3. Report on Prosthetic Fitting, Mobility, and Overall Satisfaction after Major Limb Amputation at a German Maximum Care Provider.

Abstract

Background: Satisfaction with prosthesis plays a key role in regaining mobility and is important for optimizing prosthetic usage, mobility, and increasing compliance with medical regimen. Despite unchangeable factors like age and comorbidities, other factors, like pain, received rehabilitation, satisfaction with assistive devices, service, and information, can be changed and might contribute to a better usage and acceptance of the prosthesis and amputees' mobility. Objectives: The aim of the study was to analyze mobility, pain, supply of assistive devices, and additional therapies received after major limb amputations. Furthermore, a correlation of those parameters was evaluated. Methods: Retrospective identification of patients with major limb amputation (operation and procedures classification system (OPS)) and relevant related demographics within the clinical documentation system during a four-year observation time. In addition, we undertook prospective assessment of mobility (K-level), pain qualities, additional therapies, self-rated overall quality of life (QoL) and degree of adaptation to the life after amputation, dependency from caregivers, and satisfaction with the provided assistive devices (QUEST). Results: A total of 164 patients (mean age 68, age range: 19 to 97 years) underwent major limb amputation. A total of 27 questionnaires were returned and analyzed. All those traumatic and nontraumatic amputees received assistive devices. Although mobility and QoL decreased significantly after amputation, a high satisfaction with provided prosthetic and assistive devices and care was found. Conclusions: Amputation registries are becoming elementary to allow for nationwide comparisons of clinics, to identify the requirements of amputees, and to design an interdisciplinary care model for a successful comprehensive approach. Abstract

Introduction: The C-Brace microprocessor stance and swing control orthosis was designed to overcome safety and functional limitations of traditional knee-ankle-foot orthoses (KAFOs) for individuals with lower limb paresis. However, a systematic comparison to established KAFO types has not been performed in a bigger sample. Methods: International multicenter, randomized, controlled, cross-over clinical trial. Legacy KAFO users at risk of falling were randomized to KAFO/C-Brace or C-Brace/KAFO use for three months with each orthosis. Primary outcome was balance assessed with the Berg Balance Scale (BBS). Secondary outcomes were falls, mobility, function, and quality of life. Results: Intention-to-treat analysis with 102 participants. With the C-Brace, the BBS improved by 3.3 ± 6.3 points (p < 0.0001). Significantly fewer participants presented BBS scores <40 indicative of increased fall risk (16 vs. 36, p = 0.018). Mean falls reduced from 4.0 ± 16.8 to 1.1 ± 3.3 (p = 0.002). Outcomes for function, mobility, and quality of life showed significant improvements with the C-Brace. Discussion: The improvements in fall risk and mobility can be attributed to the stumble recovery and controlled knee flexion during weight bearing of the C-Brace and have a positive impact on the quality of life of users. Conclusion: The C-Brace represents an option for KAFO users with increased fall risk and reduced mobility. IMPLICATIONS FOR REHABILITATION: When prescribing traditional knee-ankle-foot orthoses (KAFOs), their known limitations, such as limited function and mobility, and the requirement to walk with compensatory mechanisms, especially on non-level terrains, should be considered. For patients with compromised balance and increased risk of falling when using a traditional KAFO, a microprocessor stance and swing control orthosis (MP-SSCO) may be considered as an orthotic option to reduce their fall risk. For patients with mobility restrictions using a traditional KAFO, a MP-SSCO may be considered to improve function, mobility, reintegration into normal living, and quality of life. Abstract

INTRODUCTION Myoelectric hands progressed from single grip hands (traditional myoelectric devices (TH)) to be multi-grip hands (MGH) which are hypothesized to bring more degrees of freedom, greater range of motion and improved grasping capabilities1,2. Their impact on patients’ lives has been documented in only a few case studies. Abstract

INTRODUCTION By enabling users to flex the orthotic leg during swing phase and safely lock it during stance phase, stance control orthosis (SCO) offers clear benefits compared to locked knee-ankle-foot-orthosis (KAFO)1. Since such orthoses do not offer dampened knee flexion in the weight-bearing condition, this represents a limitation in everyday activities such as ramp and stair descent. C-Brace, a microprocessor controlled stance and swing orthosis (SSCO), overcomes many of those problems. Maximum knee flexion angle in stance and swing phase during level walking are closer to physiological values with C-Brace compared to conventional KAFOs2. The patients are with C-Brace able to descend stairs and ramps reciprocally2. Furthermore, patients report of safer and easier ability to perform activities of daily living3. The main aim of next generation C-Brace is a reduction in size and an increase of adaptability to the patient’s anatomic structure.
