Abstract Background The importance of environmental contamination in the transmission of pathogens among hospitalized patients is universally recognized, and disinfection of surfaces is a widely accepted modality for reducing healthcare-associated infections. Nevertheless, hospital disinfection is still suboptimal. In this study, we evaluated the sustained effects of the novel formulation OxiLast™ which extends the antimicrobial effects of chlorine-based disinfectants. Methods In an experimental lab phase, PVC surfaces were coated with OxiLast™ and then inoculated with representative Gram-positive and Gram-negative pathogenic bacteria. Cells were recovered at different contact times (5, 15, 30 min) to assess the reduction in bacterial counts compared to uncoated surfaces and also subject to various challenges to assess robustness. A similar methodology was then applied in an unoccupied hospital room to evaluate the sustained effect of OxiLast™ on high-touch surfaces. Results OxiLast™ demonstrated notable activity against the range of bacterial strains tested with ≥ 4 log10 reduction in bacterial counts observed for up to seven days following one surface application, for various strains and contact times. Similar results were observed following challenges such as simulated abrasion of coated surfaces, organic contamination or successive inoculations. The results were confirmed in a simulated patient care environment. Conclusions The addition of OxiLast™ to common chlorine-based disinfectants has shown a substantial and sustained reduction in bacterial pathogen counts for up to 7 days following one application. The consistent results in the laboratory and hospital are promising and should be tested in a real-life clinical scenario.

The Lake Titicaca basin was one of the major centers for cultural development in the ancient world. This lacustrine environment is unique in the high, dry Andean altiplano, and its aquatic and terrestrial resources are thought to have contributed to the florescence of complex societies in this region. Nevertheless, it remains unclear to what extent local aquatic resources, particularly fish, and the introduced crop, maize, which can be grown in regions along the lakeshores, contributed to facilitating sustained food production and population growth, which underpinned increasing social political complexity starting in the Formative Period (1400 BCE to 500 CE) and culminating with the Tiwanaku state (500 to 1100 CE). Here, we present direct dietary evidence from stable isotope analysis of human skeletal remains spanning over two millennia, together with faunal and floral reference materials, to reconstruct foodways and ecological interactions in southern Lake Titicaca over time. Bulk stable isotope analysis, coupled with compound-specific amino acid stable isotope analysis, allows better discrimination between resources consumed across aquatic and terrestrial environments. Together, this evidence demonstrates that human diets predominantly relied on C3 plants, particularly quinoa and tubers, along with terrestrial animals, notably domestic camelids. Surprisingly, fish were not a significant source of animal protein, but a slight increase in C4 plant consumption verifies the increasing importance of maize in the Middle Horizon. These results underscore the primary role of local terrestrial food resources in securing a nutritious diet that allowed for sustained population growth, even in the face of documented climate and political change across these periods.

Simple Summary: VaIN after hysterectomy is a rare intraepithelial neoplasia strongly associated with HR-HPV infection. It is hypothesized that the laparoscopic hysterectomy procedure may cause a higher incidence of VaIN in hysterectomized women. In a retrospective study of 170 women hysterectomized due to CIN3 or due to benign uterine pathology, all cases of high-grade VaIN occurred in women with persistent HPV. The most frequent genotype was 16. All these elements suggest that it is a history of HPV-related disease of the lower genital tract and viral persistence, rather than hysterectomy itself, that should be considered risk factors for the development of high-grade VaIN. A vaginal wall biopsy under colposcopy is recommended to be routinely performed before surgery for patients undergoing hysterectomy due to CIN3 or stage IA cervical cancer, which will help determine the necessary extent of vaginal resection during the procedure. The data from the literature show that women undergoing a LEEP due to CIN3 have a greater risk of having subsequent high-grade anogenital intraepithelial neoplasia or cancer, and the risk is greater for vaginal cancer than for anal and vulvar cancers. It is hypothesized that the laparoscopic hysterectomy procedure may cause a higher incidence of VaIN in hysterectomized women. There are few studies addressing this issue, and they show mixed results. This study aimed to investigate the incidence of high-grade or severe VaIN in the population of women undergoing hysterectomy for CIN3 or benign uterine disease and illustrate the treatment options and follow-up. Methods: This retrospective study was conducted on 170 women who underwent a laparoscopic hysterectomy due to high-grade cervical intraepithelial neoplasia (CIN3) or benign gynecological disease. The follow-up strategy included performing a cotest and colposcopy with biopsy if necessary. The median time between primary treatment and a diagnosis of high-grade VaIN was 18 months. Results: High-grade or severe VaIN was found in eight patients after hysterectomy (4.7%). All cases of high-grade VaIN occurred in women with persistent HPV infection. The most frequent genotype was 16. Women hysterectomized due to CIN3 showed an eight-fold greater risk than women hysterectomized due to benign disease of developing high-grade VaIN. The risk of VaIN is low in women hysterectomized due to benign disease. The risk of developing VaIN is greater in women with viral persistence. Conclusion: All these elements suggest that it is a history of HPV-related disease of the lower genital tract and viral persistence, rather than hysterectomy itself, that should be considered risk factors for the development of high-grade VaIN. After hysterectomy, patients with a history of CIN should undergo annual screening with vaginal dome cytology and HPV testing. [ABSTRACT FROM AUTHOR]

The epikarst crustacean fauna from four Sicilian caves (Conza, Entella, Molara, and Zubbia del Cavallo caves) and four caves in the Lessinian Massif in the Venetian Prealps (Covolo della Croce, Ponte di Veja, Roverè Mille, Buso della Rana caves) was recently investigated. The two groups of caves differ in their environmental conditions: the Sicilian caves are fossil except one which has an active branch; they are all fed by strongly intermittent and scarce rainfall peaking in the fall. The Lessinian caves are fed by more abundant rainfall, with two yearly peaks (May-June and October-November); two of them are active, one has a temporary stream, one is fossil. The crustacean fauna found in the epikarst drip of each of the studied caves is characterized by interesting endemic harpacticoid and cyclopoid copepods, and one bathynellacean syncarid, often collected in only one cave. Higher diversity of stygobiotic taxa was recorded for the Lessinian caves (9 species of copepods in the Lessinian, and 6 species of copepods and one bathynellacean in Sicily); most of the taxa collected in Sicily are endemic to one cave. Spatial analysis showed very different distributions over short spatial scales (tens of kilometers) and, within each cave, the distribution also varied over distances of a few meters. Our data correspond with other studies where many epikarst crustaceans showed a distribution with a linear extent of only a few hundred meters: the epikarst fauna is not uniformly distributed but rather divided in “blocks” probably characterized by different environmental conditions and, as a consequence, by different taxocoenoses. The data highlight the epikarst as a source of “hidden” biodiversity, and the importance of management protection plans which include not only the caves, but also the epikarst overlying layer and the water sources that feed it.
