Abstract

Neonates born very preterm (before 32 weeks’ gestational age), are a significant public health concern because of their high-risk of mortality and life-long disability. In addition, caring for very preterm neonates can be expensive, both during their initial hospitalization and their long-term cost of permanent impairments. To address these issues, national and regional neonatal networks around the world collect and analyse data from their constituents to identify trends in outcomes, and conduct benchmarking, audit and research. Improving neonatal outcomes and reducing health care costs is a global problem that can be addressed using collaborative approaches to assess practice variation between countries, conduct research and implement evidence-based practices. The International Network for Evaluating Outcomes (iNeo) of neonates was established in 2013 with the goal of improving outcomes for very preterm neonates through international collaboration and comparisons. To date, 10 national or regional population-based neonatal networks/datasets participate in iNeo collaboration. The initiative now includes data on >200,000 very preterm neonates and has conducted important epidemiological studies evaluating outcomes, variations and trends. The collaboration has also surveyed >320 neonatal units worldwide to learn about variations in practices, healthcare service delivery, and physical, environmental and manpower related factors and support services for parents. The iNeo collaboration serves as a strong international platform for Neonatal-Perinatal health services research that facilitates international data sharing, capacity building, and global efforts to improve very preterm neonate care.

Objective To determine the accuracy of the National Institute of Child Health and Human Development (NICHD) calculator in predicting death and neurodevelopmental impairment in Australian and New Zealand infants. Design Population-based cohort study. setting Australia and New Zealand. Patients Preterm infants 22-25 completed weeks gestation. Interventions Comparison of NICHD calculator predicted rates of death and death or neurodevelopmental impairment, with actual rates recorded in the Australian and New Zealand Neonatal Network cohort. Main outcome measures Infant death and death or neurodevelopmental impairment rates. results A total of 714 infants were included in the study. Of these infants, 100 (14.0%) were <24 weeks, 389 (54.5%) male, 529 (74.1%) were singletons, 42 (5.9%) had intrauterine growth restriction, 563 (78.9%) received antenatal steroids and 625 (87.5 %) were born in a tertiary hospital. There were 288 deaths (40.3%), 75 infants (10.5%) with neurodevelopment impairment and 363 (50.8%) with death or neurodevelopmental impairment. The area under the curve (AUC) for prediction of death and the composite death or neurodevelopmental impairment by the NICHD calculator in our population was 0.65(95% CI 0.61 to 0.69) and 0.65 (95% CI 0.61 to 0.69), respectively. When stratified and compared with gestational age outcomes, the AUC did not change substantially for the outcomes investigated. The calculator was less accurate with outcome predictions at the extreme categories of predicted outcomes-underestimation of outcomes for those predicted to have the lowest risk (<20%) and overestimation for those in the highest risk category (>>80%). conclusion In our recent cohort of extremely preterm infants, the NICHD model does not accurately predict outcomes and is marginally better than gestational age based outcomes.Copyright © Article author(s) (or their employer(s) unless otherwise stated in the text of the article) 2017. Abstract

Illiberal Democracy in Indonesia charts the origins and development of organicist ideologies in Indonesia from the early 20th century to the present. In doing so, it provides a background to the theories and ideology that informed organicist thought, traces key themes in Indonesian history, examines the Soeharto regime and his ‘New Order’ in detail, and looks at contemporary Indonesia to question the possibility of past ideologies making a resurgence in the country. Beginning with an exploration of the origins of the theory of the organic state in Europe, this book explores how this influenced many young Indonesian scholars and ‘secular’ nationalists. It also looks in detail at the case of Japan, and identifies the parallels between the process by which Japanese and Indonesian nationalist scholars drew on European romantic organicist ideas to forge ‘anti-Western’ national identities and ideologies. The book then turns to Indonesia’s tumultuous history from the revolution to 1965, the rise of Soeharto, and how his regime used organicist ideology, together with law and terror, to shape the political landscape consolidate control. In turn, it shows how the social and economic changes wrought by the government’s policies, such as the rise of a cosmopolitan middle class and a rapidly growing urban proletariat led to the failure of the corporatist political infrastructure and the eventual collapse of the New Order in 1998. Finally, the epilogue surveys the post Soeharto years to 2014, and how growing disquiet about the inability of the government to contain religious intolerance, violence and corruption, has led to an increased readiness to re-embrace not only more authoritarian styles of rule but also ideological formulas from the past. Abstract

Background: Compared to very low gestational age (<32 weeks, VLGA) cohorts, very low birth weight (<1500 g; VLBW) cohorts are more prone to selection bias toward small-for-gestational age (SGA) infants, which may impact upon the validity of data for benchmarking purposes. Method(s): Data from all VLGA or VLBW infants admitted in the 3 Networks between 2008 and 2011 were used. Two-thirds of each network cohort was randomly selected to develop prediction models for mortality and composite adverse outcome (CAO: mortality or cerebral injuries, chronic lung disease, severe retinopathy or necrotizing enterocolitis) and the remaining for internal validation. Areas under the ROC curves (AUC) of the models were compared. Result(s): VLBW cohort (24,335 infants) had twice more SGA infants (20.4% vs. 9.3%) than the VLGA cohort (29,180 infants) and had a higher rate of CAO (36.5% vs. 32.6%). The two models had equal prediction power for mortality and CAO (AUC 0.83), and similarly for all other cross-cohort validations (AUC 0.81-0.85). Neither model performed well for the extremes of birth weight for gestation (<1500 g and >=32 weeks, AUC 0.50-0.65; >=1500 g and <32 weeks, AUC 0.60-0.62). Conclusion(s): There was no difference in prediction power for adverse outcome between cohorting VLGA or VLBW despite substantial bias in SGA population. Abstract

OBJECTIVES: To compare survival rates and age at death among very preterm infants in 10 national and regional neonatal networks. METHOD(S): A cohort study of very preterm infants, born between 24 and 29 weeks' gestation and weighing <1500 g, admitted to participating neonatal units between 2007 and 2013 in the International Network for Evaluating Outcomes of Neonates. Survival was compared by using standardized ratios (SRs) comparing survival in each network to the survival estimate of the whole population. RESULT(S): Network populations differed with respect to rates of cesarean birth, exposure to antenatal steroids and birth in nontertiary hospitals. Network SRs for survival were highest in Japan (SR: 1.10; 99% confidence interval: 1.08-1.13) and lowest in Spain (SR: 0.88; 99% confidence interval: 0.85-0.90). The overall survival differed from 78% to 93% among networks, the difference being highest at 24 weeks' gestation (range 35%-84%). Survival rates increased and differences between networks diminished with increasing gestational age (GA) (range 92%-98% at 29 weeks' gestation); yet, relative differences in survival followed a similar pattern at all GAs. The median age at death varied from 4 days to 13 days across networks. CONCLUSION(S): The network ranking of survival rates for very preterm infants remained largely unchanged as GA increased; however, survival rates showed marked variations at lower GAs. The median age at death also varied among networks. These findings warrant further assessment of the representativeness of the study populations, organization of perinatal services, national guidelines, philosophy of care at extreme GAs, and resources used for decision-making.© Copyright 2017 by the American Academy of Pediatrics.

Abstract

Background: The value of antifungal prophylaxis depends partly on the incidence of neonatal fungal infection. We compared the incidence of fungal infection in babies in neonatal units which do and do not give antifungal prophylaxis using oral nystatin. Method(s): Prospective, multi-centre surveillance study from 1993 to 2006 of invasive fungal infection, defined as positive blood or cerebrospinal fluid culture, in babies <1500 g birth weight in neonatal units in Australia and New Zealand. Result(s): There were 118 episodes of invasive fungal infection in 14 778 babies <1500 g, an incidence of 0.80% (95% confidence interval (CI) 0.66 to 0.94%). All infections were due to Candida species, mostly C. albicans (74, 62.7%) and C. parapsilosis (39, 33.1%). The mortality was 16.5%. The incidence was 0.54% (0.38 to 0.70%) for babies <1500 g in units using selective or universal oral nystatin prophylaxis and 1.23% (0.84 to 1.62%) in units using no prophylaxis (p<0.001). The incidence of infection in babies <1000 g was 1.78% (106/5948) (95% CI 1.44 to 2.12%). The incidence was 1.23% (0.92 to 1.54%) for babies <1000 g in units using nystatin prophylaxis and 2.67% (1.97 to 3.37%) in units using no prophylaxis (p<0.001). Conclusion(s): The incidence of neonatal fungal infection was low in Australia and New Zealand, even without antifungal prophylaxis. Antifungal prophylaxis with oral nystatin was associated with a significantly lower incidence of fungal infection compared with no prophylaxis.

Abstract

Few would have predicted that two of the most popular public figures in Indonesia in 1996 would be senior judges. Judges have typically been seen as gormless and corrupt functionaries who do the government’s bidding in the government’s courts. Yet in May 1995 Administrative Court judge Benyamin Mangkoedilaga became an instant celebrity after his historic ruling that President Soeharto’s powerful Information Minister had acted beyond his powers when he banned the popular weekly magazine Tempo a year earlier. Benyamin’s courtroom in Jakarta erupted in jubilation at the decision, and the judge was for months in hot demand for appearances and interviews. More recently, the spotlight has turned to Adi Andojo Soetjipto, the Supreme Court judge whose bold denunciations of high level judicial corruption have severely embarrassed Indonesia’s top legal authorities. An unprecedented move by the Chair of the Supreme Court to dismiss Adi Andojo in June 1996 only increased the level of public admiration, if not adulation, for the ‘honest judge’ who continues to speak out against his colleagues with impunity. Between them, Benyamin Mangkoedilaga and Adi Andojo have been responsible for reviving hopes among the urban middle classes that the judiciary may one day take their place, implied in the constitution, as an independent power within the state.

Since seizing power more than three decades ago, Suharto's military-backed regime has stifled every attempt to organise opposition. Now his 'New Order' government, and the country as a whole, is reaping the results. Angry crowds are out in the streets burning and looting because there are simply no viable political organisations for people to turn to in order to vent their intense frustrations about the collapsing economy. In Thailand and South Korea, the economic crisis saw old governments voted out and new reformist governments elected. This has been impossible in Indonesia because Suharto has built a political system in which power is concentrated almost completely in his own hands.

President Suharto won another five years in office from the 1,000 member People's Consultative Assembly on 10 March 1998. But his struggle to restore faith in his government's capacity to manage Indonesia's desperate currency crisis is far from over. The global spotlight is on Indonesia and the pictures are of riots, panic buying and Chinese traders grieving in their fire-gutted shops. A collapse of international confidence in the rupiah has sent the currency into free fall, leaving ordinary people angry, bewildered and uncertain about the future. At the heart of the drama is the 76 year old President Suharto and his titanic battle of wills with the world's banker of last resort, the International Monetary Fund (IMF). In October 1997, Suharto called in the IMF in an attempt to cushion Indonesia from the effects of the currency crisis then affecting Thailand, Malaysia and the Philippines. He accepted their advice to close down sixteen badly performing banks without warning and shelve several mega-projects the IMF said Indonesia could not afford.

Abstract

Positing that the key to understanding most ‘Asian values’ rhetoric lies at the level of domestic politics, this paper examines the manufacture and political uses of ‘indigenist’ discourses in Indonesia. It contrasts populist and conservative forms of indigenism and explores how President Soeharto deployed the latter against his leftist, Muslim and liberal critics. The paper suggests that the rise of an educated and globally connected middle class has eroded the government's ability to rely on old ideological formulas and led it to search for new legitimatory mechanisms. Domestic turmoil arising from the economic crisis sweeping Asia may, however, see a return to nationalism and with it a boost in the stocks of indigenism and ‘Asian values’.

26. Radiological abnormalities in infants with urinary tract infections.

An intravenous urogram and micturating cystourethrogram were carried out in 100 infants presenting with documented urinary tract infections. Ninety three cases were identified by suprapubic aspiration and 7 by culture of two voided urine samples containing greater than 100 X 10(6) organisms per litre. The urinary tract abnormalities were analysed in respect of their clinical importance, patient's age, sex, and prematurity (in the 10 preterm infants). Radiological abnormalities were found in 47% of the infants (40% of boys; 63% of girls). Twenty nine per cent of the infants had a urinary tract abnormality regarded as clinically important--namely grade 3 or 4 vesicoureteric reflux, reflux nephropathy, or obstructive lesions requiring surgery. Six of the 10 preterm infants had radiological abnormalities. Spontaneous resolution or improvement occurred within 6 months of birth in three of the four preterm infants with severe vesicoureteric reflux. [ABSTRACT FROM AUTHOR]

35. Probiotics for Prevention of Severe Necrotizing Enterocolitis: Experience of New Zealand Neonatal Intensive Care Units.

Abstract

Introduction: Necrotizing enterocolitis (NEC) affects mainly preterm infants, has a multifactorial etiology and is associated with intestinal dysbiosis and disordered immunity. Use of probiotics for prophylaxis is beneficial with studies indicating reduction in NEC ≥ stage 2, late onset sepsis (LOS) and mortality. However, not all studies have shown a reduction, there are questions regarding which probiotic to use, whether infants <1,000 g benefit and the risk of probiotic sepsis. All neonatal intensive care units in New Zealand (NZ) use probiotics and contribute to an international database (Australian and New Zealand Neonatal Network or ANZNN). Objective: To use ANZNN data to investigate the experience of NZ neonatal units with probiotics for NEC prevention in a setting where the baseline incidence of severe NEC was low, to compare results of 2 commonly used probiotic regimes and report on the extremely low birth weight subgroup. Method: Outcomes before (Pre group 2007-2010) and after (Probiotic group 2013-2015) starting routine probiotics for preterm infants <1,500 g or <32 weeks were compared. Clinicians reviewed cases to ensure they met database criteria. Five units used Infloran ( Bifidobacterium bifidum and Lactobacillus acidophilus) and 1 unit used Lactobacillus GG (LGG) and bovine lactoferrin (bLF). Results: Four thousand five hundred and twenty nine infants were included and Pre and Probiotic groups were well-balanced with regard to gestation, birth weight and gender. The incidence of NEC in the Probiotic group was 1.6 and 2.7% in the pre group (corrected OR 0.62 CI 0.41-0.94). There was one case of probiotic sepsis. There was no significant difference between the Infloran and LGG/bLF combinations in regard to observed NEC rates. Late onset sepsis rates were significantly lower in the Probiotic group ( p < 0.01). Unlabelled: Extremely immature newborns develop a self-limiting normal anion gap metabolic acidosis in early life. This study examined the natural history of this acidosis in a population of infants of gestation less than 26 weeks in the first 14 days of life. The acidosis was maximal on day 4 with a mean base deficit of 10.6 mmol/l and had resolved in 90 % of infants by day 11. Dopamine usage was the only independent predictor of the acidosis. Its use was associated with a greater degree of acidosis., Conclusion: Extremely preterm infants experience a self-limiting normal anion gap metabolic acidosis in the first 2 weeks of life which is consistent with renal tubular immaturity.

Unlabelled: Plasma aldosterone levels were measured in 50 infants of less than 30 weeks gestation at 24 h (D1) and 7 days (D7). The relationship between the plasma aldosterone level and a number of clinical and biochemical variables was explored. Plasma aldosterone levels ranged from 1000 to 30000 pmol/l and were inversely correlated with the severity of illness (D1 or D7), serum sodium (D7) and 24 h sodium intake (D1). No correlation with the serum potassium level was noted., Conclusion: Plasma aldosterone levels in this extremely premature cohort were significantly greater than those reported in more mature infants. Important determinants were severity of illness and sodium homeostasis.

Abstract

Infants of very low birthweight (VLBW) who underwent thoracotomy were given 2.0 mg.kg-1 of bupivacaine by the intrapleural route, and serial blood levels were taken to determine the pharmacokinetic profile in this group of babies. It was apparent that the half life was longer, clearance lower, and volume of distribution greater than in term infants. Although the drug did not reach toxic levels at this dose, caution should be observed when redosing as the accumulation of the drug may be unpredictable.

Abstract

Aims: To assess the impact, mortality, morbidity and economic costs, of respiratory distress syndrome severe enough to warrant ventilation in one year in New Zealand., Methods: Review of data from all five New Zealand regional neonatal intensive care units' participation in the international OSIRIS trial of exogenous surfactant (Exosurf) treatment for respiratory distress syndrome (involving 6700 infants in 21 countries), and extrapolation of these data to a full year., Results: There were 265 New Zealand infants entered in the OSIRIS trial; the mean birthweight was 1335 g and mean gestation 29 weeks; 61% of infants were less than 30 weeks gestation. Forty-seven infants (17.7%) died prior to discharge from hospital, 40 deaths being attributed to prematurity or respiratory distress syndrome. One hundred and two infants (38.5% of the cohort; 45% of surviving infants) were oxygen dependent and 36 infants (13.6%) were dead at 28 days of age. Thirty-four infants (12.8% of the cohort; 15% of surviving infants) were oxygen dependent and 40 infants (15%) were dead at the expected date of delivery. Infants were intubated for a mean 12.5 days, with surviving infants of less than 27 weeks gestation intubated for a disproportionately long period of time. Seventy-two infants (29% of the 246 infants examined) had an abnormality detected by cranial ultrasound scan at 1 or 6 weeks of age and in 23 (9%) this was a major abnormality. Of surviving infants 16 (7.5% of 213 examined) had a major abnormality on cranial ultrasound scan. Amongst infants at high risk for respiratory distress syndrome (gestation less than 30 weeks) 53% received antenatal steroids, compared with 22% in the OSIRIS trial overall. In a full year the cost of caring for infants with respiratory distress syndrome sufficiently severe enough to warrant ventilation is estimated to be NZ$12.5 million. The average cost of caring for a surviving infant was roughly NZ$52,500 and a nonsurviving infant was NZ$24,500., Conclusions: In a full year (total births 60,000) approximately 350 New Zealand infants may require ventilation for respiratory distress syndrome. Increasing the percentage of infants who receive antenatal steroids is likely to be extremely cost effective. In the era of antenatal steroids and exogenous surfactant, 85% of infants with respiratory distress syndrome requiring ventilation survive to discharge home and over 90% of survivors are likely to be healthy normal adults.

The incidence, aetiologic organism and mortality associated with neonatal septicaemia were retrospectively reviewed over a seven year period (June 1983 to July 1990). Trends with time were evaluated by dividing the study period in half. Although there was no significant change in the incidence of septicaemia during the study period (3.59 vs 3.40 per 1000 livebirths), there was a marked increase in the numbers of very low birthweight infants developing early onset group B streptococcal sepsis and an associated increase in mortality.

Abstract

Sixty-five infants with seizures in the neonatal period were followed to the age of one year. There were 27 nonsurvivors, with 14 neonatal deaths from causes directly related to the initial neurologic insult, and 13 postneonatal deaths, eight related to neurologic sequelae and five to unrelated causes. Twenty-four (63.2%) of the survivors were neurologically normal, while five (13.2%) had seizures at age one year. Multivariate stepwise analysis identified low birthweight (p less than 0.01) and the number of anticonvulsants used to control the initial seizure(s) (p less than 0.05) as being significantly associated with nonsurvival and neurodevelopmental sequelae amongst the survivors was significantly associated with the duration of initial seizure (p less than 0.05).

Abstract

A cohort of very low birthweight (VLBW) infants (ie, less than 1500 g) who were admitted to the newborn unit, Waikato Hospital between 1984 and 1988 were followed to the age of 12 months. Four hundred and twenty-five infants were admitted with 385 surviving the neonatal period. A further 29 infants died in the postneonatal period (68.2 per 1000 livebirths). Sixteen of the infants died during their initial hospitalisation - primarily from complications of prematurity, while the remaining 13 died following discharge from hospital (9 from sudden infant death syndrome). The postneonatal mortality rate was 3-5 fold greater than that described in similar studies, reflecting the generally increased rate in New Zealand and a lower neonatal mortality rate. The duration of mechanical ventilation was associated with an increased risk of postneonatal death, but no risk factors for posthospital discharge death could be identified.

Arterial blood oxygen tensions (Pao2) were compared with simultaneously obtained transcutaneous oxygen measurements (TcO2) in 15 children during an acute asthma attack and in 18 children with an illness other than asthma. A close correlation between TcO2 and PaO2 was obtained in the non-asthmatic group but not in the asthmatics. It is suggested that impaired cutaneous blood flow may explain this poor correlation.

Fifteen preterm babies (mean gestation: 26.7 weeks; mean birthweight 860 g) with significant glucose intolerance were treated with insulin infusions. During the insulin infusions there was a significant increase in both the mean energy intake (60.8 +/- 25.1 cal/kg per day to 79.9 +/- 24.5 cal/kg per day; P less than 0.001) and the mean amount of intravenous dextrose tolerated (7.0 +/- 2.7 mg/kg per min to 9.2 +/- 2.6 mg/kg per min; P less than 0.01). The infusions were initiated at a mean postnatal age of 5.3 days (range: 2-12 days) and were continued for 1.5-17.5 days. Of the 998 blood glucose estimations performed during the insulin infusions, 28 (2.8%) were less than 2 mmol/l and 216 (21.6%) greater than 8 mmol/l. We conclude that continuous insulin infusion is a safe and effective way of managing glucose intolerance in very low birthweight infants, provided adequate means for continuous monitoring of blood glucose are available.
