Physicians must be aware of typical clinical presentations (such as cervicofacial actinomycosis following dental focus of infection, pelvic actinomycosis in women with an intrauterine device, and pulmonary actinomycosis in smokers with poor dental hygiene), but also that actinomycosis may mimic the malignancy process in various anatomical sites. Bacterial cultures and pathology are the cornerstone of diagnosis, but particular conditions are required in order to get the correct diagnosis. Prolonged bacterial cultures in anaerobic conditions are necessary to identify the bacterium and typical microscopic findings include necrosis with yellowish sulfur granules and filamentous Gram-positive fungal-like pathogens. Patients with actinomycosis require prolonged (6- to 12-month) high doses (to facilitate the drug penetration in abscess and in infected tissues) of penicillin G or amoxicillin, but the duration of antimicrobial therapy could probably be shortened to 3 months in patients in whom optimal surgical resection of infected tissues has been performed. Preventive measures, such as reduction of alcohol abuse and improvement of dental hygiene, may limit occurrence of pulmonary, cervicofacial, and central nervous system actinomycosis. Abstract

In cases where surgeons consider different interventional options for flow alterations in the setting of pathological basilar artery hemodynamics, a virtual model demonstrating the flow fields resulting from each of these options can assist in making clinical decisions. In this study, image-based computational fluid dynamics (CFD) models were used to simulate the flow in four basilar artery aneurysms in order to evaluate postoperative hemodynamics that would result from flow-altering interventions. Patient-specific geometries were constructed using MR angiography and velocimetry data. CFD simulations carried out for the preoperative flow conditions were compared to in vivo phase-contrast MRI measurements (4D Flow MRI) acquired prior to the interventions. The models were then modified according to the procedures considered for each patient. Numerical simulations of the flow and virtual contrast transport were carried out in each case in order to assess postoperative flow fields and estimate the likelihood of intra-aneurysmal thrombus deposition following the procedures. Postoperative imaging data, when available, were used to validate computational predictions. In two cases, where the aneurysms involved vital pontine perforator arteries branching from the basilar artery, idealized geometries of these vessels were incorporated into the CFD models. The effect of interventions on the flow through the perforators was evaluated by simulating the transport of contrast in these vessels. The computational results were in close agreement with the MR imaging data. In some cases, CFD simulations could help determine which of the surgical options was likely to reduce the flow into the aneurysm while preserving the flow through the basilar trunk. The study demonstrated that image-based computational modeling can provide guidance to clinicians by indicating possible outcome complications and indicating expected success potential for ameliorating pathological aneurysmal flow, prior to a procedure.

Abstract

Thrombus formation in intracranial aneurysms, while sometimes stabilizing lesion growth, can present additional risk of thrombo-embolism. The role of hemodynamics in the progression of aneurysmal disease can be elucidated by patient-specific computational modeling. In our previous work, patient-specific computational fluid dynamics (CFD) models were constructed from MRI data for three patients who had fusiform basilar aneurysms that were thrombus-free and then proceeded to develop intraluminal thrombus. In this study, we investigated the effect of increased flow residence time (RT) by modeling passive scalar advection in the same aneurysmal geometries. Non-Newtonian pulsatile flow simulations were carried out in base-line geometries and a new postprocessing technique, referred to as “virtual ink” and based on the passive scalar distribution maps, was used to visualize the flow and estimate the flow RT. The virtual ink technique clearly depicted regions of flow separation. The flow RT at different locations adjacent to aneurysmal walls was calculated as the time the virtual ink scalar remained above a threshold value. The RT values obtained in different areas were then correlated with the location of intra-aneurysmal thrombus observed at a follow-up MR study. For each patient, the wall shear stress (WSS) distribution was also obtained from CFD simulations and correlated with thrombus location. The correlation analysis determined a significant relationship between regions where CFD predicted either an increased RT or low WSS and the regions where thrombus deposition was observed to occur in vivo. A model including both low WSS and increased RT predicted thrombus-prone regions significantly better than the models with RT or WSS alone.

Abstract

International audience; Introduction: Free-running whole-heart acquisitions with the capability of deriving cardiac self-gating (SG) signals present clear interest, particularly at 3T, where the ECG signal suffers from the magnetohydrodynamic effect and acquisition related noise. Our goal was to develop a novel velocity-based cardiac SG strategy for free-running radial 4D Flow imaging and to investigate the impact of cardiac SG on the velocity quantification in the thoracic aorta. Methods: We implemented a free-running interleaved (8 spokes/interleaf) 3D radial velocity mapping sequence on a 1.5T Philips Ingenia (Philips, Best, The Netherlands), based on a spiral phyllotaxis pattern [1]. Imaging was performed on the thorax of 5 healthy subjects as follows: TE/TR 2.5/6 ms, FA=6•, VENC=180-240 cm/s, 2.5 mm isotropic voxel, acquisition time: 8-12 minutes. First, a sliding-window reconstruction using 64 consecutive spokes and a step of 8 spokes yielded high temporal and low spatial resolution velocity resolved volumes. The average speed in the aorta was obtained for each volume, using a mask drawn on the fully sampled reconstruction. Then, to obtain the cardiac SG signal, the temporal evolution of the aorta average speed was filtered in the frequency domain and interpolated using multiscale kernel ridge regression [2]. Finally, the cardiac SG signal and the ECG were used to bin the data in the same number of cardiac phases (N=12-18). Cardiac and velocity resolved images were then reconstructed offline using a compressed sensing algorithm implemented in Matlab (The Mathworks, Inc, Natick, MA). Velocity measurements in the thoracic aorta were then compared between SG and ECG based gating. Results: A comparison of the frequency analysis of the temporal evolution of the aorta speed and the ECG signal is presented in Figure A for a representative case. The frequency peak corresponding to the heart rate and the first harmonic can be observed at the same location. Despite similar average RR intervals between SG and ECG, the R-peak detection precision for SG was 50ms on average. Velocity quantification showed underestimation of peak systolic values for SG compared to ECG (Figure B). In order to confirm that the underestimation is due to the R-peak detection precision, we simulated 1% and 5% variability in the R-peak location in the ECG signal by random sampling according to a uniform distribution and used this simulated signal for cardiac binning. Similar to SG, an underestimation can be observed for the same case in the average speed (Figure C). Discussion: We present here a novel approach to cardiac SG specifically dedicated to 4D Flow MRI. Our results are in agreement with previously reported results using a navigator spoke based method for cardiac SG in 3D radial [3]. While this precision does not affect the resulting anatomical images, we have shown an impact on the velocity quantification, particularly an underestimation of the peak systolic values. This underestimation was described previously for 3D radial 4D Flow [1], however its source was not completely identified. Improvement of the precision in the R-peak detection is expected with the use of more advanced methods in the sliding window reconstruction. In addition, a respiratory SG signal can be obtained in absolute values and enable respiratory motion correction (not shown). This is an advantage of our method, compared to other previous fully self-gated strategies.

International audience; Introduction: Atrial fibrillation (AF) is a strong risk factor for embolic stroke likely due to prothrombotic alterations in left atrium (LA) blood flow. Four-dimensional velocity mapping MRI (4D Flow) allows quantification of LA and pulmonary veins (PV) flows. However, in the context of AF, standard 4D Flow leads to quantification errors from the inherent averaging of MRI [1]. Our goal was to develop a respiratory motion corrected free-running 3D radial based 4D Fow sequence and to investigate the AF related variability of LA blood flows. Methods: We implemented a free-running interleaved 3D radial velocity mapping sequence on a 1.5T Philips Ingenia (Philips, Best, The Netherlands), based on a spiral phyllotaxis pattern [2]. Imaging was performed in 6 paroxysmal AF patients, 2 permanent AF, and 5 age matched volunteers as follows: TE/TR 3.5/7.5 ms, FA=6•, VENC=70-96 cm/s, 2.2-2.8 mm isotropic voxel for an acquisition time of 8-12 minutes. The respiratory self-gated signal was obtained from the k-space center as previously proposed [3], and used to bin the data in 5 respiratory phases. The 3D translation motion of the heart between endexpiration and all other phases was estimated and used to correct the phase of the k-space data. Finally, the corrected data was binned into cardiac phases using the ECG signal and a fixed temporal resolution. This strategy reduces the velocity errors related to averaging over variable RR durations. Images were reconstructed offline using a compressed sensing algorithm implemented in Matlab (The Mathworks, Inc, Natick, MA). 4D Flow data analysis was performed with PIE Medical (The Netherlands). Results: Qualitatively, respiratory motion correction improved the sharpness of the PVs and modified slightly the flow pattern in the LA (Figure 1). Streamline visualization demonstrated significant differences in flow patterns, specifically a physiological vortex for healthy subjects, and absence of vortex and lower overall LA velocities for AF patients (Figure 2). Discussion: We present here preliminary results demonstrating the feasibility of intra-atrial flow assessment using a 3D radial based 4D Flow MRI with respiratory motion correction. The initial results will be completed by an ongoing quantitative analysis of the LA flows (vorticity, vortex size). Particular interest will be paid to the impact of respiratory motion correction on these quantitative descriptors. LA flows are related to the entire cardiac circulation. Thus an added advantage of the 3D radial 4D Flow is the capability to measure the hemodynamic parameters of the entire heart in the same amount of time as a standard Cartesian 4D Flow covering only the LA, albeit with stronger under-sampling.

Abstract

Background Optimal treatment of prosthetic joint infection and chronic osteomyelitis consists of surgical removal of biofilm-embedded bacteria, followed by a 6–12 week course of antimicrobial therapy. However, when optimal surgery is not feasible, oral prolonged suppressive antibiotic therapy (PSAT) is recommended to prevent prosthesis loosening and/or relapse of infection. Since 2010, we have used infection salvage therapy using off-label subcutaneous (sc) injection of a β-lactam as PSAT for patients in whom oral PSAT is not possible. Methods A single-centre prospective cohort study (2010–18) reporting treatment modalities, efficacy and safety in all patients receiving sc PSAT. NCT03403608. Results The 10 included patients (median age 79 years) had polymicrobial (n = 5) or MDR bacterial (n = 4) prosthetic joint infection (knee, n = 4; hip, n = 3) or chronic osteomyelitis (n = 3). After initial intensive therapy, seven patients received ertapenem, three patients received ceftriaxone and one patient received ceftazidime by sc injection (one patient received 8 days of ceftriaxone before receiving ertapenem). In one patient, sc PSAT failed with recurrent signs of infection under treatment. In three patients, sc PSAT had to be discontinued due to side effects; in only one of these was the sc route implicated (skin necrosis following direct sc injection and not gravity infusion). Median treatment duration was 433 days. In six patients, sc PSAT was successful with favourable outcome at the time of writing. Interestingly, three patients with MDR bacterial carriage at baseline lost this under PSAT during follow-up. Conclusions As salvage therapy, sc PSAT delivered by gravity infusion is a safe and interesting alternative when an optimal surgical strategy is not feasible and no oral treatment is available.

Abstract

International audience; Background Daptomycin has been recognized as a therapeutic option for the treatment of bone and joint infection (BJI). Gene polymorphism of ABCB1, the gene encoding P-glycoprotein (P-gp), may influence daptomycin pharmacokinetics (PK). Objectives We aimed to examine population PK of daptomycin and its determinants, including genetic factors, in patients with BJI. Patients and methods We analysed data from patients who received daptomycin for BJI between 2012 and 2016 in our regional reference centre and who had measured daptomycin concentrations and P-gp genotyping. A population approach was used to analyse PK data. In covariate analysis, we examined the influence of three single nucleotide variations (SNVs) of ABCB1 (3435C\textgreaterT, 2677G\textgreaterT/A and 1236C\textgreaterT) and that of the corresponding haplotype on daptomycin PK parameters. Simulations performed with the final model examined the influence of covariates on the probability to achieve pharmacodynamic (PD) targets. Results Data from 81 patients were analysed. Daptomycin body CL (CLDAP) correlated with CLCR and was 23% greater in males than in females. Daptomycin central V (V1) was allometrically scaled to body weight and was 25% lower in patients with homozygous CGC ABCB1 haplotype than in patients with any other genotype. Simulations performed with the model showed that sex and P-gp haplotype may influence the PTA for high MIC values and that a dosage of 10mg/kg/24h would optimize efficacy. Conclusions Daptomycin dosages higher than currently recommended should be evaluated in patients with BJI. Gender and P-gp gene polymorphism should be further examined as determinants of dosage requirements.

We aimed to compare the outcomes of NTM-RFA and SR for intermediate-sized HCC.\ Between 2012 and 2016, 141 patients with solitary HCC ranging from 2 to 5 cm were treated by NTM-RFA or SR at a single-center. The outcomes of 128 patients were compared after using inverse probability of treatment weighting (IPTW).\ Seventy-nine patients had NTM-RFA and 62 had SR. After IPTW, the two groups were well-balanced for most baseline characteristics including tumor size, location, etiology, severity of underlying liver disease and alpha-fetoprotein level. Abstract

We studied the correlation between the parameters of hepatic venous pressure measured before and after portal vein embolization and future hypertrophy of the liver remnant after portal vein embolization.\ Between 2014 and 2017, we reviewed patients who were eligible for major hepatectomy and who had portal vein embolization. Patients had undergone simultaneous measurement of portal venous pressure and hepatic venous pressure gradient before and after portal vein embolization by direct puncture of portal vein and inferior vena cava. We assessed these parameters to predict future liver remnant hypertrophy.\ Twenty-six patients were included. After portal vein embolization, median portal venous pressure (range) increased from 15 (9-24) to 19 (10-27) mm Hg and hepatic venous pressure gradient increased from 5 (0-12) to 8 (0-14) mm Hg. Median future liver remnant volume (range) was 513 (299-933) mL before portal vein embolization versus 724 (499-1279) mL 3 weeks after portal vein embolization, representing a 35% (7.4-83.6) median hypertrophy. Post-portal vein embolization hepatic venous pressure gradient was the most accurate parameter to predict failure of future liver remnant to reach a 30% hypertrophy (c-statistic: 0.882 [95% CI: 0.727-1.000], P < 0.001). A cut-off value of post-portal vein embolization hepatic venous pressure gradient of 8 mm Hg showed a sensitivity of 91% (95% CI: 57%-99%), specificity of 80% (95% CI: 52%-96%), positive predictive value of 77% (95% CI: 46%-95%) and negative predictive value of 92.3% (95% CI: 64.0%-99.8%). On multivariate analysis, post-portal vein embolization hepatic venous pressure gradient and previous chemotherapy were identified as predictors of impaired future liver remnant hypertrophy.\ Post-portal vein embolization hepatic venous pressure gradient is a simple and reproducible tool which accurately predicts future liver remnant hypertrophy after portal vein embolization and allows early detection of patients who may benefit from more aggressive procedures inducing future liver remnant hypertrophy.
