12. Added value of [18F]FDOPA PET to the management of high-grade glioma patients after their initial treatment: a prospective multicentre study.

Abstract

Background: Diagnostic value of 3,4-dihydroxy-6-[18F]fluoro-L-phenylalanine ([18F]FDOPA) PET in patients with suspected recurrent gliomas is recognised. We conducted a multicentre prospective study to assess its added value in the practical management of patients suspected of recurrence of high grade gliomas (HGG). Methods: Patients with a proven HGG (WHO grade III and IV) were referred to the multidisciplinary neuro-oncology board (MNOB) during their follow-up after initial standard of care treatment and when MRI findings were not fully conclusive. Each case was discussed in 2 steps. For step 1, a diagnosis and a management proposal were made only based on the clinical and the MRI data. For step 2, the same process was repeated taking the [18F]FDOPA PET results into consideration. A level of confidence for the decisions was assigned to each step. Changes in diagnosis and management induced by [18F]FDOPA PET information were measured. When unchanged, the difference in the confidence of the decisions were assessed. The diagnostic performances of each step were measured. Results: 107 patients underwent a total of 138 MNOB assessments. The proposed diagnosis changed between step 1 and step 2 in 37 cases (26.8%) and the proposed management changed in 31 cases (22.5%). When the management did not change, the confidence in the MNOB final decision was increased in 87 cases (81.3%). Step 1 had a sensitivity, specificity and accuracy of 83%, 58% and 66% and step 2, 86%, 64% and 71% respectively. Conclusion: [18F]FDOPA PET adds significant information for the follow-up of HGG patients in clinical practice. When MRI findings are not straightforward, it can change the management for more than 20% of the patients and increases the confidence level of the multidisciplinary board decisions. Abstract

International audience; BackgroundDiffuse hemispheric gliomas, H3 G34-mutant (DHG H3G34-mutant) constitute a distinct type of aggressive brain tumors. Although initially described in children, they can also affect adults. The aims were to describe the characteristics of DHG H3G34-mutant in adults and to compare them to those of established types of adult WHO grade IV gliomas.MethodsThe characteristics of 17 adult DHG H3G34-mutant, 32 H3.3 K27M-mutant diffuse midline gliomas (DMG), 100 IDH-wildtype and 36 IDH-mutant glioblastomas were retrospectively analyzed.ResultsMedian age at diagnosis in adult DHG H3G34-mutant was 25 years (range: 19-33). All tumors were hemispheric. For 9 patients (56%), absent or faint contrast enhancement initially suggested another diagnosis than a high-grade glioma, and diffusion-weighted imaging seemed retrospectively more helpful to suspect an aggressive tumor than MR-spectroscopy and perfusion MRI. All cases were IDH-wildtype. Most cases were immunonegative for ATRX (93%) and Olig2 (100%) and exhibited MGMT promoter methylation (82%). The clinical and radiological presentations of adult DHG H3G34-mutant were different from those of established types of adult grade IV gliomas. Median overall survival of adult DHG H3G34-mutant was 12.4 months compared to 19.6 months (p=.56), 11.7 months (p=.45) and 50.5 months (p=0.006) in H3.3 K27M-mutant DMG, IDH-wildtype and IDH-mutant glioblastomas, respectively.ConclusionsAdult DHG H3G34-mutant are associated with distinct characteristics compared to those of established types of adult WHO grade IV gliomas. The present study supports considering these tumors as a new type of WHO grade IV glioma in future classifications.

Abstract

International audience; Aim/Introduction: The design and validation of a reliable radiomic signature is challenging when few patients are available because the disease is rare, the imaging protocol is specific and/or the classes are unbalanced. In this context, we propose an approach to identify a signature and estimate its reliability. Materials and Methods: 84 patients with a clinical and MRI suspicion of recurrent glioblastoma were retrospectively included. Each patient underwent a 18F-FDOPA PET-CT scan. For each patient, the suspicious lesion was segmented and 49 radiomic features were calculated using LIFEx [1]. Our goal was to distinguish between tumor recurrence and radiation-induced necrosis as confirmed on pathological data, or on a 3-month clinical/imaging follow-up. A screening procedure was developed to identify a signature using leave-one-out (LOO) cross-validation. The procedure involved: 1) selection of all features with a p-value of univariate Wilcoxon test lower than alpha varying from 0.005 to 0.10 for (N-1) learning patients, 2) based on these features, selection of only one feature among correlated features using a Pearson correlation cutoff R varying from 0.95 to 0.50, 3) building of a radiomic signature involving the resulting features using a linear discriminant analysis, 4) test of the model on the Nth patient, 5) characterization of the model performance using the Youden Index (Y=sensitivity+specificity-1). The final model selection was based on the consistency of Y values as a function of alpha and R, on the consistency of selected features between the different models, and favored models involving a low number of features. To test the reliability of the selected signature, we repeated the process by excluding one patient using a jackknife procedure (ie, 84 LOO of 83 patients each). We compared the results with those obtained with the same alpha and R when randomly assigning a label to each patient (sham task). Results: 61 patients had tumor recurrence and 23 had radiation necrosis. Visual interpretation yielded Y equal to 0.35 (Lizarraga scale, Se=100%, Sp=35%). Using LOO, 10/28 radiomic models had Y>0.35. The largest Y (Y=0.49, Se=62%, Sp=87%) were obtained with 4 features on average, reflecting the volume, sphericity and heterogeneity (GLCM_Correlation, GLCM_Contrast) of the lesion uptake. Using the jackknife procedure, Y was 0.47±0.06 (range: [0.32;0.56]), significantly higher (Wilcoxon p < 0.05) than for the sham task (Y=0.06±0.18, range: [-0.52;0.47]). Abstract

The Drooling Impact Scale showed good validity and sensitivity to change, especially after botulinum toxin injection. The aim of this study is to present its French translation and to explore its validity, reliability and responsiveness to change in a group of children with Cerebral Palsy. multicentre study at six rehabilitation centres in France. Children with Cerebral Palsy aged 4–18 years with sialorrhea problems were included (n = 55), either in a control group (n = 33) or in the intervention group (n = 22, with 3 drug treatment and 19 botulinum toxin injections). The French Drooling Impact Scale was administered twice, 1 month apart. The French Drooling Impact Scale total score at inclusion was meanly 53.9 (Standard Deviation 11.9) in the stable control group and 66.0 (16.1) in the intervention group (p = 0.0058). The validity of the scale was established, as well as an adequate internal consistency (Cronbach's α = 0.71); correlations between each item and the total score were found between 0.5 and 0.71 except for item 5 (r = 0.38) and item 7 (0.41). The test-retest reliability in stable children was good (Lin coefficient = 0.83, bias correction factor = 0.92 and Pearson correlation coefficient = 0.89). There was a high responsiveness to change, mean change was –40.0 in the intervention group and –3.6 in the stable group (p < 0.0001), with Standard Error of Measurement = 2.6. the French Drooling Impact Scale has shown sufficient clinometric properties to be used now by clinicians or researchers. The Drooling Impact scale has now its French version. The French version of the Drooling Impact Scale has shown its validity and a good test-retest reliability. The responsiveness to change was explored in a group of children undergoing saliva-control interventions and the scale was able to show a big change. Abstract

Graphical abstract, Genomics and transcriptomics have led to the widely-used molecular classification of breast cancer (BC). However, heterogeneous biological behaviors persist within breast cancer subtypes. Metabolomics is a rapidly-expanding field of study dedicated to cellular metabolisms affected by the environment. The aim of this study was to compare metabolomic signatures of BC obtained by 5 different unsupervised machine learning (ML) methods. Fifty-two consecutive patients with BC with an indication for adjuvant chemotherapy between 2013 and 2016 were retrospectively included. We performed metabolomic profiling of tumor resection samples using liquid chromatography-mass spectrometry. Here, four hundred and forty-nine identified metabolites were selected for further analysis. Clusters obtained using 5 unsupervised ML methods (PCA k-means, sparse k-means, spectral clustering, SIMLR and k-sparse) were compared in terms of clinical and biological characteristics. With an optimal partitioning parameter k = 3, the five methods identified three prognosis groups of patients (favorable, intermediate, unfavorable) with different clinical and biological profiles. SIMLR and K-sparse methods were the most effective techniques in terms of clustering. In-silico survival analysis revealed a significant difference for 5-year predicted OS between the 3 clusters. Further pathway analysis using the 449 selected metabolites showed significant differences in amino acid and glucose metabolism between BC histologic subtypes. Our results provide proof-of-concept for the use of unsupervised ML metabolomics enabling stratification and personalized management of BC patients. The design of novel computational methods incorporating ML and bioinformatics techniques should make available tools particularly suited to improving the outcome of cancer treatment and reducing cancer-related mortalities.

Abstract

International audience; Objective: Gait rehabilitation is a major concern for adults with an intellectual disability or a neu-ropsychological disorder. This study evaluated a collective task exercise that could complement an individual rehabilitation routine in such individuals. The movements of 3 individuals (2 patients and 1 healthy individual) were measured while walking alone and in pairs. The recovery rate, amplitude and speed of centre of mass of individuals walking alone were measured and compared with the values of the centre of mass of the system formed by pairs of individuals. Results: When individuals were walking alone, all parameter values were lower in the 2 patients than in the healthy individual. When the patients were walking in pairs, their recovery rate decreased, but their speed increased when each of them was pai-red with a healthy individual. In pairs, the recovery rate and the amplitude of the centre of mass remained the same as when walking alone. Conclusion: Gait rehabilitation does not appear to improve when intellectually disabled patients walk in pairs compared with when they walk alone. However , walking with a healthy individual seems to be more efficient. litation doctor, a physiotherapist and an occupational therapist re-educate patients. It was investigated whether, when walking alone, the patients walk in the same way as healthy individuals and whether, when walking in LAY ABSTRACT A standard method to study locomotion is to analyze the trajectory of the center of mass of walking individuals. In our paper we propose to use this method to evaluate the changes in gait efficiency when such patients are walking alone and when they are walking in pairs linked by a load they transport. We worked with two patients suffering from an intellectual disability. These two patients could be paired either together or with their physiotherapist. Our results show that when the patients were walking in pairs their gait was less efficient than when walking alone. However, when the patients were paired with their physiotherapist, gait efficiency was the same as that as when they were walking alone. We suggest that this collective work could be used as a physical, social and mental exercise they could perform with their physiotherapist and that could be included in their rehabilitation routine.

Abstract

Purpose: This study aimed to assess the therapeutic impact and diagnostic accuracy of 18F-DOPA PET/CT in patients with glioblastoma or brain metastases.Methods: Patients with histologically proven glioblastoma or brain metastases were prospectively included in this monocentric clinical trial (IMOTEP). Patients were included either due to a clinical suspicion of relapse or to assess residual tumor infiltration after treatment. Multimodality brain MRI and 18F-DOPA PET were performed. Patients' data were discussed during a Multidisciplinary Neuro-oncology Tumor Board (MNTB) meeting. The discussion was first based on clinical and MRI data, and an initial diagnosis and treatment plan were proposed. Secondly, a new discussion was conducted based on the overall imaging results, including 18F-DOPA PET. A second diagnosis and therapeutic plan were proposed. A retrospective and definitive diagnosis was obtained after a 3-month follow-up and considered as the reference standard.Results: One hundred six cases were prospectively investigated by the MNTB. All patients with brain metastases (N = 41) had a clinical suspicion of recurrence. The addition of 18F-DOPA PET data changed the diagnosis and treatment plan in 39.0% and 17.1% of patients' cases, respectively. Concerning patients with a suspicion of recurrent glioblastoma (N = 12), the implementation of 18F-DOPA PET changed the diagnosis and treatment plan in 33.3% of cases. In patients evaluated to assess residual glioblastoma infiltration after treatment (N = 53), 18F-DOPA PET data had a lower impact with only 5.7% (3/53) of diagnostic changes and 3.8% (2/53) of therapeutic plan changes. The definitive reference diagnosis was available in 98/106 patients. For patients with tumor recurrence suspicion, the adjunction of 18F-DOPA PET increased the Younden's index from 0.44 to 0.53 in brain metastases and from 0.2 to 1.0 in glioblastoma, reflecting an increase in diagnostic accuracy.Conclusion: 18F-DOPA PET has a significant impact on the management of patients with a suspicion of brain tumor recurrence, either glioblastoma or brain metastases, but a low impact when used to evaluate the residual glioblastoma infiltration after a first-line radio-chemotherapy or second-line bevacizumab. Abstract

Aims Lower urinary tract dysfunctions (LUTD) are very common in Multiple Sclerosis (MS), have a significant social impact, while the organic impact is discussed. We studied urinary complications and their risk factors in our cohort of MS patients, in order to improve the management of LUTD in MS. Methods Between 2004 and 2009, all patients affected by MS and managed for LUTD were included in a retrospective study. We studied the epidemiological data (age, gender), the clinical data (duration of MS, EDSS score, progression of MS) and the paraclinical data (urinary creatinine clearance, urine culture, urinary tract ultrasonography and in some cases urodynamic assessment and cystography). We then identified the urinary complications and their risk factors. Results Three hundred twenty eight patients, mean age 49.9 ± 12.3 years, with a MS for 14.3 ± 10 years on average and with a median EDSS score equal to 6 (1-9), were managed for LUTD. One hundred seventy eight (54%) patients developed one or more urinary complications. We identified duration of MS greater than 8.5 years and an EDSS above 7 as risk factors. Conclusion Urinary complications are common in symptomatic MS, these results imply screening and specialized care to limit the impact on the quality of life but also to prevent urinary complications.
