2. Biochemical profile differences during the transition period based on different levels of non-esterified fatty acids at 7 weeks before parturition in Mediterranean Italian dairy buffaloes (Bubalus bubalis)

Metabolic adaptations to negative energy balance, as well as lipomobilization, influence inflammatory responses, immune function, and oxidative stress in animals. This study aimed to evaluate the biochemical profile of Mediterranean buffaloes with different levels of lipomobilization from the prepartum to the postpartum period. A total of 76 Mediterranean buffaloes were enrolled, and a weekly blood sample was taken from 7 weeks before to 6 weeks after calving. The concentration of non-esterified fatty acids (NEFAs) was determined in serum and was used to categorize buffaloes into three lipomobilization groups 7 weeks before calving: mild (NEFA-I; NEFA ≤ 0.29 mEq/L; n = 18), medium (NEFA-II; 0.29 0.29 mEq/L; NEFA-II and NEFA-III) at 7 weeks before calving should be monitored more closely to reduce the risk of metabolic diseases. Furthermore, the medium (NEFA-II) and severe (NEFA-III) lipomobilization groups could be associated with differences in the animals’ ability to manage their metabolic status. Specifically, the severe mobilization group was most associated with a greater energy deficit during both the prepartum and postpartum periods without oxidative stress. On the contrary, the medium mobilization group was associated with a less severe energy deficit but was also associated with an inflammatory status and oxidative stress during the prepartum period. These distinctions highlight the need for tailored management strategies to address varying levels of metabolic stress in dairy buffaloes.

Abstract Vaginoscopy allows an intravaginal endoscopic evaluation and can help in the diagnosis of female bovine genital tract disorders. The aim of this study is to validate a new method of gynaecological examination using a Scope VOR&GDI videovaginoscope. Twenty‐six heifers were used, divided into 2 groups with 13 animals, control group (GC) and videovaginoscopic group (GV). In the CG, vaginoscopy was performed with a vaginal speculum and in the GV with a Scope VOR&GDI videovaginoscope. All heifers underwent vaginoscopy on day 0 (D0), and 9 days later, on day 9 (D9). Vaginoscopy provided adequate intravaginal inspection. In the CG, 23% (3/13) of the heifers showed discomfort during the test. In GV, vaginal inspection was better due to better image quality. The videovaginoscope is the most effective equipment for carrying out the vaginoscopy procedure in Nellore heifers, as it produces sharper and clearer images and can help in the diagnostic and therapeutic routine of veterinarians.

ObjectivesThe current European crisis in human resources in health has opened the debate about working conditions and fair wages. This is the case with Resident doctors, which have faced challenges throughout Europe. In Portugal, they account for about a third of the doctors in the Portuguese National Health Service. No studies to date objectively demonstrate the working conditions and responsibilities undertaken. This study aims to quantify the residents' workload and working conditions.MethodsObservational, retrospective cross-sectional study which involved a survey on the clinical and training activity of Portuguese residents, actively working in September 2020. The survey was distributed through e-mail to residents' representatives and directly to those affiliated with the Independent Union of Portuguese Doctors. The descriptive analysis assessed current workload, and logistic regression models analyzed associations with geographical location and residency seniority.ResultsThere were a total of 2,012 participants (19.6% of invited residents). Of the residents giving consultations, 85.3% do so with full autonomy. In the emergency department, 32.1% of the residents work 24 h shifts and 25.1% work shifts without a specialist doctor present. The use of recombinant bovine somatotropin (rbST) leads to an increase in variable amounts of milk production in buffalo, but there is a lack of information on the influence of rbST on their metabolism. This study looked at the effects of a single 500 mg dose of rbST on the lipid profile, liver and kidney function, and physical, chemical, and cellular constitution of milk in 14 buffalo over 14 days, from the 100th day of lactation, compared with 14 animals in a control group. From the first day after rbST, there was a rise in beta-hydroxybutyrate (β-HBO), possibly due to higher dry matter intake or the biotransformation of NEFA into β-HBO. The treatment did not influence blood glucose, non-esterified fatty acids (NEFAs), triglycerides, cholesterol, total protein, albumin, AST, GGT, bilirubin, urea, or creatinine levels. In 71.3% of the buffalo, there was a gradual increase in milk production, with the maximal response occurring in the first week followed by a gradual decrease, whilst in 21.4%, the increase in production occurred between 7 and 10 days. Only 7.1% of the animals did not respond. On the 3rd, 5th, 7th, and 10th days after treatment, an increase was found in daily milk production between the two groups equal to 1.04, 1.52, 1.42, and 1.06 L, respectively. In relative terms, this means an increase in milk production, respectively, of 15.1%, 21.0%, 19.8%, and 15.1%. The constitution of the milk showed no difference in the amounts of fat, lactose, total solids, or somatic cell count; however, on the third day after rbST administration, there was a decrease in protein. Notably, from the fifth day, the protein values showed no statistical difference. It can be concluded that the use of rbST in buffalo from the 100th day of lactation is metabolically safe since the treatment neither caused imbalances in fat metabolism nor overloaded the liver or renal function, and the changes in milk composition were transient and limited to a decrease in milk protein.

Abstract Background Bovine tuberculosis (bTB), is a worldwide disease caused by Mycobacterium bovis (M. bovis). The success of bTB eradication and control programs is based on early detection and the removal of reactors from a herd thus routine testing and cull strategy have been applied globally. Since the late nineteenth century, the Tuberculin Skin Test (TST) has been the primary antemortem test available to support bTB eradication campaigns. Due to the TST limitations in terms of Se and Sp, the credibility of the diagnosis is frequently questioned given the occurrence of false-positive and false-negative reactions, therefore, it is necessary to confirm reactive animals using other methods, ensuring the reliability of the diagnosis. The aim of this study was to evaluate the sensitivity and specificity of a multiplex enzyme-linked immunosorbent assay (ELISA) relative to the tuberculin test used for the diagnosis of tuberculosis in cattle in Brazil. Results Lack of agreement between comparative cervical tuberculin test and ELISA IDEXX TM was observed. The 2 animals positive on the comparative cervical tuberculin test did not react at the ELISA IDEXX TM and 22 negative reactors by comparative cervical tuberculin test were positive by the ELISA IDEXX TM. The ELISA IDEXX TM showed sensitivity that is significantly lower than the official screening test the single cervical tuberculin. ELISA IDEXX TM also detected infected animals and herds undetected by the comparative cervical tuberculin test. The parallel use of comparative cervical tuberculin test and ELISA IDEXX TM increased sensitivity and the feasibility bTB screening. Conclusions The results obtained here suggest that the ELISA IDEXX TM may be a supplemental test for the detection of Mycobacterium bovis infection in regions without routine testing and slaughter, where the disease generally progresses to more advanced stages and antibody responses are likely to be more prevalent. Evidence to support the validation of the ELISA IDEXX™ as a supplemental test for bTB eradication programs was provided.

Cutaneous squamous cell carcinoma represents the second most common nonmelanoma skin cancer, with increasing incidence. Radiation therapy has an important role in the management of these patients either in a curative, adjuvant or palliative setting. We present the case of an 88-year-old woman with an extensive periorbital squamous cell carcinoma who did not have physical conditions to make an appropriate radiation therapy treatment with adequate positioning. For that reason, an unusual approach was taken, treating the patient in the hospital stretcher with a direct electron beam of 6 MeV and a dose of 30 Gy/5 Fr. The patient achieved a complete clinical response within 4 months. This case shows that although we are facing a continuous evolution of linear accelerators, special radiotherapy techniques and increasingly effective immobilization systems, in some cases we need to rethink our strategy and use older techniques to be able to treat the patient.

This paper presents the HybriD-GM model conception, from modeling to consolidation. The D-GM environment is also extended, providing efficient parallel executions for quantum computing simulations, targeted to hybrid architectures considering the CPU and GPU integration. By managing projection operators over quantum structures, and exploring coalescing memory access patterns, the HybriD-GM model enables granularity control, optimizing hardware resources in distributed computations organized as tree data structures. In the HybriD-GM evaluation, simulations of Shor’s and Grover’s algorithms achieve significant performance improvements in comparison to the previous D-GM version, and also with other related works, for example, LIQUi|⟩ and ProjectQ simulators.

This study aimed to evaluate passive immunity transfer in healthy buffalo calves. Colostrum samples from heifers (without previous calving) and primiparous and pluriparous dams and blood samples from their offspring were obtained at calving, before colostrum intake, and at 24, 48, and 72 h after calving for determination of serum activities of gammaglutamyltransferase and alkaline phosphatase and serum concentrations of total protein (TP), immunoglobulin A (IgA) and IgG, and lactoferrin. The results were analyzed as repeated measures, and differences were considered statistically significant at P ≤ 0.05. Considering that the buffalo calves were born hypogammaglobulinemic (4.23 ± 0.33 mg/ml) and, at 24 h, the mean serum concentration of IgG was 34.5 ± 1.48 mg/ml, passive immunity transfer was successful. Moreover, colostrum IgG concentrations at 0 h were correlated with serum IgG concentrations at 24 h in buffalo calves. Additionally, TP concentrations were highly correlated with IgG in both colostrum at calving and blood in calves at 24 h. TP is recommended as a reliable indirect parameter to evaluate both colostrum quality and passive immunity transfer in buffalo calves.

The aims of this study were to establish the prevalence of anti-bovine viral diarrhea virus (BVDV) antibodies (Ab) in beef cattle raised in Pará state, to compare the prevalence of seropositive animals to BVDV using a commercial indirect enzyme-linked immunosorbent assay kit (iELISA) and the virus neutralization (VN) test, and finally, to determine the sensitivity (Se) and specificity (Sp) of the iELISA for the detection of anti-BVDV Ab using VN as a gold standard. A total of 400 serum blood samples from Nelore cows aged at least 24 months from five farms in the Pará state from two mesoregions (Metropolitan Region of Belem and Northeast of Pará) were analyzed. All animals were vaccinated against brucellosis and foot-and-mouth disease. The examination of anti-BVDV Ab with VN was performed in the Laboratory of Bovine Viruses of the Biological Institute of Sao Paulo as described in the Manual of Diagnostic Tests and Vaccines for Terrestrial Animals. For VN, bovine kidney epithelial cells from the Madin Darby Bovine Kidney (MDBK) strain were used. The determinations of anti-BVDV Ab were performed with the iELISA test at the Laboratory of Immunology and Microbiology of the Federal Rural University of Amazonia according to the manufacturer's recommendations. The results were classified as follows: (a) correct positive diagnosis, (b) incorrect positive diagnosis, (c) correct negative diagnosis, and (d) incorrect negative diagnosis, according to the results obtained from VN. From the values obtained from VN and iELISA, Se [(a ÷ a + d) × 100], Sp [(c ÷ c + b) × 100], positive predictive value [(a ÷ a + B) × 100], and negative predictive value [(c ÷ c + d) × 100] were calculated for iELISA. The frequencies (%) of seropositive animals were determined and compared both between the different tests (iELISA and VN) and between the different farms (1, 2, 3, 4, and 5). The statistical analysis was performed with a significance level of 5%. The prevalence of seropositive animals was found to be different (P < 0.0001) using VN (39.25% [157/400]) and iELISA (54.50% [218/400]). It was observed that the Se and Sp of the iELISA assay were 98.72% and 74.07%, respectively. Of the total, 25.93% (63/243) of the samples were considered false-positive and 1.27% false-negative (2/157). It was concluded that the BVDV infection is present in beef cattle herds of the state of Para. Based on the speed of execution, ease of handling, and high Se of the iELISA, it is suggested that this assay can be used as a screening test for the detection of anti-BVDV Ab with the aim of eliminating infected animals from large herds of beef cattle.

11. Flexible Fixation for Ligamentous Lisfranc Injuries

Category: Midfoot/Forefoot Introduction/Purpose: Surgical outcomes of Lisfranc injuries depend on achieving an anatomical and stable reduction. Severity of injury defines the best treatment option and surgical techniques vary from internal fixation to arthrodesis. While effective, limitations of commonly used metal implants include iatrogenic articular cartilage damage, implant breakage and need for implant removal. Additionally, these surgical techniques do not preserve tarsometatarsal (TMT) joint motion. This report demonstrates a novel, easy and inexpensive surgical technique to stabilize TMT joint instability utilizing “flexible fixation” utilized in a case series of 8 patients. Methods: A dorsal approach over the second metatarsal is performed in a standard fashion, after careful dissection and direct visualization of the joint, the TMT joint is anatomically reduced and stabilized with k-wires avoiding damage of the cartilage. Two 2.7 or 3.5 screws with washers are placed from dorsal to plantar in the base of the metatarsal and in the respective tarsal bone to serve as posts. A non-absorbable #2 FiberWire (Arthrex, Naples, Florida) is looped 3 to 5 times in a Figure-of-8 fashion around the screws beneath the washers, tensioned, knotted and secured by tightening the screws. Repeat fluoroscopic stress views are performed to ensure stability of the TMT joint. Results: N/A Conclusion: For selected patients with Lisfranc injuries demonstrating no significant comminution or gross instability, flexible fixation using sutures is a promising technique due its potential benefits over trans-articular screws or bridge-plating techniques. This technique is inexpensive, avoids iatrogenic cartilage damage and subsequent removal of hardware.

12. The Captain’s View: A Novel Radiographic View to Assess Axial Alignment of the Calcaneus

Category: Hindfoot, Trauma Introduction/Purpose: The axial alignment of the calcaneus has paramount importance in the management of these fractures. The Harris view has long stood as the recommended radiograph to assess axial alignment. However, given the obliquity at which the radiograph is obtained, it doesn´t represent a true axial view and is subject to inaccuracies secondary to rotational malpositioning of the foot and mismeasurement of angulation. Multiple reports have described the axial alignment as a surgical outcome, but usually this assessment of the residual deformity have no described method. The objectives of this study are to evaluate the capacity of Harris view to assess axial alignment in a cadaveric model and to describe the use of a true AP view of the calcaneus that we have named Captain´s view. Methods: Five below knee amputated fresh-frozen cadaveric specimens were used in the study. For each specimen, the soft tissues over the lateral side were removed to access to the lateral wall. A small wedge of the cuboid was removed to visualize the center of the calcaneocuboid articular surface. LCA-guide and a cannulated drill were used to create a tunnel in the axis of the calcaneus. An oblique osteotomy was performed in order to simulate a non-comminuted fracture. Varus deformity was created by inserting solid radiolucent wedges into the osteotomy to create models of 10, 20, and 30 degrees of varus angulation. Harris and Captain views were obtained for each specimen with 0 (control), 10, 20, and 30 degrees of varus malalignment. Measurements of the deformity were made digitally on each fluoroscopic image. Results: The average degrees of varus in Harris views were 10,9 (5,5-16); 11,5 (8,2-13,6); and 18,3 (13,3-23,6) for 10,20 and 30 degrees of deformity respectively. The average degrees of varus in Captain´s view were 13,0 (7,3-20,9); 18,4 (11,7-23,5); and 28,2 (24,4-31,1) for 10,20 and 30 degrees of deformity respectively. The average degrees of error for varus deformity in Harris views were 4,1 (41%); 8,4 (42%) and 11,6 (39%) for 10,20 and 30 degrees of deformity respectively. The average degrees of error for varus deformity in Captain´s views were 4,8 (48%); 3,6 (18%) and 2,8 (8%) for 10,20 and 30 degrees of deformity respectively. Conclusion: The results of this study show a high rate of mismeasurement for both radiographic views. Despite the average angles have a clear correlation with the severity of varus, the wide range of error observed between specimens make this assessment unreliable and inaccurate. We observed an improvement of accuracy of captain´s view for more severe deformities, but not with Harris views which maintain a 40% mismeasurement in all the settings. Therefore, intraoperative Harris views should not be used in isolation to evaluate axial alignment and Captain´s view provides an additional perspective that can be useful to rule out severe deformities.

13. The Anteromedial Cortex of the Calcaneus: A Cadaveric Study

Category: Hindfoot Introduction/Purpose: The irregular morphology of the calcaneus obstructs its clear visualization on radiographic imaging. Particularly challenging is identifying the medial cortex given its concavity and bony overlap of the calcaneal body. Iatrogenic screw penetration of the medial wall of the calcaneus comes with increased risk of injuring the neurovascular bundle and the flexor hallucis longus tendon. Despite all intraoperative precautions, it is not infrequent to see misplaced screws in postoperative imaging. The objective of the present study is to evaluate the normal proportion of the calcaneus width to radiographically define the extent of the medial cortex on axial fluoroscopic imaging. Methods: Eight left sided fresh-frozen below-the-knee amputated cadaveric specimens were utilized for this study. Each specimen was dissected on the medial side of the hindfoot to allow visualization of the medial cortex of the calcaneus. A partial cuboid ostectomy was performed to visualize the calcaneocuboid joint and to install radiopaque markers on the borders of the anterior calcaneal facet. Finally, Harris axial views were obtained. Two independent observers measured the widths of the calcaneus on digital fluoroscopic Harris views at two levels: the maximal width at the sustentaculum tali and the maximal width at the anterior process. A ratio was calculated between these measurements. Results: The ratio between the maximal width at the anterior process and the sustentaculum ranged between 0.42 and 0.52 with an average ratio of 0.47 +/- 0.04. Conclusion: Calcaneal screw misplacement, particularly from lateral to medial, can lead to damage of the neurovascular bundle and the flexor hallucis long tendon ultimately affecting clinical outcomes. The measurements in the present study suggest a lower risk of violating the medial cortex when the length of the screw placed in the anterior process of the calcaneus is less than half of the maximal width of the calcaneus at the sustentaculum tali. As the medial cortex is difficult to visualize on intraoperative fluoroscopy, the clinical use of this normal ratio in axial views could help surgeons to avoid calcaneal screw misplacement.

Com o objetivo de verificar a influência do suplemento mineral consumido na concentração dos macro e microelementos no plasma seminal e nas características do sêmen de búfalos, 60 bezerros de 10 meses de idade foram mantidos sob lotação intermitente por 14 meses, consumindo, ad libitum, dois suplementos minerais distintos e divididos aleatoriamente em dois grupos: G-Bov (suplemento mineral convencional recomendado para bovinos; n=30) e G-Buf (suplemento mineral específico para búfalos; n=30). O consumo do suplemento mineral foi calculado a partir da diferença entre a quantidade de suplemento oferecido e a sobra mensal do cocho. Dos búfalos a partir dos 22 meses de idade foram realizadas oito colheitas de sêmen de cada animal. As médias das variáveis foram analisadas por meio do teste de Kruskal-Wallis, com nível de significância de 5%, também foi analisada a correlação de Pearson entre a concentração de minerais no plasma seminal e as características do ejaculado. Observou-se que os animais do G-Buf mesmo consumindo menos suplemento mineral (G-Bov = 83,18 vs. G-Buf = 77,14 g UA-1 dia-1; P < 0,05) obtiveram maior volume do ejaculado e melhores motilidade, vigor e turbilhonamento. Esta pesquisa apresenta as concentrações de macro e microelementos no plasma seminal de búfalos suplementados com diferentes formulações minerais. Além disso, concluiu-se que as características físicas e morfológicas do sêmen de búfalos jovens são influenciadas pelo tipo de suplementação mineral, apresentando sêmen de melhor qualidade em búfalos suplementados com suplementos específicos para a espécie. Dos minerais presentes no plasma seminal de búfalos, o fósforo é o elemento que mostra maior correlação positiva com as características seminais no ejaculado.

The neonatal period is a challenging phase for calves, and during this phase constant adaptations are required. The aim of the present study was to evaluate the invasive hemodynamics with the Swan-Ganz catheter in neonate calves to understand adaptive changes during the first 30 days of life. A prospective and observational study was conducted with 10 Holstein calves. Assessments of the right atrial pressure (RAP), right ventricular pressure (RVP), pulmonary artery pressure (PAP), pulmonary capillary pressure (PW), cardiac output (CO), heart rate (HR), pulmonary vascular resistance (PVR), and blood gas levels were performed. The analyses of PAP, PVR, PW, HR, sO2, and arterial blood gases differed (p < 0.05) between the evaluated periods. Our results indicated transient pulmonary artery hypertension during the process of extrauterine adaptation during the first 30 days of life. This hypertension must be considered as physiological and consequent to the neonatal adaptation process.

O uso indiscriminado de substâncias psicoativas tornou-se um fenômeno mundial. Apesar de o consumo destas substâncias datar dos primórdios da humanidade, os problemas decorrentes do uso abusivo e da dependência química têm sido relatados recentemente. A dependência química, constituindo-se um grave problema de saúde pública, causa uma série de prejuízos socioeconômicos, ocupacionais, psicológicos e físicos aos seus usuários. Há registros de mudanças ocorridas no perfil dos consumidores de substâncias psicoativas, ao longo dos anos, em decorrência da difusão de variados tipos de drogas e da iniciação do consumo em idade cada vez mais precoce. Neste contexto, desenvolveu-se o presente artigo, com o objetivo de investigar o perfil de usuários de drogas, internos em dois Centros Terapêuticos do Estado do Rio Grande do Norte-Brasil. Buscou-se, em estudos da especializados, estabelecer conexões entre prazer e vício, para uma melhor compreensão da dependência química. A metodologia desenvolve-se sob a forma de estudo documental e a pesquisa empírica baseou-se na coleta de dados extraídos de prontuários médicos de pacientes internos, no período compreendido entre outubro de 2013 a março de 2014, após aprovação do Comitê de Ética em Pesquisa, sob protocolo nº 155/14. O estudo foi desenvolvido com quarenta e dois (n=42) pacientes do sexo masculino, adultos jovens, de meia idade e idosos, internos nos referidos Centros, destinados ao tratamento e à reabilitação de dependentes químicos. A partir das análises realizadas, pôde-se concluir que o perfil mais recorrente de dependentes de substâncias psicoativas, assistidos naqueles Centros, corresponde a indivíduos solteiros, adultos jovens, não brancos, com certo grau de escolaridade, dependentes de cocaína/crack, com histórico de mais de uma internação, consumo iniciado em idade precoce e, quase a totalidade, a partir do uso de álcool. Para se estabelecer a prevalência de anticorpos (Ac) anti-BVDV em rebanhos bovinos de corte no Estado do Pará e comparar a prevalência de animais soropositivos para o BVDV utilizando-se um kit comercial de ELISA-I (“Enzyme-linked Immunosorbent Assay” indireto) frente ao teste de VN (virusneutralização) e determinar a Se (sensibilidade) e Sp (especificidade) do ELISA-I para detecção de Ac anti-BVDV frente a VN, foram analisadas 400 amostras de soros sanguíneos de vacas Nelore com idade mínima superior a 24 meses, vacinadas contra brucelose e febre aftosa, provenientes de cinco fazendas no estado do Pará, localizadas em duas Mesorregiões (Metropolitana de Belém e do Nordeste Paraense). A pesquisa de Ac anti-BVDV determinados pela VN foi realizada no Laboratório de Viroses de Bovídeos do Instituto Biológico de São Paulo, conforme descrito no Manual of Diagnostic Tests and Vaccines for Terrestrial Animals. Foram utilizadas células epiteliais de rim bovino da linhagem Madin Darby Bovine Kidney - MDBK). Já as determinações dos Ac anti-BVDV pelo ELISA-I foram realizadas no Laboratório de Imunologia e Microbiologia da Universidade Federal Rural da Amazônia, conforme recomendações do fabricante. Os resultados verificados foram classificados como diagnóstico positivo correto (a), diagnóstico positivo incorreto (b), diagnóstico negativo correto (c), diagnóstico negativo incorreto (d), em função dos resultados obtidos na VN. A partir desses valores, calculou-se para o ELISA-I a sensibilidade [(a / a + d) x 100], a especificidade [(c / c + b) x 100], o valor preditivo positivo [(a / a + b) x 100] e o valor preditivo negativo [(c / c + d) x 100]. Foram determinadas as frequências (%) de animais positivos, tanto entre os testes (ELISA-I e VN) como entre as diferentes fazendas (1, 2, 3, 4 e 5). A estatística de inferência foi realizada com nível de significância de 5%. A prevalência de animais soropositivos foi diferente (P < 0,0001) entre os teste de VN [39,25% (157/400)] e ELISA-I [54,50% (218/400)]. Observou-se que os valores de Se e Sp do ELISA-I corresponderam a 98,72% e 74,07%, e que 25,93% (63/243) das amostras foram falso-positivas e 1,27% (2/157) falso-negativas. Concluiu-se que a infecção pelo BVDV está presente em rebanhos de corte do estado do Pará. Devido à rapidez, praticidade e alta sensibilidade do ELISA-I, sugere-se que este teste seja utilizado como teste de triagem para a detecção de Ac anti-BVDV, visando identificar os animais infectados de grandes rebanhos de bovinos de corte.

A dependência de substâncias psicoativas constitui um grave problema de saúde pública, e causa uma série de prejuízos socioeconômicos, ocupacionais, psicológicos e físicos para os usuários de tais substâncias. A demanda por tratamento tem aumentado, consideravelmente, concomitante ao desenvolvimento de pesquisas, nesta área de atuação. Tais pesquisas investem em tratamentos que viabilizem maiores taxas de abstinência e menores índices de recaídas. O topiramato vem sendo apresentado como farmacoterapia valiosa no tratamento de dependências de certas substâncias psicoativas, mesmo sem aprovação das Agências Regulamentadoras, para uso com esta finalidade. Partindo dessa constatação, o presente trabalho tem como objetivo apresentar uma revisão literária sobre o uso do topiramato no contexto do abuso e dependência de substâncias psicoativas. A metodologia empregada para a realização deste trabalho consistiu em ampla pesquisa bibliográfica, através de fontes como: livros, revistas e artigos. Foi possível concluir que o topiramato, em virtude de seus múltiplos mecanismos de ação, tem-se revelado como fármaco promissor, no contexto de abuso e dependência de substâncias psicoativas, com destaque especial ao tratamento da adição ao álcool, tabagismo, cocaína, e benzodiazepínicos.

O crack vem se apresentando como droga com elevado poder de dependência aos seus usuários e vem gerando várias complicações orgânicas, psíquicas e sociais. O desejo intenso de consumir a substância e a recordação dos efeitos agradáveis da droga, em meio à sua falta, acarretam uma ânsia extrema para o consumo, denominada fissura ou craving. Neste sentido, o presente trabalho tem como objetivo a investigação dos gatilhos que possivelmente desencadeiam a fissura dos usuários da substância. A coleta de dados para obtenção dos resultados foi realizada através de um questionário, que foi respondido, individual e voluntariamente, pelos pacientes internados para desintoxicação, em virtude do uso de crack, em um hospital psiquiátrico de referência do estado do Rio Grande do Norte. A partir do exposto, foi possível verificar que, dentre os fatores desencadeantes de fissura em usuários de crack, destaca-se o álcool. Podemos concluir que, por esta ser droga lícita e de fácil acesso, as políticas primárias em saúde mental e de educação continuada são peças fundamentais no processo de combate à epidemia de crack que assola nosso país. A identificação junto ao paciente de outros fatores desencadeantes é extremamente relevante para que sejam abordadas terapêuticas adequadas no processo de redução das recaídas por seus usuários.

OBJETIVOS: Determinar a prevalência dos fatores ambliogênicos e ambliopia na área adscrita do Programa Saúde da Família (PSF) da Lapa (RJ) e, estimar na mesma área a sensibilidade e especificidade entre métodos de medida de acuidade visual (tabelas ETDRS e LEA) em crianças pré-escolares. MÉTODOS: Estudo transversal de 93 crianças entre três a seis anos da área adscrita do PSF Lapa, RJ. Todas as crianças foram submetidas à avaliação oftalmológica completa que incluiu: anamnese dirigida, ectoscopia, medida da acuidade visual com tabela de ETDRS e LEA de forma duplo cega, reflexo vermelho, teste de Titmus, refração objetiva sob cicloplegia, refração subjetiva, avaliação da motilidade ocular, biomicroscopia do segmento anterior, fundoscopia sob midríase. Os dados foram coletados na Policlínica Ronaldo Gazolla, da Universidade Estácio de Sá. RESULTADOS: A prevalência dos fatores ambliogênicos foi encontrada e distribuída em: 8,4 % de estrabismo; 11,86% de anisometropia e 15,2% de ametropia. O teste de acuidade visual ETDRS apresentou sensibilidade de 100% e especificidade de 18% para detecção de ambliopia. O teste de acuidade visual LEA apresentou sensibilidade de 100% e especificidade de 30,9% para detecção de ambliopia. CONCLUSÃO: Ambas tabelas ETDRS e LEA podem ser usadas para a triagem de ambliopia porque tiveram 100% de sensibilidade. A especificidade foi maior para a tabela LEA. Porém ainda em nível baixo. Portanto, há necessidade da complementação com o exame oftalmológico completo para confirmar e ratificar a presença de fatores ambliogênicos em pacientes triados positivamente. A triagem visual feita pelos Agentes Comunitários de Saúde pode identificar crianças com distúrbios visuais, referenciando para exame oftalmológico completo; em um grupo etário onde a resolução dos problemas visuais se apresenta como prioridade. O Programa de Saúde da Família pode ser utilizado para avaliar o estado de visão das crianças brasileiras e seria útil que essa avaliação fosse um marcador estatístico do SUS.OBJECTIVE: To determine amblyogenic factors and amblyopia prevalence in a Health Family assisted community área of Lapa (RJ) and to estimate in the same area the sensiblity and specificity among visual acuity methods (ETDRS versus LEA optotypes) in pre-school children. METHODS: Cross section study of 93 children from three to six years old. All the children were submitted to complete ophthalmological evaluation which included: anamnesis, ectoscopy, double blind visual acuity measurements with LEA and ETDRS optotypes, red reflex, Titmus Test, objective cycloplegic refraction, subjective refraction, ocular motility evaluation, anterior segment biomicroscopy, fundoscopy. The data were collected in Ronaldo Gazolla Policlinic of Estacio de Sá University. RESULTS: The amblyogenic factors prevalence were : 8,4% of strabismus, 11,86% of anisometropia and 15,2% of ametropia. The ETDRS visual acuity test had 100% of sensibility and 18% of specificity for amblyopia detection. Lea visual acuity test had 100% sensibility and 30,9% specificity for amblyopia detection. CONCLUSION: Both optotype tests ETDRS and LEA can be used for amblyopia screening because they had 100% sensibility. The specificity was higher for LEA optotypes. However, the specificity still remained in a low level. Therefore, it is necessary to complement the visual acuity screening with a complete ophthalmological exam in the individuals screened to have amblyopia. Visual screening performed by Health Community Agents could identify children with visual acuity problems, and they could be referred to an ophthalmological complete exam, in an age group that visual acuity problems resolution is a priority. Helth Family Program can be used to evaluate visual acuity children status and it would be useful that this evaluation could be a statiscal SUS mark.

O experimento foi realizado com o objetivo de investigar a dinâmica da infecção por nematódeos gastrintestinais em vacas no periparto. Utilizaram-se 106 vacas de corte, divididas em dois grupos: o grupo 1 (G1), constituído por 42 vacas de primeira e segunda cria; e o grupo 2, (G2) por 76 vacas de terceira cria ou mais. A partir dos 120 dias do pré-parto até os 90 dias pós-parto, coletaram-se fezes para contagens de ovos por grama de fezes (OPG) e sangue para determinação do volume globular e hemoglobina de cada um dos animais a intervalos mensais. Nos mesmos intervalos, avaliaram-se os escores de condição corporal (ECC). Os valores médios ± desvios-padrão de OPG para os animais do grupo 1 foram iguais a 19,4 ± 42,9, enquanto para os do grupo 2 foram de 31,1 ± 68,0 não havendo diferenças significativas entre eles, tampouco entre os valores hematológicos que permaneceram nos padrões normais para ambos os grupos. Maiores contagens de OPG foram observadas no período pós-parto, com médias de 32,5 ± 55,5 e 51,5 ± 84,8 para os grupos G1 e G2, respectivamente. Observou-se diferença significativa (p < 0,05) nos parâmetros estudados, quando foram comparados os períodos pré e pós-parto dentro de cada um dos grupos, com diminuição dos valores hemáticos e escore corporal e aumento dos valores de OPG no pós-parto. Os resultados sugerem que as vacas podem estar mais susceptíveis à infecção por nematoides desde o parto até os 90 dias pós-parto; todavia, vacas adultas, quando bem manejadas, não constituem fator importante na epidemiologia das verminoses gastrintestinais, mesmo no período pós-parto.The experiment was conducted to investigate the dynamics of infection by gastrointestinal nematodes during the periparturition period in cows. One hundred and six beef cows were divided into two groups: G1 was formed by 42 cows of one and two parturitions, and G2 by 76 cows of three or more parturitions. From the 120 days pre partum until the 90 days post partum, feces were collected for faecal egg counts (EPG) while blood was collected to determine the packed cell volume and hemoglobin levels of each animal, with monthly intervals. In the same intervals the body condition scores (BCS) were evaluated. The mean values ± standard deviation of the EPG for G1 were equal to 19.4 ± 42.9, and for G2 31.1 ± 68.0. No significant differences were observed between G1 and G2 in relation to EPG and hematological parameters, which remained within normal patterns for both groups. The two groups had higher counts of EPG in the post partum period than in the pre partum period, with averages of 32.5 ± 55.5 and 51.5 ± 84.8 for groups G1 and G2, respectively. A significant difference (p < 0.05) in the parameters was observed when comparing the pre and post partum within each group studied resulting in declining values of blood and body score and an increase in EPG in the post partum. The results suggest that the cows may be more susceptible to infection by nematodes from giving birth up to 90 days post partum. However, adult cows, when well-managed, are not an important factor in the epidemiology of gastrointestinal nematodes, even in the post partum period.

A sífilis é uma doença sexualmente transmissível causada pela espiroqueta Treponema pallidum. É uma doença bastante remota, com mais de 500 anos de existência. Na transição para o século XVI a infecção tornou-se uma pandemia, porém com a introdução da penicilina, na década de 40, do século XX, o panorama da doença foi plenamente modificado. Nas últimas décadas, a sífilis reapareceu como um problema de saúde mundial. O presente trabalho buscou identificar os possíveis fatores que contribuem para a reemergência da patologia, na atualidade. Realizou-se uma revisão de literatura sobre o tema proposto, entre os meses de janeiro a março de 2007. Constatou-se que, mudanças observadas na sociedade, sobretudo, alterações na estrutura familiar como a liberalização dos costumes e maior valorização das relações afetivo-sexuais, têm apresentado estrita relação com a reemergência da infecção. Encontram-se relacionados, também, fatores demográficos, tais como: o grande número de jovens sexualmente ativos e a migração urbana com mudanças sócio-culturais. Destacam-se, ainda, o aumento do nível de prostituição, da utilização de drogas, tratamentos inadequados das formas primárias da doença, avanços tecnológicos, além da associação ao vírus HIV que pode acelerar e alterar o curso da doença. A sua erradicação requer um maior engajamento de autoridades públicas e de profissionais da área da saúde, tanto na busca de meios de prevenção e tratamentos mais eficazes, quanto de esclarecimentos sobre a reincidência desta patologia e de seus agravos à saúde humana.

A neurossífilis é caracterizada pelo acometimento do sistema nervoso central e definida como extensão da infecção pelo Treponema pallidum. Apresenta-se sob as formas sintomáticas e assintomáticas. Manifesta-se sob três formas: menigovascular, meníngea e parenquimatosa. Estas podem apresentar quadros mistos e, portanto, desencadear sintomatologias diversas, em geral cursando com alterações do estado mental do paciente e, sobretudo, através de manifestações demenciais. Em decorrência da redução no número de infecção pelo Treponema pallidum, após o advento da penicilina, a neurossífilis deixou de ser encarada como patologia preocupante. Logo, muitos especialistas deixaram de pensar nesta enfermidade, contribuindo para diagnósticos equivocados e terapêuticas ineficazes, tratando apenas os sintomas ou evolução clínica do paciente. Entretanto, nas últimas décadas, tem-se verificado um recrudescimento da doença devido à revolução sexual, ao advento dos anticoncepcionais orais e ao tratamento inadequado das fases primárias da sífilis e pela falta de diagnóstico. A partir do exposto, e em detrimento da eminente semelhança entre os quadros apresentados pelos portadores de demência e apresentados pelos portadores de neurossífilis, o presente trabalho buscou investigar a existência de diagnóstico diferencial para neurossífilis em pacientes portadores de demência em um hospital psiquiátrico no município de João Pessoa entre os períodos de 1980 a 2006. Foi possível constatar que, no período investigado pelo estudo, não houve diagnóstico de neurossíflis e que 100% dos pacientes (16) eram portadores de demência, mas que, em sua totalidade, não apresentavam em seus prontuários a realização dos exames para diagnóstico diferencial da demência por invasão do sistema nervoso central pelos Treponemas das demências de outras etiologias. Os exames investigados, foram: o reflexo pupilar, VDRL e análise do líquido cefalorraquidiano. Desta maneira, foi comprovada a inoperância por parte de profissionais de saúde no diagnóstico diferencial para neurossífilis, naquela instituição, durante o período recortado pelo estudo

Benzidamina é um medicamento antiinflamatório não esteroidal usado no tratamento de sintomas inflamatórios. A descoberta do uso abusivo desse fármaco na medicina com finalidade não terapêutica foi objeto de estudo do nosso trabalho. Este trabalho pretende informar à população sobre o uso inadequado do cloridrato do benzidamina, indicando os efeitos terapêuticos e adversos desse medicamento. Para alcançar os objetivos propostos foi necessária a aplicação de um questionário enfatizando alguns requisitos relevantes a serem abordados. Foram entrevistadas dezoito pessoas que fizeram uso abusivo da substância, entre jovens e adultos, todos residentes na cidade de João Pessoa. Foi verificado que os riscos do uso abusivo desse fármaco na medicina estão relacionados a uma superdosagem da droga. Um outro fator de risco esta relacionado à duração e à intensidade das alucinações provocadas pelo uso abusivo da benzidamina.

OBJETIVO: Fazer uma revisão da literatura para diagnóstico de simulação em lombalgia e avaliar sua prevalência nos pacientes que pleiteiam benefício previdenciário. MÉTODOS: Foram avaliados 100 pacientes com lombalgia referida, divididos em dois grupos de 50; no grupo I os pacientes requisitaram o relatório médico a fim de obter benefício, no grupo II os pacientes visavam apenas tratamento. Foram considerados simuladores os pacientes que apresentavam dois sinais positivos, dos três avaliados: dor lombar à compressão axial crânio, dor lombar à rotação em bloco do tronco e diferença em sintomatologia na elevação da perna em posição sentado e supina. RESULTADOS: No grupo I a porcentagem de simuladores foi 72%, no grupo II 18%, não houve diferença significativa com relação a idade ou sexo dos pacientes. DISCUSSÃO: Apesar de haver vários sinais e sintomas para avaliar simulação a maioria dos médicos os desconhece, não fazendo uma avaliação adequada dos pacientes com lombalgia referida, contribuindo assim para o inchaço do sistema previdenciário. CONCLUSÃO: Há uma elevada incidência de simuladores entre os pacientes que requerem relatório médico a fim de conseguir benefício previdenciário. Nivel de Evidência II, prospectivo comparativo.OBJECTIVE: To carry out a literature review for the diagnosis of low back pain simulation in patients claiming disability benefit from the social security. METHODS: 100 patients with reported low back pain were evaluated. Thesewere divided in two groups of 50 each; in group I, the patients requested a medical report for the purpose of obtaining disability benefit; in group II the patients were interested only in receiving treatment. Simulators were considered as patients who presented at least two positive signs of the three signs assessed: low back pain on axial cranial compression,low back pain on rotation of the pelvis, and difference in straight leg raising in the supine and sitting positions. RESULTS: In group I the percentage of simulators was 72%, in the group II, 18%. There was no significant difference in relation to age or sex. DISCUSSION: Although there are several signs and symptoms to evaluate simulation, most of the doctors ignored them. However, without them, it is difficult to make a correct evaluation of patients with reported low back pain, thereby contributing to overburdeningthe socialsecurity system. CONCLUSION: There is a high incidence of simulators among patients who request medical reports in order to obtain disability benefits. Level of Evidence: Level II, prospective comparative study.

A população de bubalinos estimada no Brasil é de aproximadamente 3 milhões de animais, encontrando-se distribuídos em todos os Estados brasileiros, com crescimento médio anual de 12%. Apesar disso, os trabalhos realizados buscando os avanços na bubalinocultura são escassos. Em função da complexidade etiológica da diarreia em bubalinos e da falta de informações recentes nesta área, o presente estudo teve como objetivo avaliar a ocorrência dos principais agentes bacterianos e parasitários envolvidos na diarreia de bezerros búfalos lactentes, de explorações leiteiras semi-intensivas e intensivas em regiões dos estados de São Paulo e Paraná. De março de 2010 a junho de 2011, foram colhidas 53 amostras para exame coproparasitológico e 46 amostras para o exame bacteriológico de animais com quadro de diarreia nos municípios paulistas de São João da Boa Vista, Dourado, Pirassununga, Registro, Pariquera Açu, Pilar do Sul e uma propriedade no estado do Paraná, município de Santana do Itararé. No exame parasitológico, 45,28% (24) foram positivos para Eimeria spp., 26,42% (14) para Strongyloidea e 1,88 (1) para Toxocara vitulorum. No exame bacteriológico, 97,83%, (45) das amostras foram positivas para E. coli, contudo, somente duas foram consideradas patogênicas (E. coliSTEC). Em uma amostra (2,17%) isolou-se Klebsiella pneumoniae; já a presença de Salmonella spp. não foi constatada. Para o presente estudo, a presença de endoparasitas foi bastante relevante, principalmente os casos Eimeria spp., sendo a higiene das instalações e falhas de manejo fatores importantes na ocorrência de diarreia em bezerros búfalos no estado de São Paulo.

Metabolic adaptations to negative energy balance, as well as lipomobilization, influence inflammatory responses, immune function, and oxidative stress in animals. This study aimed to evaluate the biochemical profile of Mediterranean buffaloes with different levels of lipomobilization from the prepartum to the postpartum period. A total of 76 Mediterranean buffaloes were enrolled, and a weekly blood sample was taken from 7 weeks before to 6 weeks after calving. The concentration of non-esterified fatty acids (NEFAs) was determined in serum and was used to categorize buffaloes into three lipomobilization groups 7 weeks before calving: mild (NEFA-I; NEFA ≤ 0.29 mEq/L; n = 18), medium (NEFA-II; 0.29 < NEFA < 0.57mEq/L; n = 20), and severe (NEFA-III; NEFA ≥ 0.57mEq/L; n = 38). Two-way repeated measures ANOVA was used to assess changes within and between the groups and over time. Significant differences were found in the concentration levels of NEFA, β-hydroxybutyrate, glucose, cholesterol, protein profile, oxygen radicals, antioxidants, lysozyme, complement, and minerals. These results suggest that both medium and severe lipomobilization groups are associated with metabolic alterations. In conclusion, buffaloes with higher NEFA levels (>0.29mEq/L; NEFA-II and NEFA-III) at 7weeks before calving should be monitored more closely to reduce the risk of metabolic diseases. Furthermore, the medium (NEFA-II) and severe (NEFA-III) lipomobilization groups could be associated with differences in the animals' ability to manage their metabolic status. Specifically, the severe mobilization group was most associated with a greater energy deficit during both the prepartum and postpartum periods without oxidative stress. On the contrary, the medium mobilization group was associated with a less severe energy deficit but was also associated with an inflammatory status and oxidative stress during the prepartum period. These distinctions highlight the need for tailored management strategies to address varying levels of metabolic stress in dairy buffaloes. [ABSTRACT FROM AUTHOR]

O rabdomioma cardíaco se destaca sendo o principal tumor primário benigno do coração. Quando múltiplos, estão fortemente correlacionados a esclerose tuberosa, uma condição genética de alteração dos genes TSC-1 e TSC-2. Essas mutações influenciam diretamente no cresciemento celular e por fim acabam produzindo tumorações pelo organismo, estando assim fortemente correlacionada aos rabdomiomas cardíacos multiplos. Com isso, essa revisão literária tem como objetivo esclarecer como essas patologias se associam, suas consequências, seu diagnóstico e tratamento. Levando em consideração o caratér maléfico das patologias associadas, destaca-se a importância do conhecimento acerca do assunto para melhor compreensão dessas condições raras, para que se possa fazer o manejo desses pacientes corretamente. Principalmente quanto ao seu diagnóstico, que deve ser realizado precocemente com ecocardiogramas na vida intra-uterina a partir da 17ª semana e seu tratamento de acordo com o comprometimento cardíaco fetal, sendo por muitas vezes necessária a utilização de tratamento cirúrgico, mesmo sendo uma patologia que pode regredir de forma expontânea. Ademais, novas estratégias terapeuticas vem sendo estudadas para o tratamento do rabdomioma associada a esclerose tuberosa com a utilização de Everolimus e Sirulimus, com expectativas otimistas acerca do desfecho desses pacientes.

Dynamic consolidation of virtual machines (VMs) is an effective way to improve resource utilization and power efficiency in cloud computing. For example, determining when it is best to relocate VMs from an overloaded host is one aspect of dynamic VM consolidation that directly influences the resource utilization and Quality of Service (QoS) offered by the system. This paper presents a new proposal with an Interval-valued Fuzzy Logic approach to address the uncertainties and inaccuracies in determining resource usage, aiming at energy savings with minimal performance degradation. The results obtained in a simulated cloud computing environment showed improvements for energy efficiency of 2.3% with Inter Quartile Range (IQR). With regard to Service Level Agreement (SLA), results of 10.1% were achieved with Threshold Policy (THR), and finally, IQR obtained 7.9% better results considering the reduction in the number of VM migrations.

Purpose: Media products meaning lies on the interpretation of its content. As productive consumers, fans reframe them. In the process, they produce new texts on these products, known as paratexts. The research aims to understand how fan’s paratexts (re)produces discourses to re-signify media products’ (para)texts. For such, we look at fan paratextualization of Game of Thrones (GoT), an economic and cultural phenomenon with huge repercussions on social media. Design/methodology/approach: Through a Foucauldian Archeology, we collected and analyzed comments posted by GoT fans on promotional videos published on the TV series' official YouTube channel. Findings: The fans' paratextual production presents different positions on the consumed cultural product: a public tribute highlighting its cultural legacy that is based on feelings of nostalgia and frustration with the conclusion of its plot. We conclude that these paratexts worked as a reckoning for the TV series’ fans. Broadly, it is possible to interpret paratexts as marketing content that allow fans to act as prosumers of media products, the entertainment industry, and fans' culture. Originality/value: The study presents how the fans’ productive consumption is materialized through the paratext (re)production that both establish the resonance of media objects in the culture of fans and lead them to assume the role of co-authors of the texts they consume. This role reinforces how fans should be understood as consumers who can act freely in relation to marketing actions. Moreover, it implies that paratexts can be produced to stimulate the discursive production that works as prosumer performance of fans.
