%), thoracic (n=14, 18%), cardiac (n=7, 9%), spinal (n=4, 5%), and other (n=5, 7%) surgeries. Surgical prehabilitation was explicitly defined in more than half of these RCTs (n=42, 55%). Our findings consolidated the following definition: 'Prehabilitation is a process from diagnosis to surgery, consisting of one or more preoperative interventions of exercise, nutrition, psychological strategies and respiratory training, that aims to enhance functional capacity and physiological reserve to allow patients to withstand surgical stressors, improve postoperative outcomes, and facilitate recovery.' CONCLUSIONS: A common definition is the first step towards standardisation, which is needed to guide future high-quality research and advance the field of prehabilitation. The proposed definition should be further evaluated by international stakeholders to ensure that it is comprehensive and globally accepted.

Abstract

This review has two parts. The first part summarizes the current observational constraints on fluctuations in the solar medium deep within the solar Radiative Zone, and shows how the KamLAND and SNO-salt data combine to make the experimental determination of the neutrino oscillation parameters largely insensitive to prior assumptions about the nature of these oscillations. As part of a search for plausible sources of solar fluctuations to which neutrinos could be sensitive, the second part of the talk summarizes a preliminary analysis of the influence of magnetic fields on helioseismic waves. Using simplifying assumptions which should apply to modes in the solar radiative zone, we find a resonance between Alfven waves and helioseismic g-modes which potentially modifies the solar density profile fairly significantly over comparatively short distance scales, too narrow to be ruled out by present-day analyses of p-wave helioseismic spectra., Comment: Plenary talk presented at AHEP 2003, Valencia, Spain, October 2003

42. Thromboprophylaxis use in paediatric inflammatory bowel disease: an international RAND appropriateness panel

Abstract

BACKGROUND AND AIMS: Thromboprophylaxis use in paediatric inflammatory bowel disease (IBD) is inconsistent. Current guidelines only support treating children with acute severe colitis with risk factors. We convened an international RAND panel to explore thromboprophylaxis in paediatric IBD inpatients in the context of new evidence. METHODS: We convened a geographically diverse 14-person panel of paediatric gastroenterologists alongside supporting experts. An online survey was sent before an online meeting. Panellists were asked to rate the appropriateness of thromboprophylaxis in hospitalised paediatric IBD patients via 27 scenarios of varying ages, gender, and phenotype, with and without thrombotic risk factors. Anonymised results were presented at the meeting. A second modified survey was distributed to all panellists present at the meeting. Results from the second survey constitute the RAND panel results. The validated RAND disagreement index defined disagreement when =1. RESULTS: The combined outcome of thromboprophylaxis being considered appropriate until discharge and inappropriate to withhold was seen in 20 of 27 scenarios, including all patients with new-onset acute severe colitis; all flares of known ulcerative colitis, irrespective of risk factors except pre-pubescent patients with limited disease and no risk factors; and all Crohn's patients with risk factors. Disagreement was seen in 5 scenarios regarding Crohn's without risk factors where outcomes were already uncertain. CONCLUSIONS: RAND panels are an established method to assess expert opinion in areas of limited evidence. This work therefore constitutes neither a guideline nor a consensus; however, the findings suggest a need to re-evaluate the role of thromboprophylaxis in future guidelines.
