Abstract

Large randomised trials have demonstrated that fibrinolytic therapy can reduce mortality in patients with suspected acute myocardial infarction (AMI). The indications for, and contraindications to, this treatment in some categories of patient are disputed, examples being late presentation, elderly patients, and those in cardiogenic shock. This overview aims to help resolve some of the remaining uncertainties. From all trials of fibrinolytic therapy versus control that randomised more than 1000 patients with suspected AMI, information was sought and checked on deaths during the first 5 weeks and on major adverse events occurring during hospitalisation. The nine trials included 58,600 patients, among whom 6177 (10.5%) deaths, 564 (1.0%) strokes, and 436 (0.7%) major non-cerebral bleeds were reported. Fibrinolytic therapy was associated with an excess of deaths during days 0-1 (especially among patients presenting more than 12 h after symptom onset, and in the elderly) but this was outweighed by a much larger benefit during days 2-35. This "early hazard" should not obscure the very clear overall survival advantage that is produced by fibrinolytic therapy. Benefit was observed among patients presenting with ST elevation or bundle-branch block (BBB)--irrespective of age, sex, blood pressure, heart rate, or previous history of myocardial infarction or diabetes--and was greater the earlier treatment began. Among the 45,000 patients presenting with ST elevation or BBB the relation between benefit and delay from symptom onset indicated highly significant absolute mortality reductions of about 30 per 1000 for those presenting within 0-6 h and of about 20 per 1000 for those presenting 7-12 h from onset, and a statistically uncertain benefit of about 10 per 1000 for those presenting at 13-18 h (with more randomised evidence needed in this latter group to assess reliably the net effects of treatment). Fibrinolytic therapy was associated with about 4 extra strokes per 1000 during days 0-1: of these, 2 were associated with early death and so were already accounted for in the overall mortality reduction, 1 was moderately or severely disabling, and 1 was not. This overview indicates that fibrinolytic therapy is beneficial in a much wider range of patients than is currently given such treatment routinely.

Abstract

Pathophysiology of unstable angina involves spasm, plaque rupture, activation of platelets, and coagulation. The incidence and frequency of intracoronary thrombus formation are presently under active assessment in order to establish the potential benefit of thrombolytic therapy. A preliminary study was conducted in patients admitted in our coronary care unit for unstable angina with typical clinical and electrocardiographic criteria and with early coronary angiogram. After exclusion of 4 patients with left main coronary stenosis or contraindications for thrombolysis, 16 patients received thrombolytic infusion and 14 underwent a second coronary angiogram. Seven patients had an intracoronary thrombus (6 nonocclusive, 1 occlusive) and at the second angiogram only 3 nonocclusive thrombi were modified (1 disappeared, 2 were reduced). Moreover, the quantitative Coronary Angiography Analysis System (CAAS) in the 11 cases suitable for analysis did not show any significant changes, especially in the Ambrose type IIB lesions. In-hospital clinical outcome was not influenced by thrombolytic therapy (5 ischemic recurrences, 1 fatal myocardial infarction, 4 emergency and 4 elective revascularization procedures). This short series is in agreement with the literature data. Only one third of patients with active unstable angina remains refractory to conventional therapy. The transient benefit of thrombolysis is limited to patients with demonstrated intracoronary thrombi. Clinical or angiographic improvement are not always in correlation and until now do not seem able to prevent short-term recurrences or the need for revascularization procedures.

26. Effects of a comprehensive rehabilitation programme in patients with three-vessel coronary disease.

Abstract

The aim of the study was to assess the effects of rehabilitation in 46 consecutive three-vessel coronary disease patients who were considered to have no possibility of revascularization; there were 45 males and one female (mean age58) sent in the third week after acute myocardial infarction (N=31) or after unstable angina (N=15). Left ventricular ejection fraction (EF) was normal in 50% of the patients, but 15% had an EF≤0.30. Three patients could not begin their rehabilitation because of unstable angina (N=2) or severe pulmonary oedema (N=1). After a 4-week rehabilitation programme, the comparison of stress tests revealed an increase in functional capacities (maximal work-load = 103.6±27 W before rehabilitation, 126.4±31 W after rehabilitation, P<0.001), and an improvement of the ischaemic threshold [82 ± 32 W before rehabilitation, 91±31 W after rehabilitation, P≤0.05].During long-term follow up [20.8 months], four patients died of cardiac events [8.7%]; all of them had an EF<0.45. Among the 42 living patients 61.9% were asymptomatic, 28.7% had exertional angina, and 9.4% had cardiac complications, and coronary surgery was performed in two cases with good results. The level of return to work was 85% with the mean delay of 1.7 months after rehabilitation. So, rehabilitation in three-vessel coronary disease patients is safe under medical control; improvements in exertional capacities are obvious and give the patients a better self confidence as assessed by the good score of return to work after rehabilitation. 30. Les débits sanguins régionaux dans l'insuffisance cardiaque congestive et l'inhibition de l'enzyme de conversion

Abstract

Resume L'insuffisance cardiaque congestive associe une defaillance myocardique a un dereglement neurohormonal qui comporte une stimulation reflexe du systeme nerveux sympathique et du systeme renine angiotensine aldosterone. Ces deux systemes ont une action synergique; il en resulte une vasoconstriction peripherique avec elevation des resistances vasculaires d'ou une majoration des contraintes hemodynamiques pour le ventricule gauche. L'utilisation des inhibiteurs de l'enzyme de conversion (IEC) permet de mieux comprendre le role du systeme renine angiotensine aldosterone. En effet, la vasodilatation systemique obtenue avec les IEC privilegie les lits vasculaires qui dependent de l'action vasopressive de l'angiotensine II et en premier lieu le systeme vasculaire renal. L'administration des IEC ameliore la filtration glomerulaire et augmente l'excretion sodee. Les autres debits sanguins regionaux dependent peu du systeme renine angiotensine aldosterone et l'administration d'IEC n'entraine pas de vasodilatation cutanee, musculaire ou splanchnique. Neanmoins le flux sanguin cerebral se maintient voire s'accroit en depit de la baisse de pression arterielle systemique et le flux sanguin coronaire est conserve. Les IEC permettent d'obtenir une vasodilatation puissante mais selective et sans tachycardie reflexe. Le rein en est le principal beneficiaire. Il en resulte une excretion sodee accrue qui contribue a l'efficacite therapeutique a long terme des IEC dans le traitement de l'insuffisance cardiaque congestive.

Abstract

Myocardial images with thallium 201 are recorded on a computer with the ECG tracing and the cardiac cycle divided into 16 consecutive images. The patients then receive 99Tcmalbumin for first pass and a subsequent ECG gated series for comparison with the myocardial studies.These images can be superimposed and allow the topographic relationships of thallium uptake and ventricular contractility as reflected by the blood pool, to be seen. Quantitative parameters such as ejection fraction and stroke volume are also obtained. Examples in clinical practice are given.

Abstract

A new type of vicinal base lesion was isolated from the photosensitization of TpdG in aerated aqueous solution. One- and two-dimensional NMR measurements were used together with mass spectrometry to accurately characterize the new adduct. Chemical detection of guanidine provided additional structural information on the base moiety at the 3‘-OH terminal end. Altogether the experiments results were indicative of the occurrence of a covalent bonding between the pyrimidine ring on the 5‘-OH terminal end and the imidazole ring on the 3‘-OH terminal end through a methylene bridge. Photosensitization studies of TpdG, thymidine, and 2‘-deoxyguanosine in the presence of either benzophenone, menadione, or riboflavin associated with isotopic labeling experiments using enriched oxygen and water provided relevant information on the mechanism of formation of the adduct. The results of these experiments clearly demonstrated that the initial event leading to the formation of the lesion is the abstraction of a hydrogen atom from the methyl group of the thymine base moiety of TpdG. This is followed by the addition of the methyl-centered radical to the C-4 atom of the guanine ring which gives rise to the vicinal lesion after reaction with molecular oxygen and subsequent rearrangement.

Abstract

In a case of right ventricular infarction, a negative lung image was observed on the resting thallium-201 scan, with a pulmonary activity much lower than the surrounding background. The myocardial image showed only a moderate inferior hypoactivity and the left ventricular ejection fraction was normal. This seems to be related to right ventricular failure with normal left ventricular function. It represents the opposite mechanism to the increased pulmonary thallium uptake commonly seen in severe left ventricular impairment.

Abstract

Experimental studies showed that CY 222 (MW 2500 - 250 U AXAIC/ mg - 25 Ul/mg) kept the profibrinolytic properties of heparin with a considerable reduction of bleeding risk and that the optimal dose was about 1 000 U AXAIC/kg/day in rabbits. We have tested the dose related effectiveness and tolerance of this drug in 47 patients (pts) with acute pulmonary embolism (PE) less than 5 days. Pts were divided into 3 groups receiving : Group I : 500 (n = 16), Group II : 750 (n = 17), Group III : 1 000 (n = 14) U AXAIC/kg/day by continuous intravenous infusion for ten days. Effectiveness was appreciated by Miller’ s index with pulmonary angiography (PA) performed before, on the 5th day and on the 10 th day of treatment. The decrease of Miller’ s index mean value was comparable in the 3 groups. But the improvement was faster in the group IIIWe noticed 5 recurrent PE (2 in Group I, 1 in Group II and 2 in Group III) of which one was fatal (Group I), and 2 hematoma at the venous punction site in Group III.In conclusion, CY 222 is a safe treatment of PE and the most effective dose seems to be 1 000 U AXA IC/kg/day in spite of minor bleeding.

Abstract

Objective: To examine the association between low-intensity smoking (10 or less cigarettes per day) and all-cause and cause-specific mortality risk among women who smoke and by age at cessation among women who previously smoked., Methods: In this study, 104 717 female participants of the Mexican Teachers' Cohort Study were categorised according to self-reported smoking status at baseline (2006/2008) and were followed for mortality through 2019. We estimated HRs and 95% CIs for all-cause and cause-specific mortality using multivariable Cox proportional hazards regression models with age as the underlying time metric., Results: Smoking as few as one to two cigarettes per day was associated with higher mortality risk for all causes (HR: 1.36; 95% CI 1.10 to 1.67) and all cancers (HR: 1.46; 95% CI 1.05 to 2.02), compared with never smoking. Similarly, slightly higher HRs were observed among participants smoking ≥3 cigarettes per day (all causes HR: 1.43; 95% CI 1.19 to 1.70; all cancers HR: 1.48; 95% CI 1.10 to 1.97; cardiovascular disease HR: 1.58; 95% CI 1.09 to 2.28)., Conclusions: In this large study of Mexican women, low-intensity smoking was associated with higher mortality risk for all causes and all cancers. Abstract

Objective: To determine the exposure to aflatoxin B1 (AFB1) in southern Mexico and the presence of the aflatoxin signature mutation in hepatocellular carcinoma (HCC) tissue from patients from a cancer referral center., Materials and Methods: We estimated the prevalence and distribution of AFB1 in a representative sample of 100 women and men from Chiapas using the National Health and Nutrition Survey 2018-19. We also examined the presence of the aflatoxin signature mutation in codon 249 (R249S), and other relevant mutations of the TP53 gene in HCC tissue blocks from 24 women and 26 men treated in a national cancer referral center., Results: The prevalence of AFB1 in serum samples was 85.5% (95%CI 72.1-93.1) and the median AFB1 was 0.117 pg/μL (IQR, 0.050-0.350). We detected TP53 R249S in three of the 50 HCCs (6.0%) and observed four other G>T transversions potentially induced by AFB1., Conclusion: Our analysis provides evidence that AFB1 may have a relevant role on HCC etiology in Mexico.

Abstract

Objective: To determine the magnitude of mortality due to acute lymphoblastic leukemia (ALL) nationally and by age group, sex, state of residence and insurance status, as well as to evaluate time trends during the period 1998-2018 Materials and methods. We obtained ALL mortality data and estimated age-standardized national, state-level and health insurance mortality rates. We conducted a joinpoint regression analysis to describe mortality trends across the study period and estimate the average annual percent change (AAPC)., Results: In a 20-year period, age-standardized ALL mortality rates increased from 1.6 per 100 000 in 1998 to 1.7 in 2018. Nationally, a constant annual increase in mortality was observed for both sexes (1998-2002 AAPC 0.6 in boys, and 1998-2002 AAPC 0.3 in girls). We observed heteroge-neity in childhood ALL at a state level., Conclusion: Our results reflect the social, economic, geographic diversity of the country. Monitoring and surveillance of this disease is crucial to assess quality of care.

Abstract

Objectives: To describe the prevalence and correlates of depressive symptoms, generalized anxiety disorder (GAD), and perceived negative mental health impact during the SARS-Cov-2 pandemic in Mexico City and evaluate their association with adherence to stay-at-home directives. Methods: Baseline data from a cohort study of 2,016 Mexico City government employees were analyzed using multivariable logistic regression models. Results: Among participants, 17.2% had clinically significant depressive symptoms, 21.6% had probable GAD, and 15.2% reported that the pandemic has had a major impact on their mental health. Factors including the presence of COVID-19 symptoms, self-isolation, and economic difficulties were associated with poor mental health. The presence of depressive symptoms and general anxiety were associated with non-adherence to public health directives, particularly among those who might have experienced these symptoms for the first time during the pandemic. Abstract

Developing methods to segment the liver in medical images, study and analyze it remains a significant challenge. The shape of the liver can vary considerably from one patient to another, and adjacent organs are visualized in medical images with similar intensities, making the boundaries of the liver ambiguous. Consequently, automatic or semi-automatic segmentation of liver is a difficult task. Moreover, scanning systems and magnetic resonance imaging have different settings and parameters. Thus the images obtained differ from one machine to another. In this article, we propose an automatic model-based segmentation that allows building a faithful 3-D representation of the liver, with a mean Dice value equal to 90.3% on CT and MRI datasets. We compare our algorithm with a semi-automatic method and with other approaches according to the state of the art. Our method works with different data sources, we use a large quantity of CT and MRI images from machines in various hospitals and multiple DICOM images available from public challenges. Finally, for evaluation of liver segmentation approaches in state of the art, robustness is not adequacy addressed with a precise definition. Another originality of this article is the introduction of a novel measure of robustness, which takes into account the liver variability at different scales., (Copyright © 2019.
