Abstract

Summary Background Patients with chronic obstructive pulmonary disease (COPD) have few options for treatment. The efficacy and safety of the phosphodiesterase-4 inhibitor roflumilast have been investigated in studies of patients with moderate-to-severe COPD, but not in those concomitantly treated with longacting inhaled bronchodilators. The effect of roflumilast on lung function in patients with COPD that is moderate to severe who are already being treated with salmeterol or tiotropium was investigated. Methods In two double-blind, multicentre studies done in an outpatient setting, after a 4-week run-in, patients older than 40 years with moderate-to-severe COPD were randomly assigned to oral roflumilast 500 μg or placebo once a day for 24 weeks, in addition to salmeterol (M2-127 study) or tiotropium (M2-128 study). The primary endpoint was change in prebronchodilator forced expiratory volume in 1 s (FEV 1 ). Analysis was by intention to treat. The studies are registered with ClinicalTrials.gov, number NCT00313209 for M2-127, and NCT00424268 for M2-128. Findings In the salmeterol plus roflumilast trial, 466 patients were assigned to and treated with roflumilast and 467 with placebo; in the tiotropium plus roflumilast trial, 371 patients were assigned to and treated with roflumilast and 372 with placebo. Compared with placebo, roflumilast consistently improved mean prebronchodilator FEV 1 by 49 mL (p 1 was noted in both groups. Furthermore, roflumilast had beneficial effects on other lung function measurements and on selected patient-reported outcomes in both groups. Nausea, diarrhoea, weight loss, and, to a lesser extent, headache were more frequent in patients in the roflumilast groups. These adverse events were associated with increased patient withdrawal. Interpretation Roflumilast improves lung function in patients with COPD treated with salmeterol or tiotropium, and could become an important treatment for these patients. Funding Nycomed.

12. Pupil filters for moderate superresolution

Abstract

Some superresolving pupils, featuring a central maximum narrower than Airy’s core and a first secondary maximum of a few percent, have been computed, following Frieden’s formula and his criterion of maximum superresolution for circular symmetry. A superresolution factor of about 28% may be achieved without much sacrifice of illumination. Other pupils have been investigated where the SF reaches 35%. The behavior of the sidelobes in the point impulse image is examined. The corresponding encircled energy and point impulse response are given.

13. Réalisation D'un Filtrage Super-résolvant

Abstract

On présente la réalisation d'une pupille super-résolvante présentant un accroissement de 20 pour cent en résolution, un facteur d'éclairement au centre (coefficient de Strehl) de 0,11 et ne comportant pas d'anneau déphasant. Les maximums secondaires de la réponse percussionnelle associée sont tous inférieurs à 4 pour cent (champ non restreint). Les tolérances de réalisation ont été évaluées en calculant la réponse percussionnelle de filtres différent légèrement du filtre théorique: elles apparaissent suffisamment larges pour permettre un réalisation par voie photographique. Les performances atteintes sont très proches des prévisions de calculs et laissent entrevoir qu'un gain en résolution de 40 pour cent en champ non restreint pourrait être atteint par cette voie.

14. Atmospheric birefringence under wind speed gradient shear

Abstract

The birefringence of the atmosphere under the onset of a wind speed gradient has been studied near flat ground. Some characteristics and performances of the ellipsometer are given. The setup of the experiment is described; the results show that a birefringent effect is measurable. The numerical value of β = Δn/G is in good agreement with a previous experiment, but to an improved accuracy.

15. Elastooptical antenna for detection of gravitational radiation

Abstract

Using the photoelastic effect in transparent solids to detect stresses induced by a gravitational wave is proposed. The induced birefringence is measured by a polarimeter. The phase difference is amplified by a Fabry-Perot cavity containing the medium. The sensitivity of this optical strain sensor is discussed, and that of the antenna is estimated.
