15. Validity and reliability of the Turkish version of the ALBA screening instrument for Lewy body dementia in older adults.

Abstract

ALBA screening instrument (ASI) has been demonstrated to be an effective, cheap, and noninvasive clinical instrument to screen for Lewy body dementia (LBD). We aimed to determine the validity and reliability of the Turkish version of ASI (ASI-T) in patients with LBD and to investigate the discriminative power of the test in patients with Alzheimer's Disease (AD), LBD, and cognitively healthy older adults (controls). 172 older adults over 60 years of age (43 with LBD, 41 AD, and 88 controls) were included. The sensitivity and specificity of the instrument were determined. A significant difference was found in ASI-T total score between people with LBD versus the controls (t=-9.259; p < 0.001), and versus patients with AD (t = 3.490; p = 0.001). Internal consistency of the ASI-T was good(Cronbach's alpha = 0.81). The cutoff score of 7 showed sensitivity (86%) and specificity (81%) (AUC= 0.888,CI0.95, p < 0.001) compared to controls. Also, compared to AD, it showed sensitivity (86%) and specificity(70%) (AUC = 0.590,CI.95, p < 0.001). Moreover, ASI-T demonstrated a significant concurrent validity with MMSE (r = −0.62; p < 0.001) and MoCA (r = −0.54; p = 0.003). In factor analysis, the five subscales accounted for 60% of the total variance. Our findings suggested that the ASI-T is a reliable, valid, and effective instrument for screening LBD. 16. Low Serum Beta-2 Microglobulin Level: A Possible Biomarker for Sarcopenia in the Elderly Population.

Abstract

Background and Objectives: One of the most critical problems regarding sarcopenia is the difficulty of the diagnosis process. This study aimed to determine the prevalence and investigate the role of serum beta-2 microglobulin level as a biomarker for diagnosing sarcopenia. Materials and Methods: This nested case–control study was conducted between 2023 and 2024 on 251 older adults. Muscle strength was measured using a hand dynamometer, and muscle mass was assessed using the bioelectrical impedance method. Individuals with low muscle strength and low muscle mass were accepted as having definitive sarcopenia. Results: The mean age of the 251 older adults included in the study was 72.19 ± 6.11 years. The prevalence of sarcopenia in individuals aged 65 years and over was found to be 5.2%. Serum beta-2 microglobulin levels were statistically significantly lower in sarcopenic participants compared to the control group (p = 0.002). The optimal cut-off value for serum beta-2 microglobulin level was 2.26 mcg/mL, and values lower than this point were found to be diagnostic for sarcopenia. Regarding the cut-off value, the sensitivity was 92.31% and the specificity was 80.77%, the positive predictive value was 70.59%, the negative predictive value was 95.45%, the Youden index was 0.731, and the area under the curve value was 0.901. Individuals who had beta-2 microglobulin levels below 2.26 mcg/mL were found to have a 10.75 times higher risk of sarcopenia. Conclusions: A low serum beta-2 microglobulin level has the potential to be an important candidate biomarker for the diagnosis of sarcopenia. [ABSTRACT FROM AUTHOR]

Abstract

Aim: Dementia, sarcopenia, and urinary incontinence (UI) are common geriatric syndromes. UI is a condition that affects the quality of life, results in social isolation, causes falls and, causes morbidity and mortality due to falls. UI also increases caregiver burnout and the burden of care in dementia patients. Continence requires an intact genito-urinary system, peripheral and central nervous system, and cognitive health. In addition, the importance of the pelvic floor muscles from the striated muscle group and the skeletal system in continence cannot be ignored. In the light of these facts, we aimed to evaluate the relationship between UI and sarcopenia in patients with dementia.Materials and Method: Dementia patients with sarcopenia who applied to the DEU Geriatrics unit between January 2015 and December 2021 were included. Patients with CDR 3 dementia and those with acute problems were excluded. Patients were grouped according to their UI status and evaluated for sarcopenia using the EWGSOP-2 criteria.Results: According to the presence of UI, no significant difference was found in demographic and laboratory findings between groups. The frequency of anti-parkinsonian drug usage and depression was more common in the UI group. While, the frequency of probable sarcopenia, severe sarcopenia, slow gait speed, and frailty was higher in the UI group; Barthel's score was lower (p, Amaç: Demans, sarkopeni ve üriner inkontinans (Üİ) sık görülen geriatrik sendromlardır. Ayrıca demans hastalarında bakım yükünü ve bakım veren tükenmişliğini artırır. Kontinans için sağlam bir genito-üriner sistem, periferik ve santral sinir sistemi ve de biliş gerekmektedir. Bunun yanında çizgili kas grubundan pelvik taban kaslarının ve iskelet sisteminin de kontinanstaki önemi göz ardı edilemez. Çalışmamızda demans hastalarında Üİ ile sarkopeni arasındaki ilişkiyi değerlendirmeyi planladık. Materyal ve Metot: Ocak 2015 ve Aralık 2021 tarihleri arasında Dokuz Eylül Üniversitesi Geriatri birimine başvuran sarkopeni değerlendirmesi olan demans hastaları çalışmaya dahil edilmiştir. Hastalar Üİ durumlarına göre iki gruba ayrılmıştır. Sarkopeni tanısı EWGSOP-2 kriterleri kullanılarak konulmuştur. Abstract

Amaç:Alzheimer Hastalığı (AH) hala büyük bir küresel sorundur ve farklı mekanizmalara sahip ajanlar umut verici birterapötik fırsatı temsil etmektedir. Yüksek oranda emilebilir bir kurkumin formülasyonu olan Theracurminin,demansı olmayan kişilerde hafızayı ve dikkati geliştirdiği gösterilmiştir. Bu çalışma amacı; Theracurminin hafif bilişselbozukluğu (HBB) ve AH olan yaşlı hastalarda hastalık seyri üzerindeki etkisini araştırmaktır.Gereç ve Yöntem:Bu takip çalışması retrospektif olarak HBB veya AHli 93 hasta üzerinde gerçekleştirilmiştir. Tüm hastalara başlangıçtave 6. ayın sonunda Mini Mental Durum Muayenesi (MMSE), Montreal Bilişsel Değerlendirme (MOCA), saat çizmetesti, günlük yaşam aktiviteleri (GYA) dahil olmak üzere kapsamlı geriatrik değerlendirme yapıldı. AHli 19 hasta veHBBli 17 hasta, 180 mg/gün p.o Theracurmin ile tedavi edildi.Bulgular:Terakurmin ile tedavi edilmeyen AH hastalarında MMSE, MOCA ve enstrümantal GYA skorları gerilerken (sırasıylapSonuç:Terakurmin, AH ve HBBli yaşlı hastalar için, bilişsel işlevlerde ve GYAlarda ilerleyici kaybı önleyerek hastalık seyrininstabilizasyonunu sağlayan bir tedavi seçeneği gibi görünmektedir.
