Abstract

Objective: Management of recurrent laryngeal cancer presents a major challenge, and salvage laryngectomy is complicated by previous oncologic treatments. Thyroidectomy as part of salvage laryngectomy adds a nonnegligible degree of morbidity. The purpose of this study is to assess the rate of thyroid gland invasion in patients undergoing salvage laryngectomy to determine relevant predictive factors. Study Design: Case series with chart review. Setting: Department of Otorhinolaryngology, Head and Neck Surgery, Guy's Hospital, London, United Kingdom. Methods: A retrospective review of patients undergoing salvage laryngectomy between 2009 and 2019 was undertaken. Preoperative cross-sectional imaging and histopathological analysis were performed to define evidence and predictors of thyroid gland invasion (TGI). Results: Fifty-one patients had salvage laryngectomy. Histological evidence of TGI was found in 4 patients (7.8%). No significant relationship was found between histological TGI and subsite of primary carcinoma, degree of differentiation, T staging, or radiological TGI. Preoperative computed tomography had a high negative predictive value for TGI. Conclusion: Thyroidectomy should be carefully considered in patients undergoing salvage laryngectomy, and its extent should be defined on an individual basis. Total thyroidectomy should not routinely be performed in salvage laryngectomy or pharyngolaryngectomy in patients with no preoperative radiological evidence of TGI on cross-sectional imaging, unless there is intraoperative evidence of TGI. 19. Alternances de codi en parlants bilingües amb L1 català i L1 castellà

Abstract

Treball de fi de grau en Llengües aplicades. Tutora: Montserrat González Condom. Les alternances de codi són fruit de situacions de contacte de llengües. Un clar exemple és Catalunya, on el context social fa palès la convergència del castellà i el català. Alguns autors han defensat que aquest fenomen posa de manifest les mancances lingüístiques que té un parlant bilingüe respecte d’una de les llengües. Tanmateix, d’altres autors consideren que hi ha un seguit de funcions discursives, pragmàtiques i ideològiques que no s’han de passar per alt. Per tal de comprovar a quins d’aquests usos responen les alternances de codi, s’ha dut a terme l’anàlisi qualitativa d’una conversa oral i espontània entre parlants bilingües amb L1 català i L1 castellà. Un altre dels objectius és observar quina de les dues llengües té una major presència en una conversa bilingüe en què els parlants tenen L1 diferents. Els resultats preliminars de la mostra suggereixen com la producció d’alternances de codi podria estar relacionada amb un domini equilibrat de bilingüisme. Paral·lelament, s’ha detectat que la llengua que més parlants comparteixen com a L1 és la que s’estableix com a base de la interacció. Les dades conclouen que els patrons que es tracen depenen de les actituds i preferències lingüístiques dels parlants d’un grup en concret, i que per extrapolar-los a un grup representatiu s’hauria de fer un estudi més exhaustiu. Las alternancias de código son fruto de situaciones de contacto de lenguas. Un claro ejemplo es Cataluña, donde el contexto social hace evidente la convergencia del castellano y el catalán. Algunos autores han defendido que este fenómeno manifiesta las carencias lingüísticas que tiene el hablante en cuestión respecto alguna de las lenguas. No obstante, otros consideran importante tener en cuenta una serie de funciones discursivas, pragmáticas e ideológicas. Con el objetivo de comprobar a qué usos responden las alternancias de código, se ha llevado a cabo el análisis cualitativo de una conversación oral y espontánea entre hablantes bilingües con L1 catalán y L1 castellano. Otro de los objetivos es observar cuál de las dos lenguas tiene mayor presencia en una interacción en la que los hablantes tienen L1 diferentes. Los resultados preliminares de la muestra sugieren que la producción de alternancias de código podría estar directamente relacionada con un dominio equilibrado de bilingüismo. Por otra parte, se ha detectado que la lengua que más hablantes comparten como L1 es la que se establece como base de la interacción. Los resultados concluyen que los patrones que se forman dependen de las actitudes y preferencias lingüísticas de los hablantes de un grupo en concreto, y que para hacer generalizaciones de un grupo representativo se tendría que hacer un estudio más exhaustivo. Codeswitching is the result of a situation of languages in contact. Catalonia is one clear example, where the social context leaves evidence of the convergence between the Spanish and the Catalan languages. Many authors believe that this phenomenon proves the lack of linguistic abilities that a bilingual speaker has in one of the languages. Others are in favour of the more discursive, pragmatic, and ideological functions. In order to verify which of these uses follows codeswitching, a qualitative analysis of an oral and spontaneous conversation between bilingual speakers with L1 Catalan and L1 Spanish has been conducted. Another aim is to observe which of the two languages has more presence in a bilingual conversation in which the speakers have different L1s. The preliminary results suggest that codeswitching could be directly related to a symmetric bilingualism. Furthermore, the language that is shared as an L1 between more speakers is the one that is established as the basis of the interaction. The results conclude that the patterns traced depend on the linguistic attitudes and preferences of the speakers of a specific group, and it would be relevant to carry out a more exhaustive study before making generalizations.
