Die Dissertation ist eine explorative Analyse von soziodemographischen und klinischen Faktoren für die Veränderung der Lebensqualität und der psychischen Belastung bei Patientinnen mit Mammakarzinom an der Frauenklinik des MRI zwischen 2018 und 2021. Zu drei Erhebungszeitpunkten innerhalb des erstens Jahres nach Diagnose wurde mithilfe dreier etablierter Fragebögen ein umfassendes Bild über den Zustand der Patientinnen gezeichnet. Ziel der Arbeit war es, Prädiktoren zu identifizieren, die die Lebensqualität und die psychische Belastung der Patientinnen beeinflussen., Changes in quality of life and psychological distress in patients with breast carcinoma were evaluated with surveys and an explorative analysis of sociodemographic and clinical factors at the Department of Gynecology at the Technical University Munich. At three survey points within the first year after diagnosis, three established questionnaires were used to assess the patients' condition. The aim of the work was to identify predictors that influence the patients' quality of life and psychological burden in order to improve the support offered to these patients in the future.

They primarily affect adolescents and young women in their fertile years. Therefore, fertility and pregnancy outcome are of pivotal relevance in this patient collective. Apart from the guidelines of the European Society of Human Reproduction and Embryology (ESHRE) on the management of endometriosis in general, there are no specific treatment recommendations for CA and, as far as our research shows, no data illustrating the behavior of a CA over the course of pregnancy. Thus, we report the case of a 32-year-old 1-gravida, 1-para, preconceptionally diagnosed with a CA by ultrasound. After thoroughly discussing further treatment options, the decision was made to opt for a more conservative approach and not perform surgery before attempting a next pregnancy. The patient conceived spontaneously and sonographic monitoring of the CA throughout pregnancy showed complete regression of the cystic component during the second trimester. A healthy baby was delivered at term by an uncomplicated elective cesarean section. Following a review of the literature and taking into account the course of our case, we propose the feasibility of a conservative, non-surgical approach in women with a CA and the desire to conceive.Keywords: cystic adenomyoma, endometriosis, conservative management, pregnancy, fertility

Die Arbeit untersuchte das Ausmaß der psychosozialen Belastung sowie den Wunsch nach bzw. die Inanspruchnahme von psychosozialer Unterstützung bei Patientinnen mit gynäkologischen Krebserkrankungen. Außerdem wurden mögliche Einflussfaktoren hierauf analysiert. Dafür wurde das Distress-Screening der gynäkologischen Ambulanz der Klinik und Poliklinik für Frauenheilkunde der Technischen Universität München evaluiert. Es zeigte sich eine mit zunehmendem Alter und zunehmender Erkrankungsdauer abnehmende Belastung. Patientinnen mit Progress oder Unterstützungswunsch waren belasteter., The present study investigated the extent of psychological distress, the desire for and use of psychosocial support in patients with gynecological malignancies. In addition, possible factors influencing the above were analyzed. For this purpose, the psychological distress screening performed at the outpatient clinic of the Department of Gynecology, Technical University of Munich, Germany, was evaluated. A decrease in distress was found with increasing age and duration of disease. Patients with progression of disease or desire for support were significantly more distressed.

In dieser Studie wurden 340 Patientinnen mit senologischem oder gynäkologischem Karzinom zu ihrem Interesse und Bedarf an Integrativer Medizin (IM) befragt. Die Daten belegen insgesamt eine hohe Nutzung und Nachfrage der IM. Zudem wurden 144 Patientinnen in ein spezifisches IM-Programm eingeschlossen. Dieses Programm während der systemischen Therapie wurde von Patientinnen mit Brust- oder gynäkologischem Krebs gut angenommen und konnte den subjektiven Gesundheitszustand verbessern., In this study, 340 patients with with breast or gynecological cancer were surveyed about their interest and need for integrative medicine (IM). Overall, the data demonstrate high use and demand for IM. In addition, 144 patients were enrolled in a specific IM program. This program during systemic therapy was well accepted by patients with breast or gynecological cancer and was able to improve patient´s subjective health condition.

Der Einsatz einer perioperativen Antibiotikaprophylaxe (PAP) stellt ein wirksames Mittel dar, um die Entstehung postoperativer Wundinfektionen (PWIs) zu vermindern. Es herrscht jedoch Unklarheit über die optimale Dauer der verabreichten Medikation. Ziel der Studie war es, Brustkrebspatientinnen, welche im Rahmen eines Implantat-basierten Brustwiederaufbaus eine PAP von mehr als 24 Stunden bzw. weniger als 24 Stunden erhielten, hinsichtlich Entstehung von PWIs zu vergleichen und mögliche Risikofaktoren zu identifizieren., The use of a perioperative antibiotic prophylaxis (PAP) is an effective method to prevent surgical site infections (SSIs). The optimal duration of the antibiotics is, however, still controversial. This study compares different PAP regimes (less than 24h vs. more than 24h) in breast cancer patients undergoing implant-based breast reconstruction, analyses the development of SSIs, and identifies possible risk factors.

Diese publikationsbasierte Arbeit hatte zum Ziel, den Bedarf an integrativen Therapiekonzepten wie Ernährungsberatung, Entspannungstechniken, psychologische Beratung sowie die Nutzung von Komplementär- und Alternativmedizin (CAM) bei onkologischen und geburtshilflichen Patientinnen der Frauenheilkunde zu eruieren. Die Studie wurde mittels eines pseudoanonymen Fragebogens in der Frauenklinik rechts der Isar durchgeführt. Die Ergebnisse unserer Studie belegen sowohl einen weit verbreiteten Gebrauch von komplementärmedizinischen Behandlungsmethoden als auch eine starke Nachfrage nach integrativen therapeutischen Ansätzen von Seiten der Patientinnen., This publication-based thesis aims to assess the demand for integrative therapeutic approaches including Complementary and Alternative Medicine (CAM), body and mind practices, nutritional counselling, and psychological assistance among gynaecological cancer patients and women in pregnancy and childbed. The survey was conducted by means of a pseudo-anonymous questionnaire at the Department of Gynecology and Obstetrics of the Klinikum rechts der Isar. The results of this study demonstrate that there is a considerable demand for integrative medicine and widespread use of CAM among the surveyed patients.
