The present randomized, single-center, and single-blinded clinical trial tested the hypothesis that tele-supervised home-based exercise training (exercise) is an effective strategy for improving cardiovascular, respiratory, and functional capacity parameters in individuals who were hospitalized due to coronavirus disease 2019 (COVID-19).Thirty-two individuals (52 ± 10 yr; 17 were female) randomly assigned to exercise ( n = 12) or control groups ( n = 20) had their anthropometric (weight, body mass index), hemodynamic (brachial and central blood pressure), vascular (arterial stiffness), ventilatory (pulmonary function and respiratory muscle strength), and functional parameters (handgrip strength, five-time sit to stand, timed up and go test, and 6-min walking test) assessed at baseline (30-45 d of hospital discharged) and after 12 wk of follow-up.Both groups similarly increased ( P0.001) forced vital capacity (absolute and percent of predicted), forced expiratory volume in the first second (absolute and percent of predicted), and handgrip strength during follow-up. However, only the exercise group reduced carotid-femoral pulse wave velocity (-2.0 ± 0.6 m·s -1 , P = 0.048) and increased ( P0.05) resting oxygen saturation (1.9% ± 0.6%), mean inspiratory pressure (24.7 ± 7.1 cm H 2 O), mean expiratory pressure (20.3 ± 5.8 cm H 2 O), and percent of predicted mean expiratory pressure (14% ± 22%) during follow-up. No significant changes were found in any other variable during follow-up.Present findings suggest that tele-supervised home-based exercise training can be a potential adjunct therapeutic to rehabilitate individuals who were hospitalized due to COVID-19.

Methods: A total of 109 hospitalized COVID-19 patients in noncritical status were cross-sectionally studied. Clinical data (age, sex, comorbidities, and medications) and BMI were assessed. Laboratory tests included serum hemoglobin, leukocytes, lymphocytes, platelets, C-reactive protein, ferritin, D-dimer, and creatinine. Physical status was evaluated using a handgrip dynamometer. Endothelial function was assessed noninvasively using the flow-mediated dilation (FMD) method. Results: The sample average age was 51 years, 51% of patients were male, and the most frequent comorbidity was obesity (62%). Univariate analysis showed association of lower FMD with higher BMI, hypertension, use of oral antihypertensive, higher blood levels of creatinine, and larger baseline artery diameter. After adjusting for confounders, the multivariate analysis showed BMI (95% CI: −0.26 to −0.11; p < 0.001) as the major factor associated with FMD. Other factors associated with FMD were baseline artery diameter (95% CI: −1.77 to −0.29; p = 0.007) and blood levels of creatinine (95% CI: −1.99 to −0.16; p = 0.022). Conclusions: Increased BMI was the major factor associated with endothelial dysfunction in noncritically hospitalized COVID-19 patients. This may explain one of the pathways in which obesity may increase the risk for severe COVID-19. Contextualização: o exercício muscular inspiratório com carga linear (EI) vem sendo amplamente inserido no tratamento fisioterápico respiratório para diversas anormalidades e situações clínicas respiratórias. Entretanto, é possível notar que há diferentes protocolos de prescrição referente à forma de execução, por tempo ou repetição. Objetivo: revisar sobre as formas de execução do EI nas diferentes condições de saúde e analisar as respostas das variáveis estudadas. Método: três revisores buscaram ensaios clínicos controlados e randomizados nas bases de dados PubMed, SciELO (Scientific Eletronic Library), PEDro, Scopus, Cochrane e Bireme, avaliando também sua qualidade metodológica (escala de PEDro). Resultados: foram encontrados 340 artigos e, após verificar os critérios de elegibilidade, foram incluídas 17 pesquisas que avaliaram o efeito do EI. Não houve singularidade nas variáveis estudadas. Nos estudos analisados, havia protocolos de EI realizado por tempo ou repetição: sendo, em média, 15 minutos ou 3 a 5 séries de 10 respirações respectivamente. Os desfechos mais evidentes foram aumento na pressão inspiratória e expiratória máxima (PImáx e PEmáx), aumento na capacidade funcional e redução na sensação de dispneia. Conclusão: para a execução do EI, há variedade de dispositivos; e as prescrições são por tempo ou número de repetições, com predomínio à repetição. Os desfechos são significativos e relevantes à prática clínica, porém, não foram identificados, até o presente momento, estudos que têm avaliado designadamente os efeitos nas formas de execução do EI.

Objetivo: analisar a resposta aguda das variáveis cardiorrespiratórias entre o exercício respiratório realizado por número de repetições e por tempo em sujeitos com Distúrbios Respiratórios Obstrutivos (DRO). Método: estudo uni-cego e cruzado com 31 sujeitos – VEF1/CVF inferior a 0,70 do previsto. Foram mensurados: dispneia, pressão arterial sistêmica (PA), pressão inspiratória máxima (PImáx), capacidade vital lenta (CVL), frequência de pulso (FP) e respiratória (FR), e índice de amplitude (IA) toracoabdominal. Como Eexp, foi realizada a oscilação oral e alta frequência (OOAF) com shaker®. Resultados: no EexpT, houve redução da FP e PAS, e aumento no volume minuto (VM). Já no EexpR, foram observados redução na PAS e aumento na PImáx. No EexpT, a variação foi maior no VM (1,78±2,77 L?min), PEmáx [-20,00(-36,00-0,00)]. No EexpR, a maior variação foi na PImáx (11,61±21,84). Conclusão: o Eexp no DRO promove repercussões cardiorrespiratórias, sendo que a execução por tempo ocasiona maiores mudanças no número de variáveis e, principalmente, na variação do VM, pressão inspiratória e expiratória máxima.

RESUMO Na unidade de terapia intensiva (UTI) pediátrica, a falha de extubação pode aumentar o risco de mortalidade. Este estudo objetivou: (1) verificar a taxa de falha de extubação na UTI pediátrica de um hospital público do município de Bauru (São Paulo, Brasil); (2) identificar a principal causa atribuída à falha de extubação; (3) avaliar se características como a idade e o tempo de ventilação mecânica invasiva (VMI) estão associadas à falha de extubação; (4) avaliar se o tempo de permanência na UTI e hospital é maior entre os pacientes que apresentaram falha de extubação. Foi realizado estudo de coorte retrospectivo com 89 pacientes internados de maio de 2017 até julho de 2018. Os resultados mostraram taxa de falha de extubação correspondente a 16%. A principal causa atribuída à falha de extubação foi o estridor laríngeo, totalizando 57% dos casos. A comparação intergrupos (sucesso vs. falha de extubação) não mostrou diferenças em relação à idade (p=0,294) e ao tempo de VMI (p=0,228). No entanto, observamos que o grupo falha de extubação apresentou maior tempo de UTI (p=0,000) e hospital (p=0,010). Desta forma, concluímos que a taxa de extubação está de acordo com a observada em outros estudos. O estridor laríngeo foi responsável por mais da metade dos casos de falha de extubação. Embora a idade e o tempo de VMI não tenham sido características associadas à falha de extubação, esta contribuiu para o maior período de permanência na UTI e no hospital.

Introduction: Breast cancer is the second most common malignant neoplasm in women. Surgical intervention is one of the recommended treatments, which can lead to significant physical and sensorial sequelae. Objective: To analyze the musculoskeletal, cardiorespiratory, anthropometric and sensorial functions of women who underwent breast cancer surgery. Methods: An observational, cross-sectional study with women who underwent surgical resection of breast tumors at Amaral Carvalho, a reference hospital in the countryside of the São Paulo State, Jaú/SP, Brazil. Three assessments were obtained: pre-surgery (Ass1), one day after the surgery (Ass2) and 15 days after surgery (Ass3). Sociodemographic and gynecological data were collected, and anthropometric, cardiovascular, dermal sensitivity, range of motion (ROM) of the upper limbs, peripheral oxygen saturation (SpO2), inspiratory muscle strength (PImax), peak expiratory flow (PEF) and thoraco-abdominal mobility assessments were performed. Data were evaluated by repeated measures ANOVA and Wilcoxon statistical test with a Bonferroni correction (p < 0.05). Results: Eighteen women, with a mean age of 57.44 ± 9.35 years, mainly with the left side affected (61.1%) and lymphadenectomy performed in 50% of the cases. Differences were found in systolic and diastolic blood pressure, PEF and SpO2, axillary ROM index (Ass1>Ass2), PImax (Ass3>Ass2), perimetry (Ass2>Ass3), ROM in all axes of shoulder motion and wrist flexion (Ass2Ass2 and Ass1). Conclusion: The surgery for breast cancer excision resulted in hemodynamic and respiratory changes, especially on the first day after the procedure, returning to baseline values approximately 15 days later. Purpose. To evaluate the influence of previous levels of physical activity on hemodynamic, vascular, ventilatory, and functional outcomes after coronavirus disease 2019 (COVID-19) hospitalization. Methods. Sixty-three individuals with COVID-19 had their clinical status and previous levels (12 month) of physical activity (Baecke Questionnaire of Habitual Physical Activity) assessed at hospital admission. Individuals were then allocated to lower levels of physical activity (ACTLOWER; N = 22 ), intermediate levels of physical activity (ACTINTERMEDIATE; N = 22 ), or higher levels of physical activity (ACTHIGHER; N = 19 ) groups, according to tertiles of physical activity. Resting hemodynamic (heart rate and brachial/central blood pressures) and vascular (carotid-femoral pulse wave velocity, augmentation index, and brachial artery flow-mediated dilation) variables, pulmonary function (spirometry), respiratory muscle strength (maximal respiratory pressures), and functional capacity (handgrip strength, five-time sit-to-stand, timed-up and go, and six-minute walking tests) were measured at 30 to 45 days after hospital discharge. Results. ACTLOWER showed lower levels ( P < 0.05 ) of forced vital capacity, forced expiratory volume in the first second, maximal voluntary ventilation, and maximal expiratory pressure than ACTHIGHER. ACTLOWER also had lower ( P = 0.023 ) walking distance (~21%,) and lower percentage of predicted walking distance (~20%) at six-minute walking test during follow-up than ACTINTERMEDIATE. However, hemodynamic and vascular variables, handgrip strength, five-time sit-to-stand, and timed-up and go were not different among groups. Conclusion. ACTLOWER showed impaired ventilatory parameters and walking performance when compared with ACTHIGHER and ACTINTERMEDIATE, respectively. These results suggest that previous levels of physical activity may impact ventilatory and exercise capacity outcomes 30 to 45 days after COVID-19 hospitalization discharge.

Objetivo: conhecer o perfil epidemiologico e socio-demografico dos portadores desta patologia internados em hospital geral de referencia no periodo de agosto de 2009 a agosto de 2010. Metodo: Estudo prospectivo, transversal, exploratorio, descritivo, com abordagem qualitativa, utilizando o metodo estudo de caso conforme Robert K. Yin. Resultados: foram realizadas 22 entrevistas semi-estruturadas, que forneceram subsidios para reflexao da pratica assistencial desenvolvidas pelos enfermeiros aos portadores desta patologia, a fim de detectar as necessidades biopsicossociais afetadas e aplicar acoes dependentes, independentes. Conclusao: A ampla distribuicao da doenca na area urbana se mostra um problema de saude publica sendo necessario desenvolver acoes educativas e preventivas por meio da participacao da sociedade civil organizada e poder publico qualificando os profissionais da saude em todos os niveis de atencao para diagnostico precoce e continuidade do caso conforme preconizado no PCLV.

Introdução: A área da pneumologia tem dado importância para a associação da função respiratória com a obesidade. As transformações orgânicas decorrentes da obesidade implicam em alterações dos sistemas respiratório, imunomodulador, metabólico (musculatura esquelética, consumo de oxigênio e produção de gás carbônico) e circulatório. Este trabalho tem por objetivo revisar sobre o tema obesidade abdominal e sistema respiratório. Métodos: Revisão de literatura usando as bases de dados Scielo, Bireme, Pubmed e Scopus tendo como descritores: circulatory and respiratory physiological phenomena; respiratory function tests e abdominal obesity, sem limite para o ano de publicação e idioma. Resultados: A obesidade pode afetar o sistema respiratório independente da condição do parênquima pulmonar. Em indiví­duos obesos, o mecanismo de respiração está prejudicado, pois o excesso de adiposidade que reveste o tórax e ocupa o abdome dificulta a ação da musculatura respiratória. A obesidade abdominal não é apenas um fator estético ou metabólico, mas também um fator mecânico que pode comprometer a função da musculatura respiratória e consequentemente a função pulmonar. As alterações na função respiratória mais frequentemente encontradas na obesidade são: redução do volume de reserva expiratório, capacidade vital, capacidade residual funcional e capacidade pulmonar total. Além do impacto mecânico, respiratório e cardiovascular, a obesidade também interfere para o estí­mulo e controle do sistema nervoso autônomo. Conclusão: A obesidade abdominal está interligada a fatores respiratórios desde o controle central respiratório até limitação dos constituintes respiratórios podendo ocasionar distúrbio respiratório restritivo e obstrutivo.Palavras-chave: obesidade, sistema respiratório, obesidade abdominal.

High-resolution magic-angle spinning nuclear magnetic resonance spectroscopy (HR-MAS NMR) for the identification of the metabolic profile of plants under conditions very close to that in which the compounds are present in the matrix, the herbal medicines in this case. This enables selectivity in the determination of the active principle and other biomarkers present in the complex matrix, avoiding degradation products, which may occur in the extractive processes required in several analytical methods. In this study, HR-MAS analysis was applied in the quality control of seven Passiflora-based herbal medicines, using metabolic fingerprinting to confirm plant species and identify biomarkers. Vitexin and isovitexin were identified as major compounds in three of these herbal medicines (0.17 to 0.55%), while salicin was the majority in two others. On the other hand, no significant flavonoid contents were observed in the remaining two. In addition, it was possible to detect ethanol, a non-target compound, in all herbal medicines in concentrations varying between 0.009 and 0.342%. In this way, combined with chemometrics 1H HR-MAS NMR proved to be suitable for the qualitative and quantitative study of Passiflora biomarkers, using a minimal pre-treatment of the sample.

Introduction: Regular physical activity prevents cardiovascular diseases (CVD) and atherosclerosis, in addition to improving lipid levels and functional capacity in older adults. Objective: To evaluate and compare the lipid levels, functional performance and ankle brachial index (ABI) of sedentary and active older adults. Methods: This was a comparative cross-sectional study with 84 elderly (≥60 years) male and female subjects, divided into two groups: sedentary group (SG, n = 50) and active group (AG, n = 34) according to the International Physical Activity Questionnaire (IPAQ). Anthropometric and hemodynamic assessments, ankle brachial index (ABI) calculations, and functionality tests (Short Physical Performance Battery - SPPB and 6-minute walking test - 6MWT) were performed. Type 2 diabetes (T2D) is a prevalent disease in older people and it is associated with reduced pulmonary function, decline of functional capacity and worst quality of life. Multicomponent exercise training is recommended to improve motor skills of older persons, but their effects in treatment of T2D are not certain. Therefore, this study aimed to verify the effects of a multicomponent training on pulmonary function, functional capacity and quality of life in older adults with T2D. Eight diabetics aged >60 years performed a multicomponent training for 12 weeks, 3 days/week and 80 min/day. Before and after training program, the subjects were evaluated in relation to pulmonary function (spirometry, manovacuometry and ventilometry), functional capacity [timed up and go test (TUG) and 6-min walk test (6MWT)] and quality of life, with the Short-Form Health Survey (SF-36) questionnaire. Training induced significant improvement in maximal voluntary ventilation (p = 0.0009), forced vital capacity (p = 0.03), forced expiratory volume in the first second (p = 0.0004), TUG test (p = 0.02), 6MWT (p = 0.04), and in five SF-36 domains (functional capacity, physical aspects limitation, pain, vitality and social aspects). No changes in respiratory maximal pressure values and slow vital capacity were observed. Multicomponent training can be an alternative to improve functional capacity, quality of life and some respiratory parameters in older adults with T2D.

No. of bitstreams: 0 Previous issue date: 2016-08-01 Purpose: To compare the responses of aquatic exercise with dry land training and control inactive group on B-type natriuretic peptide and plasma renin activity levels in postmenopausal hypertensive women. Methods: Forty postmenopausal hypertensive women were randomly assigned to 3 groups: water group, dry land group and a non-intervention control group. The exercises programs consisted of consisted of 50-min sessions, three-times a week and was performed for 12 weeks. Samples of B-type natriuretic peptide and plasma renin activity levels were collected pre- and post-intervention. In the dry land exercise program, heart rate was maintained at around 50–60 % of the heart rate reserve, deducting 17 heart beats for aquatic exercise. Results: There was a statistically significant reduction in B-type natriuretic peptide in the water group (37 ± 11.1 ng/L) to (22 ± 9.4 ng/L), pre and post intervention, respectively. There was no difference in plasma renin activity levels between groups and moments. Conclusion: This study demonstrated the effectiveness of aquatic exercise in reducing B-type natriuretic peptide values in postmenopausal hypertensive women. Plasma renin activity levels values were not influenced by either of the interventions. Objective To evaluate the β-hydroxy-βmethylbutyrate supplementation influence on the morphological and morphometric characteristics of the diaphragm muscle of rats and verify whether there are sex differences. Methods Experimental study with 48 Wistar rats (24 of each sex) divided into 3 groups: Control Group: in which a daily diet with saline solution was offered; Experimental Group: the same amount of food from the Control group consumed in the previous day and 0.3g/kg of β-hydroxy-βmethylbutyrate; Ad libitum Experimental Group: ad libitum feeding with the same dose of the supplement. The analysis consisted of histomorphometry and classification in diaphragm muscle fiber area bands. The procedures occurred 30 days after the start of the experiment. Methods: Quasi-experimental study including 26 men split into two groups according to AC(cm): 1) < 102 (ACrisk-free); and 2) ≥ 102 (ACrisk). Heart rate variability (HRV), diastolic blood pressure (DBP), oxygen saturation (SpO2), FEV1/FVC, slow vital capacity (SVC), inspiratory capacity (IC), maximal inspiratory pressure (PImax), thoracoabdominal amplitude (AI) were measured: before (M1); 5 min after the physiotherapy (i.e. breathing exercises for airway clearance and active kinesiotherapy) (M2); and at follow-up, 30 min after physiotherapy (M3). Results: The groups differed in age, body mass index and body fat %. At M2 IC was different between groups (ACrisk-free< ACrisk). There was an increase in HRV indexes, PImax, SpO2, axillary AI, FEV1/FVC, and reduction in HR for ACrisk-free. There was a decrease in AI and an increase in DBP for ACrisk. Conclusion: In men with obstructive respiratory disorder, increased AC measurement limited the thoracoabdominal expansibility and induced a rise of the DBP. Respiratory physiotherapy promotes an increase of cardiac modulation and inspiratory capacity for men with obstructive respiratory disorder. Esta dissertação analisa a autonomia municipal na implementação de programas federais. Partiu-se da teoria sobre relações intergovernamentais no Brasil e implementação de políticas públicas para analisar a implementação do Mais Educação/Ministério da Educação (MEC) no Município de São Paulo, considerando o período de 2011 a 2016. Para realizar a análise, foi construído um quadro analítico que considera elementos topdown e bottom-up das teorias de implementação de políticas públicas. Foram considerados o desenho da política federal e o desenho da política municipal, incluindo seus incentivos institucionais, as relações interorganizacionais e a burocracia de médio escalão (BME). O método da pesquisa foi o estudo de caso único, operacionalizado empiricamente por meio de análise documental e de entrevistas semiestruturadas. Foram analisados o desenho do programa federal, a implementação dos programas municipais de expansão da jornada de ensino e as características do Mais Educação/MEC na rede de ensino. Verificamos que os programas municipais influenciaram a implementação da política federal por trazerem uma série de incentivos institucionais a este programa. Além disso, verificamos que o município levou cerca de 3 anos para construir outros incentivos institucionais para a implementação do programa federal em sua rede de ensino, conseguindo, assim, coordenar o programa. De 2011 a 2014, a adesão das unidades escolares ao programa foi marcada por diferenças inter-regionais, sobretudo de 2011 a 2013. Como principais conclusões, a pesquisa mostra que o Município de São Paulo construiu um arranjo de implementação colegiado junto ao MEC por ter criado uma série de incentivos institucionais para a implementação do programa federal em sua rede de ensino; e que a atuação empreendedora de parte da BME explica, pelo menos em parte, as diferenças inter-regionais em um contexto de pouca coordenação por parte da SME This dissertation analyzes municipal autonomy in the implementation of federal programs. It was based on the theory of intergovernmental relations in Brazil and the implementation of public policies to analyze the implementation of More Education/Ministry of Education (MEC) in the city of São Paulo, considering the period from 2011 to 2016. To carry out the analysis, an analytical framework that considers topdown and bottom-up elements of public policy implementation theories. The design of the federal policy and the design of the municipal policy, including its institutional incentives, interorganizational relations and the middle-level bureaucracy (MLB) were considered. The research method was the single case study, operationalized empirically through documentary analysis and semi-structured interviews. The design of the federal program, the implementation of the municipal programs of expansion of the teaching day and the characteristics of the More Education/MEC in the education network were analyzed. We verified that the municipal programs influenced the implementation of the federal policy by bringing a series of institutional incentives to this program. In addition, we found that the municipality took about 3 years to build other institutional incentives for the implementation of the federal program in its educational network, thus managing to coordinate the program. From 2011 to 2014, school units membership of the program was marked by interregional differences, especially from 2011 to 2013. As a key finding, the survey shows that the Municipality of São Paulo has built a collegial implementation arrangement with the MEC for having created A series of institutional incentives for the implementation of the federal program in its educational network; and that entrepreneurial activity on the part of BME explains, at least in part, the interregional differences in a context of little coordination by the SME

Foram coletadas as informações clínicas referentes ao período de 19 meses. A estratificação para os grupos foi realizada com base na ocorrência ou não da PAV, sem_PAV e PAV, respectivamente. Foi aplicado o teste T2 de Hotelling com 95% de confiança e Qui-Quadrado utilizando o Software “R” e os resultados apresentados em média ± desvio padrão e distribuição absoluta e relativa (p < 0,05).Resultados: A casuística foi de 322 sujeitos; o grupo PAV constou de 73 (22,67%), sendo 54,79% do sexo masculino, idade: 62,31 ± 16,96 anos e APACHE II: 29,98 ± 8,64. O grupo NAV teve maior tempo de VM e na UTI se comparado ao grupo sem_NAV; ainda neste grupo, a maior incidência de óbito na UTI ocorreu entre o 16° ao 20° dia de internação. O grupo sem_PAV era mais velho e 50% deste tiveram alta hospitalar.Conclusão: A PAV e suas interfaces ainda causam impacto sobre a evolução clínica dos sujeitos principalmente quanto ao fator tempo de ventilação mecânica e de internação na UTI. A maior incidência de óbito na UTI ocorre nas primeiras semanas. AbstractIntroduction: Ventilator-Associated Pneumonia (VAP) is a common complication found in the Intensive Care Unit (ICU) and is associated with increased mortality, length of hospital stay and mechanical ventilation (MV) time.Objective: To determine the incidence of VAP and its impact on the clinical course of the subject undergoing invasive MV in the ICU.Methods:This is a cohort study of hospitalized subjects in the general adult ICU of the State Hospital of Bauru / SP. The clinical information for the period of 19 months were collected. Stratification for the groups was based on the presence or absence of VAP, free_VAP and VAP, respectively. The Hotelling T² with 95% confidence, chi-square and the Mann-Whitney tests were executed using the R software and the results showed as mean ± standard deviation and absolute and relative distribution (p < 0.05).Results:The sample was of 322 subjects; the VAP group consisted of 73 (22.67%), 54.79% male, age: 62.31±16.96 years and the APACHE II: 29.98 ± 8.64. The VAP group had longer time of the MV and of the ICU compared to free VAP group; even in this group, the highest incidence of death in the ICU occurred between the 16th and 20th day of hospitalization. The free VAP group was older and 50% of the patients discharged from hospital.Conclusion: VAP and their interfaces still impact on the clinical evolution of the subjects mainly on the time factor of MV and ICU stay. The highest incidence of death in the ICU occurs in the first weeks. A diabetes mellitus tipo 2 (DM2) e uma doenca prevalente em idosos e esta associada a incapacidade funcional e anormalidades do metabolismo glicemico. O treinamento multicomponente, composto por exercicios de forca, equilibrio, coordenacao, marcha, agilidade e propriocepcao, e recomendado para melhorar a funcao fisica de idosos, porem seus efeitos no tratamento da DM2 nao estao claros. Objetivou-se investigar o efeito de um protocolo de treinamento multicomponente na aptidao funcional e parâmetros glicemicos de idosos com DM2. Foram incluidos 13 idosos com DM2 e idade de 68 ± 6 anos. Antes e apos o periodo de intervencao foram realizados os seguintes testes: chair stand , arm curl , sentar e alcancar, teste de caminhada de seis minutos (TC6M), glicemia de jejum e hemoglobina glicada (HbA1C). O programa de treinamento foi realizado por 16 semanas, tres vezes/semana, em dias nao consecutivos. Cada sessao consistiu de 10 minutos de aquecimento, 50 minutos de exercicios multicomponentes (coordenacao, forca muscular, flexibilidade, equilibrio e agilidade) e 10 minutos de alongamento e relaxamento. To evaluate cardiorespiratory alterations due to a single session of hyperbaric oxygen therapy.Randomized study with patients: a control group and hyperbaric oxygen therapy. Evaluations occurred in the beginning, during, and after exposure to pure oxygen above atmosphere for 2 hours. Systemic blood pressure, peripheral oxygen saturation, pulse rate, lung volume and lung capacity, and maximal inspiratory and expiratory pressures were evaluated. Peripheral oxygen saturation, pulse rate, and systemic blood pressure were evaluated during the pressurizing in the first hour. Data were evaluated by means of ANOVA, Mann-Whitney, and independent t-test (p0.05).A total of 14 adult patients were evaluated. In the group under therapy (seven subjects), aged: 49.57±14.59 years, there was a decrease in the pulse rate of 16 beats per minute after 35 minutes of therapy (intragroup analysis), and the peripheral oxygen saturation was higher within the same period compared to the control group.The hyperbaric oxygen therapy promotes cardiorespiratory alterations with the increase of the peripheral oxygen saturation and decrease of the pulse rate, without altering blood pressure levels and the strength, volumes, and respiratory capacities.Avaliar modificações cardiorrespiratórias em decorrência de sessão única de oxigenoterapia hiperbárica.Estudo aleatorizado com pacientes: grupos-controle e oxigenoterapia hiperbárica. As avaliações ocorreram no início, durante e após a exposição ao oxigênio puro acima de uma atmosfera, durante 2 horas. A pressão arterial sistêmica, saturação periférica de oxigênio, frequência de pulso, volume e capacidade pulmonar, pressões inspiratória e expiratória máximas foram avaliadas. A saturação periférica de oxigênio, frequência de pulso e pressão arterial sistêmica foram avaliadas durante a pressurização na primeira hora. Os dados foram avaliados pelo teste de ANOVA, Mann-Whitney e teste t independente (p0,05).Foram avaliados 14 pacientes adultos. La saturación periférica de oxígeno, frecuencia de pulso y presión arterial sistémica fueron evaluadas durante la presurización en la primera hora. Los datos fueron evaluados por el test de ANOVA, Mann Whitney y prueba t independiente (p0,05).Fueron evaluados 14 pacientes adultos. En el grupo bajo terapia (siete sujetos), edad: 49,57±14,59 años, hubo reducción de la frecuencia de pulso de 16 latidos por minuto tras 35 minutos de terapia (análisis intragrupo), y la saturación periférica de oxígeno fue mayor en ese mismo período si comparado con el grupo de control.La oxigenoterapia hiperbárica proporciona alteraciones cardiorrespiratorias con el aumento de la saturación periférica de oxígeno y la reducción de la frecuencia de pulso, sin alterar los niveles de presión arteriales y la fuerza, volúmenes y capacidades respiratorios.

INTRODUCTION High-resolution magic-angle spinning nuclear magnetic resonance (HR-MAS NMR) spectroscopy enables the analysis of the metabolic profile of plant and animal tissues under close to natural conditions, as well as of other heterogeneous natural or synthetic materials. Neither sample pretreatment is required after fragmentation nor powdering of the sample before insertion into the rotor. However, the efficiency of the method depends strongly on the sample preparation, rotor insertion procedure, and analysis conditions. OBJECTIVE To identify some of the variables that affect the spectral data and to propose solutions that minimise their impact on the quality of the analyses and results. METHODS Dried plant tissues were powdered, weighed, and homogenised in a 50 μL rotor with an optimised volume of deuterated solvent and sample in order to prevent material from escaping during spacer insertion, avoiding variations in magnetic susceptibility. Factors affecting the quality of HR-MAS NMR analysis such as particle size, sample and solvent amounts, solvent polarity, swelling time, rotor manipulation and pulse sequence setting were evaluated. RESULTS A strong correlation was observed between the signal area and the particle size of the powdered sample. The spectral profile varied depending on the deuterated solvent used. An incubation period was necessary to achieve adequate swelling of the sample and to ensure good data reproducibility. Proper sealing of the rotor, number of cycles and τ time on cpmgpr1d pulse sequence were found to affect the signal areas. CONCLUSION The study highlights the need for standardised sample preparation and instrumental setup protocols in order to achieve high reproducibility and obtain reliable data from HR-MAS NMR analyses.

Background and Purpose The evidence of the benefits from regular physical activity to hypertensives is based on dry land training studies. Therefore, the aim of this study is to compare the effect of aquatic exercise with dry land training on hypertensive women. Methods This is a randomized controlled study with 52 post-menopausal hypertensive women. The patients were randomly allocated in three groups: water aerobic training group (n = 19), dry land aerobic training group (n = 19) and a non-intervention control group (n = 14). The training protocol was performed by 12 weeks. Results There were no differences among the three groups concerning basal blood pressure (BP) and biochemical variables. In water group, there was a statistically significant reduction of systolic BP from 136 ± 16 mm Hg at zero week to 124 ± 18 mm Hg at 11th week and 124 ± 15 mm Hg at 12th week. In dry land training group, there was a statistically significant reduction of systolic BP from 138 ± 15 mm Hg at zero week to 125 ± 10 mm Hg at 7th week, 127 ± 10 mm Hg at 10th week and 126 ± 9 mm Hg at 12th week. The control group presented no change in any of the assessed variables. No changes were carried out in any antihypertensive medications during study. Discussion This is a randomized controlled study that demonstrates the antihypertensive efficacy of aerobic aquatic exercise. Copyright © 2013 John Wiley & Sons, Ltd.

A estimulação elétrica diafragmática tradicional é capaz de gerar benefícios ao pneumopata; poderia a corrente russa ser outra opção? Objetivou-se identificar as alterações após estimulação diafragmática elétrica transcutânea pela corrente russa em indivíduos portadores de DPOC. Trata-se de estudo prospectivo, quase experimental, com os seguintes critérios de inclusão: estabilidade medicamentosa, cessação tabágica, DPOC grau III e IV e manutenção do estilo de vida. Foram avaliadas medidas antropométricas, respiratórias e funcionais. A estimulação diafragmática se deu pelo Endophasys R ET 9701 por quatro meses, duas vezes por semana, com 30 sessões. Um caso para ensino é um relato de situações da vida organizacional, com o objetivo de desenvolver habilidades de resolução de problemas. Este estudo de caso para ensino é contado sob a ótica do Sargento Ciriaco, comandante da base. Desde 2007, a Base da Polícia Militar de Vila Formosa, localizada na zona leste do município de São Paulo, faz parte do programa de policiamento comunitário. Em agosto de 2008, no início da gestão do Sargento Ciriaco, foi adotada a filosofia da integração com a comunidade. Parcerias com a polícia comunitária do Japão, comerciantes locais, e outros órgãos do governo foram realizadas com objetivo de viabilizar recursos financeiros, físicos e humanos para os projetos. Apesar dos recursos escassos e poucos incentivos à inovação, o Sargento Ciriaco e seus comandados se superaram para proporcionar resultados de integração e segurança no bairro de Vila Formosa, através de diversos projetos e ações apresentados neste trabalho. A base apresenta-se como um case de sucesso dentro da corporação, que além de atingir a integração proposta, interagiu com muita dinâmica e criatividade com a sociedade. Os resultados obtidos apresentam-se através da redução do número de delitos na região e da constatação de um sentimento diferente da população acerca do papel da polícia como um parceiro da comunidade. Após a apresentação do caso, seguem notas de ensino para aplicação do caso em sala de aula. A experiência relatada foi uma das premiadas na categoria inovação pela 7ª edição do Prêmio Mário Covas (2010-2011), realizado pela Secretaria Estadual de Gestão Públicas em parceria com a Fundação do Desenvolvimento Administrativo (FUNDAP) e a Fundação Mário Covas. O objetivo do prêmio é reconhecer e premiar iniciativas de excelência ou inovação no setor público estadual paulista em diversas categorias.

Introdução: A hipertensão arterial sistêmica (HAS) e a diabetes mellitus tipo 2 (DM2) são patologias que frequentemente coexistem. A hipotensão pós-exercício (HPE) é bem documentada em indivíduos com HAS, porém não é certo se ocorre na coexistência de DM2.Objetivos: Investigar se a coexistência de DM2 influencia no efeito hipotensor pós-exercício aeróbio em indivíduos com HAS.Materiais e Métodos: Foram incluídos 33 indivíduos, distribuídos em dois grupos: Grupo HAS (n=17) e Grupo HAS+DM2 (n=16). Ambos submetidos às sessões exercício e controle. O protocolo de exercício consistiu em 10 minutos de aquecimento, 40 minutos de exercício aeróbio moderado em cicloergômetro e 10 minutos de alongamentos. No pré e pós-exercício (sete horas subsequentes), foram avaliados quanto à pressão arterial (PA), frequência cardíaca (FC) e duplo produto (DP). Neste trabalho amostra de óleo diesel foi separada por cromatografia em camada delgada utilizando, como fase estacionária, sílica, sílica impregnada com solução de quitosana em diferentes concentrações e sílica combinada com quitina, com a finalidade de verificar a influência destes polímeros na separação dos componentes do óleo diesel. Da separação da amostra de óleo diesel se observa que os melhor eluente foi o hexano. A presença de quitosana e de quitina não mostrou mudanças significativas na separação dos componentes do óleo diesel por análise cromatográfica em camada delgada, porém os compostos saturados apresentaram uma menor afinidade com a sílica impregnada com quitosana do que com a sílica pura.

RESUMO Introdução: O exercício físico tem sido proposto como tratamento não farmacológico do diabetes por seu efeito hipoglicemiante. Objetivo: Verificar o efeito agudo do exercício sobre a glicemia capilar em indivíduos diabéticos que fazem uso de insulina ou antidiabéticos orais. Métodos: Foram estudados dia-béticos em uso de hipoglicemiantes orais (G1, n=7), não diabéticos (G2, n=8, grupo controle) e diabéticos em uso de insulina (G3, n=8) da Associação de Diabéticos de Bauru (ADB). Foram submetidos a avaliações clínicas, bioquímicas, pressóricas, antropométricas e a uma sessão de exercício aeróbio (60% a 80% FCmáx). A glicemia capilar foi mensurada em oito momentos durante a sessão (M1 ao M8). A análise estatística foi descritiva (média ± desvio padrão), os testes utilizados foram de Kruskal Wallis e Friedman, não paramétricos. Resultados: Participaram 23 indivíduos com idade média 59,35 ± 14,59 anos, 17 do gênero feminino e seis masculino. As taxas de glicemia do M2 ao M8 foram comparadas ao M1, sendo observadas diminuições significativas nos momentos 4, 5, 6, 7, 8 (p≤0,05). Na análise inicial dos grupos G1 e G3 eram obesos e G2 sobrepeso. Na análise da variação da glicemia durante o exercício foi observado que G1 e G3 diferiram nos momentos 2, 3 e 5, G2 e G3 em todos os momentos (p
