PURPOSE Limited access to adequate cancer surgery training is one of the driving forces behind global inequities in surgical cancer care. Affordable virtual reality (VR) surgical training could enhance surgical skills in low- and middle-income settings, but most VR and augmented reality systems are too expensive and do not teach open surgical techniques commonly practiced in these contexts. New low-cost VR can offer skill development simulations relevant to these settings, but little is known about how knowledge is gained and applied by surgeons training and working in specific resource-constrained settings. This study addresses this gap, exploring gynecologic oncology trainee learning and user experience using a low-cost VR simulator to learn to perform an open radical abdominal hysterectomy in Lusaka, Zambia. METHODS Eleven surgical trainees rotating through the gynecologic oncology service were sequentially recruited from the University Teaching Hospital in Lusaka to participate in a study evaluating a VR radical abdominal hysterectomy training designed to replicate the experience in a Zambian hospital. Six participated in semi-structured interviews following the training. Interviews were analyzed using open and axial coding, informed by grounded theory. RESULTS Simulator participation increased participants’ perception of their surgical knowledge, confidence, and skills. Participants believed their skills transferred to other related surgical procedures. Having clear goals and motivation to improve were described as factors that influenced success. CONCLUSION For cancer surgery trainees in lower-resourced settings learning medical and surgical skills, even for those with limited VR experience, low-cost VR simulators may enhance anatomical knowledge and confidence. The VR simulator reinforced anatomical and clinical knowledge acquired through other modalities. VR-enhanced learning may be particularly valuable when mentored learning opportunities are limited.

- Abstract
Recent developments of transition-edge sensors (TESs), based on extensive experience in ground-based experiments, have been making the sensor techniques mature enough for their application on future satellite cosmic microwave background (CMB) polarization experiments. LiteBIRD is in the most advanced phase among such future satellites, targeting its launch in Japanese Fiscal Year 2027 (2027FY) with JAXA’s H3 rocket. It will accommodate more than 4000 TESs in focal planes of reflective low-frequency and refractive medium-and-high-frequency telescopes in order to detect a signature imprinted on the CMB by the primordial gravitational waves predicted in cosmic inflation. The total wide frequency coverage between 34 and 448 GHz enables us to extract such weak spiral polarization patterns through the precise subtraction of our Galaxy’s foreground emission by using spectral differences among CMB and foreground signals. Telescopes are cooled down to 5 K for suppressing thermal noise and contain polarization modulators with transmissive half-wave plates at individual apertures for separating sky polarization signals from artificial polarization and for mitigating from instrumental 1/f noise. Passive cooling by using V-grooves supports active cooling with mechanical coolers as well as adiabatic demagnetization refrigerators. Sky observations from the second Sun–Earth Lagrangian point, L2, are planned for 3 years. An international collaboration between Japan, the USA, Canada, and Europe is sharing various roles. In May 2019, the Institute of Space and Astronautical Science, JAXA, selected LiteBIRD as the strategic large mission No. 2.

A 6.5 Tesla superconducting gas-filled solenoid (SOLITAIRE) has been developed at the Heavy Ion Accelerator Facility at the ANU as a reaction product separator. Key features of the device allowing its application for precise measurement of heavy ion fusion cross sections are described. The physical separation of beam particles and the high efficiency (~80%) transport of heavy ion fusion products open up applications in nuclear structure physics, and in materials science. Finally, the developments to allow its application to providing beams of light radioactive isotopes (SOLEROO) are described.

A highly efficient superconducting solenoidal fusion product separator has been developed at the Australian National University in order to enable separation and detection of evaporation residues following heavy-ion collisions. The determination of absolute fusion cross-sections requires an accurate knowledge of the transmission efficiency of evaporation residues through the superconducting solenoid, which in turn depends on the angular distribution of evaporation residues exiting the target. Two methods have been developed to extract the angular distributions using the radial distribution and the velocity distribution of the evaporation residues exiting the solenoid. The angular distributions are compared with existing direct measurements of evaporation residue angular distributions.

- Abstract
We studied harms related to cervical cancer screening and management of screen-positive women in the United States (US) and the Netherlands. We utilized data from four US integrated health care systems (SEARCH), the US National Health Interview Survey, New Mexico state, the Netherlands national histopathology registry, and included studies on adverse health effects of cervical screening. We compared the number of Papanicolaou (Pap) smear tests, abnormal test results, punch biopsies, treatments, health problems (anxiety, pain, bleeding and discharge) and preterm births associated with excisional treatments. Results were age-standardized to the 2007 US population. Based on SEARCH, an estimated 36 million Pap tests were performed in 2007 for 91 million US women aged 21–65 years, leading to 2.3 million abnormal Pap tests, 1.5 million punch biopsies, 0.3 million treatments for precancerous lesions, 5 thousand preterm births and over 8 million health problems. Under the Netherlands screening practice, fewer Pap tests (58%), abnormal test results (64%), punch biopsies (75%), treatment procedures (40%), preterm births (60%) and health problems (63%) would have occurred. The SEARCH data did not differ much from other US data for 2007 or from more recent data up to 2013. Thus compared to the less intensive screening practice in the Netherlands, US practice of cervical cancer screening may have resulted in two- to threefold higher harms, while the effects on cervical cancer incidence and mortality are similar. The results are also of high relevance in making recommendations for HPV screening. Systematic collection of harms data is needed for monitoring and for better incorporation of harms in making screening recommendations.

This paper presents the first cosmological results based on Planck measurements of the cosmic microwave background (CMB) temperature and lensing-potential power spectra. We find that the Planck spectra at high multipoles (ℓ ≳ 40) are extremely well described by the standard spatially-flat six-parameter ΛCDM cosmology with a power-law spectrum of adiabatic scalar perturbations. Within the context of this cosmology, the Planck data determine the cosmological parameters to high precision: the angular size of the sound horizon at recombination, the physical densities of baryons and cold dark matter, and the scalar spectral index are estimated to be θ∗ = (1.04147 ± 0.00062) × 10-2, Ωbh2 = 0.02205 ± 0.00028, Ωch2 = 0.1199 ± 0.0027, and ns = 0.9603 ± 0.0073, respectively(note that in this abstract we quote 68% errors on measured parameters and 95% upper limits on other parameters). For this cosmology, we find a low value of the Hubble constant, H0 = (67.3 ± 1.2) km s-1 Mpc-1, and a high value of the matter density parameter, Ωm = 0.315 ± 0.017. These values are in tension with recent direct measurements of H0 and the magnitude-redshift relation for Type Ia supernovae, but are in excellent agreement with geometrical constraints from baryon acoustic oscillation (BAO) surveys. Including curvature, we find that the Universe is consistent with spatial flatness to percent level precision using Planck CMB data alone. We use high-resolution CMB data together with Planck to provide greater control on extragalactic foreground components in an investigation of extensions to the six-parameter ΛCDM model. We present selected results from a large grid of cosmological models, using a range of additional astrophysical data sets in addition to Planck and high-resolution CMB data. None of these models are favoured over the standard six-parameter ΛCDM cosmology. The deviation of the scalar spectral index from unity isinsensitive to the addition of tensor modes and to changes in the matter content of the Universe. We find an upper limit ofr0.002< 0.11 on the tensor-to-scalar ratio. There is no evidence for additional neutrino-like relativistic particles beyond the three families of neutrinos in the standard model. Using BAO and CMB data, we find Neff = 3.30 ± 0.27 for the effective number of relativistic degrees of freedom, and an upper limit of 0.23 eV for the sum of neutrino masses. Our results are in excellent agreement with big bang nucleosynthesis and the standard value of Neff = 3.046. We find no evidence for dynamical dark energy; using BAO and CMB data, the dark energy equation of state parameter is constrained to be w = -1.13-0.10+0.13. We also use the Planck data to set limits on a possible variation of the fine-structure constant, dark matter annihilation and primordial magnetic fields. Despite the success of the six-parameter ΛCDM model in describing the Planck data at high multipoles, we note that this cosmology does not provide a good fit to the temperature power spectrum at low multipoles. The unusual shape of the spectrum in the multipole range 20 ≲ ℓ ≲ 40 was seen previously in the WMAP data and is a real feature of the primordial CMB anisotropies. The poor fit to the spectrum at low multipoles is not of decisive significance, but is an “anomaly” in an otherwise self-consistent analysis of the Planck temperature data.

Introduction: Impaired healing and non-union of skeletal fractures is a major public health problem, with morbidity exacerbated in patients with diabetes mellitus (DM). DM is prevalent worldwide and affects approximately 25.8 million US adults, with >90% having obesity-related type 2 DM (T2DM). While fracture healing in type 1 DM (T1DM) has been studied using animal models, an investigation into delayed healing in an animal model of T2DM has not yet been performed. Methods: Male C57BL/6J mice at 5 weeks of age were placed on either a control lean diet or an experimental high-fat diet (HFD) for 12 weeks. A mid-diaphyseal open tibia fracture was induced at 17 weeks of age and a spinal needle was used for intra-medullary fixation. Mice were sacrificed at days 7, 10, 14, 21, 28, and 35 for micro-computed tomography (μCT), histology-based histomorphometry and molecular analyses, and biomechanical testing. This article has an erratum: [erratum: http://www.aanda.org/10.1051/0004-6361/201220040e].-- et al., Taking advantage of the all-sky coverage and broadfrequency range of the Planck satellite, we study the Sunyaev-Zeldovich (SZ) and pressure profiles of 62 nearby massive clusters detected at high significance in the 14-month nominal survey. Careful reconstruction of the SZ signal indicates that most clusters are individually detected at least out to R500. By stacking the radial profiles, we have statistically detected the radial SZ signal out to 3 × R500, i.e., at a density contrast of about 50-100, though the dispersion about the mean profile dominates the statistical errors across the whole radial range. Our measurement is fully consistent with previous Planck results on integrated SZ fluxes, further strengthening the agreement between SZ and X-ray measurements inside R500. Correcting for the effects of the Planck beam, we have calculated the corresponding pressure profiles. This new constraint from SZ measurements is consistent with the X-ray constraints from XMM-Newton in the region in which the profiles overlap (i.e., [0.1-1] R 500), and is in fairly good agreement with theoretical predictions within the expected dispersion. At larger radii the average pressure profile is slightly flatter than most predictions from numerical simulations. Combining the SZ and X-ray observed profiles into a joint fit to a generalised pressure profile gives best-fit parameters [P0,c500,γ, α,β] = [6.41,1.81,0.31,1.33,4.13]. Using a reasonable hypothesis for the gas temperature in the cluster outskirts we reconstruct from our stacked pressure profile the gas mass fraction profile out to 3 R500. We present the final results from the XMM-Newton validation follow-up of new Planck galaxy cluster candidates. We observed 15 new candidates, detected with signal-to-noise ratios between 4.0 and 6.1 in the 15.5-month nominal Planck survey. The candidates were selected using ancillary data flags derived from the ROSAT All Sky Survey (RASS) and Digitized Sky Survey all-sky maps, with the aim of pushing into the low SZ flux, high-z regime and testing RASS flags as indicators of candidate reliability. Fourteen new clusters were detected by XMM-Newton, ten single clusters and two double systems. Redshifts from X-ray spectroscopy lie in the range 0.2 to 0.9, with six clusters at z > 0.5. Estimated masses (M500) range from 2.5 × 1014 to 8 × 1014 MM·. We discuss our results in the context of the full XMM-Newton validation programme, in which 51 new clusters have been detected. This includes four double and two triple systems, some of which are chance projections on the sky of clusters at different redshifts. We find thatassociation with a source from the RASS-Bright Source Catalogue is a robust indicator of the reliability of a candidate, whereas association with a source from the RASS-Faint Source Catalogue does not guarantee that the SZ candidate is a bona fide cluster. Nevertheless, most Planck clusters appear in RASS maps, with a significance greater than 2σ being a good indication that the candidate is a real cluster. Candidate validation from association with SDSS galaxy overdensity at z > 0.5 is also discussed. The full sample gives a Planck sensitivity threshold of Y500 4 × 10-4 arcmin2, with indication for Malmquist bias in the Y X-Y500 relation below this threshold. The corresponding mass threshold depends on redshift. Systems with M500 > 5 × 1014 MM at z > 0.5 are easily detectable with Planck. The newly-detected clusters follow the YX-Y500 relation derived from X-ray selected samples. Compared to X-ray selected clusters, the new SZ clusters have a lower X-ray luminosity on average for their mass. There is no indication of departure from standard self-similar evolution in the X-ray versus SZ scaling properties. We present an interesting Sunyaev-Zel'dovich (SZ) detection in the first of the Arcminute Microkelvin Imager (AMI) 'blind', degree-square fields to have been observed down to our target sensitivity of 100{\mu}Jy/beam. In follow-up deep pointed observations the SZ effect is detected with a maximum peak decrement greater than 8 \times the thermal noise. No corresponding emission is visible in the ROSAT all-sky X-ray survey and no cluster is evident in the Palomar all-sky optical survey. Compared with existing SZ images of distant clusters, the extent is large (\approx 10') and complex; our analysis favours a model containing two clusters rather than a single cluster. Our Bayesian analysis is currently limited to modelling each cluster with an ellipsoidal or spherical beta-model, which do not do justice to this decrement. Fitting an ellipsoid to the deeper candidate we find the following. (a) Assuming that the Evrard et al. (2002) approximation to Press & Schechter (1974) correctly gives the number density of clusters as a function of mass and redshift, then, in the search area, the formal Bayesian probability ratio of the AMI detection of this cluster is 7.9 \times 10^4:1; alternatively assuming Jenkins et al. (2001) as the true prior, the formal Bayesian probability ratio of detection is 2.1 \times 10^5:1. (b) The cluster mass is MT,200 = 5.5+1.2\times 10^14h-1M\odot. (c) Abandoning a physical model with num- -1.3 70 ber density prior and instead simply modelling the SZ decrement using a phenomenological {\beta}-model of temperature decrement as a function of angular distance, we find a central SZ temperature decrement of -295+36 {\mu}K - this allows for CMB primary anisotropies, receiver -15 noise and radio sources. 28 páginas, 22 figuras, 3 tablas.-- Planck Collaboration: et al., We present the first all-sky sample of galaxy clusters detected blindly by the Planck satellite through the Sunyaev-Zeldovich (SZ) effect from its six highest frequencies. This early SZ (ESZ) sample is comprised of 189 candidates, which have a high signal-to-noise ratio ranging from 6 to 29. Its high reliability (purity above 95%) is further ensured by an extensive validation process based on Planck internal quality assessments and by external cross-identification and follow-up observations. Planck provides the first measured SZ signal for about 80% of the 169 previously-known ESZ clusters. Planck furthermore releases 30 new cluster candidates, amongst which 20 meet the ESZ signal-to-noise selection criterion. At the submission date, twelve of the 20 ESZ candidates were confirmed as new clusters, with eleven confirmed using XMM-Newton snapshot observations, most of them with disturbed morphologies and low luminosities. The ESZ clusters are mostly at moderate redshifts (86% with z below 0.3) and span more than a decade in mass, up to the rarest and most massive clusters with masses above 1 × 1015 M⊙.

14 páginas, 10 figuras, 6 tablas.-- Planck Collaboration: et al., All-sky data from the Planck survey and the Meta-Catalogue of X-ray detected Clusters of galaxies (MCXC) are combined to investigate the relationship between the thermal Sunyaev-Zeldovich (SZ) signal and X-ray luminosity. The sample comprises ~1600 X-ray clusters with redshifts up to ~1 and spans a wide range in X-ray luminosity. The SZ signal is extracted for each object individually, and the statistical significance of the measurement is maximised by averaging the SZ signal in bins of X-ray luminosity, total mass, or redshift. The SZ signal is detected at very high significance over more than two decades in X-ray luminosity (1043ergs-1 ≲ L500E(z)−7/3 ≲ 2 × 1045ergs-1). The relation between intrinsic SZ signal and X-ray luminosity is investigated and the measured SZ signal is compared to values predicted from X-ray data. Planck measurements and X-ray based predictions are found to be in excellent agreement over the whole explored luminosity range. No significant deviation from standard evolution of the scaling relations is detected. For the first time the intrinsic scatter in the scaling relation between SZ signal and X-ray luminosity is measured and found to be consistent with the one in the luminosity – mass relation from X-ray studies. 14 páginas, 6 figuras, 2 tablas.-- Planck Collaboration: et al., We present precise Sunyaev-Zeldovich (SZ) effect measurements in the direction of 62 nearby galaxy clusters (z < 0.5) detected at high signal-to-noise in the first Planck all-sky data set. The sample spans approximately a decade in total mass, 2 × 1014 M⊙ < M500 < 2 × 1015 M⊙, where M500 is the mass corresponding to a total density contrast of 500. Combining these high quality Planck measurements with deep XMM-Newton X-ray data, we investigate the relations between DA2 Y500, the integrated Compton parameter due to the SZ effect, and the X-ray-derived gas mass Mg,500, temperature TX, luminosity LX,500, SZ signal analogue YX,500 = Mg,500 × TX, and total mass M500. After correction for the effect of selection bias on the scaling relations, we find results that are in excellent agreement with both X-ray predictions and recently-published ground-based data derived from smaller samples. 20 páginas, 12 figuras, 2 tablas.-- Planck Collaboration: et al., We present the XMM-Newton follow-up for confirmation of Planck cluster candidates. Twenty-five candidates have been observed to date using snapshot (~10ks) exposures, ten as part of a pilot programme to sample a low range of signal-to-noise ratios (4 < S/N < 6), and a further 15 in a programme to observe a sample of S/N > 5 candidates. The sensitivity and spatial resolution of XMM-Newton allows unambiguous discrimination between clusters and false candidates. The 4 false candidates have S/N ≤ 4.1. A total of 21 candidates are confirmed as extended X-ray sources. Seventeen are single clusters, the majority of which are found to have highly irregular and disturbed morphologies (about ~70%). The remaining four sources are multiple systems, including the unexpected discovery of a supercluster at z = 0.45. For 20 sources we are able to derive a redshift estimate from the X-ray Fe K line (albeit of variable quality). The new clusters span the redshift range 0.09 ≲ z ≲ 0.54, with a median redshift of z ~ 0.37. A first determination is made of their X-ray properties including the characteristic size, which is used to improve the estimate of the SZ Compton parameter, Y500. The follow-up validation programme has helped to optimise the Planck candidate selection process. It has also provided a preview of the X-ray properties of these newly-discovered clusters, allowing comparison with their SZ properties, and to the X-ray and SZ properties of known clusters observed in the Planck survey. Our results suggest that Planck may have started to reveal a non-negligible population of massive dynamically perturbed objects that is under-represented in X-ray surveys. 7 páginas, 3 figuras, 2 tablas.-- Planck Collaboration: et al., We present first results on PLCKG266.6−27.3, a galaxy cluster candidate detected at a signal-to-noise ratio of 5 in the Planck All Sky survey. An XMM-Newton validation observation has allowed us to confirm that the candidate isa bona fide galaxy cluster. With these X-ray data we measure an accurate redshift, z = 0.94 ± 0.02, and estimate the cluster mass to be M500 = (7.8 ± 0.8) × 1014 M⊙. PLCKG266.6−27.3 is an exceptional system: its luminosity of LX [0.5−2.0 keV] = (1.4 ± 0.05) × 1045 erg s-1 equals that of the two most luminous known clusters in the z > 0.5 universe, and it is one of the most massive clusters at z ~ 1. Moreover, unlike the majority of high-redshift clusters, PLCKG266.6−27.3 appears to be highly relaxed.
