Mit Wirkung zum 1. März 2020 wird eine nicht-namentliche, laborbasierte Meldepflicht gemäß § 7 Abs. 3 IfSG auf den Nachweis von Infektionen mit N. gonorrhoeae mit verminderter Empfindlichkeit gegenüber Azithromycin, Cefixim oder Ceftriaxon eingeführt. Wie das Epidemiologische Bulletin 10/2020 ausführt ist dabei bereits die verminderte Empfindlichkeit gegen nur eines der drei angegebenen Antibiotika ausreichend und verpflichtet zur Meldung. Zur Meldung verpflichtet sind in Deutschland diagnostizierende Labore. Darüber hinaus sind die behandelnden Ärzte und Ärztinnen zur Ergänzung weiterer klinischer und epidemiologischer Angaben verpflichtet.

Background: Different treatment alternatives exist for psychological disorders. Both clinical and cost effectiveness of treatment are crucial aspects for policy makers, therapists, and patients and thus play major roles for healthcare decision-making. At the start of an intervention, it is often not clear which specific individuals benefit most from a particular intervention alternative or how costs will be distributed on an individual patient level. Objective: This study aimed at predicting the individual outcome and costs for patients before the start of an internet-based intervention. Based on these predictions, individualized treatment recommendations can be provided. Thus, we expand the discussion of personalized treatment recommendation. Methods: Outcomes and costs were predicted based on baseline data of 350 patients from a two-arm randomized controlled trial that compared treatment as usual and blended therapy for depressive disorders. For this purpose, we evaluated various machine learning techniques, compared the predictive accuracy of these techniques, and revealed features that contributed most to the prediction performance. We then combined these predictions and utilized an incremental cost-effectiveness ratio in order to derive individual treatment recommendations before the start of treatment. Results: Predicting clinical outcomes and costs is a challenging task that comes with high uncertainty when only utilizing baseline information. However, we were able to generate predictions that were more accurate than a predefined reference measure in the shape of mean outcome and cost values. Questionnaires that include anxiety or depression items and questions regarding the mobility of individuals and their energy levels contributed to the prediction performance. We then described how patients can be individually allocated to the most appropriate treatment type. For an incremental cost-effectiveness threshold of 25,000 €/quality-adjusted life year, we demonstrated that our recommendations would have led to slightly worse outcomes (1.98%), but with decreased cost (5.42%). Conclusions: Our results indicate that it was feasible to provide personalized treatment recommendations at baseline and thus allocate patients to the most beneficial treatment type. This could potentially lead to improved decision-making, better outcomes for individuals, and reduced health care costs.

Smartphones are increasingly utilized in society and enable scientists to record a wide range of behavioral and environmental information. These information, referred to as Unobtrusive Ecological Momentary Assessment Data, might support prediction procedures regarding the mood level of users and simultaneously contribute to an enhancement of therapy strategies. In this paper, we analyze how the mood level of healthy clients is affected by unobtrusive measures and how this kind of data contributes to the prediction performance of various statistical models (Bayesian methods, Lasso procedures, etc.). We conduct analyses on a non-user and a user level. We then compare the models by utilizing introduced performance measures. Our findings indicate that the prediction performance increases when considering individual users. However, the implemented models only perform slightly better than the introduced mean model. Indicated by feature selection methods, we assume that more meaningful variables regarding the outcome can potentially increase prediction performance.

I examine the architecture of émigré artist Frederick Romberg (1913-1992) through the lens of émigré photographer, Wolfgang Sievers (1913-2007). This multidisciplinary approach of analyzing Romberg’s architecture through the photographs of Wolfgang Sievers serves to better understand Australian migrant art and just as well, the migrant experience and migrant identity. Romberg and Sievers fled the rise of Nazism in Germany and arrived in Australia in 1938, bringing with them the influence of German Modernist traditions. The professional relationship between architect and photographer resulted in a multitude of photographs whose nature of architectural form and designs communicate not just the physical characteristics of buildings, but also the experience of exile, constant artistic interaction, collaboration, and active promotion of modernist aesthetics in Melbourne. Both Romberg’s deliberate use and dependence of Sievers’ Bauhaus-trained trained photographic practice to capture architecture, and likewise, Sievers’ selective photographic captures of Romberg’s Modern architectural forms, provide insight as to what and whom the Modern artist in exile depends on, engages with, and seeks once in a foreign landscape. Focusing on Sievers’ photographs of Romberg’s designs for Stanhill Flats and Newburn flats, the paper contends that documentary photographs further provide a historical reality of the past and architectural practice. The photographs put us, the viewers, in the setting Romberg found himself in and at around the time his buildings were executed; they illuminate the emerging phenomenon of German modernism in Australia during the 1950’s. These photographs, providing flat, almost 360-degree views of architectural forms throughout Romberg’s artistic career, allow for an analysis of the modernist traditions that Romberg employed in his designs. Ultimately, I hope to demonstrate how Romberg and Sievers diligently and assertively worked together in becoming successful artists in Australia once off of the Mosel and Comorin ships with fresh Modernism in their bags.

According to WHO estimates, Neisseria gonorrhoeae accounted for 106 million infections in 2008, a number of cases approximately matching those of Chlamydia infections. Gonococcal infections thus represent one of the most common causes of sexually transmitted diseases worldwide, second only to trichomonas vaginalis infections.This guideline aims to improve patient care by optimizing the diagnosis and treatment of infections due to Neisseria gonorrhoeae and to raise awareness among medical professionals of current treatment options. Infektionen mit Neisseria gonorrhoeae sind nach globalen Schätzungen der WHO mit 106 Mio. Fällen in 2008 etwa gleich häufig wie Chlamydien-Infektionen und gehören damit nach Trichomonas vaginalis-Infektionen zu den häufigsten sexuell übertragenen Infektionen (STI) in der Welt.Ziele der Leitlinie ist die Verbesserung der Versorgung der Patienten durch Optimierung von Diagnostik und Therapie bei Infektionen mit Neisseria gonorrhoeae sowie die Verbesserung der Kenntnisse von Ärztinnen und Ärzte über aktuelle Therapieoptionen.
