Abstract

ObjectivesTo assess the reliability and the minimal clinically important differences (MCID) for FVC% predicted in the Scleroderma Lung Study I and II.MethodsUsing data from SLS I and II (N=300), we evaluated the test-retest reliability for FVC% predicted (FVC%; screening vs. baseline) using intra-class correlation (ICC). MCID estimates at 12 months were calculated in the pooled cohort (SLS-I and II) using 2 anchors: Transition Dyspnea Index (≥change of 1.5 units for improvement and worsening, respectively) and the SF-36 Health Transition question: "Compared to one year ago, how would you rate your health in general now?", where "somewhat better" or "somewhat worse" were defined as the MCID estimates. We next assessed the association of MCID estimates for improvement and worsening of FVC% with patient reported outcomes (PROs) and computer-assisted quantitation of extent of fibrosis (QLF) and of total ILD (QILD) on HRCT.ResultsReliability of FVC%, assessed at a mean of 34 days, was 0.93 for the pooled cohort. The MCID estimates for the pooled cohort at 12 months for FVC% improvement ranged from 3.0 % to 5.3% and for worsening from -3.0% to -3.3%. FVC% improvement by ≥MCID was associated with either statistically significant or numerical improvements in some PROs, QILD, and QLF, while FVC% worsening ≥MCID was associated with statistically significant or numerical worsening of PROs, QILD, and QLF.ConclusionFVC% has acceptable test-retest reliability, and we have provided the MCID estimates for FVC% in SSc-ILD based changes at 12 months from baseline in two clinical trials. 4. Prevention of Incident and Recurrent Major Depression in Older Adults With Insomnia : A Randomized Clinical Trial

Abstract

IMPORTANCE: Older adults with insomnia have a high risk of incident and recurrent depression. Depression prevention is urgently needed, and such efforts have been neglected for older adults. OBJECTIVE: To examine whether treatment of insomnia disorder with cognitive behavioral therapy for insomnia (CBT-I) compared with an active comparator condition, sleep education therapy (SET), prevents major depressive disorder in older adults. DESIGN, SETTING, AND PARTICIPANTS: This assessor-blinded, parallel-group, single-site randomized clinical trial assessed a community-based sample of 431 people and enrolled 291 adults 60 years or older with insomnia disorder who had no major depression or major health events in past year. Study recruitment was performed from July 1, 2012, to April 30, 2015. The trial protocol was modified to extend follow-up from 24 to 36 months, with follow-up completion in July 2018. Data analysis was performed from March 1, 2019, to March 30, 2020. INTERVENTIONS: Participants were randomized to 2 months of CBT-I (n = 156) or SET (n = 135). MAIN OUTCOMES AND MEASURES: The primary outcome was time to incident major depressive disorder as diagnosed by interview and Diagnostic and Statistical Manual of Mental Disorders (Fifth Edition) criteria. Secondary outcome was sustained remission of insomnia disorder before depression event or duration of follow-up. RESULTS: Among 291 randomized participants (mean [SD] age, 70.1 [6.7] years; 168 [57.7%] female; 7 [2.4%] Asian, 32 [11.0%] Black, 3 [1.0%] Pacific Islander, 241 [82.8%] White, 6 [2.1%] multiracial, and 2 [0.7%] unknown), 156 were randomized to CBT-I and 135 to SET. A total of 140 participants (89.7%) completed CBT-I and 130 (96.3%) participants completed SET (chi2 = 4.9, P = .03), with 114 (73.1%) completing 24 months of follow-up in the CBT-I group and 117 (86.7%) in the SET group (chi2 = 8.4, P = .004). 5. Outcomes of neonatal congenital diaphragmatic hernia in a non-ECMO center in a middle-income country: a retrospective cohort study

Abstract

Background: Most studies examining survival of neonates with congenital diaphragmatic hernia (CDH) are in high-income countries. We aimed to describe the management, survival to hospital discharge rate, and factors associated with survival of neonates with unilateral CDH in a middle-income country. Methods: We retrospectively reviewed the medical notes of neonates with unilateral CDH admitted to a pediatric intensive care unit (PICU) in a tertiary referral center over a 15-year period, from 2003–2017. We described the newborns’ respiratory care pathways and then compared baseline demographic, hemodynamic, and respiratory indicators between survivors and non-survivors. The primary outcome measure was survival to hospital discharge. Results: Altogether, 120 neonates were included with 43.3% (52/120) diagnosed antenatally. Stabilization occurred in 38.3% (46/120) with conventional ventilation, 13.3% (16/120) with high-frequency intermittent positive-pressure ventilation, and 22.5% (27/120) with high frequency oscillatory ventilation. Surgical repair was possible in 75.0% (90/120). The overall 30-day survival was 70.8% (85/120) and survival to hospital discharge was 66.7% (80/120). Survival to hospital discharge tended to improve over time (p > 0.05), from 56.0% to 69.5% before and after, respectively, a service reorganization. For those neonates who could be stabilized and operated on, 90.9% (80/88) survived to hospital discharge. The commonest post-operative complication was infection, occurring in 43.3%. The median survivor length of stay was 32.5 (interquartile range 18.8–58.0) days. Multiple logistic regression modelling showed vaginal delivery (odds ratio [OR] = 4.8; 95% confidence interval [CI] [1.1–21.67]; p = 0.041), Apgar score ≥ 7 at 5 min (OR = 6.7; 95% CI [1.2–36.3]; p = 0.028), and fraction of inspired oxygen (FiO2) < 50% at 24 h (OR = 89.6; 95% CI [10.6–758.6]; p < 0.001) were significantly associated with improved survival to hospital discharge. Conclusions: We report a survival to hospital discharge rate of 66.7%. Survival tended to improve over time, reflecting a greater critical volume of cases and multi-disciplinary care with early involvement of the respiratory team resulting in improved transitioning from PICU. Vaginal delivery, Apgar score ≥ 7 at 5 min, and FiO2 < 50% at 24 h increased the likelihood of survival to hospital discharge.

13. Development and Validation of a Phenotyping Computational Workflow to Predict the Biomass Yield of a Large Perennial Ryegrass Breeding Field Trial

Abstract

Increasing dry matter yield (DMY) is the most important objective in perennial ryegrass breeding programs. Current yield assessment methods like cutting are time-consuming and destructive, non-destructive measures such as scoring yield on single plants by visual inspection may be subjective. These assessments involve multiple measurements and selection procedures across seasons and years to evaluate biomass yield repeatedly. This contributes to the slow process of new cultivar development and commercialisation. This study developed and validated a computational phenotyping workflow for image acquisition, processing and analysis of spaced planted ryegrass and investigated sensor-based DMY yield estimation of individual plants through normalized difference vegetative index (NDVI) and ultrasonic plant height data extraction. The DMY of 48,000 individual plants representing 50 advanced breeding lines and commercial cultivars was accurately estimated at multiple harvests across the growing season. NDVI, plant height and predicted DMY obtained from aerial and ground-based sensors illustrated the variation within and between cultivars across different seasons. Combining NDVI and plant height of individual plants was a robust method to enable high-throughput phenotyping of biomass yield in ryegrass breeding. Similarly, the plot-level model indicated good to high-coefficients of determination (R 2) between the predicted and measured DMY across three seasons with R 2 between 0.19 and 0.81 and root mean square errors (RMSE) values ranging from 0.09 to 0.21 kg/plot. The model was further validated using a combined regression of the three seasons harvests. This study further sets a foundation for the application of sensor technologies combined with genomic studies that lead to greater rates of genetic gain in perennial ryegrass biomass yield.

Abstract

Objective People living with HIV (PLWH) exhibit accelerated and premature cognitive aging in comparison to age-matched, seronegative controls. Although opioid use is elevated among PLWH, literature regarding the neurocognitive effects of opioid use across the lifespan of PLWH is limited. This cross-sectional study examines the neurocognitive effects of opioid use across the mid-older adult lifespan of PLWH. Methods One-hundred fifty-two PLWH (72% Latinx; 71% Male; Mdn(IQR) Age = 47(43, 51) years; M Education = 133 years) completed comprehensive neurocognitive, neuromedical, quality of education (Wide Range Achievement Test- 4 [WRAT-4]), psychiatric/substance-use (Composite International Diagnostic Interview [CIDI]), and urine toxicology assessments. Opioid users were defined by DSM diagnostics for lifetime opioid use disorder. A series of LSR tested the interactive effects of age and opioid use across seven neurocognitive domains. Results After controlling for covariates (e.g., CD4 cell count; WRAT-4; comorbid substance use), a least-squares regression demonstrated significant interactive effects between age and opioid use upon verbal fluency, such that older opioid users exhibited greater verbal fluency scores (F [11] = 4.28, p Discussion The moderate observed effect sizes indicate a positive relationship between older age and lifetime opioid use upon verbal fluency among PLWH. These findings may be representative of a selective survival bias among opioid users within this population. Nonetheless, verbal fluency could serve as a marker of greater survivability among PLWH with opioid use histories. Future directions should examine this interaction longitudinally and evaluate differences in the severity/duration of opioid use.

Objective The relationship between self-reported mental health (MH), physical health (PH), and neurocognition (NC) is vastly understudied among Latinx people living with HIV (PLWH). Evidence of increased somatization in this group suggests that self-reported MH and PH may contextualize cognitive outcomes among Latinx PLWH. Thus, this study examines the relationship between self-reported MH/PH and NC in a sample of Latinx PLWH. Method This study included 76 Latinx PLWH (33% female; M age = 46.0, SD = 7.08) who completed the Medical Outcomes Study HIV Health Survey (MOS-HIV) and a comprehensive neurocognitive battery. MH and PH were defined by the MOS-HIV, where higher MH and PH summary scores indicate better perceived health. Demographically-corrected T-scores were used to compute average global NC and domain scores. Results Within Latinx PLWH, partial correlations controlling for age and education found PH was associated with better processing speed (r = .28, p = .03), and MH was related to better global NC, fluency, learning, and processing speed (ps Conclusions Findings indicate that MH and PH correlated with global and domain-specific NC within Latinx PLWH. Better self-reported MH may improve attention/working memory and global NC in Latinx PLWH. Somatization is important when considering MH/PH and cognition within Latinx PLWH; physicians should consider the impact of patients’ self-reported MH/PH on NC in Latinx PLWH.

Abstract

BACKGROUND AND OBJECTIVES: Multiple processes have been identified as potential contributors to chronic pain, with increasing evidence illustrating an association with aberrant levels of neuroimmune mediators. The primary objectives of the present study were to examine central nervous system cytokines, chemokines, and growth factors present in a chronic pain population and to explore patterns of the same mediator molecules over time. Secondary objectives explored the relationship of central and peripheral neuroimmune mediators while examining the levels of anxiety, depression, sleep quality, and perception of pain associated with the chronic pain patient experience. METHODS: Cerebrospinal fluid (CSF) from a population of majority postlaminectomy syndrome patients (N = 8) was compared with control CSF samples (N = 30) to assess for significant differences in 10 cytokines, chemokines, and growth factors. The patient population was then followed over time, analyzing CSF, plasma, and psychobehavioral measures. RESULTS: The present observational study is the first to demonstrate increased mean CSF levels of interleukin-8 (IL-8; P < 0.001) in a small population of majority postlaminectomy syndrome patients, as compared with a control population. Over time in pain patients, CSF levels of IL-8 increased significantly (P < 0.001). CONCLUSIONS: These data indicate that IL-8 should be further investigated and psychobehavioral components considered in the overall chronic pain paradigm. Abstract

Background: Selective kinase and immune checkpoint inhibitors, and their combinations, have significantly improved the survival of patients with advanced metastatic melanoma. Not all patients will respond to treatment however, and some patients will present with significant toxicities. Hence, the identification of biomarkers is critical for the selection and management of patients receiving treatment. Biomarker discovery often involves proteomic techniques that simultaneously profile multiple proteins but few studies have compared these platforms. Methods: In this study, we used the multiplex bead-based Eve Technologies Discovery assay and the aptamer-based SomaLogic SOMAscan assay to identify circulating proteins predictive of response to immunotherapy in melanoma patients treated with combination immune checkpoint inhibitors. Expression of four plasma proteins were further validated using the bead-based Millipore Milliplex assay. Results: Both the Discovery and the SOMAscan assays detected circulating plasma proteins in immunotherapy-treated melanoma patients. However, these widely used assays showed limited correlation in relative protein quantification, due to differences in specificity and the dynamic range of protein detection. Protein data derived from the Discovery and Milliplex bead-based assays were highly correlated. Conclusions: Our study highlights significant limitations imposed by inconsistent sensitivity and specificity due to differences in the detection antibodies or aptamers of these widespread biomarker discovery approaches. Our findings emphasize the need to improve these technologies for the accurate identification of biomarkers.

Abstract

To assess the efficacy of mycophenolate mofetil (MMF) and cyclophosphamide (CYC) on modified Rodnan skin score (MRSS) in participants enrolled in the Scleroderma Lung Study (SLS) I and II. SLSI participants received daily oral CYCor matching placebo for 1 year, whereas SLS IIparticipants received daily MMFfor 2 years or daily oral CYCfor 1 year followed by placebo for second year. We assessed the impact of MMFand CYCon the MRSSin SLS IIover a 24‐month period. We also compared the change in MRSSin patients with diffuse cutaneous systemic sclerosis (dcSSc) assigned to CYCand MMFin SLS IIand SLSI versus placebo in SLSI over a 24‐month period using a linear mixed model. In SLS II, the baseline mean ± SD MRSSwas 14.0 ± 10.6 units for CYCand 15.3 ± 10.4 units for MMF; 58.5% were classified as dcSSc. CYCand MMFwere associated with statistically significant improvements in MRSSfrom baseline over the period of 24 months in dcSSc (P< 0.05 at each time point), but there were no differences between the 2 groups. In the dcSSc subgroup, the change in MRSSfrom baseline to all 6‐month visits was similar in SLS IIgroups (MMF,CYC, pooled cohort [MMF+ CYC]) and in the SLSI CYCgroup and showed statistically significant improvements compared to SLSI placebo at 12, 18, and 24 months (P< 0.05). In SLS II,MMFand CYCtreatment resulted in improvements in MRSSin patients with dcSSc over 24 months. In addition, MMFand CYCtreatment resulted in statistically significant improvements in MRSSin patients with dcSSc when compared with the SLSI placebo group.

Abstract

The measurement of electromagnetic fields and related quantities in a lightning environment is a challenging problem, especially at high frequencies and/or in the immediate vicinity of the lightning arcs and corona. This paper reviews the techniques for accomplishing such measurements in these regimes with examples. These sensors are often the same as for the nuclear electromagnetic pulse (EMP), but significant differences also appear.

Abstract

Electromagnetic sensors for general lightning applications in measuring environment are discussed as well as system response to the environment. This includes electric and magnetic fields, surface current and charge densities, and currents on conductors. Many EMP sensors are directly applicable to lightning measurements, but there are some special cases of lightning measurements involving direct strikes which require special design considerations for the sensors. The sensors and instrumentation used by NMIMT in collecting data on lightning at South Baldy peak in central New Mexico during the 1978 and 1979 lightning seasons are also discussed. The Langmuir Laboratory facilities and details of the underground shielded instrumentation room and recording equipment are presented.

Abstract

Electromagnetic data recorded from lightning strikes are presented. The data analysis reveals general characteristics of fast electromagnetic fields measured at the ground including rise times, amplitudes, and time patterns. A look at the electromagnetic structure of lightning shows that the shortest rise times in the vicinity of 30 ns are associated with leader leader streamers. Lightning location is based on electromagnetic field characteristics and is compared to a nearby sky camera. The fields from both leaders and return strokes were measured and are discussed. The data were obtained during 1978 and 1979 from lightning strikes occuring within 5 kilometers of an underground metal instrumentation room located on South Baldy peak near Langmuir Laboratory, New Mexico. The computer controlled instrumentation consisted of sensors previously used for measuring the nuclear electromagnetic pulse (EMP) and analog-digital recorders with 10 ns sampling, 256 levels of resolution, and 2 kilobytes of internal memory.

Abstract

Two hundred and seven persons over 65 years of age were interviewed to determine the impact of crime on their lives. The setting was during a much publicised series of assaults on the rural elderly in their homes. Seventy two per cent were upset, 40% of the total were moderately upset and 26% suffered a major upset. Females and those over 75 years old were most upset. Some aspect of change of lifestyle was found in the vast majority. peer-reviewed
