Abstract

A 21-year-old woman was diagnosed with desquamative interstitial pneumonitis. Desquamative interstitial pneumonitis is a chronic inflammation of the lungs characterized by shortness of breath and bluish skin. It differs from interstitial pneumonitis in the type of cells involved and response to treatment. She had a history of respiratory problems from birth. Lung biopsy samples taken at 39 months revealed interstitial pneumonitis. Associated shortness of breath and bluish skin were treated with anti-inflammatory drugs for nine years. She was hospitalized for fever and cough at age 19 and discharged on her normal program along with oxygen and cardiopulmonary physical therapy. She received a lung transplant 18 months later. After surgery, biopsies revealed accumulated fibrous tissue in the new lung. Inflammation and other complications were treated with high dose corticosteroids and antibiotics. She may have had a second pneumonia-related illness.

Abstract

A 23-year-old man with asthma was diagnosed with allergic bronchopulmonary aspergillosis. Allergic bronchopulmonary aspergillosis is an allergic reaction to Aspergillus fumigatus characterized by periodic collapse of pulmonary lobes, recurrent fevers and mucous in the sputum. He was admitted to the hospital because of right-sided-chest pain, increased cough, night sweats and weight loss. He was initially treated for pneumonia with antibiotics and his tuberculin skin test was negative. X-rays of his chest showed changes typical of enlarged lymph nodes. His x-rays also revealed the presence of dilated bronchi, which is a diagnostic criterion of aspergillosis. He started corticosteroid therapy and, by two months, most of his symptoms had decreased.
