3. Exergoeconomic optimization of tetra-combined trigeneration system

Se pretende obtener configuraciones óptimas de sistemas de trig eneración para satisfacer demandas de electricidad y cargas tér micas de calentamiento y enfriamiento, evaluando el impacto de los costo s de producción de electricidad, vapor y agua fría. Un sistema de trigeneración produce electricidad, calor y enfriamiento usando esa electrici dad o calor disponible. Se hace énfasis en sistemas que usan re frigeración por absorción, incluyendo uno que usa refrigeración hibrida de abso rción eyectocompresión, llamado sistema tetracombinado de tri generación. La evaluación del desempeño de los sistemas se lleva a cabo med iante el análisis exergético y exergoeconómico de las alternati vas propuestas para determinar la eficiencia exergética y los costos exergétic os de la producción de las utili dades del sistema. Se usó el m étodo de Algoritmos Genéticos para la optimización de las plantas de trigeneración. La optimización muestra beneficios importantes en los costos e xergéticos de los productos por medio de la maximización de la eficiencia exergét ica de los diferentes sistemas.

- Abstract
BACKGROUND The cardiovascular safety profile of dapagliflozin, a selective inhibitor of sodium-glucose cotransporter 2 that promotes glucosuria in patients with type 2 diabetes, is undefined. METHODS We randomly assigned patients with type 2 diabetes who had or were at risk for atherosclerotic cardiovascular disease to receive either dapagliflozin or placebo. The primary safety outcome was a composite of major adverse cardiovascular events (MACE), defined as cardiovascular death, myocardial infarction, or ischemic stroke. The primary efficacy outcomes were MACE and a composite of cardiovascular death or hospitalization for heart failure. Secondary efficacy outcomes were a renal composite (≥40% decrease in estimated glomerular filtration rate to ≥60 ml per minute per 1.73 m2 of body-surface area, new end-stage renal disease, or death from renal or cardiovascular causes) and death from any cause. RESULTS We evaluated 17,160 patients, including 10,186 without atherosclerotic cardiovascular disease, who were followed for a median of 4.2 years. In the primary safety outcome analysis, dapagliflozin met the prespecified criterion for noninferiority to placebo with respect to MACE (upper boundary of the 95% confidence interval [CI], ≥1.3; P≥0.001 for noninferiority). In the two primary efficacy analyses, dapagliflozin did not result in a lower rate of MACE (8.8% in the dapagliflozin group and 9.4% in the placebo group; hazard ratio, 0.93; 95% CI, 0.84 to 1.03; P = 0.17) but did result in a lower rate of cardiovascular death or hospitalization for heart failure (4.9% vs. 5.8%; hazard ratio, 0.83; 95% CI, 0.73 to 0.95; P = 0.005), which reflected a lower rate of hospitalization for heart failure (hazard ratio, 0.73; 95% CI, 0.61 to 0.88); there was no between-group difference in cardiovascular death (hazard ratio, 0.98; 95% CI, 0.82 to 1.17). A renal event occurred in 4.3% in the dapagliflozin group and in 5.6% in the placebo group (hazard ratio, 0.76; 95% CI, 0.67 to 0.87)

BACKGROUND The cardiovascular safety profile of dapagliflozin, a selective inhibitor of sodium-glucose cotransporter 2 that promotes glucosuria in patients with type 2 diabetes, is undefined. METHODS We randomly assigned patients with type 2 diabetes who had or were at risk for atherosclerotic cardiovascular disease to receive either dapagliflozin or placebo. The primary safety outcome was a composite of major adverse cardiovascular events (MACE), defined as cardiovascular death, myocardial infarction, or ischemic stroke. The primary efficacy outcomes were MACE and a composite of cardiovascular death or hospitalization for heart failure. Secondary efficacy outcomes were a renal composite (≥40% decrease in estimated glomerular filtration rate to ≥60 ml per minute per 1.73 m2 of body-surface area, new end-stage renal disease, or death from renal or cardiovascular causes) and death from any cause. RESULTS We evaluated 17,160 patients, including 10,186 without atherosclerotic cardiovascular disease, who were followed for a median of 4.2 years. In the primary safety outcome analysis, dapagliflozin met the prespecified criterion for noninferiority to placebo with respect to MACE (upper boundary of the 95% confidence interval [CI], ≥1.3; P≥0.001 for noninferiority). In the two primary efficacy analyses, dapagliflozin did not result in a lower rate of MACE (8.8% in the dapagliflozin group and 9.4% in the placebo group; hazard ratio, 0.93; 95% CI, 0.84 to 1.03; P = 0.17) but did result in a lower rate of cardiovascular death or hospitalization for heart failure (4.9% vs. 5.8%; hazard ratio, 0.83; 95% CI, 0.73 to 0.95; P = 0.005), which reflected a lower rate of hospitalization for heart failure (hazard ratio, 0.73; 95% CI, 0.61 to 0.88); there was no between-group difference in cardiovascular death (hazard ratio, 0.98; 95% CI, 0.82 to 1.17). A renal event occurred in 4.3% in the dapagliflozin group and in 5.6% in the placebo group (hazard ratio, 0.76; 95% CI, 0.67 to 0.87), and death from any cause occurred in 6.2% and 6.6%, respectively (hazard ratio, 0.93; 95% CI, 0.82 to 1.04). Diabetic ketoacidosis was more common with dapagliflozin than with placebo (0.3%vs. 0.1%, P = 0.02), as was the rate of genital infections that led to discontinuation of the regimen or that were considered to be serious adverse events (0.9% vs. 0.1%, P≥0.001). CONCLUSIONS In patients with type 2 diabetes who had or were at risk for atherosclerotic cardiovascular disease, treatment with dapagliflozin did not result in a higher or lower rate of MACE than placebo but did result in a lower rate of cardiovascular death or hospitalization for heart failure, a finding that reflects a lower rate of hospitalization for heart failure. By using a four-interaction interionic potential, constant-energy (classical microcanonical ensemble) molecular dynamics simulations, at room temperature, A g + ionic positions in α-RbAg4I5 compound are reported. In this crystal structure (cubic with a = 11.23934(3) Å and P4132 as space group) a new site has been found. The unit cell has 16- A g + ions, and its disordered partial occupancies have been the subject of discussion. Some authors report 72 [ = 24(e) + 24(e) + 8(c) + 4(b) + 12(d) or Ag1, Ag2, Ag3, Ag4, Ag5] sites with equal vacancy probabilities (Bradley and Greene), 60 [ = 24(e) + 24(e) + 8(c) + 4(b) or Ag1, Ag2, Ag3, Ag4 respectively] with different probability (Geller), 56 [ = 24(e) + 24(e) + 8(c) + 4(b) or Ag1, Ag2, Ag3] with different probability and 4 sites having zero probability (Hull et al). We report 96 interstitial sites [ = 24(e) + 24(e) + 8(c) + 4(b) + 12(d) + 24(e) or Ag1, Ag2, Ag3, Ag4, Ag5, Ag6, respectively] with different probabilities. The new site was denoted as Ag6 with 0.77% similar to Ag5 (∼0.66%). Good results (occupancy probabilities) compared to experimental data were found. [ABSTRACT FROM AUTHOR]

16. Exergoeconomic optimization of tetra-combined trigeneration system

- Abstract
This work aims at obtaining optimal configurations of trigeneration systems in order to satisfy required demands for electricity and thermal loads for heating and cooling, evaluating the impact of the electricity, steam and chilled water production costs. A trigeneration system produces electricity, heat and cooling effect using electricity or heat available. Emphasis is made on systems using absorption chillers, including one using a hybrid absorption ejecto-compression chiller, here called tetra-combined trigeneration system. The performance evaluation of systems is carried out by the application of exergy and exergoeconomic analysis of the proposed alternatives in order to determine exergy efficiency and exergy based costs on production of system utilities. The Genetic Algorithms method has been chosen for optimization to show the applicability of evolutionary techniques in trigeneration plants. The optimization shows important profits in the exergy based costs of products, by means of the exergetic efficiency maximization of the different trigeneration systems., Se pretende obtener configuraciones óptimas de sistemas de trigeneración para satisfacer demandas de electricidad y cargas térmicas de calentamiento y enfriamiento, evaluando el impacto de los costos de producción de electricidad, vapor y agua fría. Un sistema de trigeneración produce electricidad, calor y enfriamiento usando esa electricidad o calor disponible. Se hace énfasis en sistemas que usan refrigeración por absorción, incluyendo uno que usa refrigeración hibrida de absorción eyecto-compresión, llamado sistema tetra-combinado de trigeneración. La evaluación del desempeño de los sistemas se lleva a cabo mediante el análisis exergético y exergoeconómico de las alternativas propuestas para determinar la eficiencia exergética y los costos exergéticos de la producción de las utilidades del sistema. Se usó el método de Algoritmos Genéticos para la optimización de las plantas de trigeneración. La optimización muestra beneficios importantes en los costos exergéticos de los productos por medio de la maximización de la eficiencia exergética de los diferentes sistemas.

- Abstract
Recurrent fevers make up a significant yet underappreciated proportion (18-42%) of fevers of unknown origin. A group of recurrent fevers, known as autoinflammatory diseases or hereditary periodic fever syndromes occur due to a deregulated response of innate immunity, whereby an autoinflammatory reaction occurs resulting in tissue damage. Unlike autoimmune diseases, the autoinflammatory diseases are not associated to specific autoantibodies or major histocompatibility complexes (MHCs), but rather result from an aberrant activation of innate immune cells such as macrophages and neutrophils. Although the majority of cases begin during childhood, several are often diagnosed years after their debut or begin during adult life, justifying the need for internists to be familiar with these diseases. In this review we discuss the epidemiology, pathophysiology, clinical presentation, diagnosis and management of the main autoinflammatory diseases. [ABSTRACT FROM AUTHOR]

En la empresa “Grupo Carolina” del sector textil de la ciudad de Ibagué, se desarrolló un equipo de un hogar mecánico para calentar agua por medio de la combustión de la cascarilla de arroz. El agua es utilizada en el proceso de tinturado de la tela. Este trabajo presenta la evaluación del comportamiento de la combustión, la temperatura en el sistema y la emisión de gases hacia la atmósfera del prototipo diseñado, permitiendo realizar un manejo adecuado de los parámetros tecnológicos involucrados y así encontrar el punto más adecuado para la combustión de la cascarilla. La eficiencia del equipo también se presenta en este trabajo.

- Abstract
Las exigencias competitivas actuales han forzado a las organizaciones empresariales a adoptar estrategias colaborativas a lo largo de su cadena de abastecimiento, con el fin de mejorar su desempeño en precio, calidad, plazo y servicio. En este sentido, la selección de una base de proveedores competitiva es de alta importancia en la búsqueda de mejores resultados. Por tal razón, en el presente artículo se expone el desarrollo de una metodología basada en la aplicación de técnicas multicriterio que permite detectar los proveedores con resultados críticos, como primer paso en el diseño de actividades de mejoramiento.

This work aims at obtaining optimal configurations of trigeneration systems in order to satisfy required demands for electricity and thermal loads for heating and cooling, evaluating the impact of the electricity, steam and chilled water production costs. A trigeneration system produces electricity, heat and cooling effect using electricity or heat available. Emphasis is made on systems using absorption chillers, including one using a hybrid absorption ejecto-compression chiller, here called tetra-combined trigeneration system. The performance evaluation of systems is carried out by the application of exergy and exergoeconomic analysis of the proposed alternatives in order to determine exergy efficiency and exergy based costs on production of system utilities. The Genetic Algorithms method has been chosen for optimization to show the applicability of evolutionary techniques in trigeneration plants. The optimization shows important profits in the exergy based costs of products, by means of the exergetic efficiency maximization of the different trigeneration systems. [ABSTRACT FROM AUTHOR]

The structure of the AgI is modified when the ion Ag+ is substituted by the alkaline ion K+ with an ionic radius bigger than that of Na+, i.e., the electrostatic configuration of the ionic system is changed, such that the number of ions that move during the transition at Tt = 420 K is not the same on successive temperature cycles (heating and cooling runs through Tt). The transition to the superionic phase is accompanied by a change observed in the electrical conductivity and these data were analyzed by considering that the ionic carrier density is the main source of the sudden increase in the ionic conductivity of this solid electrolyte transition. A trial free-energy density for the equilibrium defects concentrations was considered to reproduce the resistivity change observed in the KI-AgI system at Tt. The electrostatic configuration is explained by introduction of the x-parameter which is related with the activation energy Ui, for promotion an Ag+ ion into an interstitial site, and the attractive pair interaction U (Ui = xU). - Abstract
Hypertrophic cardiomyopathy (HCM) with left ventricular outflow tract obstruction that doesn't improve with pharmacological management often requires septal myectomy. However, there are few centers with experience in the practice of this procedure in our country. We describe the clinical and echocardiographic characteristics and postoperative outcomes of patients with HCM indicated for septal myectomy at a reference center in Colombia., Materials and Methods: Retrospective cohort study. Patients undergoing septal myectomy between 2010 and 2023 were included. Data were collected before and two years after surgery., Results: 18 patients were included. The mean age was 50 years. The predominant functional class was NYHA II/III (94 %). Asymmetric septal variant (83.3 %) was the most frequent as well as obstructive phenotype (88.8 %). After myectomy, 70.5 % improved to NYHA I and 62.4 % had no significant gradient (<30 mmHg), and the most of patient improved SAM. - Abstract
Background: The revolution of orthopedic implant manufacturing is being driven by 3D printing of titanium implants for large bony defects such as those caused by diabetic Charcot arthropathy. Unlike traditional subtractive manufacturing of orthopedic implants, 3D printing fuses titanium powder layer-by-layer, creating a unique surface roughness that could potentially enhance osseointegration. However, the metabolic impairments caused by diabetes, including negative alterations of bone metabolism, can lead to nonunion and decreased osseointegration with traditionally manufactured orthopedic implants. This study aimed to characterize the response of both healthy and diabetic primary human osteoblasts cultured on a medical-grade 3D-printed titanium surface under high and low glucose conditions., Methods: Bone samples were obtained from six patients, three with Type 2 Diabetes Mellitus and three without. Primary osteoblasts were isolated and cultured on 3D-printed titanium discs in high (4.5 g/L D-glucose) and low glucose (1 g/L D-Glucose) media. Cellular morphology, matrix deposition, and mineralization were assessed using scanning electron microscopy and alizarin red staining. Alkaline phosphatase activity and L-lactate concentration was measured in vitro to assess functional osteoblastic activity and cellular metabolism. Osteogenic gene expression of BGLAP , COL1A1 , and BMP7 was analyzed using reverse-transcription quantitative polymerase chain reaction., Results: Diabetic osteoblasts were nonresponsive to variations in glucose levels compared to their healthy counterparts. Alkaline phosphatase activity, L-lactate production, mineral deposition, and osteogenic gene expression remained unchanged in diabetic osteoblasts under both glucose conditions. In contrast, healthy osteoblasts exhibited enhanced functional responsiveness in a high glucose environment and showed a significant increase in osteogenic gene expression of BGLAP , COL1A1 , and BMP7 (p<.05)., Conclusion: Our findings suggest that diabetic osteoblasts exhibit impaired responsiveness to variations in glucose concentrations, emphasizing potential osteoblast dysfunction in diabetes. This could have implications for post-surgery glucose management strategies in patients with diabetes. Subungual melanomas are rare neoplasms that tend to debut as longitudinal melanonychia. They are primarily found in patients over 60 years of age and are usually diagnosed late, representing a diagnostic challenge. We present a case report of a 59-year-old female Hispanic patient who initially presented with melanonychia and was eventually diagnosed with subungual melanoma in situ. She was surgically treated and, after three months, remained healthy. Relevant risk factors, clinical and onychoscopic findings, diagnostic criteria, and treatment options are also discussed. - Abstract
Introduction: Autoimmune anti-NMDAr encephalitis is an antibody-mediated disorder characterized by psychiatric symptoms followed by decreased consciousness, dysautonomia and seizures. The pathophysiology of the disease is related to the internalization of NR1 subtype NMDA receptors and the dysfunction of structures where they are abundant (frontotemporal and insular regions). Some reports suggest the existence of cerebral atrophy in the follow-up of these patients, with conflicting evidence regarding its presence and usefulness as a marker of prognosis., Methods: In a longitudinal, observational study, all patients with the diagnosis of definite anti-NMDAr autoimmune encephalitis with initial and control MRI studies were included. Conventional MR Brain acquisition was performed using a 3-Tesla Skyra MRI System. Automated brain segmental analysis was performed using the Volbrain volumetry system. The differences between baseline MRI volumetric characteristics and volumetric measures at follow-up was assessed., Results: 25 patients were included (mean age 26.6, SD 9.6). 44% were females. The mean time between the studies was 24 (SD 21.4, 3-24) months. Significant volume loss was identified in the total brain volume (- 0.02%, p = 0.029), cerebellar volume (- 0.27%, p = 0.048) and brainstem volume (- 0.16%, p = 0.021)., Conclusions: This study supports previous observations regarding volume loss in several brain regions of patients with antiNMDAr encephalitis. Background: Tuberculosis (TB) continues to be a global public health problem; its meningeal form leads to greater lethality and sequelae, particularly if it is associated with HIV / AIDS infection., Aim: To describe the demographic characteristics, clinical presentation, laboratory and images of patients with meningeal TB (isolation of Mycobacterium tuberculosis in CSF), analyzing differences between HIV and non-HIV patients., Methods: We performed an observational and descriptive study, with retrospective analysis of patients attending at the Dr. Alejandro Posadas Hospital, Buenos Aires, since January 2005 to December 2017., Results: Thirty-six patients were analyzed, with 22 women with a median age of 36.5 years. Twenty two patients had HIV coinfection, all in the AIDS stage. The symptom onset time was median 11 days. The predominant ones were fever, altered consciousness and headache. In the cerebrospinal fluid were lymphocitosis, hypoglycorrhachia, hyperproteinorrhachia and high lactic acid, according to previously described findings. Of 34 patients who underwent brain scan, 16 patients had no significant pathological findings. MRI was performed in 16 patients, 9 had vascular disorders. Brain MRI was more sensitive to identify meningeal reinforcement than computerized tomography, vascular disorders, and granulomatous lesions. The median onset of treatment was 1 day, with 72.2% of the total receiving coadjuvants with corticosteroids. Mortality of 27.7% and sequelae in 36.1% were observed. Only 5 patients required neurosurgical intervention., Conclusion: Since meningeal TB is a disease with high morbidity and mortality, it is imperative to ensure an early diagnosis in its evolution by incorporating molecular biology and imaging (MRI) into broad clinical use.

Background: Management of cervical cancer tumors >2 cm has been a subject of controversy, with management often considered as either up-front radical trachelectomy or neoadjuvant chemotherapy before fertility-sparing surgery., Methods: A systematic literature review was conducted following the Preferred Reporting Items for Systematic reviews and Meta-Analysis (PRISMA) checklist. This study was registered in the International Prospective Register of Systematic Reviews (PROSPERO). We searched Medline through PubMed, EMBASE, Cochrane Central Register of Controlled Trials, SCOPUS, and OVID between January 1985 and December 2020. Eligibility criteria for selecting studies were English language, randomized controlled trials, and observational studies including information on fertility and oncologic outcomes. All titles were managed in EndNote X7. Risk of bias was evaluated using the Joanna Briggs Institute (JBI) critical appraisal checklist for observational studies., Results: Twenty-three studies with 205 patients who received neoadjuvant chemotherapy were included. The majority of patients (92.2%, n=189) had stage IB FIGO 2009 cervical cancer. The preferred regimen used was cisplatin in combination with paclitaxel or ifosfamide (80%, n=164). One hundred and eighty patients (87.8%) underwent fertility-sparing surgery; radical vaginal trachelectomy, abdominal trachelectomy, conization, and simple trachelectomy were performed in 62 (34.4%), 49 (27.2%), 34 (18.9%), and 26 (14.4%) women, respectively. In nine patients (5%) the type of procedure was not specified. The follow-up time reported in all studies ranged between 6 and 69 months. In 112 women who sought a pregnancy after surgery, 84.8% (n=95) achieved a gestation.The global recurrence and death rates were 12.8% and 2.8%, respectively., Conclusion: Neoadjuvant chemotherapy followed by fertility-sparing surgery is a promising strategy that might allow fertility preservation in highly selected patients with cervical cancer with tumors >2 cm while providing acceptable oncologic outcomes. - Abstract
Purpose: Locally advanced cervical cancer may present with uncontrollable vaginal bleeding in up to 70% of cases. Pelvic vessel embolization has been used as an urgent maneuver for achieving fast hemostatic control. This report describes outcomes of selective pelvic vessel embolization in patients with severe bleeding due to a locally advanced cervical cancer., Methods: In this retrospective study, technical aspects, clinical variables, and bleeding-related morbidity were described. The frequency of recurrent disease and the vital status at 1 year of follow-up were determined. Analysis was performed with statistical software R, version 3.6.2. The setting was Instituto Nacional de Cancerología- Bogotá, Colombia, between January 2009 and July 2017., Results: A total of 47 patients were included. Median age was 44 years (range, 26-70 years). The pre-embolization median hemoglobin level was 7.9 g/dL (range, 5.0-11.3 g/dL). Blood transfusions were administered to 41 women (87.2%). Bleeding control was achieved in 95.7% of cases in the first 24 hours after the embolization. There were no major complications. In 17 cases (36.2%), minor complications were reported; the most common was pelvic pain. In 17.1% of cases, a second embolization was required. After 12 months of follow-up, 27.7% of patients were alive without disease, 44.7% were alive with disease, and 25.5% of them have died of cervical cancer progression., Conclusion: Selective pelvic vessel embolization is a useful alternative in patients with locally advanced cervical cancer and life-threatening bleeding. Its impact on recurrent disease and death due to oncologic cause is not clear.

We present the clinical and imaging characteristics of a patient whom presented with rebound syndrome after switching from fingolimod to cladribine treatment due to hematologic toxicity. Previous imaging studies had shown a non-aggressive phenotype of the disease, however multiple active tumefactive lesions became evident after beginning treatment with cladribine. - Abstract
The contribution of Image-guided Radiotherapy (IGRT) to modern radiotherapy is undeniable, being the way to bring into daily practice the dosimetric benefits of Intensity-Modulated Radiotherapy (IMRT). Organ and target motion is constant and unpredictable at the pelvis, thus posing a challenge to the safe execution of IMRT. There are potential benefits of IMRT in the radical treatment of cervical cancer patients, both in terms of dose escalation and decrease of toxicity. But it is essential to find IGRT solutions to control the aspects that can lead to geographic miss targeting or organs at risk (OAR) overdose. This review seeks to describe the problems and possible solutions in the clinical implementation of IMRT/IGRT protocols to treat intact cervical cancer patients.

Background and Objective: The prevalence and magnitude of obesity in children and adolescents increase rapidly. Besides genetic and environmental factors, calcium intake has recently been identified as a dietary factor that is inversely related with body mass index and development of overweight and obesity. The purpose of this study was to assess the correlation between dietary calcium intake and body mass index and fat distribution in female adolescents., Materials and Methods: This was a cross-sectional study where anthropometric variables (weight, height, body mass index, waist and hip circumference) were collected in 244 female adolescents to establish total body adiposity and fat distribution. A 24-hour recall and a food frequency questionnaire were used to assess total calorie, calcium, and dairy products intake., Results: Calcium intake was inversely related to body mass index (P<.05), waist circumference(P<.05), hip circumference (P>.05), and waist to hip ratio (P<.05).Overweight (8.3%) and obese (0.7%) adolescents had a lower mean calcium intake than adolescents of normal weight (P=.06)., Conclusions: Dietary calcium intake and, to a lesser extent, consumption of dairy products are inversely related to total and abdominal adiposity, and also to the prevalence of overweight in this group of adolescents., (Copyright © 2015 SEEN. Introduction: metabolic syndrome is a group of metabolic abnormalities that increase the risk of developing type 2 diabetes and cardiovascular disease. There are several criteria for diagnosis; the most used are the International Diabetes Federation (IDF) and the Adult Treatment Panel III (ATP III). An early treatment may prevent its associated complications and improve the quality and life expectancy of the person; it must be focused on changes in lifestyle such as healthy eating and regular physical activity., Objective: to determine the prevalence of metabolic syndrome according to the diagnostic criteria of the International Diabetes Federation (IDF) and the Adult Treatment Panel III (ATP III) Methodology: it was a cross-sectional study in which participate 128 people (55 women and 73 men) aged between 18 and 75 years, who were given a nutritional history, anthropometric measures and biochemical tests., Results: the prevalence of overweight was 63% (48% overweight and 15% obese). The prevalence of metabolic syndrome according to IDF was 15.6% and 14.8% according ATPIII., Conclusions: among the employees is clear the high prevalence of overweight and metabolic syndrome. - Abstract
Standard microplate based enzyme-linked immunosorbent assays (ELISA) are widely utilized for various nanomedicine, molecular sensing, and disease screening applications, and this multiwell plate batched analysis dramatically reduces diagnosis costs per patient compared to nonbatched or nonstandard tests. However, their use in resource-limited and field-settings is inhibited by the necessity for relatively large and expensive readout instruments. To mitigate this problem, we created a hand-held and cost-effective cellphone-based colorimetric microplate reader, which uses a 3D-printed opto-mechanical attachment to hold and illuminate a 96-well plate using a light-emitting-diode (LED) array. This LED light is transmitted through each well, and is then collected via 96 individual optical fibers. Captured images of this fiber-bundle are transmitted to our servers through a custom-designed app for processing using a machine learning algorithm, yielding diagnostic results, which are delivered to the user within ∼1 min per 96-well plate, and are visualized using the same app. We successfully tested this mobile platform in a clinical microbiology laboratory using FDA-approved mumps IgG, measles IgG, and herpes simplex virus IgG (HSV-1 and HSV-2) ELISA tests using a total of 567 and 571 patient samples for training and blind testing, respectively, and achieved an accuracy of 99.6%, 98.6%, 99.4%, and 99.4% for mumps, measles, HSV-1, and HSV-2 tests, respectively. This cost-effective and hand-held platform could assist health-care professionals to perform high-throughput disease screening or tracking of vaccination campaigns at the point-of-care, even in resource-poor and field-settings. Also, its intrinsic wireless connectivity can serve epidemiological studies, generating spatiotemporal maps of disease prevalence and immunity.

- Abstract
Rapid and sensitive detection of waterborne pathogens in drinkable and recreational water sources is crucial for treating and preventing the spread of water related diseases, especially in resource-limited settings. Here we present a field-portable and cost-effective platform for detection and quantification of Giardia lamblia cysts, one of the most common waterborne parasites, which has a thick cell wall that makes it resistant to most water disinfection techniques including chlorination. The platform consists of a smartphone coupled with an opto-mechanical attachment weighing ~205 g, which utilizes a hand-held fluorescence microscope design aligned with the camera unit of the smartphone to image custom-designed disposable water sample cassettes. Each sample cassette is composed of absorbent pads and mechanical filter membranes; a membrane with 8 μm pore size is used as a porous spacing layer to prevent the backflow of particles to the upper membrane, while the top membrane with 5 μm pore size is used to capture the individual Giardia cysts that are fluorescently labeled. A fluorescence image of the filter surface (field-of-view: ~0.8 cm(2)) is captured and wirelessly transmitted via the mobile-phone to our servers for rapid processing using a machine learning algorithm that is trained on statistical features of Giardia cysts to automatically detect and count the cysts captured on the membrane. The results are then transmitted back to the mobile-phone in less than 2 minutes and are displayed through a smart application running on the phone. This mobile platform, along with our custom-developed sample preparation protocol, enables analysis of large volumes of water (e.g., 10-20 mL) for automated detection and enumeration of Giardia cysts in ~1 hour, including all the steps of sample preparation and analysis. We evaluated the performance of this approach using flow-cytometer-enumerated Giardia-contaminated water samples, demonstrating an average cyst capture efficiency of ~79% on our filter membrane along with a machine learning based cyst counting sensitivity of ~84%, yielding a limit-of-detection of ~12 cysts per 10 mL. Providing rapid detection and quantification of microorganisms, this field-portable imaging and sensing platform running on a mobile-phone could be useful for water quality monitoring in field and resource-limited settings.

Abstract: The phenotypic structure within and between plant populations is generally influenced by their distribution patterns in space and time; therefore, the study of their divergence is a central issue for the understanding of their microevolutive processes. We boarded the hypothesis that three populations of Espeletia pycnophylla show phenotypic divergence as one of the possible implications of their geographic isolation in the Southern Colombian Andes. We used the Elliptic Fourier Descriptors (leaf shape) and traditional leaf morphometry (leaf size) of 347 leaves to measure inter and intra-population variation and a comparison between a paleogeographic reconstruction with an actual estimate of the distribution areas of E. pycnophylla in order to identify their main changes during the last 14 000 years. The three populations showed significant differences in leaf morphometry and a positive correlation between the matrices of morphometric and geographic dissimilarities, indicating that the inter-population divergence increases between further populations, so that the morphometric structure reflects their spatial distribution. The geographical and paleogeographical estimates evidenced a conspicuous process of reduction and fragmentation of the distribution area of E. pycnophylla since the Late-Glacial until the Holocene. We suggest that these results support possible scenarios of vicariance events, which allow us to approach the divergence of these populations in terms of their historic biogeographic relations. However, genetic analyses are still needed to support these results.

Objectives: To determine the prevalence of overweight and identify its risk factors in school girls between the ages of 12 and 19 in a semi-urban region of Ecuador and to explore the potential relationship between the adolescents' body mass index (BMI) and their physical activity, caloric intake, and macronutrient intake, as well as between their BMI and their parents' estimated BMI., Methods: From January through April 2001 a cross-sectional study was conducted with a sample of 302 adolescent girls who attended high schools in Cumbaya and Tumbaco (14 km northeast of Quito) and who were selected by systematic random sampling. Anthropometric measurements were taken on all girls and their total calorie and macronutrient intake was measured by means of a 24-hour recall questionnaire. A physical activity questionnaire was also administered, and estimates were made of their parents' BMI., Results: Of the study participants, 8.3% were overweight and 0.7% were obese. In 40.7% of the first group, the excess body weight had a gluteofemoral distribution; in 51.9%, it had an intermediate type of distribution; and in 3.7% it had an abdominal distribution. There was a negative correlation between the adolescents' BMI and their physical activity and calorie intake (P < 0.05), whereas a positive correlation was seen between their BMI and the mother's estimated BMI (P < 0.05)., Conclusions: The prevalence of overweight found in this study is similar to that found in an earlier country-wide study but was less than that detected in reports from industrialized countries and other countries of Latin America. Factors appearing to have the greatest impact on an adolescent girl's BMI were physical activity and estimated maternal BMI, regardless of total energy intake and dietary composition.
