Abstract

Introduction : ce sont des hémopathies malignes chroniques résultant de l’atteinte de la cellule souche hématopoïétique pluripotente et se traduisant par prolifération monoclonale, sans blocage de la maturation cellulaire, touchant au moins l’une des trois lignées myéloïdes (granuleuse, érythroblastique, mégacaryocytaire). ils rassemblent quatre pathologies : leucémie myéloïde chronique, polyglobulie de Vaquez, myélofibrose primitive et thrombocytémie essentielle. Matériels et méthodes : Il s’agit d’une étude rétrospective réalisée sur une période de 6 ans allant de 2002 à 2007. Les données ont été colligées à partir des registres du laboratoire. Le diagnostic de ces différents syndromes myéloprolifératifs était basé sur les données de l’hémogramme, du frottis sanguin, du myélogramme et éventuellement par l’étude cytogénétique des cellules myéloïdes. Résultat : Cinquante cas de syndromes myéloprolifératifs sont rapportés, répartis comme suit : 44 cas de leucémies myéloïdes chroniques, quatre de polyglobulie de Vaquez et deux de myélofibrose primitive. Il n’a pas été noté de cas de thrombocytémie essentielle. Les moyennes d’âge étaient respectivement de 42 ± 15 ans, 56 ± 14 ans et de 43 ± 5 ans. Il y’avait une prédominance masculine. Discussion : Il y’ a peu de données sur l’épidémiologie de ces affections, leur incidence est estimée dans les pays occidentaux à 5 à 10 nouveaux cas par millions d’habitants, chaque année, selon le type. Dans notre série l’âge de survenue de ces syndromes est plus précoce par rapport aux séries européennes. 7. ANALYSE CRITIQUE DE LA PRESCRIPTION DU DOSAGE DE LA TROPONINE Ic

Abstract

Introduction : La troponine Ic est un bon outil diagnostic et pronostic dans la prise en charge des pathologies ischémiques cardiaques. Sa prescription dans le service des urgences dépasse souvent le cadre des recommandations internationales. Notre objectif est d’évaluer les pratiques concernant la prescription du dosage de troponinr Ic au service des urgences. Matériels et méthodes : Il s’agit d’une étude propective sur une période d’ un mois (01 /04 /2009 au 30 /04/2009). Soixante seize patients ont bénéficié d’une prescription du dosage de la troponine Ic au cours de la période d’étude. Au total, 130 dosages ont été réalisés, avec la modalité d’un premier dosage à l’admission, puis un deuxième 6 à 9 heures après, en cas de négativité du premier. Résultats : Les motifs d’admission ayant conduit à la prescription du dosage sont : douleur thoracique (63 %), dyspnée (18,4 %), troubles de conscience (7,8%), douleurs épigastriques (6,8%), dysarythmie (4%). Le nombre de résultats pathologiques de la troponinr Ic en fonction des symptômes d’appel est de 20 pour les douleurs thoraciques, 3 pour la dyspnée, 4 pour les troubles de conscience. Le nombre de résultats pathologiques de la troponine Ic en fonction de la pathologie diagnostiquée est de 17 pour 27 cas de syndrome coronarien aigu, 6 pour 15 cas d’insuffisance cardiaque, 2 pour 7 cas de cardiopathie ischémique, 1 pour 18 cas de douleurs thoraciques atypiques et 1 pour 9 cas d’oedème aigu des poumons. Discussion : Le dosage de la troponine Ic est demandé à mauvais escient dans un grand nombre de cas vu le pourcentage de résultats pathologiques (20,8 %). Ce dosage comme celui de tout marqueur biologique n’est qu’une aide au diagnostic. Conclusion : Ces résultats suggèrent qu’une meilleure maîtrise de l’utilisation de la troponine Ic, est indispensable de la part des prescripteurs., Maroc Médical, Vol. 32, No 2 (2010)

Abstract

Introduction : Les candiduries sont en recrudescence depuis ces dernières années en particulier en milieu hospitalier. Pourtant, il n’existe toujours pas de critères absolus pour différencier une colonisation d’une vraie infection. Notre objectif est de déterminer la fréquence des candiduries, leurs places dans ces infections urinaires nosocomiales, ainsi que la sensibilité des espèces isolées aux principaux antifongiques. Matériels et Méthodes : Cette étude est réalisée au laboratoire de parasitologie-mycologie sur une période de deux années avec 2988 patients inclus. Résultats : cinquante sept cas d’infection urinaire à Candida sont diagnostiqués. La fréquence des infections urinaires tout germes confondu est de 29% et celle des infections urinaires à Candida est de 2%. La place de la candidurie dans les infections urinaires est de 7%. Le Candida albicans a été isolé chez 31 patients (54%) suivi du Candida glabrata (24%), Candida tropicalis (17%) et Candida parapsilosis (7%). l’antifongigramme a montré une résistance de Candida glabrata dans 33% des cas, et Candida tropicalis dans 20% des cas au fluconazole. Conclusion : Les candiduries sont devenues de plus en plus fréquentes surtout en milieu de réanimation, du faite de l’émergence de nouvelles espèces non albicans. Cette candidurie peut être le témoin d’une simple colonisation, comme elle peut être un signe d’alarme d’une infection profonde, primaire ou secondaire à une dissémination hématogène., Maroc Médical, Vol. 31, No 1 (2009)

Abstract

Introduction : La toxoplasmose est une zoonose cosmopolite due à un protozoaire toxoplasma gondii. Habituellement bénigne, elle est redoutable chez le fœtus et le patient immunodéprimé. Dans 90% des cas, la réactivation du parasite a lieu au niveau cérébral, conduisant à des abcès cérébraux. Observation : Nous rapportons le cas d’un patient de 45 ans, VIH positif qui est admis en réanimation dans un tableau comateux. Le diagnostic de toxoplasmose est évoqué devant l’image radiologique en cocarde de la TDM cérébrale. Le bilan biologique retrouve une anémie macrocytaire, une thrombopénie et des images de grands lymphocytes hyper basophiles, faisant penser à un syndrome mononucléosique. Une ponction lombaire et un prélèvement sanguin sont réalisés pour la détermination du titre des Ac Ig G antitoxoplasmiques dans le sérum et le LCR en vue de calculer la charge immunitaire et le coefficient de Desmonds. Le CD‘ 4 est en faveur d’une synthèse intrathécale d’anticorps antitoxoplasmiques, témoignant d’une infection cérébrale évolutive. Un traitement par la Pyriméthamine – Clindamycine est instauré avec une évolution favorable. Conclusion : La toxoplasmose cérébrale doit être systématiquement évoquée devant des manifestations neurologiques centrales chez un patient VIH positif. Le diagnostic biologique précoce est basé sur la mise en évidence d’une synthèse intrathécale d’anticorps antitoxoplasmique., Maroc Médical, Vol. 29, No 3 (2007)

Abstract

Trichosporon asahii is a basidiomycete yeast responsible for white piedra and onychomycosis in the immunocompetent host. In the immunocompromised patients, invasive infections are reported; their diagnosis is difficult and they are associated with high mortality rate. Urinary infection due to Trichosporon Asahi is rare but its incidence increasing. We report the case of a 58 year old diabetic patient. The yeast was isolated from urine samples of three consecutive crops in pure form. The patient improved after antifungal therapy.

Abstract

Objectives: This study had for objective to assess the frequency of resistance to fluoroquinolones and to third generation cephalosporin in E. coli isolated from urines of consulting and hospitalized patients and to detect the rate of multiresistant E. coli strains., Design: A retrospective survey was made over 3 years (1(st) January 2005 to 31(st) December 2007). Eight hundred and nineteen patients presented with UTI confirmed in the Rabat Cheikh Zayd Teaching Hospital., Results: E. coli was the etiologic agent in 57% of reported UTI. The frequency of E. coli resistance to fluoroquinolones was 27% with a higher rate among hospitalized patients. We found that ten E. coli strains were producing extended-spectrum beta-lactamase and resistant to aminosides and fluoroquinolones., Conclusions: The resistance of E. coli to fluoroquinolones is becoming worrying among consulting and hospitalized patients. Ten strains multiresistant to fluoroquinolones and third generation cephalosporins, probably because of plasmids, were isolated. This increasingly frequent resistance mechanism should lead to a more careful use of first line fluoroquinolones for UTI.
