Background. Doctors and patients hold varying beliefs concerning illness and treatment. Patients' and families' explanatory models (EMs) vary according to personality and sociocultural factors. In a multi-ethnic society, it is becoming increasingly significant that doctors understand the different beliefs of their patients in order to improve patient/doctor communication as well as patient adherence to treatment. Methods. Twelve focus groups were formed, consisting of 40 children diagnosed with asthma, as well as 28 mothers of these children. These groups included mothers and children of different ethnicities who were living in Amsterdam, the Netherlands. In order to understand the beliefs that both mothers and children hold regarding asthma and its treatment, the explanatory models were analysed and compared. Results. Study findings show that mothers and children, regardless of ethnicity and age, have their own EMs. Overall, there is a great deal of uncertainty related to the causes, consequences, problems, and symptoms of asthma and its treatment. It also seems that many concerns and feelings of discomfort are the result of lack of knowledge. For instance, the fact that asthma is not seen as a chronic disease requiring daily intake of an inhaled corticosteroid, but rather as an acute phenomenon triggered by various factors, may be very relevant for clinical practice. This particular belief might suggest an explanation for non-adherent behaviour. Conclusion. A thorough understanding of the mothers' and children's beliefs regarding the illness and its treatment is an important aspect in the management of asthma. Gaining an understanding of these beliefs will provide a foundation for a solid clinician-patient/family partnership in asthma care. Although ethnic differences were observed, the similarities between the mothers' and children's beliefs in this multi-ethnic population were striking. In particular, a common belief is that asthma is considered an acute rather than a chronic condition. In addition, there is a lack of knowledge about the course and the self-management of asthma. Background: In the context of a growing interest in developmental care (DC) this study explores the effect of the basic elements of DC and the additional effect of the individual approach of the Newborn Individualized Developmental Care and Assessment Program (NIDCAP) on the health-related quality of life (HRQoL) of very preterm infants at 1 year of age. The basic elements of DC in this study were defined as the use of standardized nests and incubator covers whose protective characteristics were hypothesized to have a positive effect on the infant's HRQoL. The individualized approach of the NIDCAP was thought to further increase HRQoL. Methods: Very preterm (≤32 weeks) born infants in a Dutch Neonatal Intensive Care Unit at two locations were included in two consecutive randomized controlled trials (RCT) comparing controls (standard care) with basic DC (standardized nests and covers) in the first RCT and basic DC with NIDCAP in the second RCT. Parents completed a questionnaire (RCT 1, n = 136; RCT 2, n = 128) regarding their infant's HRQoL (TNO-AZL Preschool Quality of Life Questionnaire) at 1 year of age, corrected for prematurity. Because of multiple testing a P-value of below 0.01 was chosen to indicate significance. Results: HRQoL scores ranged from good to optimal for most infants. No significant differences were found between basic DC vs. controls and NIDCAP vs. basic DC on the child's HRQoL as reported by parents at 1 year of age. Conclusions: These two RCT show that the basic elements of DC and the more individualized NIDCAP do not improve HRQoL of very preterm infants at 1 year of age. © 2008 The Authors Journal compilation © 2008 Blackwell Publishing Ltd.

Doel. Beschrijven van risicogedrag en opvattingen over voeding, beweging en opvoeding in gezinnen met jonge kinderen, in relatie tot specifieke risicogroepen voor overgewicht.

Objective: This study assesses the reliability and validity of the European KIDSCREEN-52 generic health-related quality of life (HRQoL) questionnaire for children and adolescents. Research Design: The KIDSCREEN-52, which measures HRQoL in 10 dimensions, was administered to a representative sample of 22,827 children and adolescents (8 to 18 years) in 13 European countries. Psychometric properties were assessed using the Classical Test Theory approach, Rasch analysis, and structural equation modeling (SEM). A priori expected associations between KIDSCREEN scales and sociodemographic and health-related factors were examined. Test-retest reliability was assessed in 10 countries. Results: For the overall sample, Cronbach's alpha values ranged from 0.77 to 0.89. Scaling success (Multitrait Analysis Program) was >97.8% for all dimensions and Rasch analysis item fit (INFITmsq) ranged from 0.80 to 1.27. The intraclass correlation coefficients ranged from 0.56 to 0.77. No sizeable differential item functioning (DIF) was found by age, sex or health status. Four items showed DIF across countries. The specified SEM fitted the data well (root mean square error of approximation: 0.06, comparative fit index: 0.98). Correlation coefficients between Pediatric Quality of Life Inventory, Child Health and Illness Profile-Adolescent Edition, and Youth Quality of Life Instrument scales and KIDSCREEN dimensions assessing similar constructs were moderate for those (r = 0.44 to 0.61). Statistically significant differences between children with and without physical and mental health problems (Children with Special Health Care Needs screener: d = 0.17 to 0.42, Strengths and Difficulties Questionnaire: d = 0.32 to 0.72) were found in all dimensions. All dimensions showed a gradient according to socioeconomic status. Conclusions: The KIDSCREEN-52 questionnaire has acceptable levels of reliability and validity. Further work is needed to assess longitudinal validity and sensitivity to change. © 2007, International Society for Pharmacoeconomics and Outcomes Research (ISPOR).

This study explored the effects of basic developmental care on the behaviour of very pre-term infants and parental stress at I and 2 years of corrected age. A randomized controlled trial was done to compare basic Developmental Care (standardized nests and incubator covers) and controls (standard care). Parents of infants born < 32 weeks of gestation completed questionnaires measuring child behaviour and parental stress at 1 year (N = 139) and 2 years (N = 133) of the child's age. Parental stress was measured using the Nijmegen Parenting Stress Index and child behaviour was measured using the Infant-Toddler Social and Emotional Assessment (ITSEA) and the Child Behaviour Checklist 2-3. At 1 year of age, children in the basic developmental care group had significantly higher behaviour scores on the total competence domain (p = .009) and the competence subscale mastery motivation (p = .002) of the ITSEA questionnaire, meaning that the infants showed more curiosity, persistence, obedience and enjoyment with small accomplishments. No significant effects were found on problem behaviour or parenting stress. We conclude that introducing a basic form of developmental care in the neonatal intensive care unit has a positive influence on the child's competence behaviour at I year of age. © 2008 The British Psychological Society.

Doel. Beschrijven van risicogedrag en opvattingen over voeding, beweging en opvoeding in gezinnen met jonge kinderen, in relatie tot specifieke risicogroepen voor overgewicht. Opzet. Dwarsdoorsnedeonderzoek. Methode. Op consultatiebureaus voor zuigelingen en peuters verspreid over het land werden 534 ouders benaderd voor het invullen van vragenlijsten over voeding, beweging en opvoeding. Er werd gevraagd naar achtergrondkenmerken en ook werd een aantal stellingen getoetst. Via een bivariate en multivariate analyse werd per stelling onderzocht of de antwoorden gekoppeld waren aan specifieke risicogroepen. Resultaten. De respons bedroeg 73 (390 ouders). Bij 15 van de 2-4-jarigen was er sprake van overgewicht, inclusief 3,7 met obesitas. De ouders hadden relatief vaak opvattingen en gedragingen die kenmerkend zijn voor een ongezonde leefstijl van jonge kinderen. Zo was men in 1 op de 7 gezinnen niet gewend om te ontbijten. Van de respondenten beschouwde 43 gezoete melkdrank als een goede vervanger voor melk en had 39 van de kinderen altijd frisdrank binnen handbereik. Een vijfde van de ouders gaf aan niet voldoende tijd te hebben om met hun kind naar buiten te gaan. Ongeveer 1 op de 10 peuters van 2-4 jaar had een tv op de eigen kamer; 22 mocht zelf bepalen of hij of zij tv keek en 9 hoelang dat gebeurde. Vooral in gezinnen met niet-westerse moeders, laagopgeleide moeders of moeders met een uitkering kwamen deze specifieke risicogedragingen frequenter voor. Het gebruikmaken van kinderopvang of peuterspeelzaal leek op enkele punten beschermend te werken. Conclusie. In gezinnen met kinderen van 0-4 jaar kwam frequent gedrag voor dat op latere leeftijd in verband staat met overgewicht. Gezien de toename van overgewicht en obesitas bij jonge kinderen is het starten van onderzoek waarin opvoeding wordt geoptimaliseerd gerechtvaardigd, met speciale aandacht voor het gedrag van ouders ten aanzien van 5 speerpunten: borstvoeding, meer buiten spelen en bewegen, regelmatig en goed ontbijten, minder consumeren van gezoete drank en minder tv-kijken en computeren. Door interventies in te zetten bij jonge leeftijdsgroepen kan worden voorkomen dat ongezonde leef- en opvoedingsstijlen ontstaan

This study aimed to identify and explain differences in health-related quality of life (QoL) between immigrant and non-immigrant children with asthma. In 274 children (7-17 years of age) generic and asthma-related QoL were assessed. The association between ethnicity and QoL was studied in linear regression model analyses. For the asthma-related QoL, unadjusted analyses showed significant ethnic differences. The non-immigrant children had the highest scores, which implies a better QoL. After adjusting for asthma control and socioeconomic status (SES), ethnic differences disappeared. These results suggest that immigrant children have a similar QoL to that of non-immigrant children from a comparable SES, when their asthma is under control. Copyright © 2007 Informa Healthcare.

We describe and analyze five consecutive modes of governance in the Dutch social housing sector. We compare these institutional forms with the five policy values that the sector was assumed to realize. For that purpose we propose and use a new, analytical framework based on a separation principle that is applied to the coercion domain (the governance of transactions) as well as to the interaction domain (the welfare values). The consecutive modes of governance were introduced to cope with the changing welfare policies, but with varying results. In this paper we show that the implemented modes of governance in the Dutch social housing sector did not always realize the values that the sector was supposed to deliver. The framework we propose also facilitates the discussion about the concept of a social enterprise and the embedding of a service of general interest in the European Union.

Objective: To assess the construct and criterion validity of the KIDSCREEN-27 health-related quality of life (HRQoL) questionnaire, a shorter version of the KIDSCREEN-52. Methods: The five-dimensional KIDSCREEN-27 was tested in a sample of 22,827. For criterion validity the correlation with and the percentage explained variance of the scores of the KIDSCREEN-52 instrument were examined. Construct validity was assessed by testing a priori expected associations with other generic HRQoL measures (YQOL-S, PedsQL, CHIP), indicators of physical and mental health, and socioeconomic status. Age and gender differences were investigated. Results: Correlation with corresponding scales of the KIDSCREEN-52 ranged from r = 0.63 to r = 0.96, and r2 ranged from 0.39 to 0.92. Correlations between other HRQoL questionnaires and KIDSCREEN-27 dimensions were moderate to high for those assessing similar constructs (r = 0.36 to 0.63). Statistically significant and sizeable differences between physically and mentally healthy and ill children were found in all KIDSCREEN-27 dimensions together with strong associations with psychosomatic complaints (r = -0.52). Most of the KIDSCREEN-27 dimensions showed a gradient according to socio-economic status, age and gender. Conclusions: The KIDSCREEN-27 seems to be a valid measure of HRQoL in children and adolescents. The effect of a short-term intervention with parents in the Neonatal Intensive Care Unit (NICU) on their knowledge of infant behavioral cues and confidence in caregiving was examined. Ten sets of parents with a total of 22 premature infants born < 32 weeks gestational age admitted to a NICU were enrolled in a time-lag control trial over an 8-month period. The intervention group was given 4 sessions of instructions on preterm infant behavior for a period of 2 weeks. The control group did not receive the instructions. All parents completed two subscales of the Mother and Baby Scale (MABS) at weeks 1 and 3 and a short questionnaire concerning nursing support at week 3. Intervention parents completed a pre- and post-test on knowledge of preterm infant behavioral cues at weeks 1 and 3. There was a significant improvement in the post-test scores concerning knowledge of preterm infant behavioral cues and a higher nursing support score for mothers in the intervention group. Intervention mothers showed no significant improvement in confidence in caregiving. Only half of the intervention group fathers participated in the sessions and there were no significant differences in fathers' scores. While the intervention significantly increased maternal knowledge of infant behavioral cues, there was no significant effect on mothers' confidence in caregiving. Very few fathers participated in the entire intervention. A longer, more intensive program with a larger sample size and finding ways of incorporating more participation from fathers is recommended. © 2007 Elsevier Ireland Ltd. All rights reserved.

Objectives: The aim of this study is to assess the structural and cross-cultural validity of the KIDSCREEN-27 questionnaire. Methods: The 27-item version of the KIDSCREEN instrument was derived from a longer 52-item version and was administered to young people aged 8-18 years in 13 European countries in a cross-sectional survey. Structural and cross-cultural validity were tested using multitrait multi-item analysis, exploratory and confirmatory factor analysis, and Rasch analyses. Zumbo's logistic regression method was applied to assess differential item functioning (DIF) across countries. Reliability was assessed using Cronbach's alpha. Results: Responses were obtained from n = 22,827 respondents (response rate 68.9%). For the combined sample from all countries, exploratory factor analysis with procrustean rotations revealed a five-factor structure which explained 56.9% of the variance. Confirmatory factor analysis indicated an acceptable model fit (RMSEA = 0.068, CFI = 0.960). The unidimensionality of all dimensions was confirmed (INFIT: 0.81-1.15). Differential item functioning (DIF) results across the 13 countries showed that 5 items presented uniform DIF whereas 10 displayed non-uniform DIF. Reliability was acceptable (Cronbach's α = 0.78-0.84 for individual dimensions). Conclusions: There was substantial evidence for the cross-cultural equivalence of the KIDSCREEN-27 across the countries studied and the factor structure was highly replicable in individual countries. Further research is needed to correct scores based on DIF results. The KIDSCREEN-27 is a new short and promising tool for use in clinical and epidemiological studies. © 2007 Springer Science+Business Media B.V.

This study explored the opinions of (para)medical and nursing staff in two Dutch Neonatal Intensive Care Units (NICU's). A questionnaire was used that measured: a) the perceived impact of NIDCAP on several NICU conditions, b) attitudes, subjective norm, perceived behavioral control, knowledge and abilities of using the NIDCAP method (based on the Theory of Planned Behavior) and c) training interest, requirements, information sources and the relevance of the NIDCAP method for different groups of NICU patients. Respondents were positive about NIDCAP and felt that using NIDCAP is fulfilling and leads to improvement of the infant's development, health and well-being. However, NIDCAP was also thought to be time-consuming and might worsen job conditions. The nursing staff, compared to the medical staff, had a more positive attitude (p = .004), higher perceived behavioral control (p = .004) and perceived a more positive impact of NIDCAP on NICU conditions (p = .008). © 2007 Elsevier Ireland Ltd. There is increasing interest in the public health sector in the health-related quality of life (HRQL) of healthy children. However, most HRQL instruments are developed for children with a chronic illness. In addition, existing questionnaires are mostly based on expert opinion about what constitutes HRQL and the opinions and views of healthy children are seldom included. In the European project KIDSCREEN, a generic questionnaire was developed for children between the ages of 8 and 18 on the basis of children's opinions about what constitutes HRQL. Focus group discussions were organised in six European countries to explore the HRQL as perceived by children. There were six groups in each country, stratified by gender and age. The age groups were 8-9 years, 12-13 years, and 16-17 years, with 4-8 children in each group. Experienced moderators guided the discussions. The full discussions were audiotaped, transcribed and content-analysed. The discussions went smoothly, with much lively debate. For the youngest group, the most important aspect of their HRQL was family functioning. For both younger and older adolescents, social functioning, including the relationship with peers, was most important. Children in all groups considered physical and cognitive functioning to be less important than social functioning. These key findings were taken into account when designing the KIDSCREEN HRQL questionnaire for healthy children and adolescents, with more emphasis being placed on drawing up valid scales for family and social functioning. In addition, items were constructed using the language and lay-out preferred by the youngsters themselves. We conclude that focus groups are a useful way of exploring children's views of HRQL, showing that an emphasis should be placed on constructing valid social and family scales. © 2006 Springer Science+Business Media B.V.

This study describes the development and reports the first psychometric results of the European KIDSCREEN-52 generic health-related quality-of-life questionnaire for children and adolescents. The KIDSCREEN-52, including ten dimensions, was applied in a European survey involving 12 countries (i.e., Austria, Switzerland, Czech Republic, Germany, Greece, Spain, France, Hungary, The Netherlands, Poland, Sweden and the UK) and 22,110 children and adolescents aged between 8 and 18 years of age. Questionnaire development included a literature search, expert consultation, and focus group discussions with children and adolescents. After definition of dimensions and collection of items, a translation process following international translation guidelines, cognitive interviews and a pilot test were performed. Analysis regarding psychometric properties showed Cronbach-α ranged from 0.77 to 0.89. Correlation coefficients between KINDLR and KIDSCREEN-52 dimensions were high for those assessing similar constructs (r = 0.51-0.68). All KIDSCREEN-52 dimensions showed a gradient according to socioeconomic status and most dimensions showed a gradient according to psychosomatic health complaints. The first results demonstrate that the KIDSCREEN-52 questionnaire is a promising cross-cultural measure of health-related quality-of-life assessment for children and adolescents in Europe. © 2005 Future Drugs Ltd.

Purpose: To determine the extent of differences and similarities in content between heath-related quality of life (HRQOL) instruments for children and adolescents. Methods: A descriptive and explanatory qualitative approach was carried out. Instruments specifically designed for use with children or adolescents were included. To assure the validity of the findings a triangulation of the analysis and 'member checking' were performed. Results: Ten questionnaires were analyzed. All of them included items referring to physical, psychological, and social aspects of health. A relatively low number of categories explained the content of the questionnaires. Conclusions: A reasonably coherent notion of HRQOL underlies instruments available for children and adolescents. HRQOL measurement in young people is still in its developmental step. © Society for Adolescent Medicine, 2004.

This report is meant as a short manual for the TNO-AZL Preschool Children’s Quality of Life questionnaire (TAPQOL). This short manual should be used in conjunction with the reference data and data entry forms, also provided on the CD. The CD_ROM also provides Dutch, English and French versions of the TAPQOL questionnaires. This short manual describes the TAPQOL questionnaire and the concepts it wants to measure. It provides information on psychometric properties and practical guidelines on how data should be handled and how scale scores should be calculated. In addition, it gives information on the reference sample. Using the TAPQOL in non-commercial studies is free of charge. Using the TAPQOL in a commercial setting is not free.
