Monitoring of esophageal pH is a major component of the evaluation of gastroesophageal reflux disease. A recent innovation has led to the replacement of the intraluminal probe by a wireless capsule. This capsule is attached to the esophageal wall and transmits the appropriate data to a patient-worn receiver. After a short period of time the capsule detaches itself from the wall and is eliminated. We recently encountered a patient in which the capsule did not detach in a timely manner. It was imaged during a preoperative esophagogram.

The anomalous arrangement of bile ducts in the Crocodylia has not been fully appreciated. A clear understanding of biliary anatomy is necessary in order to create complete bile drainage in these reptiles. The object of this study was to clarify the anatomy of the bile ductal system and to establish total bile fistulas in the American alligator, Alligator mississippiensis. Bile duct anatomy was studied in 104 juvenile alligators, and bile fistulas were constructed in seven alligators. In 93 out of 104 (89%) of the juveniles dissected there was an interconnection between the right and left hepatic duct before the right hepatic duct emptied into the gallbladder. The left hepatic duct then entered the duodenum independently of the cystic duct which drained the gallbladder directly into the duodenum. In 8% of the animals, the left hepatic duct did not enter the duodenum but joined with the right duct, forming a common hepatic duct that emptied into the gallbladder. In 3% of the cases, the right hepatic duct emptied into the gallbladder, while the left duct had no communication with the right hepatic duct and drained separately into the duodenum. This arrangement of bile ducts is similar to that seen in birds and reflects the common ancestry of crocodiles and birds. In other reptiles, the biliary system shows much more variability and is different from the alligator. In five of seven alligators in which total biliary diversion was attempted, the biliary catheter remained in place and stayed patent from 2–7 weeks. Bile flow was extremely low (1.5–2.5 ml/24 h) when compared to that of mammals (80–100 ml/24 h). This study demonstrates the variable nature of the biliary ductal system in Alligator mississippiensis and suggest a method for constructing an effective total bile fistula in these animals. J. Exp. To determine the prevalence and appearance of cardiophrenic angle (CPA) varices at computed tomography (CT) in patients with portal hypertension (PHT).A retrospective review was performed of 148 consecutive contrast material--enhanced abdominal CT scans of patients with PHT. The paracardiac region was assessed for tubular structures suggestive of varices. Variceal diameter and CT attenuation relative to adjacent liver were noted.Tubular structures consistent with CPA varices were noted in 29 cases and were more common on the right side than on the left. Mean CPA variceal diameter was 2.6 mm. In three cases, right CPA varices measured 10-13 mm in diameter, but no variceal enhancement was noted on initial dynamic CT images. Delayed CT demonstrated contrast enhancement that reflected delayed enhancement of the portal venous system.CPA varices, particularly on the right side, are not uncommon in patients with PHT. Varices should be considered and excluded as a cause of CPA masses, particularly before percutaneous biopsy. Delayed CT may be necessary to correctly delineate CPA varices.

This article reviews the computed tomography (CT) findings of miscellaneous regional and diffuse small bowel disorders. CT technique and potential pitfalls are discussed. Several categories of regional and diffuse small bowel conditions are reviewed, with representative CT images. These disorders often have relatively nonspecific CT appearances, and correlation with the history, clinical, and laboratory findings in each specific case is critical. In selected conditions, the CT findings are highly specific. The imaging literature of some of the common as well as some of the less common entities is reviewed, and clues to narrowing the differential diagnosis are provided.

We present a case of a 75-year-old male who presented with lower back pain found to have an incidental finding of intramural vesicular fat on an unenhanced computed tomography of the pelvis. This relatively uncommon finding of a normal entity should not be mistaken for other causes of pathology within the urinary bladder.

The postprocedural period is a critical time in which serious complications can manifest. Localization of suspected complications following abdominal and pelvic procedures can be difficult on clinical evaluation alone. For example, abdominal pain after a colonoscopy may vary in etiology and can result from simple colonic spasm to colonic perforation, hemoperitoneum, or even splenic rupture. Vague abdominal pain following a renal biopsy may be due to minimal postprocedural bleeding into and around the kidney or may be due to potentially life-threatening hemorrhage. In such patients, computed tomography can play a crucial role in the rapid identification of complications as well guidance of subsequent patient management. The purpose of this article is to demonstrate the benefit of computed tomography-assisted diagnosis of complications associated with routine procedures performed on or throughout the abdomen and pelvis, including cardiac catheterization, colonoscopy, endoscopy, percutaneous biopsy, and interventional radiology procedures.

Multiple opportunistic infections and neoplasms have been described in the AIDS population over the past 20 years. Four cases of AIDS-related hepatic smooth muscle tumors (leiomyomas) have been described, and of these, three were in children. The fourth case was in a single patient with two hepatic lesions. We describe the imaging features of two adult cases with biopsy-proven hepatic leiomyomas. In one of the cases, the gadolinium-enhanced MRI findings are described, which have not previously been reported.

Acute thermal injury to the esophagus has not been reported previously in the radiographic literature. We present a case of a young adult who developed an intramural "blister" that ultimately communicated with the esophageal lumen. A double-contrast esophagogram outlined the resulting mucosal flap. A brief review of other injuries to the esophagus is included.

Enteral alimentation is a crucial component of care for the malnourished patient who cannot eat. Until recently, long-term alimentation was delivered through nasogastric tubes or gastrostomy tubes placed at surgery. In the past few years, percutaneous endoscopic gastrostomy (PEG) has almost completely supplanted these traditional methods. PEG is a safer and better-tolerated procedure. The advantages of PEG over nasogastric tubes include greater social acceptance and improved cosmetic appearance, increased ease of feedings, and decreased nasal alar deformities and gastroesophageal reflux. Complications are less common with PEG than with open gastrostomy but still occur in as many as 15% of cases [1-3]. Percutaneous gastrostomies performed using fluoroscopic guidance have complications in approximately 10% of cases [4]. Despite a rapid increase in the use of percutaneous gastrostomies and their placement by radiologists [4], few published reports have described imaging findings after the placement of such tubes. This pictorial essay illustrates a spectrum of normal and abnormal imaging findings observed with the use of PEG tubes, including tube migration and misplacement, infection, tumor seeding along the PEG tube track, and a variety of gastric wall defects and pseudomasses.

We report an unusual case of a large esophageal inflammatory fibroid polyp in a man infected with the human immunodeficiency virus complaining of dysphagia. Barium studies and computed tomography demonstrated a long, submucosal-appearing, distal esophageal mass which extended into a hiatal hernia. Inflammatory fibroid polyps should be considered in the differential diagnosis of submucosal and polypoid esophageal masses, although distinctive radiographic features are not found.

The cholecystohepatic duct of Luschka is demonstrated. This anomaly directly connects the gallbladder to the bile ducts, draining a portion of the right lobe of the liver. The pertinent embryology is reviewed. When accidentally severed, it may cause a bile leak leading to biloma formation. It should be recognized before surgery to alert the surgeon.

Elastofibroma, although unusual, should be considered in the diagnosis of a periscapular mass. The ability of magnetic resonance to detect and diagnose these lesions is demonstrated.

Enterolithiasis is a rarely encountered clinical and radiologic entity. Stasis of intestinal contents is the underlying cause of the stone formation. Four cases are presented, 3 in the colon and 1 in the small bowel. The chemical composition of these stones varies according to their site of origin. The clinical setting varies widely as does the radiologic differential diagnosis.

This article describes a severe allergic reaction to a barium sulfate compound used during double-contrast colon examination. This unusual complication is probably induced by chemical agents used as additives in the commercially available barium preparations. The pertinent literature is briefly reviewed.
