Utility values are quantitative estimates of the quality of life experienced in specific health states and are a key component of cost-effectiveness modelling. However, existing health state utility values in MS typically focus on physical ability and are generally derived using generic (not disease-specific) measures of quality of life. The objective of the current study was to generate health state utility values for levels of cognitive impairment. We used a direct utility elicitation approach called the time trade-off (TTO) methodology.Materials and Methods: Health state descriptions were created following interviews with healthcare professionals, patients, and caregivers in the United States (n=35), and with healthcare professionals in the UK (n=5). Three health states (mild, moderate, and severe impairment) were defined based upon a well-established and validated test for cognitive dysfunction called the Symbol Digit Modalities Test (SDMT) and described using qualitative interview findings. Next, interviews with members of the general public in the UK were conducted to estimate utility values for each health state using the TTO methodology. The procedure was based on the established Measurement and Valuation of Health (MVH) protocol, which generates values on a scale from 0.0 to 1.0.Results: Mean health state utility values were 0.77 ± 0.24 in “mild impairment” (SDMT 43-40), 0.57 ± 0.26 in “moderate impairment” (SDMT 39-32), and 0.34 ± 0.28 in “severe impairment” (SDMT ≤ 31).Discussion: Results indicate that the public perceives that health states of cognitive slowing (as observed in MS) are associated with a substantial reduction in affected individuals’ health-related quality of life, quantified using the TTO methodology. Health state utility values are required for activities such as modelling disease burden or the cost-effectiveness of new interventions. The current study aimed to generate utility values for health states of varying levels of functional vision in RP, with members of the general public in the UK.Methods: Five health states were defined according to standard clinical measures of visual ability. Health state descriptions were developed following interviews with patients with RP in the UK (n=5). Further interviews were conducted for confirmation with healthcare professionals with specific experience of managing patients with RP in the UK (n=2). Interviews with members of the general public in the UK were conducted to value health states. A time trade-off (TTO) process based on the established Measurement and Valuation of Health (MVH) protocol was used. Due to the ongoing COVID-19 pandemic, all interviews were web-enabled and conducted 1:1 by a trained moderator.Results: In total, n=110 TTO interviews were conducted with members of the UK general public. Mean TTO utility values followed the logical and expected order, with increasing visual impairment leading to decreased utility. Mean values varied between 0.78 ± 0.20 (“moderate impairment”), and 0.33 ± 0.26 (“hand motion” to “no light perception”). Supplementary visual analogue scale (VAS) scores also followed the logical and expected order: mean VAS values varied between 47.95 ± 15.38 (“moderate impairment”) and 17.22 ± 12.49 in (“hand motion” to “no light perception”).Discussion: These data suggest that individuals living with RP have substantially impaired quality of life. Utility values for RP have been elicited here using a method and sample that is suitable for economic modelling and health technology assessment purposes.Keywords: retinitis pigmentosa, inherited retinal disease, health state utility, health-related quality of life, time trade-off, vignette study

19. Use of sedation to relieve refractory symptoms in dying patients

Abstract

Objectives. To document the use of sedation for refractory symptoms in patients admitted to an independent palliative care unit.Method. A prospective descriptive study.Setting. The 7-bed inpatient unit at Sungardens Hospice, Pretoria.Subjects. Patients who required sedation for refractory symptoms in addition to normal palliative care treatment between January and June 2002.Findings. Twenty of 100 consecutive patients admitted required sedation. All had advanced cancer. Their mean age was 68 years. Thirty-six per cent were men and 64% women.Indications. Agitated delirium was the most common reason (45%) for using sedation, followed by intractable vomiting due to inoperable malignant intestinal obstruction in 25% of patients. Three patients with persistent convulsions or myoclonic jerking and 2 patients with severe refractory dyspnoea required some sedation. Intractable pain was the main reason for sedation in only 1 patient.Survival. Mean survival following the start of sedation was 92 hours/3.8 days (range 6 - 369 hours/0.25 - 19.4 days). The combined mean survival recorded in 9 other studies was 57 hours/2.4 days (range 36 - 93.6 hours/1.5 - 3.9 days).Medication. The main drugs used for sedation were midazolam and haloperidol. The mean dosage for midazolam was 18.5 mg/24 hours (range 7.5 - 40 mg) and for haloperidol 8 mg/24 hours (range 5 - 10 mg). For pain relief the mean daily dose of parenteral morphine was 76 mg (range 15 - 260 mg).Conclusion. Use of sedation for the relief of refractory symptoms at Sungardens Hospice is in line with several studies reported in the international literature.

24. The Manchester and Salford 3-D Superficial Deposits Model : a guide to the model and its applications

Abstract

This report summarises the 3-D superficial geology beneath Manchester and Salford (including Trafford Park) and should be used in conjunction with the digital 3-D geological model supplied within the Subsurface Viewer (© Insight Geologische Softwaresysteme GmbH). The area is underlain by a complex sequence of superficial deposits comprising glacial, post-glacial and anthropogenic (man-made) material. For example, the area beneath Trafford Park is characterised by over 40 m of superficial material infilling an incised valley cut into the underlying bedrock. 3-D geological modelling in the shallow sub-surface provides a powerful tool for understanding and predicting potentially difficult engineering ground conditions and in Trafford Park, a powerful tool for assessing aquifer vulnerability and recharge to the underlying Sherwood Sandstone. The geological model shows the top, base and thickness for each natural and artificial deposit within the area. Each unit is coloured according to its lithological composition, lithostratigraphy, inferred permeability and engineering classification. 2-D synthetic cross-sections and 1-D synthetic predictive borehole logs can be generated from the model in any area. For example, a cross-section can be drawn along the route of an existing or proposed road or around a proposed development area. Synthetic borehole logs can be generated to predict potential ground conditions at a point location. The 3-D geological model of superficial deposits in the area is not intended as a replacement for any invasive site investigation. It provides an additional tool for the development of conceptual ground models for a range of users. The model provides a powerful means of assessing the nature of the ground, but with additional 3-D information, not previously available from any existing 2-D geological map. It provides essential applied geoscience information relevant to any requirement for information on ground conditions in the shallow sub-surface.

Abstract

A 3-D geological model of the area between Doncaster and Retford was created in order to characterise the thickness and distribution of superficial deposits to allow hydrogeological domains to be derived above the Sherwood Sandstone aquifer. The overarching aim of the study was to use a domains approach, derived from the output of the 3-D model to assess potential recharge to the Sherwood Sandstone aquifer. The results of the study are intended to help the Environment Agency meet its regulatory requirements under the Water Framework Directive and Catchment Abstraction Management Strategy (CAMS) and form part of its overall East Midlands – Yorkshire Sherwood Sandstone Groundwater Study. The 3-D model revealed a complex sequence of pre-glacial, glacial and post-glacial sediments deposited on a rockhead surface that extends in paces to –26mOD as a result of deep incision into the bedrock. These channels are orientated north-west, south-east or east-west. The sequence of superficial deposits is generally less than 10m thick, increasing to 25m in places to the east and southeast of the project area. The area from Hatfield Moors in the north-east to Misson in the south-east is characterised by a sequence of peat, variably underlain by Blown Sand, Glaciolacustrine silt and clay and “Older River Gravel”. The Glaciolacustrine silt and clay is most thickly developed beneath Hatfield Moors, but is laterally discontinuous. Elsewhere, to the west of the study area, the superficial deposits comprise sand, gravel and till of limited extent and are generally less than 10m thick, except in some fluvial valleys. Seven hydrogeological domains were identified from the 3-D model and other published data sources. The domains were defined in terms of the potential for recharge to occur either directly into the Sherwood Sandstone or through the sequence of superficial deposits. Abstract

The Environmental Information System for Planners (EISP) is a proof of concept web-based system designed to support decision making within the UK planning framework by making information on environmental issues more widely accessible. It incorporates relevant outputs from the Natural Environment Research Council (NERC) Urban Regeneration and the Environment (URGENT) research programme and from research directly commissioned by the Office of the Deputy Prime Minister (ODPM). It supports three principal planning functions carried out by local authorities: pre-planning enquiries, development control decisions and strategic planning. Eleven environmental science themes are incorporated: Air quality, Shallow undermining, Landslide susceptibility, Groundwater protection, Flood risk, Drainage, Land contamination, Proximity to landfill, Biodiversity, Natural and Man-made heritage. Decision flow diagrams represent detailed analysis of workflow in each theme, taking account of best practice, regulatory responsibilities and planning guidance. Industry-standard web technologies integrate the flows and provide access to the system via secure web pages. Underpinning the system is an environmental geographical information system (GIS) containing up-to-date data, information and models relevant to each theme. The modular system design allows new legislation and local priorities and datasets to be easily incorporated. Web technology delivers information and research data that have hitherto been difficult for the non-specialist to access and have therefore been under-exploited. The study has demonstrated a successful application of the principles of e-Governance in an area where informed decisions commonly require specialist information. The system, if rolled out nationally, offers potential economic benefits and efficiency savings for both planners and developers

Abstract

This report summarises the results of a collaborative study jointly funded by the North-west Region of the Environment Agency (the Agency) and the British Geological Survey (BGS). BGS was commissioned to carry out a geological and hydrogeological characterisation of the superficial deposits of the Central Manchester and Salford district as a contribution to a regional groundwater study of the Manchester and Cheshire aquifer being undertaken by the Agency. The overall objective of the study was to use a three-dimensional (3D) model of the superficial deposits to examine potential groundwater-surface water interactions. By integrating the modern 1:10,000 geological map with sub-surface (site investigation) information, the relationships within the quaternary deposits have been characterised and potential hydrogeological pathways between the surface water bodies and the deeper sandstone aquifer were identified. The work has shown that the Permo-Triassic aquifer is largely protected by glacial clay and silt deposits. However, the distribution and thickness of these deposits varies greatly across the area and hydraulic windows have been identified in the lowland areas where recharge may occur. Extensive sandy outwash sheet deposits, which are locally in contact with the underlying Permo- Triassic aquifer, offer the potential for lateral migration of groundwater in perched aquifers. Large areas and thicknesses of man-made material have been described and modelled and these could form potential sources of pollution, especially where they are in direct contact with the major aquifer or perched aquifers. Abstract

The provision of reliable and up-to-date geoscientific information for the urban environment has assumed increasing importance in recent years as legislative changes have forced developers, planning authorities and regulators to consider more fully the implications and impact on the environment of large-scale development initiatives. To comply with the principles of sustainable development, developers are increasingly required to demonstrate that proposals are based on the best possible scientific information and analysis of risk. Nowhere is this more relevant than in the context of urban regeneration, where planning policy guidance gives priority to re-use of previously developed (brownfield) land. In England, brownfield sites suitable for re-development cover an area equivalent to half the size of London.

Abstract

This report, commissioned by Telford and Wrekin Council, provides a strategy for investigating the instability of the Lloyd's Coppice landslip, near Ironbridge. Recommendations are given for a drilling programme that will assist in modelling the geological sequence exposed in the sides of the Coppice, and form a basis for a stability assessment. The recommendations are based on a limited desk study using information provided by the Council, and a one day walkover survey.

Abstract

This Sheet Description provides an account of the geology of the district covered by Geological Sheet 153 Wolverhampton. The district extends from the edge of the Black Country conurbation in the south-east, to Telford in the west, and includes a large tract of rural Green Belt on the Staffordshire–Shropshire borders. The solid rocks at outcrop range from Precambrian to Triassic in age. The oldest rocks, pyroclastic tuffs, originated in an island-arc setting in south polar latitudes; the youngest are continental red-beds that formed in inland sabkha environments, when Britain lay just north of the equator. Although the geological record spans about 560 Ma, the modern landscape has been shaped largely by geological processes that operated during the last two million years. Successive glaciations have modified the landscape, and the superficial deposits of till and glacial outwash that blanket much of the district are the product of a Late Devensian glaciation. The Ironbridge Gorge and other meltwater channels also date from this period. Since the ice retreated about 13 000 years ago, the postglacial history has been one of drainage development, valley incision and terrace aggradation. The district has a long industrial heritage dependent, until recently, on the mineral wealth of the Carboniferous rocks exposed in the South Staffordshire and Coalbrookdale coalfields. During the 18th and 19th centuries, settlements grew and prospered as industries were established, founded on a plentiful supply of coal, clay, ironstone and limestone. The relicts of these industries have left parts of the district with a legacy of difficult ground conditions, aspects of which are described in the applied section of this report. Today, mineral extraction is of less importance but brickclay, fireclay and some opencast coal are still produced, and the Triassic conglomerates are an important source of aggregate. Abstract

This report summarises the results of a collaborative study jointly funded by the Environment Agency and the British Geological Survey (BGS). BGS was commissioned to carry out a geological and hydrogeological characterisation of the superficial drift deposits of the Severn valley, upstream of Shrewsbury. The principal aims of the project were identified as follows: • to assess the heterogeneity of the superficial deposits within the buried channel systems that underlie this part of the River Severn • to identify hydrogeologically significant drift lithologies with respect to ground water storage and aquifer recharge, and • to assess their spatial distribution. A model of the sub-drift topography was constructed, based on information from over 700 boreholes. These revealed an irregular rockhead surface, deeply dissected locally by steep-sided, buried channels and broader, over-deepened depressions. The deepest of these is the Severn Trench, which probably links eastwards with a separate south-easttrending system of buried channels that passes beneath Shrewsbury. The drift varies considerably in thickness but on the lower ground commonly exceeds 40 m and in places reaches 120 m. In the Severn Trench, the sediment infill is dominantly one of clays and silts, but in the more easterly channels there are substantial sand bodies, which provide hydraulic connection between the River Severn and the underlying Permo-Triassic aquifer. A detailed assessment of the drift variation across the district was carried out from a study of borehole records. Despite the marked lateral and vertical variability encountered, it proved possible, by constructing a series of stratigraphic cross-sections, to rationalise and classify the drift into a relatively small number of hydrogeological domains and sub-domains. 43. Wolverhampton urban environmental survey : an integrated geoscientific case study

Abstract

Geoscience and land use information for the Metropolitan Borough of Wolverhampton have been collated, with the assistance of the Metropolitan Authority, and interpreted with the aid of a Geographical Information System (GIS). The digital data can be rapidly manipulated within the GIS to produce customised thematic maps of the urban area. The report is a summary of a study undertaken in the Borough over a period of three years, and is supported by a number of satellite reports which describe in more detail different aspects of the work. The Borough of Wolverhampton lies on the western edge of the South Staffordshire Coalfield. The Western Boundary Fault bisects the Borough and marks the edge of the exposed coalfield. To the west of this structure the Coal Measures are concealed by a thick sequence of Upper Carboniferous and Permo-Triassic redbeds. Glacial deposits, mainly clay-rich till with subordinate sands, gravels and laminated clays, cover much of the area. The till is of variable thickness but due to its impermeability it affords some protection to the migration of potential contaminants. Spoil and waste (made ground) from former coal workings cover much of the eastern part of the Borough which is also extensively undermined. Thematic maps indicating the extent and depth of the made ground, depth of till, extent of undermining and location of landfill sites provide useful planning information.

Abstract

Background: Women account for an increasing proportion of patients with HIV-1 but remain underrepresented in antiretroviral clinical trials.Objective: To evaluate sex-based differences in efficacy and adverse events in treatment-experienced, HIV-positive women and men receiving darunavir-ritonavir therapy over 48 weeks.Design: Multicenter, open-label, phase 3b study designed to enroll a high proportion of women, with sample size determined on the basis of a noninferiority design with a maximum allowable difference of 15% in virologic response favoring men. (ClinicalTrials.gov registration number: NCT00381303)Setting: 65 sites in the United States, Puerto Rico, and Canada.Patients: 287 women and 142 men.Intervention: Patients received darunavir-ritonavir, 600/100 mg twice daily, plus an investigator-selected optimized background regimen.Measurements: Virologic response (HIV RNA <50 copies/mL using a time-to-loss of virologic response [TLOVR] algorithm) and adverse events were assessed over 48 weeks.Results: 67% of patients were women; 84% of patients were black or Hispanic. A higher proportion of women discontinued treatment than men (32.8% vs. 23.2%; P = 0.042); more women than men discontinued treatment for reasons other than virologic failure. Response rates in women and men at week 48 were 50.9% and 58.5%, respectively (intention-to-treat TLOVR), and 73.0% and 73.5%, respectively (TLOVR censored for patients who withdrew for reasons other than virologic failure). The absolute difference in response, based on logistic regression and adjusted for baseline log(10) viral load and CD4(+) cell count, was -9.6 percentage points (95% CI, -19.9 to 0.7 percentage points; P = 0.067) for intention-to-treat TLOVR and -3.9 percentage points (CI, -13.9 to 6.0 percentage points; P = 0.438) for TLOVR population that censored patients who withdrew for reasons other than virologic failure. Adverse events were similar between the sexes. The most common grade 2 to 4 adverse events that were considered at least possibly treatment related in women and men were nausea (5.2% and 2.8%, respectively), diarrhea (4.5% and 4.9%, respectively), and rash (2.1% and 2.8%, respectively).Limitation: Baseline characteristics differed between sexes.Conclusion: Nonsignificant, sex-based differences in response were found during the 48-week study; however, these differences were probably due to higher discontinuation rates in women, suggesting that additional efforts are needed to retain women in clinical trials.
