An optimized digital RT-PCR (RT-dPCR) assay for the detection of human norovirus GI and GII RNA was compared with ISO 15216-conform quantitative real-time RT-PCR (RT-qPCR) assays in an interlaboratory study (ILS) among eight laboratories. A duplex GI/GII RT-dPCR assay, based on the ISO 15216-oligonucleotides, was used on a Bio-Rad QX200 platform by six laboratories. Adapted assays for Qiagen Qiacuity or ThermoFisher QuantStudio 3D were used by one laboratory each. The ILS comprised quantification of norovirus RNA in the absence of matrix and in oyster tissue samples. On average, results of the RT-dPCR assays were very similar to those obtained by RT-qPCR assays. The coefficient of variation (CV%) of norovirus GI results was, however, much lower for RT-dPCR than for RT-qPCR in intra-laboratory replicates (eight runs) and between the eight laboratories. The CV% of norovirus GII results was in the same range for both detection formats. Had in-house prepared dsDNA standards been used, the CV% of norovirus GII could have been in favor of the RT-dPCR assay. The ratio between RT-dPCR and RT-qPCR results varied per laboratory, despite using the distributed RT-qPCR dsDNA standards. The study indicates that the RT-dPCR assay is likely to increase uniformity of quantitative results between laboratories.

Mensen kunnen ziek worden van voedsel. Als twee of meer mensen tegelijk ziek worden na het eten van hetzelfde voedsel, dan heet dat een uitbraak door een voedselgerelateerde ziekteverwekker. In 2022 zijn 1.165 uitbraken met 4.470 zieken gemeld; in 2021 waren er 838 meldingen en 3.517 zieken. Het aantal meldingen is daarmee sinds 2016 (594 meldingen met 2.731 zieken) verdubbeld. Vaak is de oorzaak van de gemelde uitbraken niet bekend. Dat komt omdat niet altijd duidelijk is welk voedselproduct besmet kan zijn geweest of omdat er geen ziekteverwekker wordt gevonden. Bekend is dat het norovirus, Salmonella en Campylobacter in 2022, net als in eerdere jaren, nog steeds het meest zijn aangetoond als oorzaak van een uitbraak. Wel zijn er voor het derde jaar achter elkaar veel minder uitbraken van het norovirus gezien dan in de jaren ervoor. Dit komt omdat er tegenwoordig minder snel op dit virus wordt getest. De informatie over deze uitbraken komt van de Nederlandse Voedsel- en Warenautoriteit (NVWA) en de GGD’en. Zij registreren en onderzoeken voedselgerelateerde infecties en vergiftigingen. Daartoe proberen ze vanuit hun eigen werkveld te achterhalen waar mensen besmet zijn geraakt en door welke ziekteverwekker. De NVWA onderzoekt de plaats waar het voedsel is bereid of verkocht of waar het voedsel vandaan komt. Zij laat bij Wageningen Food Safety Research (WFSR) onderzoeken of daar ziekteverwekkers in zitten. De GGD richt zich op de personen die mogelijk via voedsel een infectie hebben opgelopen en probeert via hen te achterhalen waardoor ze zijn besmet. Het doel van deze werkwijze is meer zieken te voorkomen door het product uit de handel te halen, of maatregelen te nemen om herhaling te voorkomen. Het RIVM voegt de meldingen van de twee instanties samen en analyseert ze als één geheel. National Reference Laboratories (NRLs) are part of the system responsible for controlling and enforcing EU food and feed legislation. Wageningen Food Safety Research (WFSR) has been designated as NRL for thirteen areas of competence including milk and milk products, marine biotoxins, animal proteins in feedingstuffs, certain substances and residues thereof1, additives for use in animal nutrition (feed additives), genetically modified organisms in food and feed, pesticides, metals and nitrogenous substances in feed and food, mycotoxins and plant toxins in food and feed, processing contaminants, halogenated persistent organic pollutants in food and feed, food-borne viruses and water content of poultry. The tasks of WFSR’s NRLs depend on their respective fields of research. This annual report summarises all the activities carried out by WFSR’s NRLs in 2022. Staying updated on the latest developments is a crucial aspect of NRL’s responsibilities. The European Union Reference Laboratories (EURLs) arrange one or two workshops annually to facilitate this objective. Attendance at these workshops is mandatory for EURL-NRL members. In 2022, WFSR’s NRLs attended all the workshops. They actively participated in the EURL working groups to enhance their analytical methods. To assess the analytical capabilities of NRLs, EURLs conduct proficiency tests. Since the scope of these tests is sometimes limited, WFSR’s NRLs also participated in proficiency tests organised by other relevant organisations. The majority of the proficiency tests yielded satisfactory results, with only a few instances of ‘questionable’ or ‘unsatisfactorily’ outcomes. These were addressed through follow-up actions. To ensure the performance of Official Laboratories (OLs), their results in proficiency tests organised by WFSR’s NRLs were checked, or assurance samples were provided. Hepatitis E virus genotype 3 (HEV-3) is a food-borne pathogen causative of hepatitis E infections in humans. In Europe, HEV-3 is mainly transmitted through the consumption of raw or undercooked pork. In order to determine the effectiveness of control measures that can be taken in the industry or by the consumer, it is pivotal to determine the infectivity of HEV present in pork products after thermal food-processing steps. First, we implemented a method for the detection of infectious HEV-3c and HEV-3e in a cell culture medium and in extracts from inoculated pork products. Next, we investigated the effect of the thermal inactivation of HEV by mimicking food-processing steps specific for dried sausage and liver homogenate matrices. After four weeks, HEV-inoculated dried sausage subjected to 21 °C or lower temperatures was still infectious. For the liver homogenate, the highest HEV-3c/e inactivation of the conditions tested was observed at 71 °C for five min or longer. Finally, our method was able to successfully detect and estimate viral loads of infectious HEV in naturally infected pig livers. Our data provide a basis for the future use of the quantitative microbial risk assessment of infectious HEV in pork products that are subjected to thermal food processing steps.

RIVM rapport:Food can make people ill. If two or more people fall ill at the same time after eating the same food, it is called a food-related pathogen outbreak. In 2022, a total of 1,165 outbreaks were reported, with 4,470 people falling ill. In 2021, there were 838 reports with 3,517 people falling ill. Since 2016 (594 reports with 2,731 people falling ill), the number of reports has doubled. The cause of the reported outbreak is often unknown. This is because it is not always clear which food product may have been contaminated, or because no pathogen is found. It is known that the norovirus and the Salmonella and Campylobacter bacteria were still the most commonly identified causes of outbreaks in 2022, as they were in previous years. However, for the third year in a row there were far fewer outbreaks of the norovirus than in the years prior. This is because testing for this virus is less common nowadays. The information on food-related outbreaks comes from the Netherlands Food and Consumer Product Safety Authority (NVWA) and the Municipal Public Health Services (GGDs). They register and investigate food-related infections and poisoning. To this end, they each use their specific expertise to try to find out where people have become infected and by which pathogen. The NVWA examines the place where the food was prepared or sold, or where the food came from. It commissions Wageningen Food Safety Research (WFSR) to examine whether the food contains pathogens. The GGDs focus on the people who may have contracted an infection from food and try to find out through them what caused them to become infected. The aim of this approach is to prevent more cases of illness by removing the product from the market, or by taking measures to prevent a repeat incident. RIVM combines the reports from the two institutions and analyses them as a whole.
