Objetivos: Revisar las cirugías de prótesis totales de cadera realizadas en nuestro hospital, determinar el origen de la artrosis e identificar cuántas se colocaron por coxartrosis secundarias a enfermedad de Legg-Calvé-Perthes. Materiales y Métodos: Se realizó un estudio retrospectivo en el que se revisaron todas las cirugías de prótesis totales de cadera desde 2008 hasta diciembre de 2021. Se evaluaron las radiografías prequirúrgicas para determinar la etiología de la artrosis, y se consideraron variables, como lateralidad, sexo y edad en el momento de la intervención. Resultados: Se revisaron 1103 caderas en 935 pacientes. El 81% correspondía a coxartrosis primaria. En 11 caderas de 10 pacientes (1%), se detectó coxartrosis secundaria a la enfermedad de Legg-Calvé-Perthes. La media de la edad de estos pacientes era de 61 años. Conclusiones: Hay evidencia de que las alteraciones del crecimiento de la fisis femoral proximal o el sobrecrecimiento del trocánter mayor, propias de la enfermedad de Legg-Calvé-Perthes, pueden contribuir a la aparición de un choque femoroacetabular, con su consiguiente coxartrosis precoz. Es posible que algunas “mal clasificadas” coxartrosis primarias fueran identificadas así porque no existía otro dato sugerente de coxartrosis secundarias, y escondieran otra etiología evolucionada. Asimismo, proponemos el seguimiento del paciente joven con enfermedad de Legg-Calvé-Perthes, más allá del final del crecimiento, para identificar el choque femoroacetabular en sus inicios y poder ofrecer opciones terapéuticas artroscópicas.

IntroductionHematopoietic stem cell transplantation (HCT) can cure chronic granulomatous disease (CGD). However, transplant-associated morbidity or mortality may occur, and it is still controversial which patients benefit from this procedure. The aim of this retrospective study was to evaluate the outcome of pediatric patients who received HCT in one of the Spanish pediatric transplant units. ResultsThirty children with a median age of 6.9 years (range 0.6–12.7) were evaluated: 8 patients received a transplant from a sibling donor (MSD), 21 received a transplant from an unrelated donor (UD), and 1 received a haploidentical transplant. The majority of the patients received reduced-intensity conditioning regimens based on either busulfan plus fludarabine or treosulfan. Relevant post-HCT complications were as follows: i) graft failure (GF), with a global incidence of 28.26% (CI: 15.15–48.88), 11.1% in patients with MSD (1.64–56.70) and 37.08% in unrelated donors (19.33–63.17); and ii) chronic graft-versus-host disease (GVHD), with an incidence of 20.5% (8.9–43.2), 11.1% in patients with MSD (1.64–56.70) and 26.7% in unrelated donors (10.42–58.44). Post-HCT infections were usually manageable, but two episodes of pulmonary aspergillosis were diagnosed in the context of graft rejection. The 2-year OS was 77.3% (55.92–89.23). There were no statistically significant differences among donor types.DiscussionHCT in patients with CGD is a complex procedure with significant morbidity and mortality, especially in patients who receive grafts from unrelated donors. These factors need to be considered in the decision-making process and when discussing conditioning and GVHD prophylaxis.

Preeclampsia is a pregnancy-related multisystem disorder characterized by altered trophoblast invasion, oxidative stress, exacerbation of systemic inflammatory response, and endothelial damage. The pathogenesis includes hypertension and mild-to-severe microangiopathy in the kidney, liver, placenta, and brain. The main mechanisms involved in its pathogenesis have been proposed to limit trophoblast invasion and increase the release of extracellular vesicles from the syncytiotrophoblast into the maternal circulation, exacerbating the systemic inflammatory response. The placenta expresses glycans as part of its development and maternal immune tolerance during gestation. The expression profile of glycans at the maternal–fetal interface may play a fundamental role in physiological pregnancy changes and disorders such as preeclampsia. It is unclear whether glycans and their lectin-like receptors are involved in the mechanisms of maternal–fetal recognition by immune cells during pregnancy homeostasis. The expression profile of glycans appears to be altered in hypertensive disorders of pregnancy, which could lead to alterations in the placental microenvironment and vascular endothelium in pregnancy conditions such as preeclampsia. Glycans with immunomodulatory properties at the maternal–fetal interface are altered in early-onset severe preeclampsia, implying that innate immune system components, such as NK cells, exacerbate the systemic inflammatory response observed in preeclampsia. In this article, we discuss the evidence for the role of glycans in gestational physiology and the perspective of glycobiology on the pathophysiology of hypertensive disorders in gestation.

Treatment targeting CD19 by a chimeric antigen receptor expressed on T cells (anti-CD19 CAR-T) has led to a breakthrough in the management and treatment of relapsed and refractory B- cell acute lymphoblastic leukemia (B-ALL). After infusion, the efficacy of anti-CD19 CAR-T is monitored by bone marrow negative minimal residual disease and the absence of peripheral CD19+ B lymphocytes (B-cell aplasia). In patients who have received an allogenic Hematopoietic Stem Cell Transplantation (HSCT) prior to treatment with anti-CD19 CAR-T, monitoring lineage-specific chimerism could be helpful. We found that on 4 patients who received anti-CD19 CAR-T cells after HSCT and achieved early complete response, CD19+ lineage mixed chimerism but not CD3+ lineage mixed chimerism monitored by molecular techniques anticipated earlier than B-cell aplasia determined by flow cytometry, lack of effectiveness of anti-CD19 CAR-T and leukemia relapse. Donor lymphocyte infusions (DLIs) did not prevent relapse but recovered CD3+ full donor chimerism. We suggest that continuous lineage chimerism analysis should be done routinely in patients who receive anti-CD19 CAR-T cells after HSCT and achieve complete remission because it can support early treatment intervention. However, the role of DLI in this setting is unclear, so further prospective studies should be developed.

Background: Fertility diagnosis and treatment have an impact on the quality of life (QoL) of individuals. FertiQoL is an internationally developed questionnaire to measure fertility-specific QoL. Obtaining reliable information on the impact of fertility issues enables social and health care systems to develop value-centered integrated care. Objective: The aim of the present study is to examine the dimensionality, validity and reliability of Core FertiQol in a sample of Spanish women undergoing fertility treatment. Methods: A cross-sectional study was conducted, administering the Core FertiQoL questionnaire to 564 women with fertility problems. Descriptive analyses were performed on sociodemographic and clinical data of the sample. The psychometric properties of the FertiQoL were tested by performing confirmatory factor analyses, calculating the average values of the variance extracted, validity, reliability and correlation coefficients between scales. Results: Four factors (Emotional, Mind/Body, Relational and Social) were obtained. All statistics presented adequate values (RMSEA and SRMR lower than 0.09 and CFI and TLI higher than 0.9). Reliability was demonstrated with the CR index of each factor higher than 0.7 and the AVE higher than 0.5. Conclusions: Core FertiQol presents an acceptable adjustment for Spanish women with fertility problems. It offers a communication channel between professionals and patients, as well as accurate information in its subscales on areas that may reflect QoL impairment. Core FertiQol is an instrument that allows the identification of areas requiring support, providing valuable information for the design of efficient socialhealth value-centered programs for people with infertility. Introduction. The inclusion of students with disabilities in higher education is a fundamental right recognised by the legal system since its recognition in the United Nations Convention on the Rights of Persons with Disabilities. However, the measures adopted by European countries to promote their incorporation are not always accompanied by parallel training actions that provide university professors with the necessary knowledge to incorporate people with intellectual disabilities into the classroom with the same guarantees and opportunities as people without intellectual disabilities. Objective. This paper aims to provide specific data on the self-perceived training needs of university teaching staff and thus lay the foundations for a specific training programme. Methods. A cross-sectional study was carried out by means of a survey designed to collect the teachers' perceptions of their own competences and the effectiveness of their knowledge, as well as the importance they attached to some aspects of intellectual disability. The survey was administered to teachers in Serbia, the Netherlands, Portugal, Italy and Spain, with a total sample of 1009 teachers. Results. The results obtained showed that the perception of self-perceived competence in educational skills is dependent on three main factors: previous specific training, teaching experience with people with intellectual disabilities and own personal experiences. Conclusion. The present study demonstrated the concern and need of the teaching staff to obtain specific training on people with intellectual disabilities in higher education.

Objetivo: Esta revisión busca proporcionar información actual de los avances de los últimos diez años reportados para mejorar el diagnóstico de enfermedad renal con marcadores como la cistatina C, en comparación con otros analitos convencionales utilizados en la práctica clínica.Materiales y métodos: Se llevó a cabo una búsqueda de artículos originales y de revisión en idioma inglés y español de los últimos diez años en las siguientes bases de datos: MedLine, Library Plus, ProQuest, NCBI y ScienceDirect.Resultados: Para la evaluación de la función renal se usan marcadores convencionales como la creatinina y el nitrógeno ureico en sangre. Sin embargo, estas pruebas están sometidas a diferentes fuentes de variabilidad biológica, razón por la cual se ve la necesidad de identificar otros marcadores que permitan detectar de forma precoz la lesión renal, y que no se vean influenciados por factores dependientes del paciente como la cistatina C, una proteína de bajo peso molecular propuesta como un marcador de función renal más exacto, específico y sensible que la creatinina en la estimación de la tasa de filtración glomerular.Conclusión: Se considera que la cistatina C cumple con características que la hacen un biomarcador más útil en la determinación de lesión renal, ya que muestra mayor utilidad diagnóstica que la creatinina sérica para detectar daño renal temprano, sin embargo, no se utiliza como prueba rutinaria en el diagnóstico de patologías renales por su alto costo al momento de realizar un montaje en el laboratorio clínico.

Scholarly work on the conquest of Muslim cities in the so-called Castilian Reconquista has focused largely on political consequences rather than conquest rituals. Against the previous background, this article turns attention toward civil and religious rituals associated with the Christian conquest of Muslim cities as an expression of triumph. Among these rituals, the conversion of the congregational mosques has been discussed in chronicles and liturgical books that reveals the role of liturgy to understand both appropriation and sacralization of the mosque to remove these places from Muslim control, restoring the Christian faith in the new churches. These rituals are an evident legacy of Roman law modified in late antiquity, and this paper’s main aim is to highlight the re-use of preexisting Church consecration ceremonies gathered in the Roman Pontifical in order to clean up the “Mohammedan filth” applied to post-Reconquista churches.

Objective. To examine the expression of hypoxia-inducible factor-1α (HIF-1α), TfR1, and TfR1-attached terminal monosaccharides in placentas of women with IDAP and severe preeclampsia. Methods. TfR1 and HIF-1α were detected by western blot. Immunoadsorption of TfR1 was performed to characterize the terminal monosaccharides by specific lectin binding. Results. There was no difference in the expression of TfR1 and HIF-1α between groups. Lectin blot analysis pointed out an overexpression of galactose β1-4 N-acetylglucosamine (Gal-GlcNAc) and mannose in severe preeclampsia. Conclusion. The increase in Gal-GlcNAc may be due to the increased presence of antennary structures and the mannose glycans of TfR1 may indicate the presence of misfolded or incomplete proteins. These findings may be associated with the low expression of placental TfR1 in women with preeclampsia.

Este articulo muestra algunos problemas epistemológicos derivados de la explotación de los textos recogidos en el proceso e creación de la base de datos del proyecto POCRAM, relativa a la cuestión de la «conversión religiosa» entre la antigüedad tardía y el fin de la edad moderna. Así mismo se hace un balance de la evolución de las humanidades numéricas (especialmente en Francia), así como de los proyectos precedentes en que se inspira la conceptualización de la base. Presentamos nuestro Corpus y las consecuencias de su explotación gracias al uso de diferentes software que provocan una doble ruptura, epistemológica y conceptual en la forma de entender la historia.

Single-field inflation, arguably the simplest and most compelling paradigm for the origin of our Universe, is strongly supported by the recent results of the Planck satellite and the BICEP2 experiment. The results from Planck, however, also confirm the presence of a number of anomalies in the Cosmic Microwave Background (CMB), whose origin becomes problematic in single-field inflation. Among the most prominent and well-tested of these anomalies is the Cold Spot, which constitutes the only significant deviation from Gaussianity in the CMB. Planck's non-detection of primordial non-Gaussianity on smaller scales thus suggests the existence of a physical mechanism whereby significant non-Gaussianity is generated on large angular scales only. In this Letter, we address this question by developing a localized version of the inhomogeneous reheating scenario, which postulates the existence of a scalar field able to modify the decay of the inflaton on localized spatial regions only. We demonstrate that if the Cold Spot is due to an overdensity in the last scattering surface, the localization mechanism offers a feasible explanation for it, thus providing a physical mechanism for the generation of localized non-Gaussianity in the CMB. If, on the contrary, the Cold Spot is caused by a newly discovered supervoid (as recently claimed), we argue that the localization mechanism, while managing to enhance underdensities, may well shed light on the rarity of the discovered supervoid.
