Extreme field gradients intrinsic to relativistic laser-interactions with thin solid targets enable compact MeV proton accelerators with unique bunch characteristics. Yet, direct control of the proton beam profile is usually not possible. Here we present a readily applicable all-optical approach to imprint detailed spatial information from the driving laser pulse onto the proton bunch. In a series of experiments, counter-intuitively, the spatial profile of the energetic proton bunch was found to exhibit identical structures as the fraction of the laser pulse passing around a target of limited size. Such information transfer between the laser pulse and the naturally delayed proton bunch is attributed to the formation of quasi-static electric fields in the beam path by ionization of residual gas. Essentially acting as a programmable memory, these fields provide access to a higher level of proton beam manipulation.

- Abstract
We report on recent experimental results deploying a continuous cryogenic hydrogen jet as a debris-free, renewable laser-driven source of pure proton beams generated at the 150 TW ultrashort pulse laser Draco. Efficient proton acceleration reaching cut-off energies of up to 20 MeV with particle numbers exceeding 109 particles per MeV per steradian is demonstrated, showing for the first time that the acceleration performance is comparable to solid foil targets with thicknesses in the micrometer range. Two different target geometries are presented and their proton beam deliverance characterized: cylindrical (∅ 5 μm) and planar (20 μm × 2 μm). In both cases typical Target Normal Sheath Acceleration emission patterns with exponential proton energy spectra are detected. Significantly higher proton numbers in laser-forward direction are observed when deploying the planar jet as compared to the cylindrical jet case. This is confirmed by two-dimensional Particle-in-Cell (2D3V PIC) simulations, which demonstrate that the planar jet proves favorable as its geometry leads to more optimized acceleration conditions.

In 1866 Gregor Mendel published a seminal paper containing the foundations of modern genetics. In 1936 Ronald Fisher published a statistical analysis of Mendel's data concluding that "the data of most, if not all, of the experiments have been falsified so as to agree closely with Mendel's expectations." The accusation gave rise to a controversy which has reached the present time. There are reasonable grounds to assume that a certain unconscious bias was systematically introduced in Mendel's experimentation. Based on this assumption, a probability model that fits Mendel's data and does not offend Fisher's analysis is given. - Abstract
Objective: To investigate the association between body mass index (BMI), obesity, clinical outcomes, and mortality following coronary artery bypass grafting (CABG) in Brazil using a large sample with one year of follow-up from the Brazilian Registry of Cardiovascular Surgeries in Adults (or BYPASS) Registry database. Methods: A multicenter cohort-study enrolled 2,589 patients submitted to isolated CABG and divided them into normal weight (BMI 20.0-24.9 kg/m2 ), overweight (BMI 25.0-29.9 kg/m2 ), and obesity (BMI > 30.0 kg/m2 ) groups. Inpatient postoperative outcomes included the most frequently described complications and events. Collected post-discharge outcomes included rehospitalization and mortality rates within 30 days, six months, and one year of follow-up. Results: Sternal wound infections (SWI) rate was higher in obese compared to normal-weight patients (relative risk [RR]=5.89, 95% confidence interval [CI]=2.37–17.82; P=0.001). Rehospitalization rates in six months after discharge were higher in obesity and overweight groups than in normal weight group (χ2 =6.03, P=0.049); obese patients presented a 2.2-fold increase in the risk for rehospitalization within six months compared to normal-weight patients (RR=2.16, 95% CI=1.17–4.09; P=0.045). Postoperative complications and mortality rates did not differ among groups during time periods. Conclusion: Obesity increased the risk for SWI, leading to higher rehospitalization rates and need for surgical interventions within six months following CABG. Age, female sex, and diabetes were associated with a higher risk of mortality. The obesity paradox remains controversial since BMI may not be sufficient to assess postoperative risk in light of more complex and dynamic evaluations of body composition and physical fitness. Objective: This study aims to investigate the ability of the six-minute walk distance (6MWD) as a prognostic marker for midterm clinical outcomes three months after coronary artery bypass grafting (CABG), to identify possible predictors of fall in 6MWD in the early postoperative period, and to establish the percentage fall in early postoperative 6MWD, considering the preoperative baseline as 100%. Methods: A prospective cohort of patients undergoing elective CABG were included. The percentage fall in 6MWD was assessed by the difference between preoperative and postoperative day (POD) five. Clinical outcomes were evaluated three months after hospital discharge. - Abstract
Introduction: Handball’s evolution has promoted its complexification, so coaches try to mitigate the effects that the diversity of variables can cause in obtaining the success of their teams. Thus, the creation of a strategic plan for each game goes beyond the theoretical and ideological fields of the coach. Objectives: To assess the perception of coaches regarding influencing and inhibitors factors for the development of actions in numerical superiority (7x6). Methods: A semi-structured interview script was designed with predefined categories. Results: The use of actions in numerical superiority 7x6 is due to the difficulties felt in creating finishing situations in numeric equality (6x6), the need to lower the game’s pace and reduce contact between defenders and athletes of the first offensive line and the need to counter deep defensive systems with individual marking. The interviewees restrict or inhibit the use of 7x6 actions due to the insufficiency of their training volume and frequency and when the result is balanced or against fast teams exploring the various forms of offensive transition. Conclusion: Given the competition game’s uncertainty there is no specific formula to try to solve the details that the game unravels. Respondents choose decision-making as a key element for success in each game. The way the coach analyzes the behavior of his team and that of the opposing team exerts a huge influence on the decisions made by the athletes in the game., Introducción: La evolución de Balonmano ha promovido su complejización, por lo que los entrenadores tratan de mitigar los efectos que la diversidad de variables puede causar en la obtención del éxito de sus equipos. Así, la creación de un plan estratégico para cada partido va más allá de los campos teóricos e ideológicos del entrenador. Objetivos: Evaluar la percepción de los entrenadores a respecto sobre los factores de influencia e inhibidores de la opción para el desarrollo de acciones en superioridad numérica 7x6. Métodos: Se diseñó un guión de entrevista semiestructurado, cuyas categorías fueron previamente definidas. Resultados: El uso de acciones en superioridad numérica 7x6 se debe a las dificultades sentidas en la creación de situaciones de finalización en igualdad 6x6, la necesidad de bajar el ritmo de juego y reducir el contacto entre defensores y atletas de la primera línea ofensiva, así como la necesidad de contrarrestar sistemas defensivos profundos con marcaje individual. Los entrevistados restringen o inhiben el uso de acciones 7x6 debido a la insuficiencia de su volumen y frecuencia de entrenamiento, su uso en momentos en que el resultado es equilibrado o contra equipos rápidos que exploran las diversas formas de transición ofensiva. Conclusiones: Dada la incertidumbre relacionada con el contexto del juego y la competición, no existe una fórmula específica para tratar de resolver los detalles que el juego desentraña. Los encuestados eligen la toma de decisiones como un elemento clave para el éxito en cada juego. La forma en que el entrenador analiza el comportamiento de su equipo y el del equipo contrario ejerce una gran influencia en las decisiones tomadas por los atletas en el juego.

Introduction: Pulmonary hypertension is a serious disease defined by increased pressure in the pulmonary vessels. It is a clinical syndrome due to increased pulmonary vascular resistance, which occurs due to vasoconstriction and/or degeneration of the vascular bed of the lungs, causing progressive loss of gas exchange capacity. The need for the development and validation of a care protocol for patients with pulmonary hypertension admitted to intensive care units is pointed out through evaluation by specialists of items that are believed to be relevant. Objective: To develop and validate a care protocol for patients with pulmonary hypertension in intensive care units. Method: This is a methodological study to validate the content of a care protocol for patients with pulmonary hypertension, through the application of the Delphi technique, based on Spinola's theoretical framework. Results:Twenty professionals were invited to participate in the evaluation of the instrument, and 13 agreed to participate; however, 9 evaluators participated in the first Delphi Round. The final construct remained with 4 topics and 23 items, and no item was removed after evaluation by the experts. Conclusion: The protocol for care ofpatients with pulmonary hypertension is a practical instrument and aims to ensure patient safety and the quality of care provided in conjunction with technological advances. It is believed that the protocol will help multidisciplinary teams to promote safe, accurate and efficient care for patients with pulmonary hypertension in intensive care units. Introducción: La hipertensión pulmonar (HP) es una enfermedad grave definida por el aumento de la presión en los vasos pulmonares. Es un síndrome clínico, por aumento de la resistencia vascular pulmonar, que se produce por vasoconstricción y/o degeneración del lecho vascular de los pulmones, provocando una pérdida progresiva de la capacidad de intercambio gaseoso. Se destaca la necesidad de elaboración y validación de un protocolo de atención a pacientes con hipertensión pulmonar internados en Unidades de Cuidados Intensivos (UCI), a través de la evaluación, por parte de especialistas, de ítems que se crean relevantes. Objetivo: Desarrollar y validar un protocolo de atención a pacientes con hipertensión pulmonar en la unidad de cuidados intensivos. Método: Se trata de un estudio metodológico para validar el contenido de un protocolo de atención a pacientes con hipertensión pulmonar, mediante la aplicación de la técnica Delphi basada en el marco teórico de Spinola. Resultados: 20 profesionales fueron invitados a participar de la evaluación del instrumento, de los cuales 13 aceptaron participar, sin embargo, 09 evaluadores participaron en la primera Ronda Delphi. El constructo final quedó con 04 temas y 23 ítems y no se eliminó ningún ítem después de la evaluación de los expertos. Conclusión: El protocolo de atención al paciente con hipertensión pulmonar es un instrumento práctico y tiene como objetivo garantizar la seguridad del paciente y la calidad de la atención brindada en conjunto con los avances tecnológicos. Se cree que el protocolo ayudará al equipo multidisciplinario a promover la atención segura, precisa y eficiente de los pacientes con hipertensión pulmonar en las unidades de cuidados intensivos. Trata-se de uma síndrome clínica, devido ao aumento da resistência vascular pulmonar, que ocorre por vasoconstrição e/ou degeneração do leito vascular dos pulmões causando perda progressiva da capacidade de trocas gasosas. Aponta-se a necessidade da elaboração e validação de um protocolo de cuidados ao paciente com hipertensão pulmonar internado em Unidades de Terapia Intensivas (UTIs) por meio da avaliação, por especialistas, de itens os quais se acredita serem pertinentes. Objetivo: Elaborar e validar um protocolo assistencial de cuidados ao paciente com hipertensão pulmonar na unidade de terapia intensiva. Método: Trata-se de estudo metodológico, de validação do conteúdo de um protocolo assistencial de cuidados ao paciente com hipertensão pulmonar, por meio da aplicação da técnica Delphi baseado no referencial teórico de Spínola. Resultados: Foram convidados 20 profissionais para participarem da avaliação do instrumento, dos quais 13 aceitaram participar, no entanto, 09 avaliadores participaram da primeira Rodada de Delphi. INTRODUCTION: Ocular traumas with intraocular foreign bodies (IOFB) can have serious ocular complications, such as retinal detachment and endophthalmitis that can greatly affect the visual outcome. Pars plana vitrectomy (PPV) is the most commonly used technique to remove IOFB from the posterior segment. The purpose of this study was to evaluate the management and outcomes of posterior segment IOFB in a tertiary ophthalmology center. METHODS: Patients that suffered a penetrating eye injury with IOFB retained in the posterior segment who underwent PPV for IOFB extraction between 2010 and 2020 were included and data was collected from patients’ archives. RESULTS: Thirty eight patients with mean age of 48.68 years old were included, 86.8% were males. A percentage of 59.5% came to the ophthalmology emergency at the same day of the accident, but 16.2% took 3 days or more to seek medical help. The most common complications on initial examination included traumatic cataract (52.6%), retinal lesions (34.2%) and hyphema (23.7%). Also, before IOFB extraction, 42.1 % of patients developed endophthalmitis. Systemic antibiotics was administered to 84.2% of patients and 71.1% received intravitreal antibiotics. Comparing the 15.8% of eyes that ended up developing phthisis bulbi with those who did not, the only statistically significant difference (p, Revista Sociedade Portuguesa de Oftalmologia, Vol. 46 N.º 2 (2022)

O processo de implantacao do liberalismo em Portugal representa, de certa forma, uma tentativa de consolidacao de realidades multiplas, regionais e locais, em algo de mais homogeneo. De uma realidade poliedrica, marcada por autonomias locais em maior ou menor grau e por particularismos regionais, o Estado liberal vai tentando caminhar no sentido de uma organizacao politica e administrativa que pretende racional e monolitica, derivada de um centro politico que define linhas, direitos e deveres comuns. Municipios e regioes integrar-se-iam num todo nacional abstrato, como pecas de um puzzle , e os individuos, antes organizados de multiplas formas (em ordens, corporacoes, redes familiares), transformar-se-iam em cidadaos, em teoria iguais perante a lei. Mas esta ideia de homogeneizacao e, em larga medida, aparente: as resistencias locais ao centro, patentes no exemplo portugues em varios momentos da primeira metade do seculo XIX, e as varias propostas de modelos politicos assentes nos exemplos virtuosos das autonomias locais do mundo antigo e da medievalidade, que nunca saem verdadeiramente do campo do debate politico, mostram justamente que a tentativa de uniformizacao racional levada a cabo pelo modelo liberal nao e encarada de forma tao linear quanto a primeira vista poderia aparentar. Como se relacionam as novas realidades espaciais e administrativas com a vivencia da res publica pelos cidadaos? Qual a relevância da dimensao local e regional neste processo? E como se cruzam estas reflexoes com a questao da cidadania em si e de como foi pensada no contexto concreto da primeira metade do seculo XIX portugues? The local and the regional in the construction of citizenship: some reflections on Portuguese liberalism The process of implanting liberalism in Portugal represents, in a way, an attempt to consolidate multiple realities, regional and local, in something more homogeneous. From a polyhedral reality, marked, to a greater or lesser degree, by local autonomies and by regional particularisms, the liberal State tries to move towards a political and administrative organization that intends to be rational and monolithic, derived from a political center that defines lines, rights and common duties. Municipalities and regions would integrate into an abstract national whole, like pieces of a puzzle, and individuals, previously organized in multiple ways (in orders, corporations, family networks), would become citizens, in theory equal before the law. But this idea of homogenization is, to a large extent, apparent: the local resistance to the center, evident in the Portuguese example at various times in the first half of the 19th century, and the various proposals for alternative political models, based on the virtuous examples of the local autonomies of the ancient world and medievality, which never really leave the field of political debate, show precisely that the attempt at rational uniformity carried out by the liberals is not as linear as it might at first appear. How are the new spatial and administrative realities related to the experience of res publica by citizens? What is the relevance of the local and regional dimension in this process? And how do these reflections intersect with the question of citizenship itself and how it was conceived in the concrete context of the first half of the Portuguese 19th century? Despite new modern RT techniques, late complications may occur such as radiation proctitis (RP). The natural history of RP is unpredictable. Minor symptoms may resolve spontaneously or require conservative treatment. On the other hand, for similar and uncomplicated clinical contexts, symptoms may persist and can even be refractory to the progressive increase in treatment measures. Over the last decades, an enormous therapeutic armamentarium has been considered in RP, including hyperbaric oxygen therapy (HBOT). Currently, the evidence regarding the impact of HBOT on RP and its benefits is conflicting. Additional prospective and randomised studies are necessary to validate HBOT's effectiveness in the 'real world' clinical practice. This article reviewed the relevant literature on pathophysiology, clinical presentation, different classifications and discuss RP management including a proposal for a therapeutic algorithm with a focus on HBOT. publishersversion published

Coronary artery bypass surgery (CABG) without cardiopulmonary bypass (CPB) may potentially reduce the number of microembolic signals (MES) associated with aortic manipulation or generated by the pump circuit, resulting in a better neurologic outcome after surgery. Our aim was to compare the frequency of MES and neurologic complications in CABG with and without CPB. Twenty patients eligible to routine CABG without CPB were randomized to surgery with CPB and without CPB and continuously monitored by transcranial Doppler. Neurologic examination was performed in all patients before and after surgery. The two groups were similar with respect to demographics, risk factors, grade of aortic atheromatous disease and number of grafts. The frequency of MES in the nonCPB group was considerably lower than in CPB patients, however, we did not observe any change in the neurologic examination during the early postoperative period. Neurologic complications after CABG may be related to the size and composition of MES rather than to their absolute numbers. A large prospective multicentric randomized trial may help to elucidate this complex issue.

INTRODUÇÃO: A sobrevida de pacientes com miocardiopatia e insuficiência mitral secundária em classe funcional IV é muito pequena em curtos períodos de observação, apesar dos progressos consideráveis obtidos com o tratamento médico. Tem sido demonstrado que o aparecimento de insuficiência mitral secundária piora o prognóstico e a qualidade de vida e que a correção da insuficiência mitral permite melhor controle do paciente. OBJETIVO: O presente trabalho propõe o implante de uma prótese em posição mitral que elimina a insuficiência mitral e remodela a base do ventrículo esquerdo e o seu eixo longitudinal. CASUÍSTICA E MÉTODOS: Analisamos 33 pacientes com insuficiência cardíaca refratária sob terapêutica máxima operados entre dezembro/95 e setembro/99. O grupo etário variou de 25 a 78 anos (mediana 58) sendo 57,7% do sexo masculino. O período de observação variou de 4 meses a 4 anos (mediana 22 meses). Quanto à etiologia, em 15 pacientes era isquêmica, em 13 dilatada, em 3 chagásica, em 1 pós-parto e em 1 viral. RESULTADOS: Ocorreram 3 óbitos na fase hospitalar (3/33) e 2 na tardia (2/30), estando 28 pacientes em observação. Neste intervalo de seguimento, 88% dos pacientes melhoraram 1 ou 2 classes funcionais, a fração de ejeção subiu de 20 a 36%, apesar da eliminação da fração regurgitante, o volume efetivo melhorou consideravelmente (58 para 80 ml) entre a aferição pré-operatória e a última evolução. CONCLUSÃO: A insuficiência cardíaca refratária com insuficiência secundária moderada a severa pode sofrer efetiva paliação com a eliminação de regurgitação e remodelação do ventrículo esquerdo, implantando-se prótese valvar no anel atrioventricular esquerdo.

Embedded on-board-optimization-based guidance is a set of guidance schemes that provide spacecraft the capability to plan and perform trajectories under several constraints autonomously. This autonomy will allow the space vehicle to more efficiently perform more challenging goals while dealing with rapidly changing environments, unmodelled/unforeseen dynamics and failure scenarios. However, the unique challenges of on-board applications raise the need for a thorough analysis of the computational efficiency, reliability, accuracy, and robustness of the solution process. This is directly related to how the guidance function is formulated, modelled, and analyzed, a process that should be founded on the understanding of the underlying physics and tailoring of the design to the specific problem. Verification and validation of such guidance schemes, wherever theoretically possible or empirical/numerically otherwise, is an important metric when evaluating the feasibility of the proposed solutions. This paper proposes a Model Predictive Control (MPC) approach to the constrained attitude guidance problem. The problem is approximated in two ways: 1) A convex quadratically constrained quadratic program (QCQP) form, to be solved numerically by means of a QCQP solver. 2) A convex quadratic program (QP) form with linear constraints, to be handled by means of a QP solver.

GUIBEAR is an on-going activity supported by the European Space Agency aimed at the demonstration of the feasibility of bearings-only (B-O) far-range rendezvous in near rectilinear Halo orbits (NROs). The studied scenario is the far-range rendezvous of the HERACLES Lunar Ascent Element (LAE) with the Lunar Orbital Platform – Gateway (LOP-G) that is orbiting the Moon in an NRO. This activity advances the readiness level of the applicable technologies through a solution based on Computational GNC, structured around a Model Predictive Control framework and tools; and an approach design, development, verification and validation based on iterative prototyping which integrates high-fidelity hosting considerations into the design from a preliminary level: concurrent GNC and software architecture design with hosting, runtime assessment and optical high-fidelity simulations. This paper: presents the context – mission, system and hosting, and algorithm-level challenges and proposed concepts; details and justifies the architecture and design solutions; outlines the methodologies employed for prototyping verification and validation; provides conclusions on the obtained results.

Este projecto emergente na Universidade de Aveiro, que congrega investigadores de áreas de conhecimento como a filosofia, o design, a retórica e a gestão do design, centra-se em dois argumentos convergentes: o primeiro deriva da descoberta/prova, feita por Hanna e António Damásio relativamente ao papel decisivo da emoção para a afirmação da capacidade decisória; o segundo deduz-se do primeiro e corresponde a necessidade sentida de investigar quais as implicações desta descoberta no âmbito da teoria e prática do design (nomeadamente no design de interacção). Neste contexto a retórica representa uma arte arquitectónica na construção do discurso da interactividade, e, simultaneamente, uma ferramenta analítica fortemente estruturada.

Existe um hiato entre o tempo das artes e ofícios / indústria tradicionais portugueses, que demonstraram historicamente ser capazes de gerar objectos de concepção própria de alta qualidade, e o momento actual que revela um espaço de desencontro para onde a indústria foi remetida, funcionando predominantemente num regime de subcontratação não criativa, que responde, na sua quase totalidade, às encomendas dos grandes operadores globais.Portugal, foi, nas últimas décadas, receptor da transferência de tecnologias avançadas ao nível produtivo, necessitando de repensar o papel do design num processo de industrialização1 que combine a identidade / diferença da concepção local (relativamente periférica), com as premissas lançadas pela globalização de que ainda não se adivinha uma conclusão pacífica.Neste âmbito apresentaremos a definição de um quadro estratégico de investigação em design cujo objectivo se prende com o aumento da competitividade da indústria portuguesa, pelo incremento da sua auto-estima, pelo reforço da sua identidade e pela criação de novas oportunidades com viabilidade económica.Este processo definirá modelos de estudo de alguns objectos artesanais / industriais portugueses que, em contextos específicos de tempo e espaço, constituiram um património de excelência.Destes estudos resultam dois campos de investigação: um claramente museológico (no sentido da interpretação do passado com um sentido de futuro), e outro que através da definição de “briefs” interactivos, constituirá a base para a intervenção de pares designers/empresas para a concepção de novos produtos, cuja expressão se traduzirá num programa de exposições temáticas de âmbito internacional com um sentido de difusão /promoção para uma futura comercialização.O quadro estratégico de investigação proposto, pode configurar hipóteses de explicação, mas também de contribuições para a construção da cultura material de um povo. Investigação pura, no sentido em que conduz à produção de novo conhecimento (cuja aplicabilidade directa não se inscreve no seu programa), mas que promove desde logo a sua tradução em investigação aplicada.Esta proposta desagua na elaboração e organização de conteúdos, dando suporte teórico à constituição de um Museu do Design Português, conclusão final deste desiderato. A recolha, catalogação e estudo de artefactos que traduzam o património português.È fundamental para alicerçar um design que revele a nossa identidade nos quadros internacionais que são conhecidos.O museu assim estruturado encerra um muito particular ideal interactivo e metalinguístico, de forma a que sirva não só para ler o passado, mas sobretudo para pensar o presente e fazer acreditar no futuro.

OBJECTIVE: To evaluate the early outcome of mitral valve prostheses implantation and left ventricular remodeling in 23 patients with end-stage cardiomyopathy and secondary mitral regurgitation (NYHA class III and IV). METHODS: Mitral valvular prosthesis implantation with preservation of papillary muscles and chordae tendinae, and plasty of anteriun cuspid for remodeling of the left ventricle. RESULTS: The surgery was performed in 23 patients, preoperative ejection fraction (echocardiography) varied from 13% to 44% (median: 30%). In 13 patients associated procedures were performed: myocardial revascularization (9), left ventricle plicature repair (3) and aortic prosthese implantation (1). Early deaths (2) occurred on the 4th PO day (cardiogenic shock) and on the 20th PO day (upper gastrointestinal bleeding), and a late death in the second month PO (ventricular arrhythmia).
