Abstract

Edgedetection is one of the most basic stages of image processing and have beenused in many areas. Its purpose is to determine the pixels formed the objects.Many researchers have aimed to determine objects' edges correctly, like as theyare determined by the human eye. In this study, a new edge detection techniquebased on spiking neural network is proposed. The proposed model has a differentreceptor structure than the ones found in literature and also does not use graylevel values of the pixels in the receptive field directly. Instead, it takesthe gray level differences between the pixel in the center of the receptivefield and others as input. The model is tested by using BSDS train dataset.Besides, the obtained results are compared with the results calculated by Cannyedge detection method., Kenaralgılama, görüntü işlemenin en temel aşamalarından biridir ve birçok farklıalanda kullanılmaktadır. Kenar belirleme yöntemlerinin amacı görüntüyüoluşturan pikselleri belirlemektir. Çoğu araştırmacı, insan gözünün belirlediğigibi nesnelerin kenarlarını doğru algılamayı hedeflemiştir. Bu çalışmada,iğnecikli sinir ağ yapısına dayalı yeni bir kenar algılama tekniğiönerilmiştir. Önerilen model, literatürde bulunanlardan farklı bir alıcıyapısına sahiptir ve doğrudan alıcı alandaki piksellerin gri seviye değerlerinikullanmamaktadır. Bunun yerine, girdi olarak alıcı alanın ortasındaki pikselile diğerleri arasındaki gri seviye farklarını kullanarak kenar algılamaişlemini gerçekleştirmektedir. Geliştirilen model, BSDS öğrenme veri setikullanılarak test edilmiştir. Ayrıca, elde edilen sonuçlar Canny kenar algılamayöntemi yardımıyla hesaplananlar ile karşılaştırılmıştır.

Abstract

Purpose: We aimed to evaluate the impact of previous unsuccessful shock wave lithotripsy (SWL) therapy on ureterorenoscopy (URS) outcomes in proximal ureteral stones and to define whether there is any optimal timing for safe URS after SWL. Methods: The patients who underwent URS for proximal ureteral stones between the years 2015 and 2018 in eight centers were included. Patients were divided into two groups according to previous SWL history; group 1 consisted of patients without SWL before URS for the stone [SWL (-)] and group 2 consisted of patients with a previous SWL for the stone [SWL (+)]. Demographics, operation outcomes and stone characteristics were compared between these two groups. Regarding the complication and success rates, optimal timing for URS after SWL for the stone was calculated with receiver operator characteristics curve analysis. Results: Totally 638 patients were included (group 1: 466 patients and group 2: 172 patients). The operation and hospitalization times, rate of ureteral stenting and complications were significantly higher in group 2. Stone free status was similar between the groups. Optimal timing for URS after SWL was calculated as 16.5 days (AUC = 0.657, p = 0.012) with a sensitivity of 68% and specificity of 72%, regarding the complication rates. Complication rates were significantly higher in patients who were operated before 16.5 days (27.7% vs 6.5%, p < 0.001). Conclusions: The optimal timing; 2–3 weeks delay of the URS procedure after unsuccessful SWL may decrease complication rates according to our results. © 2019, Springer-Verlag GmbH Germany, part of Springer Nature.

Abstract

Background: Methanol poisoning is a serious medical, social and economic problem that may cause severe illness or death. After methanol ingestion, central nervous system depression, headache, dizziness, nausea, lack of coordination, and confusion begins. Once the initial symptoms have passed, a second set of symptoms arises, 10 to 30 hours after the initial exposure to methanol, including blurring or complete loss of vision and acidosis. Methanol poisoning by ingestion is a world-wide problem, and in some regions it is connected with high morbidity and mortality. The lethal dose of methanol in humans shows pronounced individual differences ranging from 15 to 500 ml. Methods: The records of the First Specialization Board of the Council of Forensic Medicine between 2002 and 2010 were reviewed retrospectively for all methyl alcohol poisoning cases. Results: There were 383 cases recorded. 360 (94%) of total fatalities were men and 23 (6%) were women. The age range was between 17 and 89. Although patients were conscious, cooperative, oriented at first, deteriorated general health state, metabolic acidosis and neurologic sequelae with severe electrocardiographic (ECG) changes were seen in progress of time. The laboratory findings and MRI imaging method were applied to assess progress and medical treatment. Unfortunately severe acidosis, central nervous system (CNS) sequelae and a lethal outcome occurred. The methyl alcohol blood concentrations ranged from 0 to 826 mg per 100 ml. The most common macroscopic and microscopic finding was lung edema, cerebral and cerebellar hemorrhage, ischemic changes in the brain and optic neuritis. Putaminal necrosis and hemorrhage, brainstem petechial hemorrhage, myocardial acute ischemic changes, thalamic and hypothalamic hemorrhage were detected rarely. Conclusions: This is the first study to report postmortem findings, clinical reports, crime scene reports and eye witness accounts to investigate methyl alcohol poisoning cases from medico-legal point of view in Turkey. Methanol poisoning by ingestion is a world-wide problem with high morbidity and mortality. For preventing methanol deaths, both awareness and public education must be increased. © 2017, Scientific Publishers of India. All rights reserved.

Abstract

Blackwell Publishing, Sleep disorders constitute an important public health problem. Prevalence of sleep disorders in Turkish adult population was investigated in a nationwide representative sample of 5021 Turkish adults (2598 women and 2423 men, response rate: 91%) by an interviewer-administered questionnaire. Insomnia was defined by the DSM-IV criteria, habitual snoring and risk for sleep-related breathing disorders (SDB) by the Berlin questionnaire, excessive daytime sleepiness (EDS) by the Epworth sleepiness scale score, and restless legs syndrome (RLS) by the complaints according to the International Restless Legs Syndrome Study Group criteria. Mean age of the participants was 40.7 ± 15.1 (range 18 to 90) years. Prevalence rates (men/women) were insomnia 15.3% (10.5%/20.2%; P < 0.001), high probability of SDB 13.7% (11.1%/20.2%; P < 0.001), EDS 5.4% (5.0%/5.7%; P: 0.09), RLS 5.2% (3.0%/7.3%; P < 0.001). Aging and female gender were associated with higher prevalence of sleep disorders except for habitual snoring. Prevalence rates of the sleep disorders among Turkish adults based on the widely used questionnaires were close to the lower end of the previous estimates reported from different parts of the world. These findings would help for the assessment of the health burden of sleep disorders and addressing the risk groups for planning and implementation of health care. Abstract

Background: Internalized stigma, adoption of negative attitudes and stereotypes of the society regarding persons' illness, has not been studied previously in pediatric psoriasis patients. Objective: We aimed to investigate the internalized stigma in pediatric psoriasis patients and to determine differences according to factors affecting internalized stigma compared to adult psoriasis patients. Methods: This multicenter, cross-sectional, comparative study included 125 pediatric (55 female, 70 male; mean age±standard deviation [SD], 14.59±2.87 years) and 1,235 adult psoriasis patients (577 female, 658 male; mean age±SD, 43.3±13.7 years). Psoriasis Internalized Stigma Scale (PISS), Dermatology Life Quality Index (DLQI), Perceived Health Status (PHS), and the General Health Questionnaire (GHQ)-12 were the scales used in the study. Results: The mean PISS was 58.48±14.9 in pediatric group. When PISS subscales of groups were compared, the pediatric group had significantly higher stigma resistance (p=0.01) whereas adult group had higher scores of alienation (p=0.01) and stereotype endorsement (p=0.04). There was a strong correlation between mean values of PISS and DLQI (r=0.423, p=0.001). High internalized stigma scores had no relation to either the severity or localization of disease in pediatric group. However, poor PHS (p=0.007) and low-income levels (p=0.03) in both groups, and body mass index (r=0.181, p=0.04) in the pediatric group were related to high PISS scores. Conclusion: Internalized stigma in pediatric patients is as high as adults and is related to poor quality of life, general health, and psychological illnesses. Unlike adults, internalized stigma was mainly determined by psoriasis per se, rather than disease severity or involvement of visible body parts, genitalia or folds. Abstract

Poly(3-hydroxyalkanoate)s (PHAs) are a class of polymers receiving attention because of their potential as renewable, biodegradable and high-technology properties. Unlike most short chain length (scl) PHAs such as poly(3-hydroxybutyrate) (PHB), medium chain length (mcl) PHAs such as poly(3-hydroxyoctanoate) (PHO) exhibit low crystallinity and are elastomeric in character. PHB-b–PEG-b–PHO block copolymers can combine both properties in block copolymer matrix. In this study, we report the synthesis of the block copolymers combining the PHB and PHO blocks. Transamidation reactions of PHB with polyethylene glycol with primary amine yield equimolar amounts and PHB with amine ends. PHO reacts with the modified PHB containing the amine end to give PHB-b–PEG-b–PHO block copolymers. Structural analysis of the products was performed by using 1H–, 13C, heteronuclear single quantum coherence NMR techniques. Abstract

Introduction: We aimed to evaluate the predictive factors in a holistic manner for ureterorenoscopy (URS) outcomes in proximal ureteral stones by a multicenter study. Materials and Methods: The data of patients who underwent URS for proximal ureteral stones between the years 2015 and 2018 in eight centers were recorded retrospectively. Patients were divided into two groups according to URS success: Group 1 consisted of patients with successful URS, and Group 2 consisted of patients with unsuccessful URS. The two groups were compared in terms of risk factors, stone, and clinical characteristics of patients. Results: A total of 638 patients were included in the study. Group 1 consisted of 527 (82.6%) patients, and Group 2 consisted of 111 (17.4%) patients. In multivariate logistic analysis, the key risk factors for URS success was found to be age (OR = 0.980, 95% [CI] = 0.963–0.996, p = 0.018), stone area (OR = 0.993, 95% [CI] = 0.989–0.997, p = 0.002), and operation time (OR = 0.981, 95% [CI] = 0.968–0.994, p = 0.005). Conclusions: To make the treatment decision of proximal ureteral stones, it is necessary to examine several parameters including available equipment, stone, and patient characteristics. Physicians should keep these risk factors in mind in the decision of treatment options.

Abstract

Purpose: To compare the efficacy and adverse effect profiles of the first-line treatment of patients with KRAS wild type metastatic colorectal cancer (CRC) in Turkey who were treated based on regimens including bevacizumab, cetuximab and panitumumab. Methods: This retrospective multicenter observational study involved a total of 238 patients who received chemotherapy in combination with either bevacizumab or cetuximab or panitumumab as first-line therapy for KRAS wild-type metastatic colorectal cancer. Patients with full medical records having pathological diagnosis of CRC adenocarcinoma were included in the study. The demographic, laboratory, histopathological and clinical characteristics of the patients were determined, and three groups were compared based on the study variables. Results: The mean age of the entire sample (n=238) was 58±11 years, 64% of which were male. The most frequent tumor localization was the rectum (37%) and G2 was the most common tumor grade (59.7%). About 63% of the patients had metastatic disease at diagnosis, with the most common site of metastasis being lung (14.7%) and liver (52.5%). Overall survival (OS) was 63.9%, while 1-, 3- And 5-year survival rates were 91.7, 56.6 and 36.9%, respectively. The expected mean survival was 49.1 months (95% CI, 42.9-55.3). The 1-, 3- And 5-year progression-free survival (PFS) rates following first-line treatment were 65.3, 26.1 and 5.6%, respectively, while disease free survival (DFS) in patients without metastasis at diagnosis was 68.5%. Abstract

Photovoltaics is amongst the most important technologies for renewable energy sources, and plays a key role in the development of a society with a smaller environmental footprint. Key parameters for solar cells are their energy conversion efficiency, their operating lifetime, and the cost of the energy obtained from a photovoltaic system compared to other sources. The optimization of these aspects involves the exploitation of new materials and development of novel solar cell concepts and designs. Both theoretical modeling and characterization of such devices require a comprehensive view including all scales from the atomic to the macroscopic and industrial scale. The different length scales of the electronic and optical degrees of freedoms specifically lead to an intrinsic need for multiscale simulation, which is accentuated in many advanced photovoltaics concepts including nanostructured regions. Therefore, multiscale modeling has found particular interest in the photovoltaics community, as a tool to advance the field beyond its current limits. In this article, we review the field of multiscale techniques applied to photovoltaics, and we discuss opportunities and remaining challenges. © T. It is therefore essential to know the overall cardiovascular risk and reveal the predictors of metabolic control in Turkish adults with diabetes mellitus. Methods: A nationwide, multicenter survey consecutively enrolled patients who were under follow up for at least a year. Optimal control was defined as HbA1c, Turkish Society of Endocrinology and Metabolism, The TEMD study has been funded by the Turkish Society of Endocrinology and Metabolism. Abstract

WOS: 000408928500016, PubMed: 28407292, Internalized stigma is the adoption of negative attitudes and stereotypes of the society regarding a person's illness. It causes decreased self-esteem and life-satisfaction, increased depression and suicidality, and difficulty in coping with the illness. The primary aim of this study was to investigate the internalized stigma state of psoriatic patients and to identify the factors influencing internalized stigma. The secondary aim was to identify the correlation of internalized stigma with quality of life and perceived health status. This multicentre, cross-sectional study comprised 1485 patients. There was a significant positive correlation between mean values of Psoriasis Internalized Stigma Scale (PISS) and Psoriasis Area and Severity Index, Body Surface Area, Dermatological Life Quality Index and General Health Questionnaire-12 (P < 0.001 in all). Lower percieved health score (P = 0.001), early onset psoriasis (P = 0.016), family history of psoriasis (P = 0.0034), being illiterate (P < 0.001) and lower income level (P < 0.001) were determinants of high PISS scores. Mean PISS values were higher in erythrodermic and generalized pustular psoriasis. Involvement of scalp, face, hand, genitalia and finger nails as well as arthropathic and inverse psoriasis were also related to significantly higher PISS scores (P = 0.001). Our findings imply that psoriatic patients experience high levels of internalized stigma which are associated with psoriasis severity, involvement of visible body parts, genital area, folds or joints, poorer quality of life, negative perceptions of general health and psychological illnesses. Therefore, internalized stigma may be one of the major factors responsible from psychosocial burden of the disease.

Abstract

Background and Aims: To evaluate asthma phenotypes in patients with asthma from different regions of Turkey. Methods: A total of 1400 adult asthmatic patients (mean (SD) age: 44.0 (13.9) years, 75% females) from 14 centers across Turkey were included in this study and a standard questionnaire was applied between the time period of February 2011–January 2012. Results: The disease onset ≥ 40 years of age was higher percentage in obese vs. normal/overweight patients and nonallergic vs. allergic patients (P < 0.01). The percentage of patients who had FEV1 values over 80% was higher in allergic than nonallergic and normal/overweight than obese patients (P < 0.01). Uncontrolled asthmatics have more severe disease (P < 0.01). There were more frequent hospital admissions in nonallergic and uncontrolled asthmatics (P < 0.01). Chronic rhino-sinusitis was the leading comorbid disorder in normal/overweight and allergic asthma, while gastroesophageal reflux disorder was more frequent in nonallergic and uncontrolled asthma (P < 0.01). Asthma control rate was the highest (39.0%) in patients from Marmara region among all geographical regions (P < 0.05). Conclusion: In conclusion, our findings revealed existence of clinical/trigger related phenotypes based on BMI, allergic status, control level and geographical region with more frequent respiratory dysfunction and/or adverse health outcomes in uncontrolled, obese and nonallergic phenotypes. © 2015 John Wiley & Sons Ltd

Abstract

BMJ Publishing Group, Background: The purpose of this study was to estimate the current incidence of retinopathy of prematurity (ROP) and the need for treatment in preterm infants in Turkey. Methods: The study included preterm infants who had been screened for ROP between 2011 and 2013 in 49 neonatal intensive care units. Infants with birth weight (BW) ?1500 g or ?32 weeks' gestational age and those with BW >1500 g or >32 weeks' GA with an unstable clinical course were included. The incidence of any ROP or severe ROP and treatment modalities were determined. Results: The study population included 15 745 preterm infants: 11 803 (75%) with GA ?32 weeks, and 3942 (25%) with GA >32 weeks. Overall, 30% were found to have any stage of ROP, and 5% had severe ROP. Severe ROP was diagnosed in 8.2% of infants with BW ?1500 g and 0.6% of infants with BW >1500 g. Of all infants diagnosed with ROP, 16.5% needed laser photocoagulation, and 20 patients born at >32 weeks' GA required this treatment modality. Vitroretinal surgery was performed in 28 infants with severe ROP: 23 with GA ?28 weeks and 5 with GA 29-32 weeks. Conclusions: The findings of our study have the important implication that more mature babies are at risk of severe ROP requiring treatment. Abstract

The aim of this study is to examine the acute effect of different training programs applied to elite female handball players on throwing accuracy. 14 elite female players were participated in the study. The mean age of the participating players was 28.64 ± 9.88 (years), the mean height length was 176.50 ± 4.86 (cm), the mean body weight was 68.07 ± 2.12 (kg) and the mean sport age was 14.36 ± 4.89 (years). On different days, a technical training program, maximal strength training program, strength endurance training program and speed power training program have been implemented to the participating players in the research. Before and after training, the players throwing the ball from their area to the specific points of goal and the percentage of accuracy was determined. As a result; there was a significant difference between before and after maximal strength training the mean values of throwing accuracy (p 0,05). This result indicates that acute maximal strength training has a negative effect on throwing accuracy.

Cumhuriyet University Faculty of Dentistry, Amelogenesis imperfecta (AI) encompasses a complicated group of hereditary conditions that cause developmental alterations in the structure of the enamel in the absence of a systemic disorder. AI primarily affects the quality and/or quantity of dental enamel, and it may affect all or only some of the teeth in the primary and/or permanent dentition. Treatment of a patient with amelogenesis imperfecta (AI) presents a real problem from both functional and esthetic points of view. An esthetic result also will result in an improvement in the patient's quality of life. This clinical report illustrates the oral rehabilitation of a 23-year-old woman diagnosed with hypomature type of AI. The aim of treatment was to both restore esthetics and improve masticatory function. 34. Cognitive disengagement syndrome: The same as or different from attention-deficit hyperactivity disorder? An examination based on biochemical parameters.

Abstract

Objectives: The aim of this study was to determine whether there are differences in the biochemical blood parameters between individuals with Cognitive Disengagement Syndrome (CDS) and Attention Deficit Hyperactivity Disorder (ADHD), and to investigate the relationship between vitamin levels and these disorders, as well as their impact on symptom severity., Methods: The study included 35 patients with ADHD, 35 patients with both ADHD and CDS, and 35 healthy control subjects who presented to the child and adolescent psychiatry outpatient clinic between January 2022 and 2023, matched for age and gender. Children aged 7-15 were included in the study. Participants were administered a sociodemographic data form, DSM-IV Based Screening and Assesment Scale for Disruptive Behavior Disorders- Parent form (DBSAS-DBD) and the Barkley Child Attention Scale (BCAS). Hemogram parameters, iron, ferritin, Thyroid Stimulating Hormone (TSH), T3, T4, and vitamin levels (B12, folic acid, vitamin D) were analyzed for all participants., Results: Of the participants, 78 (74.3%) were male, and 27 (25.7%) were female. In the analysis, Vitamin B12 and D levels were found to be significantly lower in the ADHD + CDS group compared to both the ADHD group and the control group (p = 0.04; p = 0.03; p = 0.009 p = 0.004 respectively). Hemoglobin levels did not significantly differ between the ADHD + CDS group and the ADHD group, but were significantly higher in the ADHD + CDS group compared to the control group (p = 0.004). Ferritin levels were significantly lower in both the ADHD + CDS group and the ADHD group (p < 0.001; p < 0.001, respectively)., Conclusions: The significant decrease in vitamin B12 and vitamin D in the CDS + ADHD group may be related to daytime sleepiness and slow cognitive processes in the CDS clinic. Screening for nutritional deficiencies in individuals with CDS may be beneficial for clinical management, and identifying differences in blood parameters of CDS from ADHD will contribute to a better understanding of this clinical condition. This is an exploratory study with a rather small sample size, in which many outcomes were examined. 35. Synthesis and biological assessment of BUB1B inhibitors for the treatment of clear cell renal cell carcinoma.

Abstract

Clear cell renal cell carcinoma (ccRCC) presents substantial therapeutic challenges due to its molecular heterogeneity, limited response to conventional therapies, and widespread drug resistance. Recent advancements in molecular research have identified novel targets, such as BUB1B, which has been identified through global transcriptomic profiling and gene co-expression network analysis as critical in ccRCC progression. In this study, we synthesized 40 novel derivatives of TG-101209 to modulate BUB1B expression and activity, leading to the induction of apoptosis in Caki-1 cells. The molecular structures of all compounds were confirmed via 1 H and 13 C NMR and mass spectrometry. Computational docking studies were conducted using Schrödinger Maestro software. The efficacy of the compounds on cell viability was screened using the MTT assay and further validated by the LDH assay. The expression of the target protein BUB1B and apoptosis-related proteins was analyzed via western blotting. BUB1B activity was assessed through an enzymatic assay, and compound binding efficacy was evaluated using a cellular thermal shift assay (CETSA). The results indicated that four compounds (7h, 8h, 8i, and 8j) demonstrate stronger molecular interactions and better conformational fit within the target cavity, leading to improved binding affinity. These compounds also exhibited more potency in reducing the viability of Caki-1 cells compared to TG-101209. In particular, compound 8h was identified as the most effective, exhibiting the strongest inhibitory effect on BUB1B and inducing apoptosis. Compound 8h demonstrated intracellular binding with BUB1B, similar to TG-101209, but through a different binding moiety that destabilizes the BUB1B protein structure, whereas TG-101209 stabilizes it. Abstract

Importance: Increasing evidence supports the oncologic safety of de-escalating axillary surgery for patients with breast cancer after neoadjuvant chemotherapy (NAC)., Objective: To evaluate the oncologic outcomes of de-escalating axillary surgery among patients with clinically node (cN)-positive breast cancer and patients whose disease became cN negative after NAC (ycN negative)., Design, Setting, and Participants: In the NEOSENTITURK MF-1803 prospective cohort registry trial, patients from 37 centers with cT1-4N1-3M0 disease treated with sentinel lymph node biopsy (SLNB) or targeted axillary dissection (TAD) alone or with ypN-negative or ypN-positive disease after NAC were recruited between February 15, 2019, and January 1, 2023, and evaluated., Exposure: Treatment with SLNB or TAD after NAC., Main Outcomes and Measures: The primary aim of the study was axillary, locoregional, or distant recurrence rates; disease-free survival; and disease-specific survival. Number of axillary lymph nodes removed was also evaluated., Results: A total of 976 patients (median age, 46 years [range, 21-80 years]) with cT1-4N1-3M0 disease underwent SLNB (n = 620) or TAD alone (n = 356). Most of the cohort had a mapping procedure with blue dye alone (645 [66.1%]) with (n = 177) or without (n = 468) TAD. Overall, no difference was found between patients treated with TAD and patients treated with SLNB in the median number of total lymph nodes removed (TAD, 4 [3-6] vs SLNB, 4 [3-6]; P = .09). Among patients with ypN-positive disease, those who underwent TAD were more likely to have a lower median lymph node ratio (TAD, 0.28 [IQR, 0.20-0.40] vs SLNB, 0.33 [IQR, 0.20-0.50]; P = .03). At a median follow-up of 39 months (IQR, 29-48 months), no significant difference was found in the rates of ipsilateral axillary recurrence (0.3% [1 of 356] vs 0.3% [2 of 620]; P ≥ .99) or locoregional recurrence (0.6% [2 of 356] vs 1.1% [7 of 620]; P = .50) between the TAD and SLNB groups, with an overall locoregional recurrence rate of 0.9% (9 of 976). The initial clinical tumor stage, pathologic complete response, and use of blue dye alone as a mapping procedure were not associated with the outcome. Even though patients with TAD demonstrated an increased disease-free survival rate compared with the SLNB group, this difference did not reach statistical significance (94.9% vs 92.6%; P = .07). Factors associated with decreased 5-year disease-specific survival were cN2-3 axillary stage (cN1, 98.7% vs cN2-3, 96.8%; P = .03) and nonluminal type tumor pathologic characteristics (luminal, 98.9% vs nonluminal, 96.9%; P = .007)., Conclusions and Relevance: The short-term results suggest very low rates of axillary and locoregional recurrence in a select group of patients with cN-negative disease after NAC treated with TAD alone or SLNB alone followed by regional nodal irradiation regardless of the SLNB technique or nodal pathology. Whether TAD might provide a clear survival advantage compared with SLNB remains to be proven in studies with longer follow-up.

Abstract

Introduction: Although CRPA may test susceptible to other β-lactams such as ceftazidime (CAZ), cefepime (FEP), and piperacillin/tazobactam (TZP), reduced potency has been observed. We assessed the adequacy of EUCAST Susceptible (S) or Susceptible Increased Exposure (SIE)/(I) doses for CAZ, FEP, and TZP against CRPA clinical isolates., Methods: CRPA isolates were collected from patients at three Turkish hospitals. CAZ, FEP, and TZP MICs were determined using broth microdilution. Monte Carlo simulations were performed to determine the probability of target attainment (PTA) for a free time above the MIC (fT > MIC) targets for various doses of each agent against isolates defined as susceptible. fT > MIC targets were 70% for CAZ or FEP and 50% for TZP. Cumulative fraction of response (CFR) was calculated. Optimal PTA and CFR was 90% target achievement., Results: The percentages of isolates SIE/I to CAZ, FEP, and TZP were 49,8%, 47%, and 31,8% respectively. Reduced potency was noted with 54,1% of CAZ-S isolates having MICs of 4 or 8 mg/L. Of the FEP and TZP-S isolates, MICs at the breakpoint (8 and 16 mg/L, respectively) were the mode with 45,2 and 53,9% of isolates for each, respectively. At an MIC of 8 mg/L for CAZ, the EUCAST standard dose was insufficient (CFR of 85%). 3 h infusions of EUCAST SIE doses were required for 90% PTA at MIC of 8 mg/L and an optimized CFR of 100%. For FEP, the SIE dose of 2 g q8h 0.5 h infusion of was effective (CFR 96%), utilization of an extended 3 h infusion further optimized the PTA at 8 mg/L (CFR 99%). For TZP, the standard dose of 4.5 q6h administered as a 0.5 h infusion was inadequate (CFR 86%). A standard TZP dose with an extended infusion (4.5 g q8h over 4 h) and the SIE dose 4.5 g q6h 3 h infusion resulted in CFRs > 95%., Conclusion: These data support the EUCAST SIE breakpoints for FEP and TZP. Abstract

Background: The omission of axillary lymph node dissection (ALND) remains controversial for patients with residual axillary disease after neoadjuvant chemotherapy (NAC), regardless of the residual burden. This study evaluated the oncologic safety and factors associated with outcomes in patients with residual axillary disease. These patients were treated solely with sentinel lymph node biopsy (SLNB) or targeted axillary dissection (TAD), without ALND, after NAC., Methods: A joint analysis of two different multicenter cohorts-the retrospective cohort registry MF18-02 and the prospective observational cohort registry MF18-03 (NCT04250129)-was conducted between January 2004 and August 2022. All patients received regional nodal irradiation., Results: Five hundred and one patients with cT1-4, N1-3M0 disease who achieved a complete clinical response to NAC underwent either SLNB alone (n = 353) or TAD alone (n = 148). At a median follow-up of 42 months, axillary and locoregional recurrence rates were 0.4% (n = 2) and 0.8% (n = 4). No significant difference was found in disease-free survival (DFS) and disease-specific survival (DSS) rates between patients undergoing TAD alone versus SLNB alone, those with breast positive versus negative pathologic complete response, SLN methodology, total metastatic LN of one versus ≥2, or metastasis types as isolated tumor cells with micrometastases versus macrometastases. In the multivariate analysis, patients with nonluminal pathology were more likely to have a worse DFS and DSS, respectively, without an increased axillary recurrence., Conclusions: The omission of ALND can be safely considered for patients who achieve a complete clinical response after NAC, even if residual disease is detected by pathologic examination. Abstract

Background: The genus Bartonella is composed of Gram-negative, fastidious, facultative intracellular bacteria that can cause bacteremia in mammals and various disorders in humans. Rodents have been reported as reservoirs of more than 30 Bartonella species, seven of which cause zoonotic infections. Materials and Methods: In the present study, the isolation of Bartonella sp. was attempted from 150 spleen samples from 13 rodent species (mostly Apodemus species) from three geographically different regions in Turkey. Results: Bartonella sp. was successfully isolated from 65 of these 150 samples (43%). The prevalences of Bartonella sp. in tested rodents in the regions of Giresun, Yozgat, and Burdur were 68%, 44%, and 16%, respectively. Using polymerase chain reaction/sequence analysis of the citrate synthase-coding gene ( gltA ), Bartonellaisolates were classified seven species including B. taylorii , B. grahamii , B. birtlesii , B. mastomydis , and three putatively new Bartonella species. We performed further identification techniques for one of the three Bartonella species that were different from the validated Bartonella species according to the gltA sequence analysis. Conclusion: Here, we report the genomic and phenotypic characterization of Bartonella sp. strain G70 that was isolated from the splenic tissue of an Apodemus uralensis (Pallas 1881), the Ural field mouse, captured in the Giresun region of northeastern Turkey. Bartonella sp. strainG70 (RSKK 22001) was characterized by whole genome and partial gene ( gltA , 16S ribosomal RNA) sequencing and comparison, scanning electron microscopy, biochemical tests, and matrix-assisted laser desorption ionization time-of-flight mass spectrometry. This novel Bartonella is a Gram-negative, rod-shaped bacterium and has neither flagella nor pili. The genome from strain G70 was 1,606,969-bp-long with a G + C content of 35.7%. Bartonella rochalimae was found to be the closest phylogenetic relative of strain G70 (OrthoANI = 90.5%, digital DNA-DNA hybridization = 41.4%). We therefore propose that this new species be named Bartonella bilalgolemii sp. nov. with strain G70 T as the type strain.

Abstract

The antioxidant activity of Scorzonera parviflora Jacq. roots were assessed by measuring their ability to scavenge ABTS and DPPH radicals. Bioactivity-guided fractionation was utilized to identify the compound(s) responsible for this activity. The most active phase, ethyl acetate, was isolated using column chromatography. The resulting fractions were then purified using preparative TLC on reverse phase and semi-preparative HPLC. The structures of the pure compounds were elucidated by spectral analysis (MS and 1 H, 13 C, 2D-NMR). Three undescribed phenolic acid derivatives, namely parvifloric acid A (1), B (2), and C (3), and one new sesquiterpene lactone, parviflorin (4) together with seven known compounds were isolated and identified as scopolin (5), scopoletin (6), caffeic acid (7), protocatechuic acid (8), 4,5-O-dicaffeoylquinic acid (9) 3,5-O-dicaffeoylquinic acid (10), and 3,5-O-dicaffeoylquinic acid methyl ester (11). Finally, the pure compounds obtained were tested to evaluate their antioxidant capacities, using ABTS and DPPH radical scavenging potencies. Background: Given the growing number of older adults, it is crucial to understand geriatric depression and to determine the impact of depression on well-being and health. The aim of this study was to examine the predictive effect of geriatric depression on subjective health complaints and life satisfaction in older adults., Methods: This cross-sectional study was carried out in two family health centres in a city centre in the northwest of Türkiye. The sample consisted of 257 older adults. Structural equation modelling path analysis was utilised to examine the extent to which geriatric depression predicted subjective health complaints and life satisfaction among older adults., Results: The study found that geriatric depression was a predictor of both subjective health complaints and life satisfaction. Results showed that as geriatric depression scale score increased, subjective health complaints also increased (Std. β = 0.470, P < 0.001), while life satisfaction decreased (Std. β = -0.573, P < 0.001)., Conclusion: Geriatric depression is a complex health issue that biopsychosocially affects older adults. 42. Chemical profiling and biological activity evaluation of propolis from Çayeli-Rize, Eastern Black Sea Region, Anatolia.

Abstract

The constituents of the aqueous, ethanol, hexane, and methanol extracts of Anatolian propolis collected from the Eastern Black Sea Region (Çayeli-Rize) were investigated by GC-MS, HPLC and AAS. Interestingly, lactulose has been identified. Ten phenolic compounds, namely caffeic acid, ferulic acid, rutin, taxifolin, quercetin, kaempferol, apigenin, silicristin, silibinin and gallic acid were determined. The contents of phenolic acids and flavonoids varied between 17.04-642.59 and 1.18-2749.20 ppm, respectively. Minerals found in propolis were Na, K, Ca, Mg and Zn. The methanol extract had the highest antiproliferative activity against the A549 cell line with an IC 50 value of 0.1821 µL/mL. The extracts showed higher antibacterial activity against Gram-negative bacteria compared to Gram-positive bacteria.

43. Exploring the GSTP1 inhibition potential of photosensitizer compounds for enhanced cancer treatment in photodynamic therapy.

Abstract

Photodynamic therapy (PDT) has gained considerable attention in cancer treatment due to its non-invasive nature and the ability of photosensitizers to generate reactive oxygen species upon light activation, leading to tumor destruction. Glutathione S-transferase P1 (GSTP1) is a key enzyme in chemotherapy resistance, often overexpressed in various cancers, and its inhibition of GSTP1 presents a promising strategy to enhance cancer treatment. This study is aimed at assessing the potential of prominent photosensitizers as GSTP1 inhibitors through molecular docking analysis to strengthen the efficacy of PDT. The photosensitizers were docked into the active site of GSTP1, and their binding affinities, inhibition constants (Ki), and molecular interactions were assessed. Among the tested photosensitizers, zinc phthalocyanine, hypericin, and temoporfin emerged as the top candidates, exhibiting binding energies of - 10.8, - 10.2, and - 9.8 kcal/mol, along with Ki values of 0.012, 0.033, and 0.064 µM, respectively. These compounds outperformed the reference inhibitor ethacrynic acid, which had a binding energy of - 6.6 kcal/mol and a Ki of 14.35 µM. These findings suggest that the dual action of these photosensitizers provides a promising strategy for combating cancer and overcoming treatment resistance., Competing Interests: Declarations. 44. Impact of Plant Species on the Synthesis and Characterization of Biogenic Silver Nanoparticles: A Comparative Study of Brassica oleracea , Corylus avellana , and Camellia sinensis .

Abstract

The choice of plant species is crucial, as different plants provide unique biomolecules that influence nanoparticle characteristics. Biomolecules in plant extracts, such as proteins, amino acids, enzymes, polysaccharides, alkaloids, tannins, phenolics, saponins, terpenoids, and vitamins, act as stabilizing and reducing agents. This study explores the synthesis of silver nanoparticles (AgNPs) using leaf extracts from collard greens ( Brassica oleracea var. acephala ), hazelnut ( Corylus avellana var. avellana ), and green tea ( Camellia sinensis ). NPs were synthesized using silver nitrate (AgNO 3 ) solution at two different molarities (1 mM and 5 mM) and characterized by UV-Vis spectroscopy, XRD, TEM, and FTIR. The Surface Plasmon Resonance (SPR) peaks appeared rapidly for hazelnut and green tea extracts, within 30 and 15 min, respectively, while collard greens extract failed to produce a distinct SPR peak. X-Ray Diffraction confirmed the formation of face-centered cubic silver. TEM analysis revealed high polydispersity and agglomeration in all samples, with particle size generally decreasing at higher AgNO 3 concentrations. However, hazelnut extract showed a slight increase in size at higher molarity. Among all samples, green tea-derived AgNPs synthesized with 5 mM AgNO 3 were the smallest and least polydisperse, highlighting the significant role of plant type in optimizing nanoparticle synthesis.

45. Assessment of the Mandibular Osseous Architecture in Cleft Lip and Palate Using Fractal Dimension Analysis: A Pilot Study.

Abstract

Background/Objectives : Although there has been extensive research on the orofacial morphologic effects of cleft lip and palate (CLP), the effects of CLP on mandibular structures remain largely unknown. The aim of this study was to investigate the trabeculation differences in the mandibular osseous architecture of patients with bilateral CLP (BCLP) and left-sided unilateral CLP (UCLP) using fractal dimension (FD) analysis and to compare these findings with healthy controls without CLP. Methods : A total of 63 patients (27 females, 36 males) with a mean age of 9.69 ± 1.5 years in the pre-peak growth stage were divided into three groups (n = 21 per group): the control group (CG), the BCLP group, and the UCLP group. The FD analysis was conducted on selected regions of interest (ROIs) from the mandibular condyle, angulus, corpus, and coronoid areas in TIFF-formatted panoramic radiographs. Statistical analyses were performed using the paired t-test and ANOVA for parametric data, and the Wilcoxon and Kruskal-Wallis tests for nonparametric data. Statistical significance was set at p < 0.05. Results : The FD values obtained from the ROIs of the right condyle were found to be significantly lower in the BCLP group compared to the CG and UCLP groups ( p < 0.05). Conversely, the FD values for the left condyle were significantly higher in the CG group ( p < 0.05), while no significant differences were observed between the BCLP and UCLP groups ( p > 0.05). The FD value of the left condyle in the UCLP group was found to be significantly lower than that of the right condyle ( p < 0.05). In the CG group, the FD values for both the right and left mandibular condyle and corpus were significantly higher than those for the angulus and coronoid regions; in the UCLP group, only the FD values of the right mandibular condyle and corpus were significantly higher than those for the same regions ( p < 0.05). Conclusions : The reduced FD values in the mandibular condyle of CLP patients during the pre-peak growth stage suggest a loss of trabeculation and lower metabolic activity, while similarly, reduced FD values in the corpus region contribute to delayed tooth eruption timing, likely due to decreased masticatory forces during the intercuspal position and altered occlusal relationships. Clinical Relevance : In treating CLP patients, particularly with orthopedic face masks, the reduction in metabolic activities in these areas should be considered to achieve the optimal mandibular growth and development, and dental eruptions during the distribution of force from the chin to the corpus and condyle.

46. Novel predictor for metabolic syndrome: Para-aortic adipose tissue.

Abstract

Background: Metabolic Syndrome (MetS) is an independent risk factor for cardiovascular disease. Perivascular fat depots not only serve as energy storage but also function as endocrine organs. Para-aortic adipose tissue (PAT), a perivascular local adipose tissue, has been suggested to play a role in obesity-mediated vascular disease, and has been associated with MetS components and measures of coronary and abdominal aortic calcification. PAT was previously described and examined using tomography and magnetic resonance imaging. This study aimed to describe the features of para-aortic adipose tissue measured echocardiographically in individuals with MetS., Methods: Patients were divided into two groups according to their MetS status. The hypoechoic space in front of the ascending aorta was considered a PAT on the parasternal long-axis view. Possible covariates for the regression analysis were determined using the DAGitty diagram., Results: A total of 494 patients were enrolled in this study. The PAT was significantly higher in the MetS group [9.6 (6.1/10.6) vs. 6.1 (0.9/9) mm, p < 0.001]. Logistic regression analysis revealed that PAT (OR=2,15, p = 0,003) was significantly associated with MetS. 7.55 mm of PAT has a sensitivity of 65 % and specificity of 65 % [AUC = 0.675, p < 0.001, 95 % CI (0.623-0.726)] in predicting the presence of MetS., Conclusions: Based on the measurements obtained using this newly described modality in transthoracic echocardiography, its relationship with MetS was determined. Purpose: Phenotypic methods have been proposed for the detection of carbapenemase production. These tests can have slower turnaround times. With the sensitivity-based algorithm described by Gill et al. will be possible to detect the carbapenemase., Methods: Carbapenem-resistant Pseudomonas aeruginosa (CRPA) isolates from three hospitals between January 2017 and December 2021 were included. The modified carbapenemase-inactivation-method(mCIM) and two algorithms were used, defined as "primary algorithm, i.e. ceftazidime and cefepime non-susceptible in addition to imipenem or meropenem resistance" and "secondary algorithm, i.e. ceftolozane/tazobactam non-susceptible in addition to imipenem or meropenem resistance". PCR testing was performed on all isolates., Results: 256 CRPA isolates were included in the study. When the primary or secondary algorithm criteria were applied, there were 173 isolates that met one or both of them. Abstract

Background: For nurses working in specialized units such as burn units, having adequate levels of job satisfaction and motivation is crucial for enhanced productivity in order to maintain quality nursing care., Methods: A cross-sectional, descriptive, correlational design was used in the study. The study aimed to investigate the job satisfaction and motivation levels of nurses working in burn units, the relationship between job satisfaction and motivation levels, and the influencing factors. The study sample consisted of 73 nurse managers/clinicians who worked in burn services or intensive care units in September and December 2023. The data were collected with "Demographic and work characteristics form," "Job Satisfaction Scale for Nurses (JSSN)", "Nurse Job Motivation Scale (NJMS)". Data were analyzed using Independent Samples T-test, One-Way ANOVA, and Mann-Whitney U tests to compare the descriptive characteristics of nurses and the scale scores. Correlation and regression analyses were conducted to reveal the relationship between the descriptive characteristics and scale scores of the nurses., Results: The mean age of the nurses was 30.26 ± 7.99 years, and 60.3 % were bachelor's degree graduates. The nurses had an average of 8.64 ± 8.25 years of experience in the profession and an average of 4.77 ± 5.39 years in the burn unit. The total score average for the NJMS was found to be 66.49 ± 5.56, and the JSSN score average was 3.46 ± 0.58. A weak negative relationship was found between the JSSN subscale "Suitable Support from Seniors" and age (p < 0.05), "Perceived importance at the job" and the working years in the burn unit (p < 0.05)., Conclusions: A positive correlation was observed between job satisfaction and job motivation of the nurses. If older nurses cannot get support from their supervisors, their satisfaction will be decreased. Abstract

Objective: To compare the predictive effects of stone size and volume on the efficacy and safety of retrograde intrarenal surgery (RIRS) and to determine the cutoff values of stone volume for prediction of RIRS efficacy and safety. Methods: Patients who underwent RIRS between 2017 and 2021 in six referral centers were retrospectively included in the study. The database of the RIRsearch group, which was formed prospectively, was used for this retrospective analysis. The surgical results and complications of RIRS were evaluated according to stone size and stone volume and compared between these groups. Results: A total of 1128 patients were included. Operation time, intraoperative complication rate, and postoperative complication rate increased significantly as stone size and stone volume increased ( P < .05). Stone size and volume were significant indicators for stone-free rates, but pairwise comparison showed that stone volume was a significantly better predictor of surgical success compared with stone size ( P < .001). Stone size was not sufficient to predict postoperative complications, whereas stone volume predicted these complications with low performance. Conclusions: Stone volume was a better predictor for surgical success than stone size, and it was as reliable as stone size in predicting postoperative complications.

50. Can HALP (Hemoglobin, albumin, lymphocyte, and platelet) score distinguish malignant and benign causes of extrahepatic cholestasis in patients with extrahepatic bile duct obstruction?

Abstract

Objective: Cholestatic diseases are common and classified as benign or malignant based on their etiology. HALP is a unique nutritional immune marker that combines indicators of nutritional status, including hemoglobin and albumin, with immune function markers like lymphocyte and platelet counts. We investigated the HALP score's ability to differentiate between benign and malignant causes in extrahepatic cholestasis patients., Methods: This research was designed as cross-sectional and retrospective. Between 1 January 2020-1 January 2022, patients diagnosed with extrahepatic cholestasis were included. The diagnoses were confirmed using non-invasive imaging methods, ERCP (endoscopic retrograde cholangiopancreatography), and tissue biopsy results. Based on the type of extrahepatic biliary obstruction, either benign or malignant, the patients were divided into two groups. The HALP score was calculated by multiplying the patient's albumin (g/L), hemoglobin (g/L), and lymphocyte count (/L) and dividing by the platelet count (/L)., Results: In 121 of 216 patients, extrahepatic cholestasis was caused by benign factors, mostly choledocholithiasis, while malignant causes, predominantly pancreatic head cancer, were responsible for extrahepatic cholestasis in 95 patients. The malignant cholestasis group had significantly higher bilirubin levels (p<0.001), lower hemoglobin levels (p=0.005), lower albumin levels (p<0.001), higher lymphocyte counts (p<0.001), and higher platelet levels (p=0.001) compared to the benign cholestasis group. There was no considerable difference in the HALP score between the two groups, as indicated by a p-value of 0.741., Conclusion: The HALP score could not distinguish between benign and malignant causes of extrahepatic cholestasis., Competing Interests: No conflict of interest was declared by the authors.
