Amaç: Bu çalışmada, sıçan tibialarında titanyum implantların osseointegrasyonuna lokal ve sistemik borik asit (BA) uygulamasının etkilerini araştırmayı amaçladık. Gereç ve Yöntem: Titanyum implantlar, 32 sıçandan oluşan bir numune grubunun sağ tibia kemiğine cerrahi olarak entegre edildi. Sıçanlar dört tedavi grubuna ayrıldı: kontroller (CNT) (n=8), lokal BA (LBA) (n=8), sistemik BA (SBA) (n=8) ve sistemik ve lokal BA (SLBA) ( n=8). Kontroller, implant cerrahisinden sonra herhangi bir uygulama görmedi. LBA grubunda implant soketlerine lokal BA uygulandı, ardından implantlar soketlere yerleştirildi. SBA grubuna periton içi 8 mg/kg BA uygulandı. SLBA grubunda implant soketlerine lokal BA uygulandı, ardından implantlar soketlere yerleştirildi; ardından günde 8 mg/kg BA periton içi uygulandı. Deney periyodu 28 gün sürmüş, bundan sonra hayvanlara ötenazi uygulanmış ve implantlar çevredeki kemik dokusu ile birlite alınmıştır. Bulgular: Dekalsifiye olmayan histolojik yöntem kullanılarak kemik-implant kaynaşması (KİK) oranları (%) incelendi. SLBA ve SBA gruplarında KİK oranları, kontroller ve LBA grubu ile karşılaştırıldığında yüksekti (p0.05 p=0.38). Sonuç: Bu çalışmanın sınırlamaları dahilinde, lokal uygulama ile sistemik ve sadece sistemik BA uygulamasının titanyum implantların osseointegrasyonunu artırabileceği sonucuna varılabilir. Objective: In this study, we aimed to investigate the effects of local and systemic boric acid (BA) application on the osseointegration of titanium implants in rat tibias. Materials and Methods: The titanium implants were integrated surgically to the right tibias of a sample group of 32 rats. Rats were randomly divided into four treatment groups: controls (CNT) (n=8), local BA (LBA) (n=8), systemic BA (SBA) (n=8), and systemic and local BA (SLBA) (n=8) groups. Controls received no further treatment following implant surgery. In the LBA group, local BA was applied locally in the implant sockets, after which the implants were inserted in the sockets. In the SBA group, 8 mg/kg BA was applied intraperitoneally. In the SLBA group, local BA was applied locally in the implant sockets, after which the implants were inserted in the sockets; then, 8 mg/kg BA was applied intraperitoneally in a day. The experimental period lasted 28 days, after which the animals were euthanized and the implants collected, along with some surrounding bones from each rat. Results: Bone–implant connection (BIC) ratios (%) were observed using the non-decalcified histologic method. In the SLBA and SBA groups, BIC ratios were high compared with the controls and the LBA group (p0.05 p=0.38). Conclusion: Within the limitations of this study, it may be concluded that systemic and systemic with local BA application could increase the osseointegration of titanium implants.

Amaç: Losartanın osteoklastogenezi azalttığı ve osteoblast aktivitesini arttırdığı bildirilmiştir. Bu çalışmada sistemik losartan uygulamasının titanyum implant osseointegrasyonu üzerindeki etkisinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Titanyum implantlar, sıçanların tibiya kemiklerine cerrahi olarak yerleştirildi, daha sonra sıçanlar rasgele iki gruba ayrıldı: kontrol grubu (n=10) ve losartan tedavi grubu (n=10). Kontrollere başka bir tedavi uygulanmamışken, losartan tedavi grubuna, sekiz haftalık deney süresi boyunca 30 mg/kg losartan haftada üç kez steril serum fizyolojikte çözülerek oral gavaj ile uygulanmıştır. Deney döneminden sonra, çevredeki kemik dokuları ile titanyum implantlar alındı ve formaldehitte sabitlendi. İmplantların ve çevresindeki kemik dokusunun dakalsifiye edilmemiş histolojik analizleri yapıldı. Her bir implantın kemik implant bağlantı yüzdesi (%), implant çevresinin toplam uzunluğuna, kemikle doğrudan temas halinde olan implantın toplam çevre uzunluğunun oranı olarak hesaplandı. Kemik dolumu oranı, titanyum implanttan meziyal, distal ve apikal yönde 0.5 mm mesafedeki alan dahil olmak üzere, bölgedeki kemik dolu alanların bölgenin toplam alanına oranı (%) ile belirlenmiştir. Bulgular: Her ne kadar losartan grubunda kemik implant kaynaşması ve kemik dolumu oranları kontrol grubuna göre daha yüksek olsa da, iki grup arasındaki oranlarda istatistiksel olarak anlamlı fark yoktu (P>0.05). Sonuç: Bu çalışmanın sınırlamaları dahilinde losartan kemik implant bağlantı düzeylerini sayısal olarak artırdı, ancak istatistiksel olarak arttırmadı. Objective: Losartan has been reported to reduce osteoclastogenesis and increase osteoblast activity. The aim of this study was to evaluate the effect of systemic losartan application on titanium implant osseointegration. Materials and Methods: Titanium implants were surgically integrated in the tibial bones of rats, after which the rats were randomly divided into two groups: a control group (n=10) and a losartan treatment group (n=10). No other treatment was applied to the controls, whereas losartan was administered to the treatment group in a dosage of 30 mg/kg by oral gavage with saline solution three times per week during an eight-week experimental period. After the experimental period, titanium implants were collected with the surrounding bone tissues and fixed. Non-decalcified histological analyses of implants and surrounding bone tissue were performed. The bone implant connection ratio (%) of each implant was calculated as the ratio of the length of the implant circumference to the circumference of the total implant in direct contact with the bone. The bonefilling ratio (%) was determined by the ratio of bone-filled areas within the region to the total area of the region, including the area 0.5 mm from the titanium implant in periimplant bone. Results: Although the bone implant connection and bone-filling ratios (%) ratios were higher in the losartan group than in the controls, there was no statistically significant difference in the ratios between the two groups (P>0.05). Conclusion: Within the limitations of this study, losartan increased the osseointegration levels numerically but not statistically.

Bu çalışmanın amacı; kardiyovasküler olarak ateroskleroz tanısı ve periodontal olarak ise kronik periodontitis tanısı konulmuş bireyler ile kardiyovasküler olarak sağlıklı ve periodontal olarak ise kronik periodontitis tanısı konulmuş bireylerin periodontal tedavileri sonrası periodontal sulkusta periodontopatojen mikroorganizmaların oranının değişimi, DOS'da antimikrobiyal peptidlerin miktarının değişimi, klinik periodontal indekslerin değişimi ve aterosklerozun sistemik betirteçlerinin değişimini kullanarak karşılaştırılmasıdır. Yaşları 35-65 arasında değişen; kardiyovasküler olarak ateroskleroz tanısı konulmuş ve periodontal olarak kronik periodontitis tanısı konmuş 20 hasta test grubu, periodontal olarak kronik periodontitis tanısı konulmuş sistemik olarak sağlıklı 20 hasta kontrol grubu olacak şekilde toplam 40 hasta dâhil edilmiştir. Bütün hastalara klinik periodontal tedavi uygulanmış, başlangıç ve 6. ay kontrollerinde; klinik periodontal parametreler (SCD, PI, GI, KAS ve SK), biyokimyasal belirteçler (ADM, CCL-28, HBD-2, HBD-3 ve calprotectin) kendilerine özgü ELISA kitleri ile, kan değerleri (WBC, LDL, HDL, fibrinojen, kreatinin ve platelet), belirlenen periodontopatojen mikroorganizmaların (A. actinomycetemcomitans, P. gingivalis, T. forsythia, T. denticola, P. intermedia, P. micros, F. nucletum/periodonticum, C. rectus, E. nodatum, E. Corrodens ve C. gingivalis/ochracea/sputigena) oranları (Micro-IDentPlus® testi kullanılarak), ölçülerek kaydedilmiştir. Her iki grupta da klinik periodontal tedavi sonrası klinik parametrelerde istatistiksel anlamlı kazanç sağlanmıştır. Biyokimyasal belirteçlerde her iki grupta anlamlı azalmalar sağlanmıştır. Kan değerleri tedavi sonrası, test grubunda kontrol grubuna göre anlamlı derecede değişmiştir. Periodontopatojen mikroorganizma oranları istatistiksel anlamlı şekilde düşüş göstermiştir. Bütün belirteçlerin değerlendirilmesi ve testler neticesinde; klinik periodontal tedavinin başarılı olduğu ve periodontal durumu iyileştirmekle beraber sistemik inflamasyonu azalttığı belirlenmiştir. Lokal enfeksiyonun tedavisi ve sistemik inflamasyon belirteçlerinin azaltılması ile ateroskleroz tanı kriterlerinde de azalma sağlanmış olunabileceği düşünülmektedir. Bu çalışmanın sınırları dâhilinde varılan sonuç itibari ile ateroskleroz için bir risk faktörü olan kronik periodontitisin klinik periodontal tedavisinin ateroskleroz prognozunu pozitif etkilemiş olduğu sonucuna ulaşılabilmektedir.Anahtar sözcükler: Ateroskleroz, kronik periodontitis, biyokimyasal belirteç, periodontopatojen. The aim of this study is to compare the change in rate of sulcular periodontopathogen microorganisms, change in antimicrobial peptides in GCF, change in clinical periodontal indexes and change in systemic markers of atherosclerosis in patients diagnosed of cardiovasculary atherosclerosis and with chronic periodontitis; with patients who are cardiovascularly healthy and has chronic periodontitis. Total number of subjects is 40 aged between 35 and 65; test group is 20 patients diagnosed with cardiovascular atherosclerosis and chronic periodontitis, test group is 20 patients with no systemic diseases but had chronic periodontitis. All patients had received clinical periodontal therapy; clinical periodontal parameters (SCD, PI, GI, KAS, and SK), biochemical markers (ADM, CCL-28, HBD-2, HBD-3, and calprotectin) (with individual ELISA kits), blood count (WBC, LDL, HDL, fibrinojen, kretinin, and platelet), rates of specified periodontopathogens (A. actinomycetemcomitans, P. gingivalis, T. forsythia, T. denticola, P. intermedia, P. micros, F. nucletum/periodonticum, C. rectus, E. nodatum, E. corrodens, C. gingivalis/ochracea/sputigena) (with Micro-IDentPlus® test) had measured and recorded in startup and sixth month control sessions. Both control and test groups had gained statistically significant improvoments in clinical parameters after clinical periodontal therapy. Biochemical markers showed significant reduction in both groups. Blood count values showed significant difference in test group in comparison with control group after therapy. Falling of periodontopathogen rates was statistically significant. The evaluation of all tests and markers showed that clinical periodontal theraphy was succesful and cured periodontal situation as well as reducing systemic enflamation. Treatment of local infection and reduction of sistemic inflamation markers, had generated improvement in diagnostic criteria for atherosclerosis. Within the limitations of this study we concluded that the clinical therapy of chronic periodontitis which is a risk factor for atherosclerosis would benefit the prognosis of atherosclerosis. The effects of change in blood count, biochemical markers and rates of periodontopathogens on systemic enflamation and cure of chronic periodontitis which is a risc factor for atherosclerosis would contribute the treatment of atherosclerosis.Key words: Atherosclerosis, chronic periodontitis, biochemical markers, periodontopatogen. 169
