OBJECTIVE The primary aims of this study were to identify latent profiles of acute stress disorder (ASD) symptoms and to evaluate postconcussive symptom differences across the identified profiles as measured by the Acute Stress Disorder Scale and the Military Acute Concussion Evaluation, respectively. METHOD Participants (N = 315) in the current study were predominantly active-duty (75.0%), enlisted (97.8%) males (97.4%) serving in the U.S. Army (87.8%). Approximately, half of the sample reported being married or engaged (51.1%) and was on average 25.94 (SD = 6.31) years old. Participants were referred to the Air Force Theater Hospital, 332nd Air Expeditionary Wing, Joint Base Balad, Iraq, to be evaluated as part of routine clinical assessment for neurocognitive and psychological symptoms following exposure to a blast. RESULTS A 3-profile solution was identified as the most parsimonious and best-fitting model based on statistical model fit indices. Blast injured service members in Profile 3 had greater ASD total and subscale severity compared to the other 2 subgroups, with effect size estimates largely differing by hyperarousal and reexperiencing symptoms. Furthermore, Profiles 2 and 3 were more likely to demonstrate postconcussive symptoms compared to Profile 1. CONCLUSIONS Findings provide novel information on heterogenous ASD symptom profiles during the acute phase following a blast injury and highlight the relationship between psychological and physical symptoms. Classification of blast-injured service members may help identify at-risk individuals who would benefit from further clinical care and mitigate long-term psychological and neurocognitive issues. Between October 1975 and November 1976, Samuel Beckett began, revised and abandoned a manuscript for a prose piece entitled ‘Long Observation of the Ray’ (at least) twice. After a period of writing for television, Beckett’s next foray into prose resulted in ‘The Voice/Verbatim’, a text that – due to its partial incorporation into Company – resists definition as either draft or abandoned work. This article parses Beckett’s motivations for abandoning (and reabandoning) these pieces and demonstrates that the ‘ray’ and the ‘voice’ partake in a broader conversation in Beckett’s work concerning the visual and the audible. The 1970s, a time of unprecedented modulation between media in Beckett’s writing, see Beckett almost capriciously tailoring images to fit media, or vice versa. His writing desk between 1975 and 1977 was home to ‘ends and odds’ in various media: two stage plays, two TV plays, a litany of poems and prose. Focusing on the writing that immediately preceded his first attempt at ‘Long Observation’, the works that come between its first and second abandonment, and the inception of Beckett’s late prose ‘trilogy’, this article argues that one can witness a negotiation between writing the ‘eye’ and the ‘ear’ as he wrangles with how to put the observations of the ‘ray’ and the overheard ‘voice’ in words. This article contextualises this dynamic within discourses surrounding modernism and the senses, philosophical responses to ‘ocularcentrism’ and the ways in which manuscript research can harmonize with Deleuze’s reading of Beckett’s ‘exhaustive’ language. Alongside this, investigations into Beckett’s late allusions to Dante’s description of Virgil’s hoarse voice or faint appearance and W. B. Yeats’s ‘deepening shades’ reveal this compositional struggle seeping into (or deriving from) Beckett’s reading habits; traversing the ‘ill-heard’ and the ‘ill-seen’, these references encapsulate Beckett’s ongoing struggle to isolate in words the senses of sight and hearing.

Background: In the UK, audiologist-delivered cognitive behavioral therapy (CBT) is a key intervention to alleviate the distress caused by tinnitus and its comorbid hyperacusis. However, the availability of face-to-face CBT is limited, and such therapy involves significant costs. CBT provided via the internet provides a potential solution to improve access to CBT for tinnitus. Purpose: The aim was to perform a preliminary assessment of the effect of a specific program of non-guided internet-based CBT for tinnitus, denoted iCBT(T), in alleviating the problems caused by tinnitus alone or tinnitus combined with hyperacusis. Research Design: This was a retrospective cross-sectional study. Study Sample: The data for 28 people with tinnitus who completed the iCBT(T) program and answered a series of questions about their tinnitus and hearing status were included in the study. Twelve patients reported also having hyperacusis (including five also with misophonia). Data collection and analysis: The iCBT(T) program has seven self-help modules. Anonymous data were collected retrospectively from patients’ answers to the questions in the iCBT(T) initial and final assessment modules. Questionnaires administered within the iCBT(T) program were: 4C Tinnitus Management Questionnaire (4C), Screening for Anxiety and Depression in Tinnitus (SAD-T), and the CBT Effectiveness Questionnaire (CBT-EQ). Results: Responses to the 4C showed a significant improvement from pre- to post-treatment, with a medium effect size. The mean improvement was similar for those with and without hyperacusis. Responses to the SAD-T questionnaire also showed a significant improvement from pre- to post-treatment with a medium effect size. The improvement was significantly greater for participants with tinnitus alone than for participants who also had hyperacusis. For both the 4C and the SAD-T, the improvements were not significantly related to age or gender. Participants’ views of the effectiveness of the iCBT(T) program were assessed using the CBT-EQ. The mean score was 50 out of a maximum of 80, indicating moderately high effectiveness. CBT-EQ scores did not differ for those with and without hyperacusis. Conclusions: Based on this preliminary analysis, the iCBT(T) program showed promising result in improving the ability to manage tinnitus and decreasing symptoms of anxiety and depression. Future studies with larger samples and control group(s) are required to further assess various aspects of this program.

Enhanced recovery after surgery (ERAS) protocols aim to optimize the pre-, intra-, and postoperative care of patients to improve surgery outcomes, reduce complications, decrease length of stay, and more. We aim to perform a systematic review and meta-analysis of ERAS protocols for head and neck cancer surgery with or without microvascular reconstruction.PubMed, Embase, and Web of Science databases were queried, and abstracts were screened independently by 2 investigators.This review was conducted in accordance with the PRISMA guidelines. We included comparative observational studies but excluded animal studies, case reports, and case series.Of 557 articles initially reviewed by title and/or abstract, we identified 30 for full-text screening, and 9 met the criteria for qualitative synthesis. Meta-analysis of length of stay revealed a mean decrease of 1.37 days (95% CI, 0.77-1.96;Implementation of ERAS protocols can lead to decreases in length of stay and opioid drug utilization. However, further high-quality prospective studies of ERAS protocols are needed, especially with stratified analysis of outcomes based on the type of head and neck cancer surgery.
