2. Exploring eHealth Literacy and Patient-Reported Experiences With Outpatient Care in the Hungarian General Adult Population: Cross-Sectional Study

Abstract

BackgroundDigital health, which encompasses the use of information and communications technology in support of health, is a key driving force behind the cultural transformation of medicine toward people-centeredness. Thus, eHealth literacy, assisted by innovative digital health solutions, may support better experiences of care. ObjectiveThe purpose of this study is to explore the relationship between eHealth literacy and patient-reported experience measures (PREMs) among users of outpatient care in Hungary. MethodsIn early 2019, we conducted a cross-sectional survey on a large representative online sample recruited from the Hungarian general population. eHealth literacy was measured with the eHealth Literacy Scale (eHEALS). PREMs with outpatient care were measured with a set of questions recommended by the Organisation for Economic Co-operation and Development (OECD) for respondents who attended outpatient visit within 12 months preceding the survey. Bivariate relationships were explored via polychoric correlation, the Kruskal–Wallis test, and chi-square test. To capture nonlinear associations, after controlling covariates, we analyzed the relationship between eHEALS quartiles and PREMs using multivariate probit, ordinary least squares, ordered logit, and logistic regression models. ResultsFrom 1000 survey respondents, 666 individuals (364 females, 54.7%) were included in the study with mean age of 48.9 (SD 17.6) years and mean eHEALS score of 29.3 (SD 4.9). Respondents with higher eHEALS scores were more likely to understand the health care professionals’ (HCPs’) explanations (χ29=24.2, P=.002) and to be involved in decision making about care and treatment (χ29=18.2, P=.03). In multivariate regression, respondents with lowest (first quartile) and moderately high (third quartile) eHEALS scores differed significantly, where the latter were more likely to have an overall positive experience (P=.02) and experience fewer problems (P=.02). In addition, those respondents had better experiences in terms of how easy it was to understand the HCPs’ explanations (P.05 in all models). ConclusionsWe demonstrated the association between eHealth literacy and PREMs. The potential patient-, physician-, and system-related factors explaining the negative experiences among people with highest levels of eHealth literacy warrant further investigation, which may contribute to the development of efficient eHealth literacy interventions. Further research is needed to establish causal relationship between eHealth literacy and patient-reported experiences.

24. Development of updated population norms for the SF-36 for Hungary and comparison with 1997–1998 norms.

Abstract

Background: Hungarian SF-36 population norm data were last collected in 1997–1998 and have not been updated since, reducing their relevance and limiting their usability and comparability. This study aimed to establish contemporary normative data for the SF-36 domain and standardised summary scores in Hungary and compare them to the previous population norms. Methods: An online cross-sectional survey, including the SF-36v1, was conducted among 1,700 members of the Hungarian adult general population in 2020. The sample demonstrated good representativeness across key sociodemographic characteristics. Normative data were calculated for domains using raw scores and for summary scores using country-specific factor score coefficients derived from exploratory factor analysis. Multivariate linear regression models were performed to examine the association of domain and summary scores with sociodemographic and health-related characteristics. Raw domain scores were compared with the 1997–1998 norms. Results: Males reported higher scores (better health) in seven out of eight domains (p < 0.001). Mean standardised PCS scores decreased, whereas MCS scores increased with age (p < 0.001). Compared to the 1997–1998 population norms, the 18–24 and 25–34 age groups reported lower, while the 65 + age group reported higher scores in all eight domains. Higher scores were reported in 2020 from the 35–44 age group onward on the role physical, bodily pain, social functioning, and role emotional domains. Conclusions: This study established contemporary population norms for the SF-36 in Hungary. Our results highlight the changes in health status in the general population, particularly in young adults, compared to the 1997–1998 population norms, and provide valuable input to inform decision-makers. Abstract

This study aims to explore the measurement agreement between direct and indirect health utility measures in four chronic dermatological conditions (atopic dermatitis, hidradenitis suppurativa, pemphigus, psoriasis). Outpatients survey data collected between 2015 and 2021 were analysed. Health-related quality of life (HRQoL) outcome measures included time trade-off (TTO), EQ-5D-5L and Dermatology Life Quality Index (DLQI). Descriptive statistics were computed for the pooled sample and four diseases. Mean, standard deviation (SD), median, interquartile range (IQR), ceiling and floor effects were calculated for TTO, EQ-5D-5L and DLQI utilities. Bland‒Altman plots and intraclass correlation coefficients (ICC) were applied to investigate the agreement between health utility measures. Sociodemographic characteristics (age, sex, educational level, employment status) and health-related information (disease duration, outpatient care visits in the past 3 months and disease severity) impact on utilities was investigated by Tobit regressions. The sample includes N = 765 patient responses with a mean age of 41.5 (SD = 16.2), majority being males (52.7%). Total sample mean utilities were the highest according to TTO (0.83), followed by EQ-5D-5L and vDLQI (0.81 and 0.81) and lowest in mDLQI (0.77). Measurement agreement was found only between TTO and EQ-5D-5L. Skin-disease severity impacted all health state utilities, though only TTO differentiated utility values according to disease type. The discrepancies between the TTO and DLQI warn to compare DLQI-based utilities in different dermatological conditions with extreme caution. [ABSTRACT FROM AUTHOR]
