Background: Chronic/latent viral infections may accelerate immunological aging, particularly among people living with HIV (PLWH). We characterized chronic/latent virus infections across their lifespan and investigated their associations with leukocyte telomere length (LTL). Methods: Participants enrolled in the CARMA cohort study were randomly selected to include n = 15 for each decade of age between 0 and >60 y, for each sex, and each HIV status. Cytomegalovirus (CMV), Epstein–Barr virus (EBV), human herpesvirus 8 (HHV-8), herpes simplex virus 1 (HSV-1), and HSV-2 infection were determined serologically; HIV, hepatitis C (HCV), and hepatitis B (HBV) were self-reported. LTLs were measured using monochrome multiplex qPCR. Associations between the number of viruses, LTL, and sociodemographic factors were assessed using ordinal logistic and linear regression modeling. Results: The study included 187 PLWH (105 female/82 male) and 190 HIV-negative participants (105 female/84 male), ranging in age from 0.7 to 76.1 years. Living with HIV, being older, and being female were associated with harbouring a greater number of chronic/latent non-HIV viruses. Having more infections was in turn bivariately associated with a shorter LTL. In multivariable analyses, older age, living with HIV, and the female sex remained independently associated with having more infections, while having 3–4 viruses (vs. 0–2) was associated with a shorter LTL. Conclusions: Our results suggest that persistent viral infections are more prevalent in PLWH and females, and that these may contribute to immunological aging. 1 | INTRODUCTION By 2017, 20.9 million of the approximately 36.7 million people living with HIV had access to antiretroviral therapy (ART), representing progress towards the UNAIDS 90-90-90 targets to [...], Introduction: Heat-stable lopinavir/ritonavir (LPV/r) oral pellets were developed to overcome challenges with administration and storage experienced with previously available tablet and syrup forms of LPV/r prescribed to paediatric HIV patients. We report on the adoption of LPV/r pellets for infants living with HIV in the public sector antiretroviral therapy (ART) programme in Zimbabwe. Methods: Infants aged three months to three years who had been prescribed a LPV/r-based regimen (including ART-naive patients) in fourteen facilities across the country were eligible to receive the pellets. Caregivers were counselled on the new formulation and provided with administration guides. A caregiver questionnaire was administered three to four months after the child initiated on pellets. Data were also extracted from patient ART records. Results and discussion: One hundred and fifty-seven children were enrolled (median age: 21 months; interquartile range 11.8 to 29.4). Survey data from 74 caregivers were included for analysis. Eighty-one per cent of the caregivers preferred pellets while 19% preferred the syrup formulation. Eighty-nine per cent assessed their child's response to taking the pellets as good or very good. Overall, 46% did not report any challenges while 54% reported one or more challenges with using the pellets. Difficulties with administration included: poor taste (36%; 26 participants); swallowing pellets (16%; 12 participants); finishing the dose (14%; 10 participants); and opening the capsule (10%; seven participants). Caregivers who were not confident to instruct others on pellet administration were 5.64 (95% confidence interval 1.45 to 21.95, p = 0.013) times as likely to experience a challenge. Conclusions: A large proportion of caregivers preferred pellets to other formulations of LPV/r and reported a good response to pellets; however, they also reported challenges with administration. Counselling should focus on ensuring that caregivers can confidently administer pellets and are able to instruct others, to ensure high uptake and good adherence to treatment. LPV/r pellets may be an acceptable substitute for other available forms of LPV/r for eligible children under three years if they are currently on or in need of LPV/r-containing regimens; however, challenges with administration still highlight the need for improved drug formulations for paediatric ART patients. Background: Breastfeeding is not recommended for women living with HIV (WLWH) in Canada. We described the prevalence of breastfeeding and explored experiences of care, support, and stigma related to infant feeding. Setting: Quebec, Ontario, and British Columbia (Canada). Methods: Data were obtained from the HIV Women's Sexual and Reproductive Health Cohort Study (CHIWOS) surveys, conducted between 2013 and 2018. Results: Breastfeeding was reported by 73.5% of the 786 women who delivered before HIV diagnosis and 7.3% of the 289 women who delivered after HIV diagnosis. Among them, earlier year of delivery, delivery outside of Canada, and African, Caribbean, Black ethnicity were independently associated with increased odds of breastfeeding. Among WLWH who had a live birth during the last year, 77% (40/52) felt that they had received support regarding infant feeding practices, and 77% (23/30) were concerned that not breastfeeding could lead to them being identified as WLWH. Among 71 women within one year postpartum at any one of the study waves, 89% reported having an undetectable viral load. Conclusion: Breastfeeding experiences were common among WLWH, most often prior to HIV diagnosis. Fear of unintentional HIV status disclosure when not breastfeeding and challenges to maintain an undetectable HIV viral load are important issues to address during postpartum care. : Introduction: Women living with HIV (WLHIV) experience stigma and elevated exposure to violence in comparison with HIV‐negative women. We examined the mediating role of experiencing recent violence in the relationship between stigma and depression among WLHIV in Canada. Methods: We conducted a cohort study with WLHIV in three Canadian provinces. Recent violence was assessed through self‐reported experiences of control, physical, sexual or verbal abuse in the past three months. At Time 1 (2013‐2015) three forms of stigma were assessed (HIV‐related, racial, gender) and at Time 2 (2015‐2017) only HIV‐related stigma was assessed. We conducted structural equation modelling (SEM) using the maximum likelihood estimation method with Time 1 data to identify direct and indirect effects of gender discrimination, racial discrimination and HIV‐related stigma on depression via recent violence. We then conducted mixed effects regression and SEM using Time 1 and Time 2 data to examine associations between HIV‐related stigma, recent violence and depression. Results: At Time 1 (n = 1296), the direct path from HIV‐related stigma (direct effect: β = 0.200, p < 0.001; indirect effect: β = 0.014, p < 0.05) to depression was significant; recent violence accounted for 6.5% of the total effect. Gender discrimination had a significant direct and indirect effect on depression (direct effect: β = 0.167, p < 0.001; indirect effect: β = 0.050, p < 0.001); recent violence explained 23.15% of the total effect. Including Time 1 and Time 2 data (n = 1161), mixed‐effects regression results indicate a positive relationship over time between HIV‐related stigma and depression (Acoef: 0.04, 95% CI: 0.03, 0.06, p < 0.001), and recent violence and depression (Acoef: 1.95, 95% CI: 0.29, 4.42, p < 0.05), controlling for socio‐demographics. There was a significant interaction between HIV‐related stigma and recent violence with depression (Acoef: 0.04, 95% CI: 0.01, 0.07, p < 0.05). SEM analyses reveal that HIV‐related stigma had a significant direct and indirect effect on depression over time (direct effect: β = 0.178, p < 0.001; indirect effect: β = 0.040, p < 0.001); recent violence experiences accounted for 51% of the total effect. Conclusions: Our findings suggest that HIV‐related stigma is associated with increased experiences of recent violence, and both stigma and violence are associated with increased depression among WLHIV in Canada. Objectives: Uganda adapted its policy for prevention of vertical transmission (VT) of HIV transmission as the World Health Organization released Options A, B and B+. We assessed trends in diagnostic testing, breastfeeding practices, maternal and infant antiretroviral therapy (ART), mortality, VT and HIV‐free survival (HFS) among Ugandan infants born to women living with HIV during this period of successive guideline changes. Methods: This is is a retrospective observational study of infants attending early infant diagnosis clinics at two Ugandan hospitals. Results: A total of 1885 infants (48% female) were managed from 2009 to 2017. DNA polymerase chain reaction (PCR) for early infant diagnosis was performed on 1719 infants (92%, one or more PCR tests) and 676 infants (36%, two PCR tests). HIV serology was performed on 90 infants (4.8%). Testing increased over the study period but remained suboptimal, due to high loss to follow‐up (LTFU). A total of 93% of infants were breastfed, for a median of 9.5 months. The duration of breast milk exposure increased over the study period, consistent with guidelines that increasingly encouraged breastfeeding. Nine cases (0.48%) of suspected breast milk transmission were observed. The use of ART increased significantly over the study period. Mortality (3.5%, 2.7% and 1.1%; p = 0.0076) and VT (17%, 12% and 7.4%; p < 0.0001) decreased over the study period (2008–2010, 2011–2012 and 2013–2017, respectively). LTFU values were 31%, 49% and 59% at 6, 12 and 18 months of age, respectively, with only modest improvements over time. HFS could only be conclusively documented in 532 infants (28%) because of LTFU. Conclusions: From 2009 to 2017, outcomes improved among HIV‐exposed infants in Uganda. LTFU remains a barrier to optimal care. [ABSTRACT FROM AUTHOR]

Mother-to-child (MTC) transmission is the primary mode by which infants become infected with human immunodeficiency virus (HIV) in Canada. Transmission may occur in utero, during labour and delivery or via [...], OBJECTIVE: Despite a high uptake of HIV screening and anti-retroviral prophylaxis in Ontario, several cases of mother-to-child (MTC) transmission occur every year. We wished to examine the modifiable factors responsible for MTC HIV transmission in Ontario, in particular HIV testing, antiretroviral prophylaxis and breast-feeding. METHODS: Using the Ontario data from the Canadian Perinatal HIV Surveillance Program, we examined potential correlates of late maternal HIV diagnosis (i.e., diagnosed at or after delivery) among women delivering from 1996 to 2008. To better understand the factors responsible for MTC HIV transmission, we reviewed the medical charts of 35 HIV-infected infants born in Ontario. RESULTS: Among the 645 HIV-infected mothers, 85 (13.2%) had late HIV diagnosis. The proportion with late HIV diagnosis significantly decreased during the study period, but did not differ by race/ethnicity group or maternal exposure category. With respect to the mothers of the 35 HIV-infected infants, 27 (77%) were diagnosed with HIV at or after delivery. The reasons no prenatal HIV test was performed were: not offered, offered but refused, no prenatal care, denied HIV testing history, and offered but not done. Reasons for no or incomplete antiretroviral prophylaxis (ARP) among eight mothers diagnosed prior to or during pregnancy were: refused or non-compliant with ARP, and failed to inform care provider of HIV status. CONCLUSIONS: Despite the recommendation for universal prenatal HIV counseling and voluntary testing adopted in Ontario, MTC transmission continued to occur, mostly due to late HIV diagnosis of the mother. Future work to reduce perinatal HIV infection should focus on enhancing timely HIV testing of pregnant women. Nous avons voulu examiner les facteurs modifiables de la TME du VIH en Ontario, en particulier le depistage du VIH, la prophylaxie antiretrovirale et l'allaitement maternel. METHODE: A l'aide des donnees ontariennes du Programme de surveillance perinatale du VIH au Canada, nous avons examine les correlats possibles du diagnostic maternel tardif du VIH (c.-a-d. durant ou apres l'accouchement) chez les femmes ayant accouche entre 1996 et 2008. Pour mieux comprendre les facteurs de la TME du VIH, nous avons examine les dossiers medicaux de 35 nourrissons infectes par le VIH nes en Ontario. RESULTATS: Sur les 645 meres infectees par le VIH, 85 (13,2 %) avaient recu un diagnostic tardif. La proportion de meres ayant recu un diagnostic tardif du VIH a beaucoup diminue au cours de la periode de l'etude, mais ne differait pas selon la race/le groupe ethnique, ni la categorie d'exposition des meres. En ce qui a trait aux meres des 35 nourrissons infectes par le VIH, 27 (77 %) avaient recu un diagnostic de VIH durant ou apres l'accouchement. Les raisons de l'absence de depistage prenatal du VIH etaient les suivantes : depistage non offert; depistage offert, mais refuse; pas de soins prenatals; deni des antecedents de depistage du VIH; et depistage offert, mais non effectue. Les raisons pour lesquelles la prophylaxie antiretrovirale (PAR) etait incomplete ou absente chez les huit meres diagnostiquees avant ou pendant leur grossesse etaient les suivantes : refus ou non-observance de la PAR; et omission d'informer le fournisseur de soins de l'etat serologique relativement au VIH. CONCLUSIONS: Malgre la recommandation adoptee en Ontario d'offrir universellement le counseling prenatal sur le VIH et le depistage volontaire du VIH, la TME continue a survenir, surtout en raison du diagnostic tardif du VIH chez les meres. Les futurs efforts de reduction des infections perinatales par le VIH devraient etre axes sur l'amelioration du depistage rapide du VIH chez les femmes enceintes. Background: While treatment guidelines for HIV in adults have evolved rapidly with the advent of new antiretroviral (ARV) treatment, those for the prevention of vertical HIV transmission in pregnancy have evolved more slowly due to safety and efficacy concerns. Here we describe Canadian prescribing patterns for ARV treatments during pregnancy and compare them to perinatal HIV prescribing guidelines of the United States Department of Health and Human Services (HHS), that are commonly used in Canada and include recommendations for newly commercialized therapies. Methods: The Canadian Perinatal HIV Surveillance Program (CPHSP) captures annual medical data on mothers living with HIV and their infants from 23 sites across Canada. Women from this cohort who received an ARV treatment during pregnancy and who gave birth between 2004 and 2020 were included in the study. ARV treatments were designated as 'preferred/alternative' as per HHS HIV perinatal guidelines, or 'other than preferred/alternative'. Results: We identified 3673 pregnancies from 2720 women. The proportion of women that conceived while on ARV treatment increased from 29% in 2003 to 90% in 2020. Other than preferred/alternative ARV treatments were received in 1112 (30%) of pregnancies and this was significantly associated with having initiated ARV treatment before conception. Conclusion: In Canada during the study period, a high number of women were prescribed an other than preferred/alternative ARV treatment during pregnancy. Further optimization of ARV treatment in women of childbearing age living with HIV is warranted. [ABSTRACT FROM AUTHOR]

Perinatally infected children living with HIV (CLWH) face lifelong infection and associated inflammatory injury. Chitinase-like 3 protein-1 (CHI3L1) is expressed by activated neutrophils and may be a clinically informative marker of systemic inflammation in CLWH. We conducted a multi-centre, cross-sectional study of CLWH, enrolled in the Early Pediatric Initiation Canadian Child Cure Cohort Study (EPIC4). Plasma levels of CHI3L1, pro-inflammatory cytokines, and markers of microbial translocation were measured by enzyme-linked immunosorbent assays. Longitudinal clinical characteristics (viral load, neutrophil count, CD4+ and CD8+ T-lymphocyte counts, and antiretroviral (ARV) regimen) were abstracted from patient medical records. One-hundred-and-five (105) CLWH (median age 13 years, 62% female) were included in the study. Seventy-seven (81%) had viral suppression on combination antiviral therapy (cART). The median CHI3L1 level was 25 μg/L (IQR 19–39). CHI3L1 was directly correlated with neutrophil count (ρ = 0.22, p = 0.023) and inversely correlated with CD4/CD8 lymphocyte ratio (ρ = −0.35, p = 0.00040). Children with detectable viral load had higher levels of CHI3L1 (40 μg/L (interquartile range, IQR 33–44) versus 24 μg/L (IQR 19–35), p = 0.0047). CHI3L1 levels were also correlated with markers of microbial translocation soluble CD14 (ρ = 0.26, p = 0.010) and lipopolysaccharide-binding protein (ρ = 0.23, p = 0.023). We did not detect differences in CHI3L1 between different cART regimens. High levels of neutrophil activation marker CHI3L1 are associated with poor virologic control, immune dysregulation, and microbial translocation in CLWH on cART. [ABSTRACT FROM AUTHOR]
