Abstract

Background: In a 2013 survey, we reported distinct discrepancies in delivery of tuberculosis (TB) and HIV services in eastern Europe (EE) vs. western Europe (WE). Objectives: To verify the differences in TB and HIV services in EE vs. WE. Methods: Twenty-three sites completed a survey in 2018 (EE, 14; WE, nine; 88% response rate). Results were compared across as well as within the two regions. When possible, results were compared with the 2013 survey. Results: Delivery of healthcare was significantly less integrated in EE: provision of TB and HIV services at one site (36% in EE vs. 89% in WE; P = 0.034), and continued TB follow-up in one location (42% vs. 100%; P = 0.007). Although access to TB diagnostics, standard TB and HIV drugs was generally good, fewer sites in EE reported unlimited access to rifabutin/multi-drug-resistant TB (MDR-TB) drugs, HIV integrase inhibitors and opioid substitution therapy (OST). Compared with 2013, routine usage of GeneXpert was more common in EE in 2018 (54% vs. 92%; P = 0.073), as was access to moxifloxacin (46% vs. 91%; P = 0.033), linezolid (31% vs. 64%; P = 0.217), and bedaquiline (0% vs. 25%; P = 0.217). Integration of TB and HIV services (46% vs. 39%; P = 1.000) and provision of OST to patients with opioid dependency (54% vs. 46%; P = 0.695) remained unchanged. Conclusion: Delivery of TB and HIV healthcare, including integration of TB and HIV care and access to MDR-TB drugs, still differs between WE and EE, as well as between individual EE sites.

Abstract

Background: Early diagnosis of tuberculosis (TB) is important to reduce transmission, morbidity and mortality in people living with HIV (PLWH). Methods: PLWH with a diagnosis of TB were enrolled from HIV and TB clinics in Eastern Europe and followed until 24 months. Delayed diagnosis was defined as duration of TB symptoms (cough, weight-loss or fever) for ≥ 1 month before TB diagnosis. Risk factors for delayed TB diagnosis were assessed using multivariable logistic regression. The effect of delayed diagnosis on mortality was assessed using Kaplan–Meier estimates and Cox models. Findings: 480/740 patients (64.9%; 95% CI 61.3–68.3%) experienced a delayed diagnosis. Age ≥ 50 years (vs. < 50 years, aOR = 2.51; 1.18–5.32; p = 0.016), injecting drug use (IDU) (vs. non-IDU aOR = 1.66; 1.21–2.29; p = 0.002), being ART naïve (aOR = 1.77; 1.24–2.54; p = 0.002), disseminated TB (vs. pulmonary TB, aOR = 1.56, 1.10–2.19, p = 0.012), and presenting with weight loss (vs. no weight loss, aOR = 1.63; 1.18–2.24; p = 0.003) were associated with delayed diagnosis. PLWH with a delayed diagnosis were at 36% increased risk of death (hazard ratio = 1.36; 1.04–1.77; p = 0.023, adjusted hazard ratio 1.27; 0.95–1.70; p = 0.103). Conclusion: Nearly two thirds of PLWH with TB in Eastern Europe had a delayed TB diagnosis, in particular those of older age, people who inject drugs, ART naïve, with disseminated disease, and presenting with weight loss. Patients with delayed TB diagnosis were subsequently at higher risk of death in unadjusted analysis. There is a need for optimisation of the current TB diagnostic cascade and HIV care in PLWH in Eastern Europe.

Abstract

Background: In a 2013 survey, we reported distinct discrepancies in delivery of tuberculosis (TB) and HIV services in eastern Europe (EE) vs. western Europe (WE). Objectives: To verify the differences in TB and HIV services in EE vs. WE. Methods: Twenty-three sites completed a survey in 2018 (EE, 14; WE, nine; 88% response rate). Results were compared across as well as within the two regions. When possible, results were compared with the 2013 survey. Results: Delivery of healthcare was significantly less integrated in EE: provision of TB and HIV services at one site (36% in EE vs. 89% in WE; P = 0.034), and continued TB follow-up in one location (42% vs. 100%; P = 0.007). Although access to TB diagnostics, standard TB and HIV drugs was generally good, fewer sites in EE reported unlimited access to rifabutin/multi-drug-resistant TB (MDR-TB) drugs, HIV integrase inhibitors and opioid substitution therapy (OST). Compared with 2013, routine usage of GeneXpert was more common in EE in 2018 (54% vs. 92%; P = 0.073), as was access to moxifloxacin (46% vs. 91%; P = 0.033), linezolid (31% vs. 64%; P = 0.217), and bedaquiline (0% vs. 25%; P = 0.217). Integration of TB and HIV services (46% vs. 39%; P = 1.000) and provision of OST to patients with opioid dependency (54% vs. 46%; P = 0.695) remained unchanged. Conclusion: Delivery of TB and HIV healthcare, including integration of TB and HIV care and access to MDR-TB drugs, still differs between WE and EE, as well as between individual EE sites.

Background: Early diagnosis of tuberculosis (TB) is important to reduce transmission, morbidity and mortality in people living with HIV (PLWH). Methods: PLWH with a diagnosis of TB were enrolled from HIV and TB clinics in Eastern Europe and followed until 24 months. Delayed diagnosis was defined as duration of TB symptoms (cough, weight-loss or fever) for ≥ 1 month before TB diagnosis. Risk factors for delayed TB diagnosis were assessed using multivariable logistic regression. The effect of delayed diagnosis on mortality was assessed using Kaplan–Meier estimates and Cox models. Findings: 480/740 patients (64.9%; 95% CI 61.3–68.3%) experienced a delayed diagnosis. Age ≥ 50 years (vs. < 50 years, aOR = 2.51; 1.18–5.32; p = 0.016), injecting drug use (IDU) (vs. non-IDU aOR = 1.66; 1.21–2.29; p = 0.002), being ART naïve (aOR = 1.77; 1.24–2.54; p = 0.002), disseminated TB (vs. pulmonary TB, aOR = 1.56, 1.10–2.19, p = 0.012), and presenting with weight loss (vs. no weight loss, aOR = 1.63; 1.18–2.24; p = 0.003) were associated with delayed diagnosis. PLWH with a delayed diagnosis were at 36% increased risk of death (hazard ratio = 1.36; 1.04–1.77; p = 0.023, adjusted hazard ratio 1.27; 0.95–1.70; p = 0.103). Conclusion: Nearly two thirds of PLWH with TB in Eastern Europe had a delayed TB diagnosis, in particular those of older age, people who inject drugs, ART naïve, with disseminated disease, and presenting with weight loss. Patients with delayed TB diagnosis were subsequently at higher risk of death in unadjusted analysis. There is a need for optimisation of the current TB diagnostic cascade and HIV care in PLWH in Eastern Europe.

Abstract

Objectives: The aim of the study was to investigate the organization and delivery of HIV and tuberculosis (TB) health care and to analyse potential differences between treatment centres in Eastern (EE) and Western Europe (WE). Methods: Thirty-eight European HIV and TB treatment centres participating in the TB:HIV study within EuroCoord completed a survey on health care management for coinfected patients in 2013 (EE: 17 respondents; WE:21; 76% of all TB:HIV centres). Descriptive statistics were obtained for regional comparisons. The reported data on health care strategies were compared with actual clinical practice at patient level via data derived from the TB:HIV study. Compared with WE, treatment of HIV and TB in EE are less often located at the same site (47% in EE versus 100% in WE; P<0.001) and less often provided by the same doctors (41% versus 90%, respectively; P=0.002), whereas regular screening of HIV-infected patients for TB (80% versus 40%, respectively; P=0.037) and directly observed treatment (88% versus 20%, respectively; P<0.001) were more common in EE. The reported availability of rifabutin and second- and third-line anti-TB drugs was lower, and opioid substitution therapy (OST) was available at fewer centres in EE compared with WE (53% versus 100%, respectively; P<0.001). Conclusions: Major differences exist between EE and WE in relation to the organization and delivery of health care for HIV/TB-coinfected patients and the availability of anti-TB drugs and OST. Significant discrepancies between reported and actual clinical practices were found in EE.

Abstract

Objectives Mortality among HIV patients with tuberculosis (TB) remains high in Eastern Europe (EE), but details of TB and HIV management remain scarce. Methods In this prospective study, we describe the TB treatment regimens of patients with multi-drug resistant (MDR) TB and use of antiretroviral therapy (ART). Results A total of 105 HIV-positive patients had MDR-TB (including 33 with extensive drug resistance) and 130 pan-susceptible TB. Adequate initial TB treatment was provided for 8% of patients with MDR-TB compared with 80% of those with pan-susceptible TB. By twelve months, an estimated 57.3% (95%CI 41.5–74.1) of MDR-TB patients had started adequate treatment. While 67% received ART, HIV-RNA suppression was demonstrated in only 23%. Conclusions Our results show that internationally recommended MDR-TB treatment regimens were infrequently used and that ART use and viral suppression was well below the target of 90%, reflecting the challenging patient population and the environment in which health care is provided. Urgent improvement of management of patients with TB/HIV in EE, in particular for those with MDR-TB, is needed and includes widespread access to rapid TB diagnostics, better access to and use of second-line TB drugs, timely ART initiation with viral load monitoring, and integration of TB/HIV care.

Abstract

Electroporation is used to induce homologous recombination in the genome of the murine ES(embryonic stem) cells. Routinelly subconfluent ES cells are recommended to be used in suchexperiments. Electroporation of immunoglobulin specific targeting vectors with different length ofhomology leads to reduced number of selected colonies. The enrichment of double selected colonies ishigh and thus the amount of HM-1 ES cell colonies for the analysis is very low. Here we show that theelectroporation of confluent HM-1 ES cells leads to an increased amount of simple and doubleselected colonies.

Abstract

Management of tuberculosis in patients with HIV in eastern Europe is complicated by the high prevalence of multidrug-resistant tuberculosis, low rates of drug susceptibility testing, and poor access to antiretroviral therapy (ART). We report 1 year mortality estimates from a multiregional (eastern Europe, western Europe, and Latin America) prospective cohort study: the TB:HIV study.Consecutive HIV-positive patients aged 16 years or older with a diagnosis of tuberculosis between Jan 1, 2011, and Dec 31, 2013, were enrolled from 62 HIV and tuberculosis clinics in 19 countries in eastern Europe, western Europe, and Latin America. The primary endpoint was death within 12 months after starting tuberculosis treatment; all deaths were classified according to whether or not they were tuberculosis related. Follow-up was either until death, the final visit, or 12 months after baseline, whichever occurred first. Risk factors for all-cause and tuberculosis-related deaths were assessed using Kaplan-Meier estimates and Cox models.Of 1406 patients (834 in eastern Europe, 317 in western Europe, and 255 in Latin America), 264 (19%) died within 12 months. 188 (71%) of these deaths were tuberculosis related. The probability of all-cause death was 29% (95% CI 26-32) in eastern Europe, 4% (3-7) in western Europe, and 11% (8-16) in Latin America (p0·0001) and the corresponding probabilities of tuberculosis-related death were 23% (20-26), 1% (0-3), and 4% (2-8), respectively (p0·0001). Patients receiving care outside eastern Europe had a 77% decreased risk of death: adjusted hazard ratio (aHR) 0·23 (95% CI 0·16-0·31). In eastern Europe, compared with patients who started a regimen with at least three active antituberculosis drugs, those who started fewer than three active antituberculosis drugs were at a higher risk of tuberculosis-related death (aHR 3·17; 95% CI 1·83-5·49) as were those who did not have baseline drug-susceptibility tests (2·24; 1·31-3·83). Other prognostic factors for increased tuberculosis-related mortality were disseminated tuberculosis and a low CD4 cell count. 18% of patients were receiving ART at tuberculosis diagnosis in eastern Europe compared with 44% in western Europe and 39% in Latin America (p0·0001); 12 months later the proportions were 67% in eastern Europe, 92% in western Europe, and 85% in Latin America (p0·0001).Patients with HIV and tuberculosis in eastern Europe have a risk of death nearly four-times higher than that in patients from western Europe and Latin America. This increased mortality rate is associated with modifiable risk factors such as lack of drug susceptibility testing and suboptimal initial antituberculosis treatment in settings with a high prevalence of drug resistance. Abstract

Routine generation of chimeras relies on the production of large numbers ofblastocysts in a constant and reproducible manner and on the quality of the targetedES (embryonal stem) cells used for microinjection. We have focussed this work onthe production of large numbers of blastocysts by superovulation, in standardizedconditions. The efficiency of superovulation in mice is affected by several factors.These factors depend on the animal, on the environment and on the operator. Theeffect of seasonal variation as environmental impact was assessed. For this purpose360 prepubescent C57BL6/NCrl females were superovulated with 5 IU of PMSG(Pregnant Mare’s Serum Gonadotropin) and 5 IU hCG (Human ChorionicGonadotropin) and mated with fertile C57BL/6NCrl males. The distribution of theexperiments over the seasons was as following: 8 experiments were performed inspring, 12 in summer, 12 in autumn and 4 in winter. The influence of the seasonalvariation on the efficiency of superovulation in two age categories (23 day old and26 day old females ) was compared. The results show that the overall vaginal plugpositive rate (VP+%) of the 23 day old females (90%) is higher in comparison tothat of the 26 day old females (86,4%), whereas the blastocyst rate (B%) of 26 dayold females (45,4%) is higher compared to the 23 day old females (41%). Theinfluence of the seasonal variation on the yield of embryos and the rate ofblastocysts is relatively constant in each season. These results suggest that housingof the mice in our artificial conditions and the hormonal induction of superovulationlead to a reproducible number of blastocysts over the seasonal variation of the year.

27. The use of harmonic scalpel in head and neck surgery: a report on 161 patient

Abstract

The harmonic scalpel cuts and coagulates simultaneously using a mechanical vibration. The present article reports our experience in the use of this instrument. Over a period of 5 months, we performed 123 thyroidectomies, 13 parotidectomies and 25 tonsillectomies using the harmonic scalpel. 106 thyroidectomies, 9 parotidectomies and 30 tonsillectomies were performed with the use of conventional hemostatic techniques (electrocautery and knot tying). The use of harmonic scalpel shortened the duration of thyroid and parotid surgery by 20-30% as compared with the conventional techniques. The average incision length for those undergoing thyroidectomy with the harmonic scalpel was 1.5 cm shorter compared with the conventional thyroidectomy group. The use of harmonic scalpel shortened the average duration of tonsillectomy by 5.5 minutes. Postoperative pain and complication rate were comparable for both groups. The use of the harmonic scalpel reduces surgical time and intraoperative blood loss, and enables shorter incision length in thyroid surgery.

Abstract

Introduction: Inverted papilloma (IP) is benign tumor of nose and paranasal sinuses. Squamous cell carcinoma (SCC) appears synchronously or metachronously to IP. Authors report 3-32%of IPs to be associated with SCC. It's obvious that identification potential markers for malignant alteration of IP would be of great importance.p53 is well-known tumor suppressor gene. Its product have important role in cell cycle control and apoptosis. Mutations of p53 gene are documented in nearly 50% of all human cancers. Ki67 is one of the most prominent proliferation markers showing influence on biologic behavior or being associated with other prognostic factors in different tumors. Material and Methods: We analyzed 58 specimen of IP including 8 cases of synchronous and 2 of metachronous SCC. Patients had diagnosed IP in period from 1982-2001 at Departments of Pathology in Zagreb , Osijek and Budapest . Immunohistochemistry was performed on formalin fixed, paraffin embedded tissue, using DAKO antibodies and LSAB+ staining kit. PC based image analyzer (VAMSTECH, Zagreb ) was used to determine the rate of nuclear staining. Statistical analysis was performed. Results: p53:16 specimen stained positive on immunohistochemistry. Among all cases of SCC arising in IP, except one as well as one of two recurrences of another carcinoma. IP preceding SCC stained weakly positive in multiple areas, contrasting negative staining of overt IP. Fisher’ s exact test showed prognostic significance for malignant transformation (p=0.008).It supports role of p53 mutations in development of head and neck cancer, including SCC arising in IP.Ki67: Immunostaining was positive in all cases except one. Expression rate ranged from 0.03 in papilloma to 0.7 in carcinoma. Statistical analysis using Kruskal Wallis test showed significant difference between 3 groups (p=0.008). Wilcoxon’ s test showed significantly higher values in the group of malignant than in the benign tumors. Conclusions: Our results support the potentially important prognostic role of p53 and Ki67 staining in inverted sinonasal papilloma.

29. Role of SHH/PTCH/SMO signalling pathway in pathognesis of malignant schwanoma

Abstract

Introduction: Schwanoma is tumor which derives from schwann cells, neuroglia that wrap around axons in nervous system forming the myelin sheath. Schwannoma is relatively rare tumor, it can derive from any cranial or peripheral nerve, but mostly they came from n. statoacusticus. The genetic basis of the schwanoma tumor is not known. During development Schwann cells secrete ligand Desert hedgehog, which bind to its receptor and can be detected in the mesenchyme. Desert Hedgehog and Sonic Hedgehog are ligands for glycoprotein Patched (Ptch), receptor which with cooreceptor Smo (Smoothened) transmit signal into nucleus. Ptch is located in membrane, has 12 transmembrane domains and two big extracellular loops between which ligand Hedgehog is placed. Ptch is member of the Hedgehog/Patched/Smoothened signalling pathway (or SHH/PTCH/SMO signalling pathway). We think that mutations in Patched might disturb signal transduction, which can contribute to development of schwanoma. Material and Methods: We report findings in one family with malignant schwanoma. At least two family members are affected. Results: We found allelic loss in 9q22.3 region (where PTCH is mapped) for three polymorphic markers D9S180, D9S287 and PTCH intra of one affected family member. Two of markers are in close vicinity of PTCH gene and intragenic marker PTCH intra is located in first intron of PTCH. Although we find allelic loss we didn't find mutation in coding regions of PTCH. In our next step we shall look for mutations in promoter region of Ptch and in expression profiles of all members of the pathway. Conclusions: We intend to answer if the loss of function of PTCH gene indicated through allelic loss can be confirmed either through mutations in promoter of PTCH or through impaired expression any of members of the SHH/PTCH/SMO pathway, or caused by innapropriate methylation of promoter regions.

30. Cervical approach for substernal goiter

Abstract

Introduction: In substernal goiter at least 50 % of the gland lies inferior to the thoracic inlet. It usually occurs in fifth and sixth decade, with incidence three to four times greater in woman than in man. According to major vessels and recurrent laryngeal nerve, substernal goiter can be anterior and posterior. Substernal goiter is best diagnosed by radiological studies, ultrasound, cytological and radionuclide scintigraphic methods. Due anatomy and location behind sternum, ultrasound, cytological and scintigraphic methods are not always reliable. Computerized tomography (CT) or magnetic resonance imaging is useful for evaluation of goiter substernal extension and tracheal dislocation and compression. Surgical procedure is more difficult because of possible dislocation of anatomic structures, difficult identification of recurrent laryngeal nerve and parathyroid glands, intraoperative bleeding. Majority of substernal goiters can be removed through cervical incision, but occasionally sternotomy is necessary which prolongs postoperative recovery. Material and Methods: We will demonstrate our experience and results in surgical treatment 96 cases of substernal goiter. Results: In presentation of substernal goiter, majority had cervical mass (95.8 %), respiratory symptoms (91.6%), dysphagia (48.9 %), acute airway (11.4 %), and some of them were asymptomatic (12.5 %). Indication for surgery were pressure effects in longstanding goiter, potential for acute airway problems, potential for malignancy, unsuccessful medical treatment, and minimal operative morbidity and mortality. All substernal goiter were remover via cervical incision. There were no cases of operative deaths, vocal cord paralysis and in 3.1 % were detected hypocalcaemia. Conclusions: Transcervical approach for substernal goiter is partially different from that used for routine thyroid surgery and need experienced surgeon. The majority of the substernal goiters can be resected transcervicaly, but rarely will require a sternal split.

31. Eosinophilic granuloma of the thyroid gland: Case report

Abstract

Introduction: Eosinophilic granuloma is pathological and clinical entity characterized by abnormal proliferation of Langerhan's cells. Usually, it is mild and localized disease which most commonly occurs within bone tissue of children aged 5 to 10. It makes up 60-80% of all cases of Langerhan's cell histiocytosis. Hand-Schüller-Christian disease represents the intermediate, more agressive form characterized by multifocal, chronic involvement and Letterer-Siwe disease is the fulminant disseminated type. Here we report unusual presence of eosinophilic granuloma within thyroid gland tissue. Material and Methods: M.F., 47, was hospitalized because of large multinodal euthyroic goiter. Lobectomy of right thyroid lobe was performed, followed by histopathological analysis. Results: 20 histopathologically benign nodes which differed only in diameter were found. Surprisingly, among them, one 2 cm large node appeared different: it was gelatinous, dark red with numerous small white flecks within it. Histopathology reported predomination of histiocytes with large nuclei. Diagnosis was confirmed by immunohistochemical staining for S-100 protein, which was clearly positive. Additionally, intracellular Birbeck granules were also demonstrated by electron microscopy. Conclusions: This work reports very unusual location of eosinophilic granuloma within thyroid gland together with atypical age and sex of the patient. Thus, it adds new information to current knowledge about this disease and could help to understand its still mysterious pathogenesis.

Abstract

Among studies investigating the expression of DNa content in laryngeal SCC there are conflicting results concerning the potentuial prognostic utility of DNA parameters. Intratumoral heterogeneity in macroscopically different tumor areas has previously been demostrated for DNA content of laryngeal SCC. Also our previous investigation demonstrated heterogeneity for AgNOR in different microscopically defined tumor decided to investigate DNa parameters in different microscopically defined tumor areas of SCCD of the larynx. Thirty-one total laryngectomy specimens that were formalin fixed and paraffin embedded have been investigated. Three distinct areas were analyzed: proliferative and invasive tumor margin and tumor center. Speciments were dewaxed at room temperature, then incubated for 90 min in pepsin at 37 oC. Nuclei ewre stained with propidium iodide. Analysis was performed using a FACS calibur flow cytometer( becton Dickinson), using an excitation wavelenght of 488nm and a 15mW argon ion laser ; 20 000 thousand nuclei were counted for each sample.We performed DNA histogram analysis using ModFitLT softwere. DNa ploidy, DNA index, and S-phase fraction werw determined. A significant difference was observed between the inveswtigated atreas concerning the percentage of aneuploid samples( centra:proliferative margin:invasive margin=78:60:40) as well as S phase fraction( p=0.03). Our results, together with our previous data for AgNOR, strongly underline the fact that intratumoral heterogeneity is not to be neglected. The prognostic potential of this approach has yet to be elucidated.

38. Immunohistochemical Analysis and Prognostic Value of Cathepsin D Determination in Laryngeal Squamous Cell Carcinoma

Abstract

Cathepsin D, a protease with the capability of degrading matrix proteins, is implicated in the process of breast and colorectal cancer invasion and metastasis. Biochemical studies in laryngeal cancer have shown a potential prognostic significance of cathepsin D content determination. We studied immunohistochemical positivity of cathepsin D in tumor epithelium and stroma of 61 surgical specimens of squamous cell laryngeal cancer. Immunohistochemical reaction was quantitatively assessed using a PC-based image analysis system SFORM-VAMS. The results were correlated to clinical and morphological parameters and survival. Immunohistochemical positivity was noted in neoplastic cells and tumor stroma. Significant prognostic value for cathepsin D was established separately for epithelial tumor component and tumor stroma using log-rank test, the Cox proportional hazards regression model, and C4.5 machine learning system. In all groups, patients above the median cathepsin D staining showed significantly shorter survival time. C4.5 machine learning system extracted cutoff values for the decision tree that defines the probabilities of patients survival and death with high sensitivity (92.8% alive, 73.6% dead), 100% specificity, and 86.9% accuracy. This makes immunohistochemical cathepsin D estimation an independent prognostic parameter in laryngeal carcinomas within a 5-year period from the time of tumor surgery.

Abstract

Lecture-based teaching is still the prevalent form of teaching in almost all secondary schools in Serbia. Although active teaching cannot eliminate all the shortcomings in the teaching of Geography, this particular form gives the best opportunities for understanding individual abilities and preferences of students. The principal aim of the study was to explore which way of teaching (modern-active or traditional-passive) is more beneficial for motivating students' learning and acquiring competencies (knowledge, skills, and abilities) in Geography. Research was conducted to analyse competencies acquired by selected high school students through the application of certain didactic principles in the third grade of Grammar school 'Laza Kostić' (Novi Sad, Serbia) with the topic 'Hydrography of Serbia'. Fieldwork investigation was comprised of conducted survey questionnaire (with pupils evaluating motivation and participation in the process of teaching Geography, as well as activities and learning/acquiring certain geographical content through selected didactic principles, forms, methods, and tools) and the competency test. Descriptive statistics was used for calculating the percentage and average values. The independent t-test was used in order to determine whether there is a statistically significant difference between the experimental and control group regarding the self-evaluation of motivation, participation in teaching, and learning. The difference in terms of acquiring competencies between experimental and control groups was explored by the Chi-square test (χ2). In order to determine whether there is a correlation between self-evaluation in terms of motivation, cooperation, and learning, as well as in the knowledge test between the experimental and control groups, Pearson's correlation coefficient (r) has been calculated. The results indicate that the students have higher level of motivation, and are more interested in learning and acquiring knowledge when the teacher organises the lecture through active learning techniques. The knowledge and skills resulting from these kinds of classes are higher. Also, the results obtained from this type of experimental classes, showed that with an increase of skills and abil experiities, there is no increase in knowledge implying that modern (active) teaching presented better results than the traditional teaching format, but not in all analysed segments.

Abstract

Abstract Background Copper (Cu), zinc (Zn), cadmium (Cd), and lead (Pb) can pose serious threats to environmental health because they tend to bioaccumulate in terrestrial ecosystems. We investigated under field conditions the transfer of these heavy metals in a soil-plant-snail food chain in Banat area, Romania. The main goal of this paper was to assess the Roman snail (Helix pomatia) usefulness in environmental monitoring as bioindicator of heavy metal accumulation. Eight sampling sites, selected by different history of heavy metal (HM) exposure, were chosen to be sampled for soil, nettle leaves, and newly matured snails. This study also aimed to identify the putative effects of HM accumulation in the environment on phenotypic variability in selected shell features, which included shell height (SH), relative shell height (RSH), and whorl number (WN). Results Significantly higher amounts of HMs were accumulated in snail hepatopancreas and not in foot. Cu, Zn, and Cd have biomagnified in the snail body, particularly in the hepatopancreas. In contrast, Pb decreased when going up into the food chain. Zn, Cd, and Pb correlated highly with each other at all levels of the investigated food chain. Zn and Pb exhibited an effective soil–plant transfer, whereas in the snail body only foot Cu concentration was correlated with that in soil. There were significant differences among sampling sites for WN, SH, and RSH when compared with reference snails. WN was strongly correlated with Cd and Pb concentrations in nettle leaves but not with Cu and Zn. SH was independent of HM concentrations in soil, snail hepatopancreas, and foot. However, SH correlated negatively with nettle leaves concentrations for each HM except Cu. In contrast, RSH correlated significantly only with Pb concentration in hepatopancreas. Conclusions The snail hepatopancreas accumulates high amounts of HMs, and therefore, this organ can function as a reliable biomarker for tracking HM bioavailability in soil. Long-term exposure to HMs via contaminated food might influence the variability of shell traits in snail populations. Therefore, our results highlight the Roman snail (Helix pomatia) potential to be used in environmental monitoring studies as bioindicator of HM pollution.

Abstract

The first enantioselective [12 + 2] cycloaddition has been developed for the construction of a chiral cycl[3.2.2]azine core, a tricyclic moiety with a central ring-junction nitrogen atom, by an operationally simple one-step organocatalytic process. The reaction concept builds upon aminocatalytically generated 12π-components derived from 5H-benzo[a]pyrrolizine-3-carbaldehydes reacting with different electron-deficient 2π-components and affording the complex scaffold of benzo[a]cycl[3.2.2]azine (indolizino[3,4,5-ab]isoindole) with excellent enantio- A nd diastereoselectivity in good yields. The developed reaction is robust toward diverse substituent patterns and has been extended to different classes of electron-deficient 2π-components by minor variations in reaction setup. The obtained [12 + 2] cycloadducts are electron-deficient in nature, and their reaction with nucleophiles have been demonstrated. The enantioselective [12 + 2] cycloaddition with α,β-unsaturated aldehydes as the electron-deficient 2π-components relies upon an unconventional, simple aminocatalyst. In order to understand the high stereoselectivity of the [12 + 2] cycloaddition for this simple catalyst, combined experimental and computational investigations were performed. The investigations point to activation of both the 5H-benzo[a]pyrrolizine-3-carbaldehyde and the α,β-unsaturated aldehyde by the aminocatalyst and that the reaction proceeds by a stepwise cycloaddition, where especially the ring-closure is crucial for the stereochemical outcome. For other electron-deficient 2π-components, such as α,β-unsaturated ketoesters and nitroolefins, a more sterically demanding aminocatalyst has been applied and the corresponding [12 + 2] cycloadducts are obtained with excellent stereoselectivity. The [12 + 2] cycloaddition with vinyl sulfones afforded fully unsaturated systems, which display photoluminescence properties and for which quantum yields have been evaluated.
