Abstract

Background. Academic failure can negatively impact on the student, the university and the nursing workforce in the short term. The aim of this study is to analyze the characteristics and predictors of academic success in two cohorts of students of a Nursing Bachelor’s Degree program. Methods. This longitudinal study enrolled students who attended the Nursing Bachelor’s Degree program. The Tor Vergata University includes 21 Nursing Bachelor’s Degree programs at several Sanitary Institutions of the Rome area, with a mean annual recruitment of 800 students. We considered two cohorts of students beginning the program in the 2011-2012 and 2012-2013 academic years, respectively. The follow-up of these two cohorts ended in the 2017-2018 academic year. We considered socio-demographic and academic variables in the nursing field (pre-admission test grade, training exam grade in the first, second and third year and professional license exam grade). The outcome variable is academic success defined as graduation on time; academic failure is defined as changing degree, delay in completion of the course, attrition and dismissal (failure to complete the studies). Data were collected at baseline (T0), at the end of the first year (T1), at the end of the second year (T2) and at the end of the third year (T3) of the course. A logistic regression was performed to identify predictors of academic success. Results. 2,041 students were enrolled, with an average age of 22.0 years (± 4.6, 18-50); 67.4% were female. Regarding academic success, 30.6% of students graduated on time and so achieved academic success, while 69.4% failed to complete the course within the established time: 5.4% (110/2,041) changed the course, the attrition rate was 7.3%, 0.3% (6/2,041) overdue from the course for expiring of the terms, 18.0% (368/2,041) is attending the course and 38.4% (784/2,041) graduated after more than three years. Pre-admission test grade (OR: 10.0, 95% CI: 10.020-10.054) and training exam grade at the second and third years (OR: 10.0 95% CI: 10.027-10.139; OR: 10.2 95% CI: 10.171-10.294) predicted academic success. Discussion and Conclusions. Some nursing students changed course during the last year of the program, while the training exam grade of the second and third years had predicted academic success; this is a brand new information derived from this study. In addition, we confirmed the association of age, gender, secondary school type and grade and preadmission test with academic success. For these reasons, clinical training should be improved and the quality of internship wards, as learning environments, should be monitored. Italian universities should undertake establishing the ideal cut-off in pre-admission tests to determine which students have a high probability of academic success. [ABSTRACT FROM AUTHOR]

Abstract

University time is considered to be a period of vulnerability among nursing students for substance abuse, which can create an unsafe clinical practice. The aim of this study was to investigate the substance abuse of nursing students in the form of alcohol, drugs and tobacco use during the course of studies. In addition, another study aim was to describe a typical socio-demographic profile for substance-abusing students.A longitudinal study design based on a sample of 254 nursing students was used. Data from the same cohort of students at the baseline level (T0), at the end of the first year (T1), at the end of the second year (T2) and at the end of the third year (T3) were analysed.A decrease in the number of nursing students who smoke regularly, an increase in the number of those who use drugs to lose control and of those who drink weekly were found.The early recognition of substance-abusing students can be fundamental to their health, academic background and safe clinical practice.Il periodo degli studi universitari è considerato di estrema vulnerabilità per l'abuso di sostanze tra gli studenti infermieri; questo può determinare l’esercizio di una pratica clinica, durante il tirocinio, non sicura. Lo scopo di questo studio è quello di indagare l’uso di sostanze tra gli studenti infermieri, con particolare attenzione all’abuso di alcol, droghe e tabacco. Infine si vuole individuare il profilo socio-demografico di questi studenti.è stato utilizzato un disegno di studio longitudinale, basato su un campione di 254 studenti infermieri. Sono stati analizzati i dati della stessa coorte di studenti all’ingresso nel corso di studi (T0), alla fine del primo anno (T1), alla fine del secondo anno (T2) e alla fine del terzo anno (T3). RISULTATI: vi è una diminuzione del numero di studenti che fumano regolarmente, un aumento di coloro che usano droghe per perdere il controllo e che abusano di alcol con una frequenza settimanale. CONCLUSIONE: il riconoscimento precoce degli studenti che abusano di sostanze può essere fondamentale per la loro salute, per il loro percorso accademico e per una pratica clinica sicura.

18. Vitrification of equine expanded blastocysts

Abstract

A successful and easy method for equine embryo vitrification is currently being used for commercial purposes. Anyway, cryopreservation is more successful when embryos are less than 300 µm, day 6 after ovulation, but in commercial programs embryos are often collected on day 7 or 8, for an higher recovery rate. Recently, successful cryopreservation of equine expanded blastocysts after blastocoele collapse was obtained using a micromanipulator. The aim of this study was to test a field method to simplify collapse and cryopreservation procedures for expanded equine blastocysts. Equine blastocysts > 400 µm were collected from 8 mares 7-9 days after ovulation and cut with a 27 gauge syringe needle before standard in straw vitrification. Embryos were warmed and transferred in 6 recipients or stained with Hoechst 33342/propidium iodide. 13 embryos were collected from 20 flushes, and none of 6 produced pregnancy after warming and transfer. Only 1/7 embryo was intact after warming and had about 20% dead cells. Use of standard vitrification and solutions seems to be inadequate to assure large embryos survival. The method for blastocoele collapse used in this study should be improved to enhance fluid loss and tested with a non-standard vitrification protocol, to establish its efficacy.

Abstract

Background: Randomized controlled trials (RCTs)may be categorized as either effectiveness trials or efficacy trials, which may be categorized by the Pragmatic-Explanatory Continuum Indicator Summary (PRECIS) tool. However, no data regarding the application of the PRECIS tool in a cluster of RCTs belonging to a specific discipline such as nursing are available. Purpose: The principal aim of this study was to assess the prevailing nature (pragmatic vs. explanatory) of a cluster of clinical nursing RCTs. Evaluating the suitability of the PRECIS in the analysis of nursing RCTs was the secondary aim. Methods: All nursing RCTs published in 2010 were identified through a systematic review and extracted in full-text form. An explanatory-pragmatic (E-P) group consisting of 11 researchers trained in the use of the PRECIS tool evaluated each RCT in terms of 10 domains, respectively scored on a scale ranging from 5 (pragmatic) to 1 (explanatory). The E-P group further scored the feasibility of the PRECIS tool using a numerical rating scale (0 = not at all, 10 = entirely feasible). Results: Along the pragmatic-explanatory continuum, assuming 50 as the highest degree of pragmatism and 10 as the highest degree of explanatory, the evaluation of nursing RCTs returned an average of 31.1 (median = 31, SD = 7.18, range = 13Y44). On the pragmatic-explanatory continuum, the evaluated nursing RCTs ended to be pragmatic, which seems to be consistent with the purposes of the nursing discipline. The feasibility of the PRECIS tool in the evaluation of nursing trials as perceived by the EYP Group was, on average, 7.09 (SD = 1.09, 95% CI [6.35, 7.82]). Conclusions/Implications for Practice: Applying the PRECIS tool is perceived to be highly feasible in the critical appraisal of a cluster of RCTs in a specific discipline such as nursing.

Abstract

Supporting student nurses whilst undertaking clinical training is an important activity for both educators and students, yet there is little consensus in the literature as to what constitutes support. Also, it is difficult to find a robust instrument that provides a valid measure of the quality of the clinical learning environment in different contexts. The aim of this study was to identify factors that influence student nurses' perception of their clinical tutors. A two-part questionnaire was administered to students of a Nursing School in a large city in northern Italy; 143 students participated in the study. In replying to the questions, student nurses were asked to refer to their last one-month training period of the year 2004. Analysis of study results showed that students' perceived satisfaction with their clinical guide was greater in cases where tutors used the critical thinking method. A one-to-one relation between tutor and student was not found to be essential in assuring a satisfactory clinical experience for nursing students.
