Коментуються американські та українські рекомендації з харчування і модифікації стилю життя, результати багатоцентрових досліджень при гіпертонічній хворобі й ожирінні, європейські дієтичні раціони з доведеною ефективністю (DASH-дієта, Mediterranean Diet, MIND, TLC, дієти Орніша, Polymeal, Omni Heart, клініки Майо, Paleolithic diet, Weight Watchers, дієти LCHF, South Beach, методика MEDFICTS зі скринінгу харчових жирів), особливості їх призначення при гіпертонічній хворобі, ожирінні, цукровому діабеті 2-го типу, атеросклерозі, короткі рекомендації з розширення фізичної активності в даної категорії пацієнтів.

The article presents the data of foreign and national literature on the pathogenetic role of aldosterone (AL), levels of aldosterone synthase (AS) and gene polymorphisms of this enzyme in patients with various forms of arterial hypertension, associated with diseases of the heart and vessels, such as hypertrophic cardiomyopathy, atrial fibrillation, brain pathology etc. The activation of circulating and local renin-angiotensin system results in increased secretion of the powerful vasoconstrictor angiotensin II, which stimulates AL production in an auto- and paracrine way. Angiotensin II and AL have independent regulatory effect on the function and structure of the heart and blood vessels, kidneys and brain. Both hormones stimulate hypertrophy of cardiomyocytes and vascular smooth muscle cells, hyperplasia of myocardial fibroblasts, synthesis of connective tissue matrix. As a result, the left ventricular hypertrophy is formed, cardiac remodeling is progressing, in which processes AL is involved. The main humoral, metabolic, cellular, immune-inflammatory and profibrogenic effects of aldosterone are considered. The AS gene CYP11B2 catalyzes the last stage of the synthesis of AL from desoxycorticosterone. The gene is located in the g21 region of chromosome 8, and consists of nine exons and eight introns. Single nucleotide polymorphisms are known in the promoter region of AL and in the AS gene. The polymorphism of the 5th section of this gene, which manifests itself by replacing cytosine (C) with thymine (T) at the 344th position of the 344 T/C nucleotide sequence, rs1799998, has been most fully investigated. MiR-766 can be used to suppress the expression of the AS gene in an experiment, as well as in humans, and reduce blood pressure level in people having the –344T allele. The determination of the single nucleotide replacement of T by C at position 344 of the CYP11B2 gene is carried out by polymerase chain reaction with polymorphism of the length of restriction fragment analysis. Currently, a list of forward and reverse primers, which are used in the amplification of the CYP11B2 gene, has been compiled. In the number of studies, the relationship between AS gene polymorphism and risk factors of cardiovascular and endocrine pathology were studied, the sex, gender and ethnic characteristics of AS gene polymorphism were considered, and the results of population studies of gene polymorphism of AS in several European and Asian countries were received. The information is provided on the AS levels and gene polymorphisms of this enzyme in arterial hypertension, including its resistant form, in the combination of ischemic heart disease, postinfarction cardiosclerosis, their participation in cardiovascular remodeling, the formation of left ventricular hypertrophy, in hypertensive nephropathy, cerebral stroke, heart failu­re. The levels of AL and gene polymorphism of AS also can be used as differential diagnostic criteria for hypertension with low plasma renin activity, with renovascular and resistant hypertension, at hyperaldosteronism types 1 and 2, aldosteromas, apparent mineralocorticoid excess syndrome, congenital adrenal hyperplasia. The congenital defects of enzymatic activity of AS, insufficiency of AS and aldosteronism (arterial hypertension), which are cured by glucocorticoids, are described. The results of several studies provide conflicting data on the degree of response to antihypertensive therapy by various first-line drugs and mineralocorticoid receptor antagonists, depending on the haplotype CYP11B2, are presented. The prospects of therapeutic use of a new class of drugs — aldosterone synthase inhibitors among various categories of patients with symptoms of arterial hypertension are considered.

Подано огляд даних літератури щодо лікування артеріальної гіпертензії у хворих на цукровий діабет. Розглянуто сучасні підходи до медикаментозного лікування артеріальної гіпертензії у цієї категорії хворих. Наведено детальну характеристику основних антигіпертензивних препаратів, які рекомендовані для адекватного контролю артеріального тиску у хворих на цук-ровий діабет.

A review of foreign and native sources of literature from scientometric databases provides data on the harmful and beneficial properties of such well-known nutrients as table salt and sugar in essential hypertension and associated diseases, primarily in abdominal obesity and diabetes mellitus. The historical aspects of obtaining these nutrients, their participation in the pathogenesis of hypertension, the physiology of sodium and glucose metabolism are considered; the evidence base for prospective epidemiological studies is provided; the negative impact of these products on the intestinal microbiota, alarming statistics of salt and added free sugar abuse in population of different countries, social, economic and medical aspects of excess salt and sugar as risk factors for chronic non-communicable diseases emergence and progression of arterial hypertension are noted. The methods for assessing their amount in food, approaches to inhibiting salt and sugar-rich food abuse are given; the attention on the materials of the WHO, European, American, and Ukrainian guidelines on the safe amount of both nutrients is focused. The paper gives advice on rational nutrition of patients and options for modern diets, first of all, a low-salt DASH diet (Dietary Approaches to Stop Hypertension) from the standpoint of salt and sugar limiting., В огляді закордонних та вітчизняних джерел літератури з наукометричних баз даних наводяться дані щодо шкідливих та корисних властивостей таких загальновідомих нутрієнтів, як кухонна сіль та цукор, за умов гіпертонічної хвороби та асоційованих з нею захворювань, насамперед при абдомінальному ожирінні, цукровому діабеті. Розглядаються історичні аспекти видобування цих нутрієнтів, їх участь у патогенезі гіпертонічної хвороби, фізіологія обміну натрію та глюкози, наводиться доказова база щодо проспективних епідеміологічних досліджень, відмічається негативний вплив цих продуктів на кишкову мікробіоту, тривожна статистика щодо зловживання кухонною сіллю та доданим вільним цукром серед населення різних країн, соціальні, економічні та медичні аспекти надлишку солі та цукру як факторів ризику виникнення хронічних неінфекційних захворювань і прогресування артеріальної гіпертензії. Наводяться методи оцінки їх кількості в їжі, підходи до гальмування зловживання багатими на сіль та цукор продуктами харчування, акцентується увага на матеріалах ВООЗ, європейських, американських та українських рекомендаціях щодо безпечної кількості обох нутрієнтів, надаються поради щодо раціонального харчування пацієнтів та варіанти сучасних дієт, насамперед низькосольова дієта DASH (дієтичні підходи щодо зупинення гіпертензії), з позиції обмеження солі і цукру.

Розглядаються способи немедикаментозної та медикаментозної корекції гіпертонічної хвороби серця, а також фактори, що перешкоджають регресуванню гіпертрофованого міокарда., The review presents data from the domestic and foreign literature on the prevalence, pathogenesis, modern methods of verification of hypertensive heart disease (left ventricular hypertrophy) in patients with arterial hypertension and conditions associated with it: coronary heart disease, atrial fibrillation, metabolic syndrome, diabetes mellitus. Methods of non-drug and drug correction of hypertensive heart disease, as well as factors that prevent the reverse regression of hypertrophied myocardium are considered.

Modern approaches to the drug treatment of arterial hypertension in this category of patients are considered. The detailed characteristics of the main antihypertensive drugs that are recommended for adequate control of arterial pressure in patients with diabetes mellitus are given.

The aim of the work was a comparative study of the clinical efficacy and tolerability of S-isomer of amlodipine and lercanidipine in patients with essential hypertension (EH) and abdominal obesity (AO). It was shown that monotherapy with S-amlodipine and lercanidipine are effective types in treating mild and moderate forms of EH, including that of associated with AO. However, treatment with lercanidipine in significantly greater number of cases leads to achievement of target blood pressure as compared to S-amlodipine. Lercanidipine monotherapy resulted in a significant decrease in the level of insulin resistance and microalbuminuria in the examined patients, while the S-amlodipine monotherapy did not lead to significant positive changes in these indicators. We revealed a reliable lower incidence of any adverse events in patients with EH and AO under the influence of lercanidipine monotherapy as compared with S-amlodipine. Treatment with lercanidipine caused significantly lower incidence of edema of the lower limbs in patients with EH and AO than racemic amlodipine and S-amlodipine.

X-ray photoelectron core-level and valence-band spectra for pristine and Ar + -ion irradiated (0 0 1) surfaces of AgCd 2 GaS 4 and AgCd 2 GaSe 4 single crystals grown, respectively, by the Bridgman method and the method of direct crystallization have been measured in the present work. The X-ray photoelectron spectroscopy (XPS) results reveal high chemical stability of (0 0 1) surfaces of AgCd 2 GaS 4 and AgCd 2 GaSe 4 single crystals. Electronic structure of AgCd 2 GaS 4 has been calculated employing the full potential linearized augmented plane wave method. For the AgCd 2 GaS 4 compound, the X-ray emission bands representing the energy distribution of the valence Ag d-, Cd d-, Ga p- and S p-like states were recorded and compared on a common energy scale with the XPS valence-band spectrum. The theoretical and experimental data regarding the occupation of the valence band of AgCd 2 GaS 4 were found to be in excellent agreement to each other. Second harmonic generation (SHG) efficiency of AgCd 2 GaS 4 by using the 320 ns CO laser at 5.5 μm has been recorded within the temperature range 80–300 K. Substantial increase of the photoinduced SHG which in turn is substantially dependent on the temperature has been detected for the AgCd 2 GaS 4 compound.

Spectra of absorption in glassy HgSe(x)-GeSe₂(₁-x) (x = 42, 46, 54) alloys were studied. Sustained relaxation processes - a shift of the optical absorption edge to longer wavelength - were detected. Also observed was a thermoinduced reversible process when the absorption edge is shifted in the same direction after the samples have been annealed at 100 °С.

Results of investigations of spectral characteristics in the fundamental absorption range for the glass-like alloys HgSe - GeSe₂ are represented. To explain the phenomenon of anomaly growth of the static disorder, the model of deforming tensions is discussed. The hypothesis concerning a sharp change of physical-and-chemical properties for the transition over the double eutectic point on the stable phase diagram of the HgSe - GeSe₂ system with a changing glass-creating matrix is suggested.
