Abstract

Dopamine (DA) neurons in the ventral tegmental area (VTA) are heterogeneous and differentially regulate ingestive and locomotor behaviors that affect energy balance. Identification of which VTA DA neurons mediate behaviors that limit weight gain has been hindered, however, by the lack of molecular markers to distinguish VTA DA populations. Here, we identified a specific subset of VTA DA neurons that express neurotensin receptor-1 (NtsR1) and preferentially comprise mesolimbic, but not mesocortical, DA neurons. Genetically targeted ablation of VTA NtsR1 neurons uncouples motivated feeding and physical activity, biasing behavior toward energy expenditure and protecting mice from age-related and diet-induced weight gain. VTA NtsR1 neurons thus represent a molecularly defined subset of DA neurons that are essential for the coordination of energy balance. Modulation of VTA NtsR1 neurons may therefore be useful to promote behaviors that prevent the development of obesity.

4. Towards environmentally sustainable healthcare: using quality improvement to deliver a net zero NHS.

Abstract

The healthcare industry is a major contributor to climate change globally. There is growing interest in using quality improvement methods to improve the sustainability of healthcare. East London NHS Foundation Trust uses quality improvement as its approach to solving complex problems. This article uses a case study methodology to describe how the trust's programme of sustainability used quality improvement at both systemic and local levels to support the organisation to reduce direct greenhouse gas emissions by 40% by 2025 and indirect emissions by 40% by 2036. Using quality improvement in a structured way enabled staff and service users to consider their contribution to sustainability and develop, test and measure ideas that were within their control. This led to an organisational reduction in gas emissions of 37% and CO2 emissions by 14% in winter months. At a local level, three case studies showcase the accomplishments of teams who successfully reduced medication waste by 66.2kg CO2e per year, reduced spending on single-use plastics by 54% and lowered CO2 emissions by 10798kg per year through the return of walking aids. Supporting structures for the programme included active and visible participation from senior stakeholders, with service users and carers involved at both strategic and operational levels, and tracking data over time. Abstract

Abstract Background Despite dedicated emergency theatre, emergency surgery can be often delayed due to competing urgencies, suggesting a need for innovative theatre time management. Aim To investigate if a change in the emergency theatre prioritisation affects outcomes for a common urgent operation such as appendicectomy. Methods We prospectively recorded data from 67 patients undergoing appendicectomy, for two cohorts of patients: before and after change in theatre prioritisation: Group 1 (Jan-Mar) and 2 (Aug-Oct) respectively. Demographic and peri-operative data, time from admission to surgery, postoperative length of stay and total length of stay and complications were compared. Results The two groups were comparable with regards to gender, age, time of admission and histological confirmation of appendicitis. No differences between the two groups were found regarding time from admission to surgery (24.4 (95% CI 11.2;27.6) hours versus 16.1 (95% CI 10.4;21.7) hours, Mann-Whitney U test, p = 0.35), postoperative length of stay (90.8 (95% CI 61.4;120.1) hours versus 70 (95% CI 48.3;91.6) hours, Mann-Whitney U test, p = 0.25) and total length of stay (115.2 (95% CI 84.6;145.7) hours versus 86 (95% CI 61.6;110.4) hours, Mann-Whitney U test, p = 0.07) as well as complication or re-admission rates. Conclusion A change in the emergency theatre prioritisation does not affect outcome for appendicectomy. Provision of a second emergency theatre could be a solution to reduce the delays in acute surgical operations.
