The increase in oncology knowledge and the possibility of creating personalized medicine by selecting a more suitable therapy related to tumor subtypes, as well as the patient's management with cancer within a multidisciplinary team has improved the clinical outcomes. Early detection of cancer through screening-based imaging is probably the major contributor to a reduction in mortality for certain cancers. Nowadays, imaging can also characterize several lesions and predict their histopathological features and can predict tumor behaviour and prognosis. CT is the main diagnostic tool in oncologic imaging and is widely used for the tumors detection, staging, and follow-up. Moreover, since CT accounts for 49-66% of overall patient radiation exposure, the constant reduction, optimization, dose inter- and intraindividual consistency are major goals in radiological field. In the recent years, numerous dose reduction techniques have been established and created voltage modulation keeping a satisfactory image quality. The introduction of CT dual- layer detector technology enabled the acquisition of spectral data without additional CT x-ray tube or additional acquisitions. In addition, since MRI does not expose the body to radiation, it has become a mainstay of non-invasive diagnostic radiology modality since the 1980s.

Purpose: Ultra high-frequency ultrasound (up to 70 MHz) allows an extremely accurate assessment of the breast skin layers as well as hair follicles in axilla, areola and nipple. The aim of the study was to evaluate the normal anatomy of the breast within the first 1 cm of the skin surface using ultra high the world’s first ultra ultra high-frequency US system for routine clinical use available today and its utility as a diagnostic tool. Methods: In this study were involved 7 patients (6 females and 1 male) with a mean age of 40.3 yo (range 31-53 yo). Images were acquired on the breast skin on upper outer quadrant, on axilla and in nipple-areolar region. Results: Periareolar and subcutaneous regions are often difficult to examinate with conventional ultrasound; this method allows to ultra highlight anatomical details such as skin layers, nipple, areolar and ducts structure. Conclusion: Ultra high frequency breast ultrasound provides important information on the anatomy expecially in nipple-areolar region and in the evaluation of skin thickness and could improve the clinical diagnosis and the preoperative planning.

While fenestration and duplication are relatively common in the arteries, they are extremely rare in the venous compartment: internal jugular vein fenestration has been reported occurring in 0.4% of unilateral neck dissections. Familiarity with these morphological anomalies is important for the radiologist and for the surgeon to prevent neurovascular injury, especially in neck surgery and interventional catheterization. We present the case of a patient harboring a fenestration of the left internal jugular vein, diagnosed by magnetic resonance angiography, and a systematic review of the literature. To our knowledge, from 1985 until 2016 only 36 patients (including the present) were diagnosed as having an internal jugular vein morphological anomaly. Out of 36 patients, only 11 (30,5%) were diagnosed using radiological imaging; the high rate of intra-operative diagnoses (22/36, 62,5%) is likely related to the limited use of diagnostic imaging or to misdiagnosis/misinterpretation of a relatively unknown and rare morphological anomaly. A contrast enhanced computed tomography or magnetic resonance angiography should be considered in case of vascular procedures in a patient with known internal jugular vein anomaly.

Radiation-induced brain cavernomas have been mainly reported in children who underwent radiotherapy for medulloblastoma, leukemia, or low-grade glioma. Otherwise, the "de novo" appearance of a cav-ernoma in an elderly long-survivor patient after resection and radiotherapy of a glioblastoma is a rare event. We report the case of a 62-year-old female patient who underwent surgical resection of a right temporal glioblastoma, followed by radiation therapy of the operative field and surrounding brain and concomitant adjuvant temozolomide. Four years after the operation, a follow-up Magnetic Resonance revealed a good tumor control and a small round lesion at the superior surface of the right cerebellar hemisphere, close to the margins of the previous irradiation field. The radiological items were consistent with a cavernous angioma. Because of the small size of the malformation and the absence of related symptoms, no treatment was performed. The patient died for tumor progression 86 months after the initial operation, with unchanged cerebellar caver-noma. The occurrence of a cavernous angioma in an elderly patient after radiotherapy for brain glioblastoma is an exceptional event; the distribution of radiotherapy-induced cavernous malformations reported in current literature is presented and the mechanism of their formation is discussed. Colorectal cancer (CRC) is the third leading cause of cancer-related deaths worldwide. It has been estimated that more than one-third of patients are diagnosed when CRC has already spread to the lymph nodes. One out of five patients is diagnosed with metastatic CRC. The stage of diagnosis influences treatment outcome and survival. Notwithstanding the recent advances in multidisciplinary management and treatment of CRC, patients are still reluctant to undergo screening tests because of the associated invasiveness and discomfort (e.g., colonoscopy with biopsies). Moreover, the serological markers currently used for diagnosis are not reliable and, even if they were useful to detect disease recurrence after treatment, they are not always detected in patients with CRC (e.g., CEA). Recently, translational research in CRC has produced a wide spectrum of potential biomarkers that could be useful for diagnosis, treatment, and follow-up of these patients. The aim of this review is to provide an overview of the newer noninvasive or minimally invasive biomarkers of CRC. Here, we discuss imaging and biomolecular diagnostics ranging from their potential usefulness to obtain early and less-invasive diagnosis to their potential implementation in the development of a bespoke treatment of CRC.

The aim of this paper was to evaluate the efficacy, safety, and clinical outcomes of superselective embolization using ethylene-vinyl alcohol copolymer (Onyx Liquid Embolic System; ev3 Neurovascular, Irvine, CA, USA) as the primary treatment in active peripheral emergency arterial bleeding.

To assess the diagnostic accuracy and radiation dose of split-bolus multidetector-row computed tomography (MDCT) protocol in the detection and characterization of focal liver lesions in oncologic patients.We retrospectively analyzed triphasic CT at initial diagnosis and follow-up split-bolus 64-detector row CT protocol in 48 oncologic patients with focal liver lesions. Split-bolus MDCT protocol by i.v. injection of two boli of contrast medium combines hepatic arterial phase (HAP) and hepatic enhancement during portal venous phase (PVP) in a single pass. First bolus: 75-90 mL at 2.0 mL/sec to obtain adequate hepatic enhancement during the PVP; second bolus: 60 mL/sec at 3.5 mL/sec to ensure HAP. Each bolus is followed by 20 mL of saline solution at the same flow rate. Sensitivity, specificity, positive predictive value and negative predictive value of split-bolus MDCT protocol were calculated for detection and characterization of liver lesions. The effective radiation dose (ED) was calculated using dose-length product (DLP) values in mSv determined using a conversion factor.compared to triphasic-MDCT, split-bolus MDCT protocol confirmed all the 210 lesions identified and characterized by triphasic-MDCT technique, unchanged during the follow-up. The mean ED was 27.8 ± 6 mSv for chest-abdomen-pelvis biphasic split-bolus MDCT and 45.7 ± 13.6 mSv for triphasic-MDCT.The diagnostic efficacy of split-bolus protocol is comparable to that of triphasic protocol at MDCT with a reduction in radiation dose of approximately 35-40%.

The purpose of the study is to examine the incidence of adverse reactions caused by non-ionic contrast media in selected patients after desensitization treatment and to evaluate the safety profile of organ iodine contrast media (i.c.m.) in a multistep prevention protocol. In a population of 2000 patients that had received a CT scan, 100 patients with moderate/high risk for adverse reactions against iodinated contrast agents followed a premedication protocol and all adverse reactions are reported and classified as mild, moderate or severe. 1.7 percent of the pre-treated patients reported a mild, immediate type reaction to iodine contrast; of these five patients with allergy 0.71 percent had received iomeprol, 0.35 percent received ioversol and 0.71 percent received iopromide. The incidence of adverse reactions was reported to be higher (4 out of 5 patients) among those that referred a history of hypersensitivity against iodinated i.c.m. Although intravenous contrast materials have greatly improved, especially in terms of their safety profile, they should not be administered if there isn't a clear or justified indication. In conclusion, even if we know that the majority of these reactions are idiosyncratic and unpredictable we propose, with the aim of improving our knowledge on this subject, a multicenter study, based on skin allergy tests (prick test, patch test, intradermal reaction) in selected patients that have had previous experiences of hypersensitivity against parenteral organ iodine contrast media.

The development of various sophisticated mechanical thrombectomy devices and the amassed experience of physicians in minimal invasive therapy produced a paradigm shift in vascular access management toward percutaneous declotting procedures, using pharmaceutical thrombolysis, mechanical thrombectomy, balloon thrombectomy, and a combination of the above techniques. In this setting, in the last years, AngioJet™ (Possis, Minneapolis, MN, USA) rheolytic thrombectomy (RT) showed an increasing use in emergency and election patients. The purpose of this review is to present the current status of percutaneous rheolytic thrombectomy in different fields of applications.

BACKGROUND: FDG-PET is a diagnostic imaging procedure effective in staging primary and recurrent cancer. False-positive uptake already has been described in both inflammatory and infectious respiratory diseases, although no reports associate Chlamydia pneumoniae infection to FDG uptake. CASE REPORT: An incidental diagnosis of asymptomatic multilobar pneumonia during screening for thyroid malignancy is reported. Three areas of pulmonary consolidation strongly positive on PET/CT scan, mimicking pulmonary malignancy were identified. Both radiologic features and serum IgM antibodies for Chlamydia pneumoniae suggested the diagnosis of an unusual presentation of a Chlamydia pneumoniae respiratory infection. Specific antibiotic therapy induced a complete resolution of the areas of pulmonary consolidation. CONCLUSIONS: This case suggests that positive PET is not an absolute indicator for malignancy. Chlamydia pneumoniae respiratory infections can exhibit positive uptake on FDG-PET.
