Abstract

Introduction: Children with congenital heart disease (CHD) are at higher risk for developmental delays than the general population. The American Heart Association (AHA) published a guideline to address these concerns in 2012. This study determined the neurodevelopmental evaluation and referral practices of practitioners in central South Africa. Method: An online survey was administered to practitioners (n=45) including paediatric cardiologists (n=4), cardiothoracic surgeons (n=4) and general paediatricians (n=37). Information on practitioner characteristics, awareness of the 2012 AHA guideline; and neurodevelopmental evaluation and referral practices was collected. Results: Twenty-one practitioners responded, including paediatric cardiologists (n=4), cardiothoracic surgeons (n=2) and paediatricians (n=15). Data for 20 practitioners was included. Despite most practitioners (n=18) indicating guidelines for the management of development were important, the majority (n=16; 80%) were unaware of the guideline. Most practitioners (n=18; 90%) failed to risk stratify children to identify those to be evaluated. Children with developmental delays were referred for formal developmental evaluation (n=11; 55%) and to intervention therapies (n= 15; 75%). Conclusion: Most practitioners are unaware of the 2012 AHA guideline. Awareness of the developmental risks associated with CHD and implementation of the guideline could promote early identification of developmental delays with referral to intervention therapies.

Abstract

Introduction: Worldwide, cardiovascular disease is associated with substantial economic challenges and profound morbidity and mortality. Considering the dearth of information on cardiovascular disease for the central region of South Africa, this study aimed to assess the profile of patients who were referred to an echocardiography laboratory at a tertiary institution. Method: A hospital-based, observational, descriptive study was conducted. Demographic, anthropometric, socio-economic, clinical and echocardiographic data were collected. Standard transthoracic echocardiograms were performed. Results: The study population had a mean age of 51.8 ± 17.38 years, was predominantly black (64%) with a slight female preponderance (55%). The majority of patients were from a low-socioeconomic background (H0 - H2; 91%). Two-thirds of the participants were hypertensive (64%) and 57% had a body mass index exceeding 25kg/m2. Sixty-three percent of referrals were for routine echocardiographic assessment. Abnormal echocardiographic findings were reported in 74% of patients. Diastolic dysfunction and left ventricular hypertrophy were detected in almost half of all patients (n=1 034; 41%), followed by cardiomyopathies and systolic dysfunction in about one-third (n=804; 32%). Conclusion: This is the first study describing the profile of patients referred for echocardiography in central South Africa. A high percentage of patients had underlying cardiac pathology, especially myocardial dysfunction. [ABSTRACT FROM AUTHOR]

5. Investigation into decadal scale variability in the North Atlantic Ocean

Abstract

The subsurface density of the North Atlantic displays considerable variability on decadal timescales. In this thesis the variability is examined from two directions. First I show direct observations of oceanic Rossby waves, using historical subsurface temperature data in the North Atlantic. Previously, such waves have only been directly observed using satellite data. Subsurface temperature data are used to investigate westward propagating Rossby waves across the North Atlantic at 30.5°N to 34.5°N, during 1970-74 and 1993-97. The latter period is when high resolution satellite altimetry data are available for comparison. The comparison was very favourable, with observed phase speeds east of the mid-Atlantic ridge, 3.6 cm s-1 in the altimeter data and 3.4 cm s-1 for the corresponding period in the hydrographic data. The phase speed of Rossby waves (west of the mid-Atlantic ridge) was observed to be 6.1 cm s-1 in 1970-74, compared with 4.3 cm s-1 in 1993-00. Interannual variability of the phase speed was also observed in the altimeter data, this variability was greater in the west than the east. Data from a coupled climate model (HadCM3) are used to assess the ability of an assimilation scheme, based on temperature-salinity preservation, to represent salinity given temperature data. The assimilation scheme was amended to account for meridional frontal movements. The adjustment improves the pre-assimilated salinity in comparison to the true-salinity in all areas south of 55°N, in the North Atlantic. Finally initial condition experiments are performed to determine the predictability of sea surface temperature (SST) and heat flux. The temperature and salinity fields resulting from the assimilation scheme are used as initial conditions in a twin experiment of HadCM3. When the temperature and salinity of the initial ocean state are perfectly known, the SST anomaly forecast skill is up to 5 years. When the assimilation scheme is used to provide these initial conditions for a forecast, the skill of predicting the SST anomaly is almost 2 years. There was no skill in predicting the fluxes in HadCM3 on a seasonal or annual timescale. However the average winter flux anomaly, when averaged over 5 years was found to be predictable.

Abstract

BACKGROUND: Anomalous left coronary artery from the pulmonary artery (ALCAPA) or Bland-White-Garland syndrome is a rare congenital anomaly that has profound effects on heart function. Patients with ALCAPA present with non-specific symptoms like irritability, feeding difficulties and signs and symptoms of cardiac failure. If left untreated, ALCAPA has a 90% mortality rate in the first year of life, primarily due to myocardial ischemia and heart failure. It is therefore important to diagnose them in time and offer surgery. OBJECTIVES: The primary objective of the study was to determine the clinical presentation and severity of symptoms using the Ross classification and to describe the clinical status of the patients at last follow up, including the clinical outcome (Ross classification) and survival. METHODS: This was a retrospective descriptive study at the Department of Pediatric Cardiology between January 1998 and August 2019. All patients with the diagnosis of ALCAPA during the study period were included. Medical records of patients referred from Free State, Northern Cape and Lesotho were reviewed. Data collection included demographics of patients, clinical presentations, referral diagnosis, echocardiography before and after surgery, catheterization data, surgical data, clinical presentation at last follow up and ECHO at last follow up. Data was captured using the REDCap® online database. The researchers considered a Ross classification of more than two as clinically diagnostic of heart failure. The SF ranges were classified as follows: SF >28% = normal, SF 20-25% = mild, SF 15-20% = moderate and SF <15% =severe. RESULTS: During the study period a total of 30 patients presented to Universitas Paediatric Cardiology Unit with the diagnosis of ALCAPA. At presentation, 89% (n=25) of patients were in cardiac failure with a Ross classification for age of more than two. 37. Haemostatic and thrombotic disorders: a journey from bench to bedside

Abstract

This compilation focuses on thrombotic and haemostatic disorders, illustrating my journey from basic research on thrombotic and haemostatic disorders to the differential diagnoses of these disorders. For purposes of clarity I shall divide it in three parts. The first part includes the testing of antithrombotic agents. My scientific career started with the testing of antithrombotic drugs in a baboon model of arterial thrombosis. These antithrombotic drugs were mostly targeted at platelets and, to a lesser extent, coagulation. For my PhD, I clarified the catabolism, pharmacokinetics and exctretion of recombinant hirudin, an anti-thrombin drug. The second part includes the development of cost-effective diagnostic tests, mostly for von Willebrand disease (VWD), the most common congenital bleeding disorder. This comprises the largest part of the thesis. I developed four anti-thrombotic peptides by using Phage Display technology that I mastered during my post-doctoral study at the University of Leuven in Belgium. I was also part of the group of researchers that developed many new thrombosis models in baboons. For my M.Med Sc study, I developed a flow chamber model to study in vitro endothelial function, which was subsequently used to test the thrombogenicity of tissue-engineered small vessels. This study was the first where endothelial cells were successfully seeded onto decellularised baboon arteries. This study was undertaken in collaboration with the Department of Cardiothoracic Surgery at the University of the Free State, Bloemfontein. The third part is a spin-off from my research on VWD. I established the only Special Haemostasis laboratory of the NHLS, situated at the University of the Free State in Bloemfontein, South Africa, by developing, validating and implementing four diagnostic assays. As a result the laboratory now functions as a reference centre for von Willebrand disease; the most prevalent, but underdiagnosed bleeding disorder in South Africa.
