National audience; The provision of care for patients with end-stage chronic renal failure is an important medical and economic challenge for the Health Insurance. Previous studies have shown a lower cost for home dialysis. More recently, studies have confirmed identical short-term survival rates between haemodialysis and peritoneal dialysis. Notwithstanding, home dialysis techniques utilization remains weak in France. This work aims at: determining the average annual cost of dialysis, per patient and per technique of dialysis, and assessing the global annual cost of dialysis in France, from the Health Insurance perspective. Methodologically, this article provides a static estimation of the cost of dialysis. Costs related to co-morbidities of end-stage chronic renal failure have not been considered. Standard patient care schemes have been outlined by a multidisciplinary expert committee, for each dialysis technique, and have been valorised using publicly available data and tariffs recorded in 2005. Our result show that home dialysis techniques are the less costly, with an average annual cost per patient of 49.9, 49.7 and 50.0 k€ respectively for home haemodialysis, automated peritoneal dialysis, and continuous ambulatory peritoneal dialysis. Autodialysis, autonomous in-center haemodialysis and in-center haemodialysis respectively cost 59.5, 62.3 and 81.5 k€ per patient and per annum. The total 2005 cost of dialysis for the Health Insurance is estimated at 2.1 billion €. Therefore, the development of alternative techniques to in-center haemodialysis, such as home dialysis or autonomous in-center haemodialysis, autodialysis being already well developed, could generate savings for the Health Insurance. From the patient’s perspective, it could also allow the enlightened choice of the best customized technique, less guided by local offer than by medical or social criteria, as well as by the patient’s own opinion.; La prise en charge des patients, souffrant d'insuffisance rénale chronique terminale, représente un enjeu médical et financier important pour l’assurance maladie. Des études anciennes ont montré un coût moindre des techniques de domicile. Des études plus récentes ont confirmé des survies identiques à court terme entre hémodialyse et dialyse péritonéale. Cependant, les techniques de domicile restent encore peu développées en France. L’objectif de cette étude est double : estimer les coûts moyens annuels de la dialyse, par patient et par modalité de dialyse, et déterminer le coût annuel global de la dialyse en France, du point de vue de l’assurance maladie. Sur le plan méthodologique, le présent article propose une estimation statique du coût de la dialyse. Les coûts liés à la prise en charge des comorbidités de l’insuffisance rénale chronique terminale n’ont pas été retenus dans cette étude. Des parcours-types de soins ont été élaborés par un comité d’experts pluridisciplinaire pour chaque technique de dialyse, et valorisés à l’aide des données publiques disponibles et des tarifs de soins en vigueur en 2005. Les résultats sont les suivants : les techniques de dialyse dispensées au domicile sont les moins coûteuses avec un coût moyen annuel de 49,9, 49,7 et 50,0 k€ respectivement pour l’hémodialyse à domicile, la dialyse péritonéale automatisée et la dialyse péritonéale continue ambulatoire. L’autodialyse, l’unité de dialyse médicalisée et l’hémodialyse en centre coûtent par année et par patient respectivement 59,5, 62,3 et 81,5 k€. Le coût global pour l’assurance maladie s’élève à 2,1 milliards € pour 2005. Ainsi, le développement des techniques alternatives à l’hémodialyse en centre, comme les techniques de domicile principalement, mais aussi l’unité de dialyse médicalisée (l’autodialyse étant déjà répandue en France), seraient une piste d’économies potentielles pour l’assurance maladie. Du point de vue du patient, ce développement permettrait en outre un choix éclairé de la technique la plus adaptée, moins contingenté par l’offre locale, mais selon des considérations médicales, sociales ou de préférence du patient.

International audience; We present a summary of the experimental results, the attempts of interpretation at the present stage, and the conclusions of coupled investigations on semi-insulating, n-type and p-type vanadium-doped CdTe crystals. Combining electron paramagnetic resonance and magnetic circular dichroism permits the monitoring and the quantitative assessment of [ V 21] and [ V 31] defect concentrations. Photoabsorption results are presented and are discussed in terms of photoionization processes at various wavelengths. The temperature and the spectral dependence of the steady-state photoconductivity are carefully studied in the near infrared. The results are discussed in terms of electronic transitions for the two vanadium charge states. Finally, the photorefractive performances of the semi-insulating sample are analyzed in a two-beam-coupling experiment and are correlated to the properties of the V 21/V 31 donor states to derive photoionization cross sections
