Abstract

Introduction: Total pancreatectomy (TP) is an uncommon operation, with indications that have not been clearly defined and non-standardized postoperative results. We present a national multicentric study on TP and a comparison with the existing literature Methods: A prospective observational study using data from the national registry of patients after pancreaticoduodenectomy and TP performed for any indication during the study period: January 1 to December 31, 2015 Results: 1016 patients were included from 73 hospitals, 112 of whom had undergone TP. The percentage of TP from the total number of cases was 11%. The mean age was 63.5 years, and 57.2% were males. The most frequently suspected radiological diagnosis was pancreatic cancer (58/112 cases). The most common TP technique was "mesentery artery first" (43/112 cases). Venous resections were performed in 23 patients (20.5%). The percentage of postoperative complications within 90 days was 50%, but major complications (>IIIA) were only 20.7%. The overall 90-day mortality was 8% (9 patients). The average stay was 20.7 days. The 3 most frequent definitive histological diagnoses were: adenocarcinoma of the pancreas, intraductal papillary mucinous neoplasm and chronic pancreatitis. The R0 rate was 67.8%. Conclusions: This study shows that the morbidity and mortality results of TP in Spain are similar or superior to previous publications. Abstract

Fortescue's transformation, Clarke's transformation, and Karrenbauer's transformation are all well-known tools which can be used to conduct a modal analysis of perfectly balanced transposed three-phase lines. For untransposed asymmetrical transmission lines, however, such standard transformations fail. Nevertheless, all these transformation matrices can still play an important role in determining a first order approximation of the exact modal transformation. This paper shows how this can be done by employing a perturbation technique approach. In addition to the theoretical aspects discussed, some application examples concerning a vertical and a triangular line are also presented for illustrative purposes. Abstract

Hepatocellular carcinoma is the most common primary malignancy of the liver and one of the most frequent causes of death in patients with liver cirrhosis. Simultaneously with the recognition of the clinical relevance of this neoplasm, in recent years there have been important developments in the diagnosis, staging and treatment of HCC. Consequently, the Asociacion Espanola para el Estudio del Higado has driven the need to update clinical practice guidelines, continuing to invite all the societies involved in the diagnosis and treatment of this disease to participate in the drafting and approval of the document (Sociedad Espanola de Trasplante Hepatico, Sociedad Espanola de Radiologia Medica, Sociedad Espanola de Radiologia Vascular e Intervencionista y Sociedad Espanola de Oncologia Medica). The clinical practice guidelines published in 2009 accepted as Clinical Practice Guidelines of the National Health System has been taken as reference document, incorporating the most important advances that have been made in recent years. The scientific evidence for the treatment of HCC has been evaluated according to the recommendations of the National Cancer Institute (www.cancer.gov) and the strength of recommendation is based on the GRADE system.

Abstract

The use of osteosynthesis materials when complex fractures are presented is well known. However, the use of these materials has not achieved a correct fixation and reduction of all bone fragments. Therefore, an adhesive for bones would provide a simple and quick method to fix this kind of fractures. The aim of this work is to propose and to evaluate an adhesive based on chitosan hydrogels that could have a potential use as a bone adhesive underwater and will not develop cytotoxic effects. Ionically and covalently crosslinked hydrogels based on chitosan were used in this study. Butt joint test with bovine cancellous bone specimens were used in order to measure the tensile bond strength (TBS) in ideal (completely dry) and physiological (immersed in water at 37 °C) conditions. Additionally, TBS was estimated as a function of time of bone specimens immersed in water at 37 °C. Cell viability was studied using MTT assay and cell morphology on the adhesive surface was examined by scanning electron microscope. Mechanical studies revealed that only covalently crosslinked hydrogels maintain their TBS at physiological condition with respect to the dry environment. In addition, it was observed that the TBS, using only covalently crosslinked hydrogels adhesives, dramatically changes as a function of time and its behavior increases as calcium carbonate and hydroxyapatite is added. Finally,in vitrocell testing of covalently crosslinked hydrogel with calcium carbonate and hydroxyapatite exhibited excellent biocompatibility.
