Background: Recent data shows a 139.9% global increase in aesthetic procedures since 2011, with a notable 190.5% rise in Germany. This study analyzes the current landscape of plastic surgery in German-speaking European countries, focusing on the range of procedures offered in plastic surgery practices in Germany, Austria and Switzerland. We aimed to provide an overview of the current trends and procedures of plastic surgery in the practice setting in Germany, Austria, and Switzerland. Methods: The information sampled from the websites of n = 511 plastic surgery practices (314 German, 113 Austrian, 84 Swiss) were analyzed with regards to general information, as well as procedures offered on the websites. A set of different procedures, belonging to each pillar of plastic surgery (i.e., Aesthetic, Reconstructive, Hand, Burn surgery) as well as non-surgical aesthetic procedures was analyzed. Results: Aesthetic surgical procedures were offered most often with 98.8% (506) of all websites analyzed, followed by Hand surgery at 29.7% (152), Reconstructive surgery at 27.8% (142) and Burn surgery at 5.7% (29). Reconstructive and Hand surgery were statistically significantly most commonly offered on Austrian plastic surgery websites, with p = 0.011 and p < 0.001, respectively. Among aesthetic surgical procedures, statistically significant regional differences were found for head and neck, body and breast procedures most commonly offered on Swiss plastic surgery with p = 0.041, p = 0.010 and p = 0.003, respectively, while the procedures of the intimate region were most commonly offered on German websites with p = 0.007.Conclusions: Plastic surgery websites in German-speaking European countries predominantly advertise aesthetic surgical procedures, with other domains of plastic surgery such as reconstructive, hand, and burn surgery being less prominent, suggesting an underrepresentation. Zur Etablierung eines Behandlungsalgorithmus für Rekonstruktion komplexer Weichteildefekte des Vorfußes wurden 133 Vorfußrekonstruktionen zwischen 2008 und 2019 analysiert. Hauttransplantate konnten häufig der mechanischen Belastung nicht standhalten. Lokale Lappenplastiken verzeichneten eine niedrige Komplikationsrate und gute Funktionalität. Bei freien Muskellappen kam es seltener zu schweren Komplikationen als bei Fasziokutanlappen. Die Wahrnehmung der äußeren Ästhetik eines Menschen wird durch eine Vielzahl von Faktoren beeinflusst. In diesem Kontext spielt das Gesicht und im Besonderen die Lippenregion eine wichtige Rolle. In der vorliegenden Studie sollten interkulturelle und ethnische Präferenzen von plastisch-ästhetischen Chirurgen und medizinischen Laien bezüglich des Lippenvolumens und des Oberlippen-Unterlippen-Verhältnisses analysiert werden. Dazu wurde ein Online-Fragebogen entworfen, bei dem an einem weiblichen Gesichtsmodell die Lippenfülle und das Oberlippen-Unterlippen-Verhältnis durch die Teilnehmer der Studie verändert und bewertet werden konnten. Dieser Fragebogen wurde weltweit an mehr als 9000 plastische Chirurgen und medizinische Laien verschickt. 1011 Personen (14%) aus 35 Ländern und vier Kontinenten sandten den Fragebogen vollständig ausgefüllt zurück. Die erfassten Daten wurden gesammelt und mit Hilfe einer Varianzanalyse ausgewertet. Trotz einer bei vielen Parametern transkulturellen Übereinstimmung fanden sich unterschiedliche Ergebnisse hinsichtlich der ästhetischen Einschätzung des Lippenvolumens- Ästhetisch-plastische Chirurgen aus Asien bevorzugen im Vergleich zu den in Europa, Nord-und Südamerika tätigen Chirurgen ein signifikant größeres Lippenvolumen. Die medizinischen Laien in Asien präferierten hingegen ein geringeres Lippenvolumen, wobei in dieser Gruppe abweichend von den plastischen Chirurgen die Südamerikanische Bevölkerung ein größeres Lippenvolumen als attraktiv empfindet. In a globalized world, even though supposedly reliable ideals and surgical objectives exist in reconstructive plastic surgery, the results of our study show that individual and culturally dependent perceptions exist, when it comes to lip aesthetics. An online survey that allowed the user to vary the lip fullness on a face model by varying the volume of the upper and lower lips, as well as the upper to lower lip ratio, was devised. This survey was sent to more than 9000 plastic surgeons and people with no medical educational background. The recorded demographic data was collected and evaluated using an analysis of variance. From this data, the preferred lip volume and lip ratio were determined. Three months after sending the survey, 1011 people (14% response rate) from 35 countries and 4 continents returned the survey in full. Analysis of the surveys revealed statistically significant differences in lip volume: cosmetic surgeons from Asia prefer a larger lip volume, unlike surgeons working in Europe. The average population in Asia, on the other hand, prefers a lower lip volume, which suggests that Asian surgeons have different ideas to those of their potential patients. With regards to lip ratio, 60% of respondents considered the 1.0:1.0 lip ratio of upper to lower lip as the "ideal". Ethnic origin does not seem to play a decisive role in the preferred lip ratio. However, various preferences for the upper to lower lip ratio could be seen among the respective subcategories: age, sex and country of origin. INTRODUCTION: Metabolic diseases like diabetes mellitus often show prolonged healing and chronic wounds. Occlusive wound dressings are known to support wound closure by creating a moist environment which supports collagen synthesis, epithelialization and angiogenesis. We aimed to assess the effect of occlusion on diabetic wound fluid on the cellular level regarding fibroblast activity and angiogenetic response.MATERIAL AND METHODS: 22 split skin donor sites from 22 patients (11 patients with diabetes mellitus) were treated with occlusive dressings intraoperatively. On day 3, fluid and blood serum samples were harvested while changing the dressings. The influence of wound fluid on fibroblasts was assessed by measuring metabolic activity (Alamar Blue assay, Casey Counter), cell stress/death (LDH assay) and migration (in vitro wound healing assay) of fibroblasts. Angiogenesis of endothelial cells (HUVEC) was analyzed with the tube formation assay. Furthermore, a Magnetic Luminex Assay for multi-cytokines detection was performed focusing on inflammatory and pro-angiogenetic cytokines.RESULTS: The influence of wound fluid under occlusive dressings from diabetic patients showed a significantly increased angiogenic response and fibroblast migration compared to the non-diabetic patient group. Additionally, cell stress was increased in the diabetic group. Cytokine analysis showed an increase in VEGF-A in the diabetic group.CONCLUSION: Occlusive dressings may stimulate regenerative effects in diabetic wounds. Our in-vitro study shows the influence of wound fluid under occlusive dressings from diabetic patients on angiogenesis, migration and proliferation of fibroblasts, which are essential modulators of wound healing and scar modulation.
