Abstract

[english] Objective: In 1998 an international contest for medical students called “Benjamin Franklin Contest” was initiated by the former university hospital of Freie Universität Berlin on the occasion of the 30th anniversary of its foundation in 1968. Since then, seven teams from Germany, Austria, and Switzerland have been competing against each other every year. Five medical students per team match up in paper cases, diagnosis of medical images, multiple choice tests, and practical skills.Methods: All participants of the 11th Benjamin Franklin Contest (2009) were asked about their motivation, preparation strategy, and experience with the contest. Results: In total 92% (32 of 35) participated in the evaluation study. We show that improvement of medical knowledge is one of the key motivating factors behind participation in the competition. The teams were trained by self-study, tutorials, and learning groups. Remarkably, students invested 54.2 hours (SD 32 hours) of their free time to prepare for the contest. Conclusions: The Benjamin Franklin Contest is an innovative approach to facilitate self-directed learning and develope medical competencies. [german] Zielsetzung: Im Jahr 1998 wurde am damaligen Universitätsklinikum Benjamin Franklin der Freien Universität (FU) in Berlin der internationale Medizinstudierendenwettbewerb „Benjamin Franklin Contest“ (BFC) initiiert. Anlass war damals der 30. Jahrestag der Gründung der Universitätsmedizin an der FU. Seitdem findet jedes Jahr ein Wettbewerb zwischen sieben Teams von medizinischen Fakultäten aus Deutschland, Österreich und der Schweiz statt. Jeweils fünf Studierende messen sich in den Kategorien Papierfälle, Blickdiagnosen, Auswahlfragen und praktische Fähigkeiten.Methodik: Ziel dieser Arbeit ist die Beschreibung des Projektes BFC und eine erste Evaluation der Erfahrung von Teilnehmenden. Dazu wurden alle Teilnehmenden des 11. BFC (2009) mittels eines selbst entwickelten Fragebogens hinsichtlich ihrer Motivation, Art und Aufwand der eigenen Vorbereitung und der Erfahrung mit dem Wettbewerb befragt. Ergebnisse: Insgesamt beteiligten sich 92% (32/35) der Teilnehmenden im Jahr 2009 an der Evaluation des Wettbewerbs. Es zeigte sich, dass die Erweiterung medizinischen Wissens zu den Hauptmotiven für eine Teilnahme am BFC gehört. Zur Vorbereitung auf den Wettbewerb wurden neben dem Selbststudium insbesondere Tutorien und Lerngruppen eingesetzt. Der Aufwand lag bei durchschnittlich 54,2 Stunden (Standardabweichung 32 Stunden). Schlussfolgerungen: Das Konzept des BFC kann als innovativer Ansatz selbstmotivierten Lernens verstanden werden. Die Teilnahme am BFC hat das Potential, neben medizinischen Kompetenzen auch die Fähigkeit zur Teamarbeit zu fördern.

Abstract

Various technical developments extended the potential of angle-resolved photoemission (ARPES) tremendously during the last twenty years. In particular improved momentum, energy and spin resolution as well as the use of photon energies from few eV up to several keV makes ARPES a rather unique tool to investigate the electronic properties of solids and surfaces. With our work we present a generalization of the state of the art description of the photoemission process, the so called one-step model that describes excitation, transport to the surface and escape into the vacuum in a coherent way. In particular, we present a theoretical description of temperature-dependent ARPES with a special emphasis on spin fluctuations. Finite temperature effects are included within the so called alloy analogy model which is based on the coherent potential approximation and this way allows to describe uncorrelated lattice vibrations in combination with spin fluctuations quantitatively on the same level of accuracy. To demonstrate the applicability of our approach a corresponding numerical analysis has been applied to spin- and angle-resolved photoemission of Fe(100) at finite temperatures., Comment: Submitted to PRB

Abstract

Therapeutic targets have been defined for axial and peripheral spondyloarthritis (SpA) in 2012, but the evidence for these recommendations was only of indirect nature. These recommendations were re-evaluated in light of new insights. Based on the results of a systematic literature review and expert opinion, a task force of rheumatologists, dermatologists, patients and a health professional developed an update of the 2012 recommendations. These underwent intensive discussions, on site voting and subsequent anonymous electronic voting on levels of agreement with each item. A set of 5 overarching principles and 11 recommendations were developed and voted on. Some items were present in the previous recommendations, while others were significantly changed or newly formulated. The 2017 task force arrived at a single set of recommendations for axial and peripheral SpA, including psoriatic arthritis (PsA). The most exhaustive discussions related to whether PsA should be assessed using unidimensional composite scores for its different domains or multidimensional scores that comprise multiple domains. This question was not resolved and constitutes an important research agenda. There was broad agreement, now better supported by data than in 2012, that remission/inactive disease and, alternatively, low/minimal disease activity are the principal targets for the treatment of PsA. As instruments to assess the patients on the path to the target, the Ankylosing Spondylitis Disease Activity Score (ASDAS) for axial SpA and the Disease Activity index for PSoriatic Arthritis (DAPSA) and Minimal Disease Activity (MDA) for PsA were recommended, although not supported by all. Shared decision-making between the clinician and the patient was seen as pivotal to the process. The task force defined the treatment target for SpA as remission or low disease activity and developed a large research agenda to further advance the field.

Abstract

Background: Individuals with anti-citrullinated protein antibodies (ACPAs) and subclinical inflammatory changes in joints are at high risk of developing rheumatoid arthritis. Treatment strategies to intercept this pre-stage clinical disease remain to be developed. We aimed to assess whether 6-month treatment with abatacept improves inflammation in preclinical rheumatoid arthritis. Methods: The abatacept reversing subclinical inflammation as measured by MRI in ACPA positive arthralgia (ARIAA) study is a randomised, international, multicentre, double-blind, placebo-controlled trial done in 14 hospitals and community centres across Europe (11 in Germany, two in Spain, and one in the Czech Republic). Adults (aged ≥18 years) with ACPA positivity, joint pain (but no swelling), and signs of osteitis, synovitis, or tenosynovitis in hand MRI were randomly assigned (1:1) to weekly subcutaneous abatacept 125 mg or placebo for 6 months followed by a double-blind, drug-free, observation phase for 12 months. The primary outcome was the proportion of participants with any reduction in inflammatory MRI lesions at 6 months. The primary efficacy analysis was done in the modified intention-to-treat population, which included participants who were randomly assigned and received study medication. Safety analyses were conducted in participants who received the study medication and had at least one post-baseline observation. The study was registered with the EUDRA-CT (2014-000555-93). Findings: Between Nov 6, 2014, and June 15, 2021, 139 participants were screened. Of 100 participants, 50 were randomly assigned to abatacept 125 mg and 50 to placebo. Two participants (one from each group) were excluded due to administration failure or refusing treatment; thus, 98 were included in the modified intention-to-treat population. 70 (71%) of 98 participants were female and 28 (29%) of 98 were male. Abstract

Background: Radiographic axial spondyloarthritis (r-axSpA), formerly known as ankylosing spondylitis (AS), is a chronic, inflammatory rheumatic disease associated with symptoms such as inflammatory back pain, morning stiffness, and arthritis. First-line recommendations for patients with AS include treatment with nonsteroidal anti-inflammatory drugs (NSAIDs) for reducing pain and stiffness. Objectives: The objective of our study is to evaluate the efficacy and short-term NSAID-sparing effect of secukinumab in patients with AS currently treated with NSAIDs. Design: We assessed the clinical Assessment of SpondyloArthritis International Society (ASAS20) response to secukinumab and evaluated the extent to which the use of concomitant NSAID can be reduced between weeks 4 and 12 in r-axSpA patients treated with secukinumab 150 mg compared with placebo. Methods: ASTRUM was a prospective 24-week randomized controlled trial of adult patients with active r-axSpA [Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) ⩾4] who had a documented inadequate response to ⩾2 NSAIDs. Patients were randomized (1:1:1) to initiate treatment with subcutaneous secukinumab 150 mg from either week 0 (group 1), week 4 (group 2), or week 16 (group 3). From week 4 onward, tapering of NSAIDs was allowed in all groups. Results: This study included 211 patients (n = 71, 70, and 70 in groups 1, 2, and 3, respectively). ASAS20 response at week 12 for pooled groups 1 and 2 versus group 3 was 51.1% versus 44.3% (p = 0.35). A higher proportion of patients in groups 1 and 2 achieved ASAS40 and BASDAI50 and showed improvements in other secondary clinical outcomes as compared to group 3 at week 16. More patients in groups 1 and 2 versus group 3 stopped their NSAID intake from baseline through week 16. Conclusion: Treatment with secukinumab improved clinical outcomes and showed a short-term NSAID-sparing effect in patients with r-axSpA, even though the primary endpoint was not met.
